        L’état actuel des libertés religieuses en Corée du Nord

        평양은 세계에서 가장 고립된 국가인데다가, 종교적 자유 또한 지켜지지 않아 많은 비난을 받고 있다. 11년간 이어온 “Open Door”라는 미국 국제 기독교 선교위원회의 보고에 따르면, 북한은 최근 세계에서 가장 종교를 탄압하는 국가로 목록에 올라왔다. 일반적으로 구하기 힘든 NGO가 모은 증거 자료들을 바탕으로, 이 글을 통해 북한의 종교적 자유에 관한 현재 상황 그리고, 종교의 가르침과 억압에 대한 현실뿐만 아니라 종교적 자유의 관할권 또한 살펴볼 것이다.

        Effectiveness of anchorage with temporary anchorage devices during anterior maxillary tooth retraction: A randomized clinical trial

        Objective: This study evaluated the efficiency of anchorage provided by temporary anchorage devices (TADs) in maxillary bicuspid extraction cases during retraction of the anterior teeth using a fixed appliance. Methods: Patients aged 12 to 50 years with malocclusion for which bilateral first or second maxillary bicuspid extractions were indicated were included in the study and randomly allocated to the TAD or control groups. Retraction of the anterior teeth was achieved using skeletal anchorage in the TAD group and conventional dental anchorage in the control group. A computed tomography (CT) scan was performed after alignment of teeth, and a second CT scan was performed at the end of extraction space closure in both groups. A three-dimensional superimposition was performed to visualize and quantify the maxillary first molar movement during the retraction phase, which was the primary outcome, and the stability of TAD movement, which served as the secondary outcome. Results: Thirty-four patients (17 in each group) underwent the final analysis. The two groups showed a significant difference in the movement of the first maxillary molars, with less significant anchorage loss in the TAD group than that in the control group. In addition, TAD movement showed only a slight mesial movement on the labial side. On the palatal side, the mesial TAD movement was greater. Conclusions: In comparison with conventional dental anchorage, TADs can be considered an efficient source of anchorage during retraction of maxillary anterior teeth. TADs remain stable when correctly placed in the bone during the anterior tooth retraction phase.

        Engaging with the Enemy : China-Taiwan Economic and Strategic Relations After 2008 and Beyond

        The improvement of the China-Taiwan relationship since 2008 reshapes not only the economic ties between Taipei and Beijing but also the security dialogue over the Taiwan Strait. A new economic framework agreement (ECFA), multiple exchanges and high level meetings have characterized the re-engagement between two traditional enemies. In parallel, the development of a “Taiwanese” nationalism, that is slowly replacing the traditional “Chinese” nationalism that deeply changes the nature of the relation, as well as its finality, has been observed. This article explores the current Cross-Strait relation-ship. Instead of focusing on the possibility of a reunification, it suggests that deepening economic and cultural cooperation might boost confidence that will implement possible peace agreement in a longer term.

        Relationship between positive end-expiratory pressure levels, central venous pressure, systemic inflammation and acute renal failure in critically ill ventilated COVID-19 patients: a monocenter retrospective study in France

        Background The role of positive pressure ventilation, central venous pressure (CVP) and inflammation on the occurrence of acute kidney injury (AKI) have been poorly described in mechanically ventilated patient secondary to coronavirus disease 2019 (COVID-19). Methods This was a monocenter retrospective cohort study of consecutive ventilated COVID-19 patients admitted in a French surgical intensive care unit between March 2020 and July 2020. Worsening renal function (WRF) was defined as development of a new AKI or a persistent AKI during the 5 days after mechanical ventilation initiation. We studied the association between WRF and ventilatory parameters including positive end-expiratory pressure (PEEP), CVP, and leukocytes count. Results Fifty-seven patients were included, 12 (21%) presented WRF. Daily PEEP, 5 days mean PEEP and daily CVP values were not associated with occurrence of WRF. 5 days mean CVP was higher in the WRF group compared to patients without WRF (median [IQR], 12 mm Hg [11-13] vs. 10 mm Hg [9–12]; P=0.03). Multivariate models with adjustment on leukocytes and Simplified Acute Physiology Score (SAPS) II confirmed the association between CVP value and risk of WRF (odd ratio, 1.97; 95% confidence interval, 1.12–4.33). Leukocytes count was also associated with occurrence of WRF in the WRF group (14 G/L [11–18]) and the no-WRF group (9 G/L [8–11]) (P=0.002). Conclusions In mechanically ventilated COVID-19 patients, PEEP levels did not appear to influence occurrence of WRF. High CVP levels and leukocytes count are associated with risk of WRF.

        Radiomic analysis of abdominal organs during sepsis of digestive origin in a French intensive care unit

        Background: Sepsis is a severe and common cause of admission to the intensive care unit (ICU). Radiomic analysis (RA) may predict organ failure and patient outcomes. The objective of this study was to assess a model of RA and to evaluate its performance in predicting in-ICU mortality and acute kidney injury (AKI) during abdominal sepsis. Methods: This single-center, retrospective study included patients admitted to the ICU for abdominal sepsis. To predict in-ICU mortality or AKI, elastic net regularized logistic regression and the random forest algorithm were used in a five-fold cross-validation set repeated 10 times.Results: Fifty-five patients were included. In-ICU mortality was 25.5%, and 76.4% of patients developed AKI. To predict in-ICU mortality, elastic net and random forest models, respectively, achieved areas under the curve (AUCs) of 0.48 (95% confidence interval [CI], 0.43–0.54) and 0.51 (95% CI, 0.46–0.57) and were not improved combined with Simplified Acute Physiology Score (SAPS) II. To predict AKI with RA, the AUC was 0.71 (95% CI, 0.66–0.77) for elastic net and 0.69 (95% CI, 0.64–0.74) for random forest, and these were improved combined with SAPS II, respectively; AUC of 0.94 (95% CI, 0.91–0.96) and 0.75 (95% CI, 0.70–0.80) for elastic net and random forest, respectively.Conclusions: This study suggests that RA has poor predictive performance for in-ICU mortality but good predictive performance for AKI in patients with abdominal sepsis. A secondary validation cohort is needed to confirm these results and the assessed model.

        A new flap combination for reconstruction of lower nasal dorsum and supra-tip skin defects

        Repairing surgical defects of the nose is still challenging due to its tridimensional shape and its aesthetic concern. Difficulty in reconstructing nasal subunits lies in their contour, skin texture and limited availability of adjacent skin. For lower nasal dorsum and supra-tip regions, we design a new combined local flap as existing local flaps may give disappointing results. This combination flap was performed on two patients for reconstruction of the lower nasal dorsum area after basal cell carcinoma excision. Size of the excision ranged from 20 to 25 mm diameter and safe margins were obtained. The defects were reconstructed with a local flap that combined a rotation nasal flank flap and a V-Y advancement nasolabial flap. Excision and reconstruction were performed in a one-stage surgery under intravenous sedation and local anesthesia. There were no postoperative complications and no flap loss occurred. Aesthetic and functional results after 6 months postoperatively were satisfying without modification of nasal shape. This flap is reliable and offers interesting functional and aesthetic outcomes. It can be considered as a new reconstruction alternative for supra-tip and lower nasal dorsum skin defects performed in a one-stage procedure under local anesthesia.

        THE LICK AGN MONITORING PROJECT 2011: SPECTROSCOPIC CAMPAIGN AND EMISSION-LINE LIGHT CURVES

        <P>In the Spring of 2011 we carried out a 2.5 month reverberation mapping campaign using the 3 m Shane telescope at Lick Observatory, monitoring 15 low-redshift Seyfert 1 galaxies. This paper describes the observations, reductions and measurements, and data products from the spectroscopic campaign. The reduced spectra were fitted with a multicomponent model in order to isolate the contributions of various continuum and emission-line components. We present light curves of broad emission lines and the active galactic nucleus (AGN) continuum, and measurements of the broad H beta line widths in mean and rms spectra. For the most highly variable AGNs we also measured broad Ha line widths and velocity centroids from the nightly spectra. In four AGNs exhibiting the highest variability amplitudes, we detect anticorrelations between broad H beta width and luminosity, demonstrating that the broad-line region 'breathes' on short timescales of days to weeks in response to continuum variations. We also find that broad Ha velocity centroids can undergo substantial changes in response to continuum variations; in NGC 4593, the broad H beta velocity shifted by similar to 250 km s(-1) over a 1 month period. This reverberation-induced velocity shift effect is likely to contribute a significant source of confusion noise to binary black hole searches that use multi-epoch quasar spectroscopy to detect binary orbital motion. We also present results from simulations that examine biases that can occur in measurement of broad-line widths from rms spectra due to the contributions of continuum variations and photon-counting noise.</P>

      • THE LICK AGN MONITORING PROJECT 2011: Fe II REVERBERATION FROM THE OUTER BROAD-LINE REGION

        <P>The prominent broad Fe II emission blends in the spectra of active galactic nuclei have been shown to vary in response to continuum variations, but past attempts to measure the reverberation lag time of the optical Fe II lines have met with only limited success. Here we report the detection of Fe II reverberation in two Seyfert 1 galaxies, NGC 4593 and Mrk 1511, based on data from a program carried out at Lick Observatory in Spring 2011. Light curves for emission lines including H beta and Fe II were measured by applying a fitting routine to decompose the spectra into several continuum and emission-line components, and we use cross-correlation techniques to determine the reverberation lags of the emission lines relative to V-band light curves. In both cases, the measured lag (tau(cen)) of Fe II is longer than that of H beta, although the inferred lags are somewhat sensitive to the choice of Fe II template used in the fit. For spectral decompositions done using the Fe II template of Veron-Cetty et al., we find tau(cen)(Fe II)/tau(cen)(H beta) = 1.9 +/- 0.6 in NGC 4593 and 1.5 +/- 0.3 in Mrk 1511. The detection of highly correlated variations between Fe II and continuum emission demonstrates that the Fe II emission in these galaxies originates in photoionized gas, located predominantly in the outer portion of the broad-line region.</P>
