Purpose: To evaluate the microbial leakage at two different implant-abutment interfaces using a new sealant. Materials and Methods: Forty implants of the size 4.2 Dx11.5 L (Adin, Israel), internal hex, were selected. Twenty titanium abutments (Adin, Israel) with twenty titanium implants were used in Group 1, twenty CAD/CAM zirconia abutments with twenty titanium implants were used in Group 2. Each group was subdivided into two subgroups, subgroup A- control without sealant and subgroup B- with the new sealant. Implants were immersed in a brain heart infusion broth (Accumix Microxpress, India), and a polymicrobial solution (ATCC 10556 Streptococcus sanguis, Himedia, India, and ATCC 90030 Candida glabrata Himedia, India) was introduced into the Eppendorf tubes (Tarson, India) containing the implant assemblies of the four subgroups and incubated for 14 days. Then samples from the implant wells were taken using sterile absorbent paper points (Diadent, Korea) and cultured in Mueller Hinton agar (Himedia India). The colonies formed were counted using a digital colony counter. The results obtained were tabulated and statistically analyzed. Results: The Mann-Whitney U test was used to compare the values between the two groups. There was a significant difference in the microbial leakage in the titanium abutments with and without the new sealant for both microorganisms (P<0.05). There was also a significant difference in microbial leakage in the zirconia abutments with and without the new sealant for both microorganisms (P<0.05). There was no statistically significant difference in the microbial leakage between titanium and zirconia abutments treated with the new sealant for both the microorganisms (P>0.05). Conclusions: There was a significant reduction in microbial leakage for both microorganisms on the application of the new sealant in both the titanium and zirconia abutments over titanium implants. The new sealant used has both antibacterial and antifungal efficacy on usage with both types of abutments over titanium implants.
Blast-induced Mild Traumatic Brain Injury through Ear Canal: A Finite Element Study

Purpose The role of ear canal in transmitting blast waves tothe brain is not clear. The goal of this work is to characterizethe influence of ear canal on blast-induced mild traumaticbrain injury through a computational approach. Methods A three-dimensional human head model withsingle-side ear canal details was reconstructed from computedtomography images. The ear canal was positioned eitherfacing the incident blast wave or facing away from the blastwave. Results The blast wave-head interaction has demonstratedthat the overpressure within the ear canal was substantiallyamplified when the ear directly faced the blast wave. Whenit faced away from the blast wave, the overpressure withinthe ear canal was less than the actual incident blast pressure. Regardless of the substantial pressure differences within theear canal induced by the blast wave, the resulting intracranialpressures were almost the same for both cases. Conclusions The blast wave-head interaction has demonstratedthat the role of the ear canal in brain dynamics, and thusbrain injury, was negligible. However, the peak overpressurewithin the ear canal exceeded the documented tympanicmembrane rupture and inner ear damage thresholds. Thiswas speculated to cause the degeneration of axons along theauditory pathway up to the midbrain. This work providedfundamental understanding of the load transmission throughthe ear canal and could serve as a platform for designingbetter protective armors.
Duodenal Neuroendocrine Tumor: a Diagnostic Surprise during Workup for Anemia

Neuroendocrine tumors (NETs) arising from the pancreas and duodenum together represent 18% of all gastroenteropancreatic NETs. Duodenal neuroendocrine tumor (D-NET) comprises 1%–3% of all primary duodenal tumors, of these only 20% are resectable. Surgery remains the only curative option provided a R0 resection can be done. We present a case of D-NET in an elderly male who was being investigated for anemia who was treated with pancreaticoduodenectomy and was discharged without sequele.

Introduction: Gutkha contains harmful and carcinogenic chemicals and oral cancer caused by tobacco usage has been reported as a major preventable cause of death worldwide by the World Health Organization. The Telangana state government implemented a ban on gutkha usage starting in 2013 but how effective this ban has been remains unclear. Objectives: The objective of this study was to determine the actual impact of the gutkha ban on users and vendors. Methodology: A cross-sectional study was conducted among gutkha users and tobacco vendors in Ranga Reddy district, Telangana. Based on a pilot study the sample size was determined as 368 and 384 for users and vendors respectively. Two separate questionnaires were administered to these groups. The parameters studied mainly included knowledge regarding the ban, and its impact. Results: About 49.1% of the users were aware of the ban on gutkha. Newspapers were the main source of information regarding the ban as reported by 45.3% of users. After the ban, 29.8% of gutkha users switched to other tobacco products. Awareness of health hazards and non-availability of gutkha was the most important reason stated for quitting or reducing consumption. Conclusion: The perspective of ban when visualized from the users point of view depicted a negative impact while the vendors portrayed a positive impact. Considering the addictive potential of the ingredients of gutkha, recording the effects of the ban on regular consumers and determining whether they can still obtain the products by illicit trade, would be noteworthy for implementation of strict rules.

Camptothecin (CPT) and its derivatives are the only chemical class that targets the enzymatic activity of DNA topoisomerase I, which is involved in DNA damage and subsequent cell apoptosis. Despite CPT's advantages, its use has been restricted due to extremely poor solubility, drug resistance by several efflux pumps, short half-life, and poor bioavailability. To overcome these limitations, we designed and synthesized a water-soluble CPT-conjugated molecular transporter that successfully and rapidly delivered CPT into cells. The conjugate had affinity toward mitochondria inside cells and it was distributed mainly to kidney, lung, and spleen. MTT assay confirmed that the CPP-conjugate decreased cell viability of colon cancer cell lines to a much greater extent than the parent drug. This novel approach has the potential of improving CPT efficacy for future in vivo applications, especially for colon cancer therapy.
