      • 증식당뇨망막증에서 아르곤레이저 광응고술의 치료 효과

        Vitreous hemorrhage, which is the major cause of blindness in proliferative diabetic retinopathy, has been prevented clinically with laser photocoagulation. We reviewed 57 cases(98 eyes) of proliferative diabetic retinopathy which were treated by argon laser and followed up for more than 6 months. Of these 98 eyes, 81 eyes(83%) showed a satisfactory response after standard panretinal photocoagulation. 14 eyes(14%) revealed the vitreous hemorrhage after panretinal photocoagulation and higher incidence of vitreous hemorrhage in the cases of new vessel on the optic disc. In the cases(26 eyes) of new vessel on the optic disc, 2000 laser burns revealed lower incidence of vitreous hemorrhage(1/5, 20%) in comparison with 1500 laser burns(8/21, 38%). These results indicate that initial augmented laser therapy prevented the vitreous hemorrhage effectively.

      • Superolateral Orbital Rim Craniotomy for Middle Cerebral Artery Aneurysms

        Objective : Recently, several approaches for middle cerebral artery aneurysms (MCA) have been developed from conventional method, that is pterional approach. However, the literatures contain no report about minimally invasive surgery for the treatment of MCA aneurysms. So the purpose of our study is to describe a new technique, that is superolateral orbital rim craniotomy (SLORC) approach-minimally invasive surgery and the outcome for the treatment of MCA aneurysms. Methods : A retrospective review of 12 patients with MCA aneurysms ruptured or unruptured who were operated by SLORC approach was performed during October 1999 thru December 2001. Results : All the aneurysms were clipped successfully. The mean aneurysm size was 6.8 mm ranged from 4 to 15 mm. The average hospital stay was 15.9±9.1 days and the operation time usually consumed 3 to 4 hours. No patients died and adverse effects directly related with surgery were minimal. Cosmetic result was also achieved just a few days after operation. We never used lumbar drain or extraventricular drainage. During operation, less retraction of neural structure and no transfusion were required. The modified Rankin scores at discharge and 6 months afterwards revealed 0 grade in all assessed patients. Conclusion : The SLORC is a newly developed procedure for neurosurgeon to treat MCA aneurysms. Although this technique is not widely used, it is likely that this minimally invasive approach will become a promising operative technique in the future.

      • Gullian Barre Syndrome 患者 1例에 대한 症例報告

        Gullian Barre Syndrome is an acute symmetric polyradiculoneuropathy that typically presents as a progressive flaccid paralysis. The pathology is believed to be caused by both cellular and humoral immune processes The possibility of death is 3-5% and About 20% of patients require mechanical ventilaton. Recently We have experienced a patient who had suspected Gullian Barre Syndrome for 60days, his condition weakness and pain was impoved through the herb and acupucture Therefore I reported it for the treatment.

      • 소뇌출혈후 신경인성방광으로 배뇨장애를 주소로 한 환자 치험 1例

        In patients with Cerebral vascular accident, Voiding Dysfunction is very usual, but if with no improvement for a long time, it can cause many complexed-problems as urinarytract injury, infection or inflamation and patients suffering from pain, inconvenience and anxiety. In this case 58-year-old female with Voiding Dysfunction with Neurogenic Bladder after Cerebellum Hemorrhage, for 6 monts there was no interval change on symptom, We prescribed appropeiate herbal medication, and perform acupuncture and moxa. 5 days later after admission in Taejeon Oriental Hospital, The patien was able to have self-voiding and gradually improved. So we report it for the better treatment.

      • 혈소판 응집 억제제의 효과 측정법에 대한 고찰과 지원자에서 Triflusal의 혈소판 응집 억제능 평가

        The anti-platelet agent is a member of a class of pharmaceuticals that decreases platelet aggregation and inhibits thrombus formation. They are effective in the arterial circulation and widely used in primary and secondary prevention of thrombotic cerebrovascular or cardiovascular disease. As a method for estimating the effects of anti-platelet agent, platelet aggregation was conventionally measured using the optical method or the impedance method. Several alternate methods currently in development or recently developed were considered, including luminescence method, flow cytometry, laser-light scattering method, and Verify Now-P2Y12 assay. Principles, advantages, and disadvantages of the optical method, impedance method, and the other alternate platelet aggregation methods were discussed in this report. 15 human volunteers were recruited for the evaluation of the efficacy of triflusal using the optical method. After the oral administration of a single dose of 900mg, 15 subjects received eight doses administered at 24-hour intervals of 600mg triflusal. Using platelet rich-plasma from above subjects, we performed baseline platelet aggregation test induced by adenosine diphosphate(ADP), collagen, and arachidonic acid. The results of platelet aggregation test after triflusal administration were compared with the baseline study. Triflusal significantly inhibited platelet aggregation induced by ADP (33.0±21.3%) and arachidonic acid (99.1±1.2%), respectively. Therefore, we concluded that anti-platelet aggregation effect of triflusal can be studied successfully with the optical method. Each of the platelet aggregation methods has value for evaluating the effects by various mechanisms of the anti-platelet agents. The ideal method for estimating the platelet aggregation as it relates to safety and efficacy in patients treated with anti-platelet agents will need to be determined in clinical trials.

      • Paraquat 유도독성에 대한 Ginkgo biloba Extract의 독성경감효과 (Ⅰ)

        Reactive oxygen species (ROS) are highly reactive molecules due to their unpaired electron. They have been suspected as one of the major tissue damage inducers in biological metabolic systems. Antioxidant enzymes, such as catalase and superoxide dismutase, could not repair all the oxidative damages resulting from those excessive toxic ROS. It is, therefore, urgent to develop effective antioxidants to relieve from the oxidative damages. In this study antioxidative effects were investigated by using two flavonoids such as quercetin and naringenin and a flavonoid-rich extract, Ginkgo biloba extract in combination with paraquat that is known as a strong generator of oxygen radicals. The results are summeringed as follows: 1. To assess radical scavenging ability reduction concentrations (IC_(50)) of 1,1-diphenyl-2-picry1hydrazine (DPPH) within 15 minutes were measured. The values of the IC50 of quercetin and Ginkgo biloba extract were 15.4μM and 13.2μg/ml, respectively. Their radical removing activities showed concentration-dependent manners. 2. In the hydrogen peroxide assay by using PMS-NADH system, quercetin, naringenin and Ginkgo biloba extract led to removing hydrogen peroxide in concentration-dependent manner whose removing abilities at 100μM or 100μg/ml were 75.6, 25.8 and 26.0%, respectively. 3. In the hydrogen peroxide-induced rat blood hemolysis assay all three compounds led to similar effects whose hemolysis inhibition ratios at 100μM or 100μg/ml were 68.0, 5.14 and 55.8%, respectively. 4. In the xanthinee oxidase assay by measuring degree of NADH oxidation in the presence of hypoxanthine and xanthinee oxidase, both quercetin and Ginkgo biloba extract showed excellent activities showing 42.8 and 24.2% inhibiting xanthine oxidase activity at 100μM or 100μg/ml concentrations, respectively. 5. In the mouse liver microsomal NADPH-dependent cytochrome p450 reductase assay, paraquat consumed NADPH in a dose-dependent manner from 0 to I mM paraquat concentrations. When quercetin, naringenin or Ginkgo biloba extract added to this system, they blocked NADPH consumption rates at a concentration-dependent manner whose inhibition ratios of cytochrome p450 reductase at 100μM or 100μg/ml were 63.1, 52.2 and 52.2N6, repectively when compared with those of the 1 mM paraquat treatment. 6. Lactate dehydrogenase (LDH) is frequently used as a membrane toxicity indicator. When paraquat added to the macrophage RAW 264.7 cell live, LDH concentration was increased from 162.4 to 290.3 unit/L. When various concentration of quercetin, naringenin or Ginkgo biloba extract were added to the culture system in the presence of 1 mM paraquat, they decreased LDH concentration-dependent manner, showing 182, 5, 182.1 and 212.3 unit/l of LDH at 100μM or 100μg/ml teratment, repectively. However, in this case, only quercetin and Ginkgo biobla extract increased macrophage viability from 35.5 to 38.0 and 39.7 at the concentration of 100μM or 75 μg/ml when measured MTT assay, within 24hr, respectively. From these results flavonoids of quercetin, naringenin and Ginkgo biloba extract exerted their antioxidant effects by removing oxygen radicals, by blocking oxygen generating enzymes. As a general, naringenin, quercetin and Ginkgo biloba extract showed the useful compounds for scavenger and antioxidant on paraquat induced toxicity.

All four schools address the development and use of computer-based tools that support and aid managers in their role as decision makers. They differ, however, in terms of the nature of the decision situation, the phase of the decision process, the primary aims for a DSS development, the nature of the learning and the phase of the development process.

      • Supra Pubic Catheter를 施行한 腦卒中 患者 1例에 對한 證例報告

        According to the literary study on catheterization of bladder in one case of patient with C.V.A. who was treated by supra pubic catheterization, the results were as follows. 1. If not immediately treat ischuria, emergency disease, very great danger to life can happen by loss of kidney function. So we must treat ischuria instantly by consideration of treatment from the factor. 2. Catheterization of bladder generally acted in emergency room, is emergency treatment only in the lower part of urethral obstruction. 3. Catheterization of bladder was put on record in Son Saimiao's Qianjinyaofang, which is the first book on it. It had been developed by Luo Qianyi, Zhang Jiepin, and Wang Dao. 4. Ancient times, an urethral catheter was made of green onion leaf, pig bladder, goose feather and so on. 5. Nelaton is catheterization of bladder in functional or momentary ischuria. Indwelling catheterization is treatment keeping in urethral canal for a long time. 6. Supra pubic catheterization is a surgical operating catheterization when urethral catheterization is impossible, is very easy and can decrease risk of infection. 7. After catheterization, for decreasing risk of the infection, a complication of catheterization, it is must be taken precedence, ascetic treatment, constantly management and sterilization, anti-biotic medication. 8. At times, increasing patient of emergency disease, as C.V.A etc., the needing of catheterization of bladder is increasing also for preventing infection and bed sore by ischuria and incontinence of urine. Therefore a specialty knowledge and skill about it is necessary.

      • 저소음 도로 포장재의 현황과 문제점 및 개선에 관한 연구

        Road traffic noise is an important issue nowadays. A tire noise which is produced between tire and road is a major part of road traffic noise. This study deals with research in the field of silent pavement types and looks into examples for construction of noise reducing pavement in the world. Noise reducing pavement has problems which are decrease of noise reduction effectiveness as the time has passed, difficulty for management of road surface in the winter and high cost for road construction. To prevent lifetime decrease of silent pavement, periodic road cleaning and responsibility management system of construction company are needed. Porousness pavement must be regulated in the freezing road area to manage road surface well. When we evaluate cost of silent pavement construction, social charge decrease by the noise reduction has to be considered.

      • Selective Nitration에 의한 3-Phenyl-4-(p-aminophenyl) Sydnone의 합성

        3,4-Diphenylsydnone에 조건을 변화시켜가면서 니트로화반응을 시켰다. 반응은 4위치의 phenyl에서만 일어났으며, CH_3COOH-HNO_3조건하에서는 para에 그리고 H_2SO_4-HNO_3조건하에서는 ortho와 para에 진행하여 각각 니트로화합물을 얻었다. 생물학적으로 활성을 갖을 것으로 기대되는 아미노기를 갖는 sydnone유도체를 얻기 위하여, 이 니트로화합물을 Fe와 2%초산수용액에 의해 환원시켰다. 이들 생성물의 확인은 IR과 NMR로 검토하였으며 원소분석으로 확정지었다. Nitration of 3,4-diphenylsydnone was examined under various conditions. Reaction took place only at phenyl which was at 4-position of sydnone ring, and it was found that para-substituent was obtained under CH_3COOH-HNO_3 condition, whereas under H_2SO_4-HNO_3 condition ortho-and para-substituent was obtained. In order to obtain amino group containing sydnone derivative which was expected to be biologically active, this nitro compound was reduced by iron and 2% aqueous acetic acid. The products thus obtained were identified by IR and NMR spectroscopy and confirmed by elemental analysis.
