        Seismic Behavior of Steel Shear Wall Systems and Application of Smart Structures Technology

        Stel shear walls have been used more frequently in recent years as the lateral load resisting system in the design and retrofitof high-rise buildings. This paper concentrates on the experimental studies of an innovative steel shear wal system used intechnology into the design of steel shear wall system. The steel shear wall system studied herein consists of steel plate shearwalls welded inside a multi-bay steel moment frame. The steel moment frame consists of large concrete filed steel tubes (CFT)at the edges, internal wide flange (WF) columns, and horizontal WF beams. Most of the gravity load is resisted by the CFTcolumns, and lateral loads are resisted by the dual system consisting of the moment frame and the steel plate shear wall.

        Shear wall systems are one of the most comonly used lateral load resisting systems in high-rise buildings. This paperconcentrates on the experimental and analytical studies of two composite shear wal systems and presents a sumary anddiscussion of research results. In addition, the paper discusses application of smart structures technology into the design of thesesystems. The composite shear wal system studied herein consists of a steel boundary frame and a steel plate shear wall withe reinforced concrete wal by bolts. In the system caled “traditional” the reinforced concrete wall is in direct contact withthe boundary stel frame, while in the system caled “innovative” there is a gap in betwen.

        Outcome and Graft Patency in Coronary Artery Bypass Grafting with Coronary Endarterectomy

        Background: Controversy persists regarding the use of coronary endarterectomy (CE) in patients with severe coronaryartery disease. We compared the comorbidities and perioperative characteristics of patients undergoing coronaryartery bypass grafting (CABG) with and without CE. Methods: This study was performed in two private hospitalsin Shiraz, Iran from May 2010 to December 2011 on 967 patients who underwent CABG without CE and 84patients who underwent CABG with CE (the CE+ group). After follow-up at 9.66±3.65 months post-surgery, 28 patientsfrom the CE+ group underwent angiography to evaluate the patency of grafts and native coronary vessels. Results: Patients in the CE+ group had a more prevalent history of diabetes (48% vs. 36%) and number of diseasedvessels (2.88±0.39 vs. 2.70±0.85). The overall hospital mortality was 1.8%, and no significant difference wasobserved between the two groups. In the 28 patients who underwent reangiography, 113 vessels were bypassedand 29 endarterectomies were performed, mostly on the left anterior descending artery (12 endarterectomies) andthe right coronary artery (8 endarterectomies). In the endarterectomized vessels, a 66% patency rate was found inboth the grafts and the native vessels. The native coronary vessels were more likely to be patent when the leftinternal mammary artery was used as a conduit than when a saphenous vein bypass graft was used. Conclusion:The lack of a significant difference in postoperative complications in patients who underwent CABG with or withoutCE may indicate that CE does not expose patients to a higher risk of complications. Since most of the endarterectomizedvessels were shown to be patent during the follow-up period, we propose that endarterectomy is a viableoption for patients with severely diseased vessels.

        Amiodarone Versus Propafenone to Treat Atrial Fibrillation after Coronary Artery Bypass Grafting: A Randomized Double Blind Controlled Trial

        Background: Atrial fibrillation (AF) is one of the most common complications after cardiac surgery. Several therapeutic and preventive strategies have been introduced for postoperative AF, but the treatment and prophylaxis of AF remain controversial. We aimed to compare the efficacy of intravenous amiodarone and oral propafenone in the treatment of AF after coronary artery bypass grafting (CABG). Methods: This was a randomized controlled trial performed in two hospitals in Shiraz, Iran from 2009 to 2012. We included all patients who underwent elective CABG and developed AF postoperatively. The patients were randomly assigned to receive propafenone or amiodarone. The duration of AF, the success rate of the treatment, the need for cardioversion, the frequency of repeated AF, and the need for repeating the treatment were compared. Results: The duration of the first (p=0.361), second (p=0.832), and third (p=0.298) episodes of AF, the need for cardioversion (p=0.998), and the need to repeat the first and second doses of drugs (p=0.557, 0.699) were comparable between the study groups. Repeated AF was observed in 17 patients (30.9%) in the propafenone group and 23 patients (34.3%) in the amiodarone group (p=0.704). Conclusion: Oral propafenone and intravenous amiodarone are equally effective in the treatment and conversion of recent-onset AF after CABG.

        Bilateral Macular Hole Following Myopic Photorefractive Keratectomy

        A 42-year-old man was admitted to our clinic complaining of visual distortion in his left eye two monthsafter bilateral myopic photorefractive keratectomy (PRK). Macular optical coherence tomography (OCT)showed a stage II macular hole in the left eye. Simultaneous OCT in the right eye showed vitreous tractionand distortion of the outer retina. One month later, the patient underwent vitrectomy for the left eye, andthe macular hole was closed. Two months after that, the patient complained of visual distortion in the righteye, and OCT revealed increased traction and accentuated outer retinal distortion indicating a stage IBmacular hole. Traction attenuated later without any intervention. The short interval between PRK and holeformation, bilateral involvement, and the moderate refractive error in this case highlight the possible role ofPRK in aggravating vitreoretinal interface abnormalities. We recommend the addition of PRK to the list ofprocedures that may be associated with the formation of a macular hole.

        Characterization of the Salmonella typhi Outer Membrane Protein C

        Salmonella enterica serovar typhi, a Gram-negative food-borne pathogen, causes typhoid fever in humans. OmpC is an outer membrane porin of S. typhi expressed throughout the infection period. OmpC is potentially an attractive antigen for multivalent vaccines and diagnostic kit designs. In this study we combined in silico, in vitro and in vivo approaches to analyze various aspects of OmpC's antigenic properties. The conserved region, in addition to secondary and tertiary structures, and linear B cell epitopes, were predicted. A number of results obtained from in silico analyses were validated by experimental studies. OmpC was amplified, cloned and then expressed, with the recombinant protein then being purified. BALB/c mice were immunized by purified denatured OmpC. The titer of antibody was raised. Results of challenges with the pathogen revealed that the immunity is non-protective. Most of the theoretical and experimental results were in consensus. Introduced linear B cell epitopes can be employed for the design of diagnostic kits based on antigen-antibody interactions.
