Acute effects of one session dry needling on a chronic golfer’s elbow disability

A 40-year-old retired male elite athlete with more than 20 years’ experience in wrestling and bodybuilding with mild elbow pain and disability of 2 years presented to our clinic reporting an acute pain in medial aspect of the elbow. Physical examination revealed symptoms of left Golfer’s elbow during target-directed movements. The results of sonography in left elbow showed low level of hypo echo irregularity and increased blood flow in color Doppler mode in the common flexor origin. The pain amplitude was moderate at rest and extremely high during kinetic and intentional movements. During last 2 years, the patient tried different types of therapy including manual therapy, Transcutaneous electrical nerve stimulation and also received a variety of injections none of which were effective for reducing pain and disability. This is the first study showed that one sessions of dry needling improved both pain and disability and helped to return to a normal life and exercise training.
Hormone Receptor Expression and Clinicopathologic Features in Male and Female Breast Cancer

Background: Male and female breast cancers were investigated for variation in the clinicopathologic characteristics and expression of steroid hormone receptors in the northeast of Iran. Materials and Methods: Tumor specimens of 17 males and 338 females with breast cancer were collected at the hospitals of Mashhad University of Medical Sciences. Immunohistochemical expression of hormone receptors and clinicopathologic features of breast cancer were compared between two groups. Results: The mean age in men was 15 years higher than women (p=0.000). Males and females were mainly in stage II and III respectively (p=0.007). Although more than 60% of male and female patients were grade II, the respective figures for grade I and III were 25% and 12.5% in men but 7.1% and 27.2% in women respectively (p=0.025). ER was significantly more positive in men against women; 82.3% versus 53.4% (p=0.016). The related measures for PR was 58.8% and 50.3%, respectively (p=0.424). Males also showed significantly more ER expression than postmenopausal females; 82.3% versus 48.9% (p=0.010). Conclusions: Breast cancer in males and females contrasted in age at diagnosis, histological type, stage, grade and ER expression which emphasize they are separate diseases with different behaviors.
Effect of pozzolans on mechanical behavior of recycled refractory brick concrete in fire

Reusing building materials and concrete of old buildings can be a promising strategy for sustained development. In buildings, the performance of materials under elevated temperatures is of particular interest for determining fire resistance. In this study, the effect of pozzolan and aggregate type on properties of concrete exposed to fire was investigated. In doing so, nanosilica with cement-replacement levels of 0, 2, and 4% as well as silica fume and ultrafine fly ash with cement-replacement levels of 0, 7.5, and 15% were used to study effect of pozzolan type, and recycled refractory brick (RRB) fine aggregate replacing natural fine aggregate by 0 and 100% was utilized to explore effect of aggregate type. A total of 126 cubic concrete specimens were manufactured and then investigated in terms of compressive strength, ultrasonic pulse velocity, and weight loss at 23°C and immediately after exposure to 400 and 800°C. Results show that replacing 100% of natural fine aggregate with recycled refectory brick fine aggregate in the concretes exposed to heat was desirable, in that it led to a mean compressive strength increase of above 25% at 800°C. In general, among the pozzolans used here, silica fume demonstrated the best performance in terms of retaining the compressive strength of heated concretes. The higher replacement level of silica fume and ultrafine fly ash pozzolans in the mixes containing RRB fine aggregate led to a greater weight loss rate, while the higher replacement level of nanosilica reduced the weight loss rate.

Botnet have turned into the most serious security dangers on the present Internet framework. A botnet is most extensive and regularly happens in today's cyber-attacks, bringing about the serious risk of our system resources and association's properties. Botnets are accumulations of compromised computers (Bots) which are remotely regulated by its creator (BotMaster) under a typical Command-and-Control (C&C) framework. Botnets cannot just be implemented utilizing existing well-known applications and additionally developed by unknown or inventive applications. This makes the botnet detection a challenging issue. In this paper proposed an anomaly detection model based on genetic neural network system, which joined the significant global searching capability of genetic algorithm with the precise local searching element of back propagation feed forward neural networks to improve the initial weights of neural network.

In this paper, the effect of the type and amount of fibers on the physicomechanical properties of concrete containing fine recycled refractory brick (RRB) and natural aggregate subjected to elevated temperatures was investigated. For this purpose, forta-ferro (FF), polypropylene (PP), and polyvinyl alcohol (PVA) fibers with the volume fractions of 0, 0.25, and 0.5%, as well as steel fibers with the volume fractions of 0, 0.75, and 1.5% were used in the concrete containing RRB fine aggregate replacing natural sand by 0 and 100%. In total, 162 concrete specimens from 18 different mix designs were prepared and tested in the temperature groups of 23, 400, and 800°C. After experiencing heat, the concrete properties including the compressive strength, ultrasonic pulse velocity (UPV), weight loss, and surface appearance were evaluated and compared with the corresponding results of the reference (unheated) specimens. The results show that using RRB fine aggregate replacing natural fine aggregate by 100% led to an increase in the concrete compressive strength in almost all the mixes, and only in the PVA-containing mixes a decrease in strength was observed. Furthermore, UPV values at 800°C for all the concrete mixes containing RRB fine aggregate were above those of the natural aggregate concrete specimens. Finally, regarding the compressive strength and UPV results, steel fibers demonstrated a better performance relative to other fiber types.

Objective: The objective of this study was to compare the biomechanical differences of different rod configurations following anterior column realignment (ACR) and pedicle subtraction osteotomy (PSO) for an optimal correction technique and rod configuration that would minimize the risk of rod failure. Methods: A validated spinopelvic (L1-pelvis) finite element model was used to simulate ACR at the L3–4 level. The ACR procedure was followed by dual-rod fixation, and for 4-rod constructs, either medial/lateral accessory rods (connected to primary rods) or satellite rods (directly connected to ACR level screws). The range of motion (ROM), maximum von Mises stress on the rods, and factor of safety (FOS) were calculated for the ACR models and compared to the existing literature of different PSO rod configurations. Results: All of the 4-rod ACR constructs showed a reduction in ROM and maximum von Mises stress compared to the dual-rod ACR construct. Additionally, all of the 4-rod ACR constructs showed greater percentage reduction in ROM and maximum von Mises stress compared to the PSO 4-rod configurations. The ACR satellite rod construct had the maximum stress reduction i.e., 47.3% compared to dual-rod construct and showed the highest FOS (4.76). These findings are consistent with existing literature that supports the use of satellite rods to reduce the occurrence of rod fracture. Conclusion: Our findings suggest that the ACR satellite rod construct may be the most beneficial in reducing the risk of rod failure compared to all other PSO and ACR constructs.
