      • Perceived parenting behaviours predict young adolescents' nutritional intake and body fatness

        <P>This study investigated whether perceptions of parenting behaviours predict young adolescents' nutritional intake and body fatness. The randomly selected study sample consisted of 106 13-15 years olds from Houston Metropolitan Statistical Area. Parenting style variables were created by cluster analysis and factor analysis. A two-cluster solution for both maternal and paternal parenting style represented authoritative vs. non-authoritative parenting. Two parenting dimension factors derived were maternal/paternal nurturing and control. For adolescents' energy and nutrient intake, greater maternal nurturing appeared to be most beneficial given its association with lower consumption of total kilocalorie and lower saturated fat intake. Paternal nurturing was associated with lower sodium intake, whereas paternal control predicted lower percentage of kilocalories from carbohydrate and percentage Dietary Reference Intake for dietary fibre, and greater percentage of kilocalories from total fat. Maternal authoritative parenting and lower maternal control over their adolescents may have protective effects against having heavier and fatter adolescents given their associations with adolescents' body weight, sub-scapular skinfold, waist circumference, body mass index, and the tendencies of being at risk of overweight and being overweight. None of paternal parenting styles or dimensions appeared to be significantly related to adolescents' body fatness.</P>

        Targeting Moderate and Severe Male Stress Urinary Incontinence With Adjustable Male Slings and the Perineal Artificial Urinary Sphincter: Focus on Perioperative Complications and Device Explantations

        Purpose: To analyze perioperative complications and postoperative explantation rates for selected readjustable male sling systems and the perineal single-cuff artificial urinary sphincter (AUS) in a large, contemporary, multi-institutional patient cohort. Methods: Two hundred eighty-two male patients who underwent implantation between 2010 and 2012 in 13 participating institutions were included in the study (n=127 adjustable male sling [n=95 Argus classic, n=32 Argus T], n=155 AUS). Perioperative characteristics and postoperative complications were analyzed. The explantation rates of the respective devices were assessed using the Fisher exact test and the Mann-Whitney U-test. A Kaplan-Meier curve was generated. Potential features associated with device explantation were analyzed using a multiple logistic regression model (P<0.05). Results: We found significantly increased intraoperative complication rates after adjustable male sling implantation (15.9% [adjustable male sling] vs. 4.2% [AUS], P=0.003). The most frequent intraoperative complication was bladder perforation (n=17). Postoperative infection rates did not vary significantly between the respective devices (P=0.378). Device explantation rates were significantly higher after AUS implantation (9.7% [adjustable male sling] vs. 21.5% [AUS], P=0.030). In multivariate analysis, postoperative infection was a strong independent predictor of decreased device survival (odds ratio, 6.556; P=0.001). Conclusions: Complication profiles vary between adjustable male slings and AUS. Explantation rates are lower after adjustable male sling implantation. Any kind of postoperative infections are independent predictors of decreased device survival. There is no significant effect of the experience of the implanting institution on device survival.

        Artificial Urinary Sphincter Cuff Size Predicts Outcome in Male Patients Treated for Stress Incontinence: Results of a Large Central European Multicenter Cohort Study

        Purpose: The aim was to study the correlation between cuff size and outcome after implantation of an AMS 800 artificial urinary sphincter. Methods: A total of 473 male patients with an AMS 800 sphincter implanted between 2012 and 2014 were analyzed in a retrospective multicenter cohort study performed as part of the Central European Debates on Male Incontinence (DOMINO) Project. Results: Single cuffs were implanted in 54.5% and double cuffs in 45.5% of the patients. The cuffs used had a median circumference of 4.5 cm. Within a median follow of 18 months, urethral erosion occurred in 12.8% of the cases and was associated significantly more often with small cuff sizes (P<0.001). Multivariate analysis showed that, apart from cuff size (P=0.03), prior irradiation (P<0.001) and the penoscrotal approach (P=0.036) were associated with an increased erosion rate. Continence rate tended to be highest with median cuff sizes (4–5.5 cm). Conclusions: Apart from irradiation and the penoscrotal approach, small cuff size is a risk factor for urethral erosion. Results are best with cuff sizes of 4.5–5.5 cm.

        The Clinical Correlations between Diabetes, Cigarette Smoking and Obesity on Intervertebral Degenerative Disc Disease of the Lumbar Spine

        Study Design: Retrospective analysis of a nationwide private insurance database. Chi-square analysis and linear regression models were utilized for outcome measures. Purpose: The purpose of this study was to investigate any relationship between lumbar degenerative disc disease, diabetes, obesity and smoking tobacco. Overview of Literature: Diabetes, obesity, and smoking tobacco are comorbid conditions known to individually have effect on degenerative disc disease. Most studies have only been on a small populous scale. No study has yet to investigate the combination of these conditions within a large patient cohort nor have they reviewed the combination of these conditions on degenerative disc disease. Methods: A retrospective analysis of insurance billing codes within the nationwide Humana insurance database was performed, using PearlDiver software (PearlDiver, Inc., Fort Wayne, IN, USA), to identify trends among patients diagnosed with lumbar disc degenerative disease with and without the associated comorbidities of obesity, diabetes, and/or smoking tobacco. Patients billed for a comorbidity diagnosis on the same patient record as the lumbar disc degenerative disease diagnosis were compared over time to patients billed for lumbar disc degenerative disease without a comorbidity. There were no sources of funding for this manuscript and no conflicts of interest. Results: The total number and prevalence of patients (per 10,000) within the database diagnosed with lumbar disc degenerative disease increased by 241.4% and 130.3%, respectively. The subsets of patients within this population who were concurrently diagnosed with either obesity, diabetes, tobacco use, or a combination thereof, was significantly higher than patients diagnosed with lumbar disc degenerative disease alone (p <0.05 for all). The number of patients diagnosed with lumbar disc degenerative disease and smoking rose significantly more than patients diagnosed with lumbar disc degenerative disease and either diabetes or obesity (p <0.05). The number of patients diagnosed with lumbar disc degenerative disease, smoking and obesity rose significantly more than the number of patients diagnosed with lumbar disc degenerative disease and any other comorbidity alone or combination of comorbidities (p <0.05). Conclusions: Diabetes, obesity and cigarette smoking each are significantly associated with an increased diagnosis of lumbar degenerative disc disease. The combination of smoking and obesity had a synergistic effect on increased rates of lumbar degenerative disc disease. Patient education and preventative care is a vital goal in prevention of degenerative disc disease within the general population.

        CORE AND GLOBAL PROPERTIES OF EARLY-TYPE GALAXIES AND THEIR GLOBULAR CLUSTER SYSTEMS

        The core and global properties of the early-type ("red sequence") galaxies in the Virgo and Fornax clusters are examined using high-quality HST/ACS imaging for 143 galaxies. Rather than dividing neatly into disparate populations having distinct formation and/or evolution histories, many of the core and global properties of these galaxies show smooth and systematic variations along the galaxy luminosity function. The few examples of the rare class of compact elliptical galaxies in our sample all show properties that are strongly suggestive of tidal stripping by massive galaxies; if so, then these systems should not be viewed as populating the low-luminosity extension of so-called "normal" elliptical sequences. These results demonstrate that complete and/or unbiased samples are a pre-requisite for identifying the physical mechanisms that gave rise to the early-type galaxies we observe locally, and how these mechanisms varied with mass and environment.

      • THE EFFECT OF PRODUCT IMAGE ON COUNTRY IMAGE: THE CASE OF SOUTH KOREA

        Country image is a construct with far-reaching commercial implications (Kotler and Gertner, 2002). It is often held up as an antecedent condition of attitude formation towards products (Verlegh and Steenkamp, 1999; Laroche et al., 2005; Zeugner-Roth and Diamantopolous, 2009), as a determinant of visiting intentions for tourist destinations (Nadeau et al., 2008; Martínez and Alvarez, 2010) or even as a source of political power (van Ham, 2001; Nye, 2004; Wang, 2008). However, the way in which country image is approached in the literatures dealing with it various venues of relevance almost exclusively threat the construct as a source of some exerted effect and rarely venture into the issue of how country image is formed and how it various venues of its commercial relevance interact. Some exceptions to this general rule can be found in White (2012) who explored how product image exerts a formative effect on country image, a country-of-origin effect in the inverse. Also, Nadeau et al. (2008) observed that country image as it pertains to destination image as well as to exports intersect in an interesting way and that the two areas of commercial relevance indeed are not completely separate. In the present study we postulate that the meaning attributed to a particular country’s image is the result of an oscillation of meaning between that attributed to objects (such as for instance products, destinations or people) associated to the country and the country image itself. I.e. a product perceived as being associated to a country would derive its meaning from the country image but also exert a feedback of meaning towards the country image. Any association is not only a determinant of meaning towards one of the objects in an association dyad (cf. Keller, 1993) but this meaning also, through association, transfer in the inverse direction as well. This suggests that any venue of meaning attribution to a country could potentially exert an effect on any situation in which the country in turn exerts a formative influence. For example, a product image of a product associated to a country could potentially exert an influence on the countries attractiveness as a tourist destination through a mediated influence through country image. In order to put this mechanism to the test 500 respondents from the USA (311 female, x age 46.5, respondents of Korean origin excluded) were recruited through an online panel and subjected to a psychometric test-series featuring items pertaining to the image of South Korea, the image of South Korean products and various factors pertaining to the respondent’s attitude towards South Korea as a tourist destination. The resulting data was subjected to statistical analysis using a covariance-based structural equation model approach. The results of the data analysis suggest that attitudes and notions directed to products from South Korea exert a statistically significant effect on the image of the country as a whole. This image in turn exerts an effect on the whether consumers express interest in visiting the country in their capacity as potential tourists. These results are in support of White (2012) in that they demonstrate that attitudes towards products from a particular country contribute to the attitude towards a country as a whole. The results also support Nadeau et al. (2008) in their finding that country image as it pertains to inferring the degree of quality of exports and the attractiveness of a place as a tourist destination should not be compartmentalized, but rather treated as a facets of the same construct. In the present study the case of South Korea was used with a particular goal in mind. The rather unusual historical particularities of the transition of South Koreas economy makes it highly interesting as an example for several reasons. Not only did South Koreas transition take place in a highly compressed timeframe, but it also took a route which runs counter to the notion that tourism often serves as the starting point of economic transition (Dieke, 2003; Dritsakis, 2004; Oh, 2005; Mishra et al., 2006; Kaplan and Celik, 2008) as the rise of South Korea can be said to have been driven by industrial development and exports, prior to the country becoming a popular tourist destination. The particularities of this path to a greater degree of economic development arguably shed new light on the issue of how country image can form and how it pertains to highly variable commercial contexts and how the effect the country image construct exert transect across these various venues of commercial activity.

        Natural bioceramics: our experience with changing perspectives in the reconstruction of maxillofacial skeleton

        Objectives: Various bone graft substitute materials are used to enhance bone regeneration in the maxillofacial skeleton. In the recent past, synthetic graft materials have been produced using various synthetic and natural calcium precursors. Very recently, eggshell-derived hydroxyapatite (EHA) has been evaluated as a synthetic bone graft substitute. To assess bone regeneration using EHA in cystic and/or apicectomy defects of the jaws through clinical and radiographic evaluations. Materials and Methods: A total of 20 patients were enrolled in the study protocol (CTRI/2014/12/005340) and were followed up at 4, 8, 12, and 24 weeks to assess the amount of osseous fill through digital radiographs/cone-beam computed tomography along with clinical parameters and complications. Wilcoxon matched pairs test, means, percentages and standard deviations were used for the statistical analysis. Results: The sizes of the lesions in the study ranged from 1 to 4 cm and involved one to four teeth. The study showed significant changes in the formation of bone, the merging of material and the surgical site margins from the first week to the first month in all patients (age range, 15-50 years) irrespective of the size of the lesions and the number of teeth involved. Bone formation was statistically significant from the fourth to the eighth week, and the trabecular pattern was observed by the end of 12 weeks with uneventful wound healing. Conclusion: EHA showed enhancement of bone regeneration, and healing was complete by the end of 12 weeks with a trabecular pattern in all patients irrespective of the size of the lesion involved. The study showed enhancement of bone regeneration in the early bone formative stage within 12 weeks after grafting. EHA is cost effective and production is environment friendly with no disease transfer risks. Thus, natural bioceramics will play an important role in the reduction of costs involved in grafting and reconstruction.

        인슐린종의 임상적 분석

        Purpose: Insulinoma is a rare disease for which early diagnosis followed by proper surgical management provides a chance for cure. Analyses of clinicopathological features of patients can help optimize the surgical approach in the treatment of insulinoma. Methods: The records of 13 patients (seven male, six female mean age 44.3 years; age range 17∼62 years) who were diagnosed clinically and pathologically with insulinoma and who underwent surgery between March 1997 and April 2007 at the Department of Surgery, Seoul National University Hospital. Hospital in English please were retrospectively examined. Results: All patients had findings compatible with Whipple's triad. Mean fasting blood sugar was 40.5 mg/dl, serum insulin level was 33.5ՌU/ml, and insulin-to-glucose ratio was 0.6. A prolonged starvation test was performed on six patients. Tumors were localized in 10 patients with a computed tomography (CT) scan and in three patients with CT angiography. Five tumors were located in the pancreas head and uncinate process, five in the body, and four at the body-tail border and tail. Patients underwent resection of tumorby enucleation, distal pancreatectomy, pylorus-preserving pancreaticoduodenectomy, laparoscopic distal pancreatectomy, and duodenum-preserving resection of pancreas head. Four immediate postoperative complications (fluid collection, pancreatic fistula, delayed gastric emptying) occurred. No symptoms or recurrences were apparent during the median 15 month follow-up. Conclusion: Insulinoma is difficult to diagnose correctly without a prolonged duration of symptoms. Localization of insulinoma can be aided by a CT scan and/or CT angiography. Less aggressive operative procedures such as simple enucleation might be a sufficient and feasible procedure for curative resection of benign insulinomas. (Korean J Endocrine Surg 2010;10:99-105)
