Purpose: Optimal radiotherapy treatment volumes for patients with locally advanced hypopharynx squamous cell carcinoma should ensure maximal tumor coverage with minimal inclusion of normal surrounding structures. Here we evaluated the effectiveness of a direct 3-mm high-dose gross tumor volume to planning target volume expansion on clinical outcomes for hypopharynx cancers. Materials and Methods: We performed a retrospective analysis of patients with hypopharynx carcinoma treated between 2004 and 2018 with primary radiotherapy using a direct high-dose gross tumor volume to planning target volume expansion and with or without concurrent systemic therapy. Diagnostic imaging of recurrences was co-registered with the planning CT. Spatial and volumetric analyses of contoured recurrences were compared with planned isodose lines. Failures were initially defined as in field, marginal, elective nodal, and out of field. Each failure was further classified as central high-dose, peripheral high-dose, central intermediate/low-dose, peripheral intermediate/low-dose, and extraneous. Clinical outcomes were analyzed by Kaplan-Meier estimation. Results: Thirty-six patients were identified. At a median follow-up at 52.4 months, estimated 5-year overall survival was 59.3% (95% confidence interval [CI], 36.3%–74.1%), 5-year local and nodal control was 71.7% (95% CI, 47.1%–86.3%) and 69.9% (95% CI, 57.0%–82.6%), respectively. The most common failure was in the high-dose primary target volume. The gastrostomy tube retention rate at 1 year among patients without recurrence was 13.0% (95% CI, 3.2%–29.7%). Conclusion: Minimal high-dose target volume expansions for hypopharynx cancers were associated with favorable locoregional control. This approach may enable therapy intensification to improve clinical outcomes.
Cariprazine — an Alternative Treatment for Clozapine-resistant Schizophrenia?

Treatment-resistant schizophrenia (TRS) poses a significant therapeutic challenge in psychiatric practice. Clozapine is recognized as a treatment of choice in TRS but is not always effective in alleviating patients’ symptoms. Additionally, clozapine therapy is associated with multiple side effects and monitoring requirements that often limit its use and negatively affect patients’ compliance with the treatment. Although clozapine augmentation options are available, there is currently no alternative monotherapy proven to be effective in TRS. We present a case of a young man with TRS who failed to respond to appropriate trials of risperidone, aripiprazole and also clozapine, and who experienced impairing adverse effects of clozapine that made further clozapine treatment not only futile but also detrimental to his health. He was successfully treated with cariprazine monotherapy, which culminated in the remission of his both positive and negative symptoms of psychosis as well as in the marked improvement in social functioning. Cariprazine, a newer atypical antipsychotic endowed with a D3-preferring mode of action, may offer a better tolerated and more acceptable treatment option for patients with difficult-to-treat psychotic symptoms.

The Aronia genus (Rosaceae family, Maloideae subfamily) includes two species of native North American shrubs: Aronia melanocarpa (Michx.) Ell. (black chokeberry) and Aronia arbutifolia (L.) Pers. (red chokeberry). The fruits of A. melanocarpa have been traditionally used by Potawatomi Native Americans to cure colds. In the first half of the 20th century, cultivars of black chokeberry were introduced to the Soviet Union and other European countries, providing fruits used by food industry. At present, it is used mainly for juice, jam, and wine production, as well as an ornamental plant. Among other substances, the berries of A. melanocarpa contain anthocyanins and procyanidins, possessing strong antioxidative potential. Numerous health-promoting activities—namely, antioxidative, antimutagenic, anticancer, cardioprotective, hepatoprotective, gastroprotective, antidiabetic, anti-inflammatory, antibacterial, antiviral, radioprotective, and immunomodulatory—have been demonstrated for black chokeberry extracts by both in vitro and in in vivo studies. The presented review summarizes the information concerning botany, cultivation, chemical composition, and pharmacological activities of Aronia plants.
COVID-19: challenges and opportunities

A paper in this Journal by Choi et al. demonstrates how the technologically advanced Korean healthcare system has responded to the COVID-19 pandemic by creation of novel drive through and walk through facilities that allow rapid and safe viral testing helping to identify and isolate patients with the disease as well a track contacts to help reduce the pandemic spread. Another paper in this issue by Rola et al.4 highlights how much the traditional approach to hypoxemic respiratory failure has changed with COVID-19. While invasive mechanical ventilation has generally been used early in refractory hypoxemia, we now recognize that many patients present with profound hypoxemia (even with oxygen levels previously thought to be incompatible with life) with little distress. This phenotype has been called the L phenotype or the so-called “happy hypoxemic” patient.5 In these cases, use of oxygen by high flow nasal cannula or noninvasive ventilation together with proning may help avoid invasive mechanical ventilation or delay the need for, and duration of, subsequent invasive ventilation thus helping to preserve scarce and vital resources.

이 논문은 아리 애스터의 두 장편 영화 <유전>과 <미드소마>의 ‘가족’ 재현을 검토함으로써 그 영화적 내러티브와 신자유주의 이데올로기와의 관련성을 분석한다. 영화 <유전>이 현재 공포영화 장르의 최근 경향인 젠더 갈등과 (안티-)오이디푸스적 관계를 반영하고 있는 반면에 <미드소마>는 표면적으로는 신자유주의적 가족관계가 아닌 다른 “그 무엇”을 상상한다. 표면적으로 이 두 영화가 보여주는 가족재현이 신자유주의적 가족 이후의 어떤 것을 제안하고 있는 것처럼 보이지만, 이들 영화의 가족재현은 오히려 신자유주의 가족 이데올로기 구성의 핵심인 ‘좋은 가족’에 대한 멜랑콜리, 이성애적 미래성, 그리고 새로운 가족 언어의 부재라는 삼중구속을 드러내 보여준다. 결국 아리 애스터의 영화는 지금과는 다른 어떤 것에 대한 우리의 집단적 상상력을 제한하는 신자유주의의 힘에 대한 분석적 비판으로 읽을 수 있다.

This paper examines the tradeoff between commercial orientation and crop productivity among smallholder farm households living in drought risk-prone areas of the central rift valley of Ethiopia. The study was built on the hypothesis that there is a one-way causation between commercial orientation and productivity. The relationship between commercial orientation and crop productivity is assessed in a censored simultaneous equation model framework. Results indicate that there is a unidirectional relationship with total factor productivity being influenced by the endogenous commercial orientation factor.
Surveying the Impact of Work Hours and Schedules on Commercial Motor Vehicle Driver Sleep

Background: Given the long hours on the road involving multiple and interacting work stressors (i.e., delivery pressures, irregular shifts, ergonomic hazards), commercial drivers face a plethora of health and safety risks. Researchers goal was to determine whether and to what extent long-haul trucker work schedules influence sleep duration and quality. Methods: Survey and biometric data collected from male long-haul truck drivers at a major truckstop in central North Carolina over a six month period. Results: Daily hours worked (mean ¼ 11 hours, 55 minutes) and frequency of working over governmentmandated daily HOS regulations (23.8% “frequently or always”) were statistically significant predictors of sleep duration. Miles driven per week (mean ¼ 2,812.61), irregular daily hours worked (63.8%), and frequency of working over the daily hour limit (23.8% “frequently or always”) were statistically significant predictors of sleep quality. Conclusion: Implications of findings suggest a comprehensive review of the regulations and operational conditions for commercial motor vehicle drivers be undertaken.

<P>The azimuthal correlations of D mesons with charged particles were measured with the ALICE apparatus in pp collisions at [FORMULA OMISSION] and p–Pb collisions at [FORMULA OMISSION] at the Large Hadron Collider. [FORMULA OMISSION], [FORMULA OMISSION], and [FORMULA OMISSION] mesons and their charge conjugates with transverse momentum [FORMULA OMISSION] and rapidity in the nucleon-nucleon centre-of-mass system [FORMULA OMISSION] (pp collisions) and [FORMULA OMISSION] (p–Pb collisions) were correlated to charged particles with [FORMULA OMISSION]. The yield of charged particles in the correlation peak induced by the jet containing the D meson and the peak width are compatible within uncertainties in the two collision systems. The data are described within uncertainties by Monte-Carlo simulations based on PYTHIA, POWHEG, and EPOS 3 event generators.</P>
Design and Characterization of a Flextensional Stage Based on Terfenol-D Actuator

A machine for microforming was designed and built around a custom low-frequency, high-power Terfenol-D transducer coupled to a flextensional type lever system for displacement amplification. Separate bolt-on flexure joints were used instead of a traditional monolithic flextensional lever system design in order to reduce cost. A mathematical model of the transducer and lever system was created that matches blocked force with an 11% error and maximum displacement with a 2% error. Thermal response and hysteresis vs. frequency response were also tested. Using an 18 mm diameter 200 mm long Terfenol-D rod and a lever ratio of 10, the system is capable of up to 365 N of force and 1.6 mm of displacement with resolution of less than 1 µm.
