        Single-Port Thoracic Surgery: A New Direction

        Single-port video-assisted thoracic surgery (VATS) has slowly established itself as an alternate surgical approach for the treatment of an increasingly wide range of thoracic conditions. The potential benefits of fewer surgical incisions, better cosmesis, and less postoperative pain and paraesthesia have led to the technique’s popularity worldwide. The limited single small incision through which the surgeon has to operate poses challenges that are slowly being addressed by improvements in instrument design. Of note, instruments and video-camera systems that are narrower and angulated have made single-port VATS major lung resection easier to perform and learn. In the future, we may see the development of subcostal or embryonic natural orifice translumenal endoscopic surgery access, evolution in anaesthesia strategies, and cross-discipline imaging-assisted lesion localization for single-port VATS procedures.

        Developmental and Psychopathological Issues of Intrapsychic Loneliness and Aloneness during Midlife: A Clinical Case

        This paper begins with exploration of the origins of psychic loneliness and aloneness and presentation on a developmental line of human experiences from infancy to midlife. The theoretical section is followed by the presentation on detailed clinical materials obtained from the ongoing analysis of a fifty-one year old male patient who is unable to deal with issues related to aging. Prior to receiving treatment, the patient attempted to alleviate his increasing feeling of isolation and loneliness by leaving his wife and having affairs with other women. When his symptoms intensified, he reluctantly sought treatment. Unresolved abandonment issues lead to the formation of an elaborate compensatory fantasy life in both childhood and adulthood. The paper concludes with a detailed developmental discussion on the patient’s psychopathology and the analytic process.

        Correlation of Plasma Level of Insulin-Like Growth Factor-1 (IGF-1) with Bacterial Index on Leprosy Patients in Bali

        Background: Leprosy is an infection by Mycobacterium leprae, which influenced by cellular immunity. Leprosy tends to occur in low socio-economic and nutrition groups. Researchers try to prove the role of nutrition in the pathogenesis of leprosy. Insulin-like growth factor-1 (IGF-1) as a marker of nutritional status shown to play a role in cellular immunity. Objective: To evaluate the correlation between IGF-1 with bacterial index (BI) on leprosy patients in Bali. Methods: Cross-sectional study in Sanglah Public General Hospital, Denpasar of patients with paucibacillary (PB) and multibacillary (MB) leprosy were assessed for BI using slit-skin smear. All patients were tested for plasma IGF-1 using chemiluminescent immunometric assay Immulite. All data were analyzed using IBM SPSS ver. 24.0. The study has been approved by local Institutional Review Board with ethical clearance number 2017.02.1.0356. Results: Our study involved 44 MB and 2 PB leprosy. The common age group affected was between 31∼40 years old (23.9%), male (60.9%), and normal body mass index (BMI) (65.2%). Mean plasma IGF-1 level in PB leprosy was higher (91.07±0.74 ng/ml) than MB (82.74±6.44 ng/ml). The mean IGF-1 level decreases as BI increases in both groups (CI 95%=81.16∼85.04; p< 0.001). Pearson correlation test shows strong negative correlation (Pearson r=−0.976; p<0.001) with determinant coefficient (R2) showing 95.2% (p<0.001). Conclusion: In Balinese leprosy patients, severity of disease status measured by BI were found to be strongly correlated with the plasma IGF-1 level which may help preventing transmission in household contacts by improving nutritional status.

      • The SSP4: A world of deepening inequality

        <P><B>Abstract</B></P> <P>Five new scenarios, or Shared Socioeconomic Pathways (SSPs), have been developed, spanning a range of challenges to mitigation and challenges to adaptation. The Shared Socioeconomic Pathway 4 (SSP4), “Inequality” or “A Road Divided,” is one of these scenarios, characterized by low challenges to mitigation and high challenges to adaptation. We describe, in quantitative terms, the SSP4 as implemented by the Global Change Assessment Model (GCAM), the marker model for this scenario. We use demographic and economic assumptions, in combination with technology and non-climate policy assumptions to develop a quantitative representation of energy, land-use and land-cover, and emissions consistent with the SSP4 narrative. The scenario is one with stark differences within and across regions. High-income regions prosper, continuing to increase their demand for energy and food. Electrification increases in these regions, with the increased generation being met by nuclear and renewables. Low-income regions, however, stagnate due to limited economic growth. Growth in total consumption is dominated by increases in population, not increases in per capita consumption. Due to failures in energy access policies, these regions continue to depend on traditional biofuels, leading to high pollutant emissions. Declining dependence on fossil fuels in all regions means that total radiative forcing absent the inclusion of mitigation or impacts only reaches 6.4Wm<SUP>−2</SUP> in 2100, making this a world with relatively low challenges to mitigation. We explore the effects of mitigation effort on the SSP4 world, finding that the imposition of a carbon price has a varied effect across regions. In particular, the SSP4 mitigation scenarios are characterized by afforestation in the high-income regions and deforestation in the low-income regions. Furthermore, we find that the SSP4 is a world with low challenges to mitigation, but only to a point due to incomplete mitigation of land-related emissions.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Describes the quantification of the SSP4 storyline by the Global Change Assessment Model (GCAM), the marker model for the SSP4. </LI> <LI> Examines the implications of the SSP4 world on energy, land, emissions, and climate, with and without efforts to mitigate. </LI> <LI> Compares the GCAM SSP4 to other scenarios. </LI> </UL> </P>

      • AN OVERVIEW OF CONFUCIUS ETHICS IN BUSINESS MANAGEMENT: TEXT MINING OF JOURNAL ARTICLES DURING 1999–2016

        Aims: Using text mining techniques to investigate the understudied publication trend and thematic areas in the Confucius Business Ethics relating publications. Methods: Research articles which were published during 1999–2016, were searched in 10 business relating databases on April 9, 2017; resulting a total of 488 articles were included. The titles and abstracts were extracted for the text mining analysis using SAS Text Miner, themes were identified by the Text Topic Node. Publication trend, clustering and associations between themes were analyzed. Results: Eight groups of themes were identified. During 1999–2016, the appearance of the Themes “Confucius Leadership”, “Competitive Advantage”, “Corporate Social Responsibility (CSR), and “Satisfaction and Health” was continuously increasing; meanwhile, research relating to “Chinese Economy” was continuously decreasing; there was a fluctuation trend in that of “Global Ethics”, “Moral and Human Right”, and “Asian Studies”. In the cluster analysis, three main clusters of themes were classified: “Ethics and Leadership”, “Competition and Economy in Asia”, and “CSR and Satisfaction”. Besides, the appearance of “Confucius Leadership” and “Global Ethics”, “Confucius Leadership” and “Moral & Human Right”, “Asia” and “Competitive Advantage”, and “Asia” and “CSR was positively associated; whereas, that of “Global Ethics” and “China Economy” was negatively associated. Conclusion: The research of Confucius Business Ethics has been focusing on Eastern and Western cross-cultural business ethics, the Confucius ethics and CSR, which was associated with leadership, moral and human rights, in the Global and Asian environments. A call for future research in “Global Ethics” and “China Economy” were suggested.

      • An anisotropic tertiary creep damage constitutive model for anisotropic materials

        <P><B>Abstract</B></P><P>When an anisotropic material is subject to creep conditions and a complex state of stress, an anisotropic creep damage behavior is observed. Previous research has focused on the anisotropic creep damage behavior of isotropic materials but few constitutive models have been developed for anisotropic creeping solids. This paper describes the development of a new anisotropic tertiary creep damage constitutive model for anisotropic materials. An advanced tensorial damage formulation is implemented which includes both material orientation relative to loading and the degree of creep damage anisotropy in the model. A variation of the Norton-power law for secondary creep is implemented which includes the Hill’s anisotropic analogy. Experiments are conducted on the directionally-solidified bucket material DS GTD-111. The constitutive model is implemented in a user programmable feature (UPF) in ANSYS FEA software. The ability of the constitutive model to regress to the Kachanov-Rabotnov isotropic tertiary creep damage model is demonstrated through comparison with uniaxial experiments. A parametric study of both material orientation and stress rotation are conducted. Results indicate that creep deformation is modeled accurately; however an improved damage evolution law may be necessary.</P> <P><B>Highlights</B></P><P>► The deformation of anisotropic creeping solid is directionally dependent. ► Few constitutive models have been developed to deal with anisotropic behavior. ► A transversely-isotropic nickel base superalloy, DS GTD-111, is studied. ► A vector constitutive model based on the Kachanov-Rabotnov formulation is developed. ► The new model accurately models deformation at various orientations.</P>
