        High Production of Recombinant Norwegian Salmonid Alphavirus E1 and E2 Proteins in Escherichia coli by Fusion to Secretion Signal Sequences and Removal of Hydrophobic Domains

        The Norwegian salmonid alphavirus (NSAV)infects farmed Atlantic salmon and rainbow trout, causes pancreas disease and leads to economic losses and fish health issues for the aquaculture. Vaccines are available,but recurring infection outbreaks at Norwegian fish farms have led to endeavours in finding solutions for increased prevention. The NSAV E1 and E2 envelope proteins are potential targets for production of recombinant subunit vaccines and for generation of antibodies for diagnostics. Efficient expression of target proteins is necessary for these applications, and here we present a new strategy for expressing this kinds of viral proteins. We show that 5'-terminal fusion of signal sequences OmpA and CSP to the e1 and e2 genes and removal of the C-terminal hydrophobic interaction and transmembrane domains of E1 and E2 leads to significantly increased expression levels. Recombinant Escherichia coli strains for high-level production of E1 and E2 harbouring these modifications were established using the inducible XylS/Pm expression cassette. Furthermore,reduction of temperature to 16oC after induction leads to 4-fold increase in production for E1, and under high-celldensity cultivations we obtained production levels up to 2.3 g/L. We also show that these proteins can be purified from inclusion bodies by affinity chromatography. This demonstrates the present approach as promising for large scale production of such viral proteins.

        Background: Sarcoidosis is a systemic disease of low prevalence and unknown etiology, characterized pathologically by the presence of immunologically mediated noncaseating granulomas, mainly involves the lungs and lymph system, as a result of an exaggerated response to antigen exposure, still unidentifi ed, in genetically susceptible individuals. The acute presentation occurs only in 20-40% of cases, being a variant thereof, Lofgren`s syndrome, more prevalent in young Caucasian women, who present an association of fever, erythema nodosum, bilateral hilar lymphadenopathy, symmetrical polyarthritis and uveitis with 70% of spontaneous remission and 30% recurrence. The diagnosis is establish by clinical and radiological features consistent, histologic evidence of noncaseating epithelioid granulomas, and the reasonable exclusion of other granulomatous diseases. Corticosteroids have been shown to be the most effective drugs in the treatment of symptomatic sarcoidosis Methods and Results: Female patient 52 years old, consultation fatigue, weakness, polyarthralgia, rash pruritic on limbs and trunk, and fever records 24 hours of evolution. Physical examination shows polyarthralgia associated with functional impotence and erythema nodosum; elevated erythrocyte sedimentation rate; chest radiograph with interstitial pattern. Thorax TC evidence bilateral hilar lymphadenopathy. Mediastinal lymph node biopsy reported noncaseating granuloma (nodular sarcoidosis). After arriving at diagnosis, the patient began treatment with prednisone 40 mg/ day, with complete remission of clinical symptoms and improvement of the images. Conclusions: The mortality of patients with sarcoidosis is greater than the general population, mainly due to pulmonary fi brosis. Early recognition of symptoms such as unexplained persistent fatigue, fever, overlapping cognitive impairment, and changes in health status and quality of life have improved as a diagnostic suspicion. As regards treatment, there is no cure for sarcoidosis. Corticosteroids have been shown to be most effective drugs in the treatment of symptomatic sarcoidosis. The same, it is useful to change the clinical course of the disease and improve prognosis.

        Predictive Value of Somatosensory Evoked Potential Monitoring during Resection of Intraparenchymal and Intraventricular Tumors Using an Endoscopic Port

        Background and Purpose Intraoperative neurophysiological monitoring (IONM) using upper and lower somatosensory evoked potentials (SSEPs) is an established technique used to predict and prevent neurologic injury during intracranial tumor resections. Endoscopic port surgery (EPS) is a minimally-invasive approach to deep intraparenchymal and intraventricular brain tumors. The authors intended to evaluate the predictive value of SSEP monitoring during resection of intracranial brain tumors using a parallel endoscopic technique. Methods A retrospective review was conducted of patients operated on from 2007-2010 utilizing IONM in whom endoscopic ports were used to remove either intraparenchymal or intraventricular tumors. Cases were eligible for review if an endoscopic port was used to resect an intracranial tumor and the electronic chart included all intraoperative monitoring data as well as pre- and post-operative neurologic exams. Results 139 EPS cases met criteria for inclusion. Eighty five patients (61%) had intraparenchymal and fifty four (39%) had intraventricular tumors or colloid cysts. SSEP changes were seen in eleven cases (7.9%), being irreversible in three (2.2%) and reversible in eight cases (5.8%). Seven patients (5.0%) with intraparenchymal tumors had SSEP changes which met our criterea for significant changes while there were four (2.9%) with intraventricular (p-value=0.25). Five patients suffered post operative deficits, two reversible and two irreversible SSEP changes. Only one case exhibited post operative hemiparesis with no SSEP changes. The positive predictive value of SSEP was 45.4% and the negative predictive value was 99.2%. Conclusions Based on the high negative and low positive predictive values, the utility of SSEP monitoring for cylindrical port resections may be limited. However, the use of SSEP monitoring can be helpful in reducing the impact of endoscopic port manipulation when the tumor is closer to the somatosensory pathway.
