Twenty total hip arthroplasties were evaluated in fourteen patients who had ankylosing spondylitis. After an average period of follow-up of 4.5 years, complete pain relief was achieved in all 20 hips. Significant gains were made in total range of motion of the hip joint (postoperative total range of motion 195). The hip prosthesis seems to be durable in this young patients (average age, thirty-three years) even if the follow-up periods were short and the cases studied were not enough. The incidence of clinically important heterotropic ossification (Classes III and IV of Brooker et al.) was not high.
대퇴 근위부의 Wiring과 Compression Hip Screw를 사용한 대퇴 전자간 불안정 골절의 치료 -예비결과보고-

In the treatment of elderly hip fracture, early mobilization as a means of reducing early postoperative morbity and mortality is man@datory. For the treatment of the senile unsable intertrochanteric fracture, the Compression hip screw is like to. However, for the instability and comminution of the posteromedial fragment of the proximal femur, sub- sequent loss of fixation, head perforation, nonunion and implant failure were developed. To prevent these difficult problems, we have used proximal wiring technique with Compression Hip screw since 1989. The 21 cases were available with minimum follow-up of six months, which was healed radiologically and clinically. l. Of 21 cases of trochanteric fracture mean age was 70 years old (range 32-89 years old) and sex distribution showed ll cases of female and 10 cases of male. 2. The radiological morphometry using Singh index were showed: 11 cases had poor bone quality (below Grade III) and the fracture type classified by Kyle method showed the 16 cases of type III and 5 cases of type IY. 3. In operation method, we performed anatomical reduction of the fracture as possible and two circumferential wire were inserted through the large posteromedial fragment including as lesser trochanter, and Compression Hip screw was done as usual manner under imaginary intensifier, then for the prevention of the collapse of the fracture site, we performed circlage wiring, which through the posteromedial fragment with the compression hipscrew. 4. For measure the screw sliding between the immediate post-op X-ray and last follow-up X-ray, Doppelt's method was used. And measure the angular deformity, neck-shaft angle was used with correction factor. 5.We analysed the 21 cases with procimal wiring technique with Compression hip screrw in the treatment of the unstable trochanteric fracture to prevent the complication of the Comprssion Hip Screw, for the instability of the posteromedial fragments.

The results of twenty seven hip replacement arthroplasties, performed from Jan. 1981 to Dec. 1990 at Seoul National University Hospital in sixteen patients whose follow-up medical and radiological data were available, were reviewed. Duration of follow-up ranged from 12 to 141 months with an ave@xage of 57 months. The final diagnosis was rheumatoid arthritis in 11 patients and juvenile rheumatoid arthritis in 5 patients. The operation time and perioperative transfusion amount were not different from those of other diseases. Clinical results showed more significant improvement in pain than in function. tntraoperative femoral fracture occurred in 2 hips. There were no dislocations, but cup malposition was noted in 8 hips. Only one hip had a superficial wound infection which was cured with antibiotics only. Loosening developed in seven hips at the average of 5 years after operation and its rate was higher than that of others. Cup protrusion developed in two hips which had a preoperative acetabular protrusion and were treated with bipolar endoprosthesis and bone graft. This finding seemed to suggest bipolar endoprosthesis is not adequate for rheumatoid coxarthritis. Of 7 loosenings and 2 cup protrusions, revision was done only in 2 hips and the remaining 7 hips had not required revision. The low activity of RA patients is thought to be a possible explanation for this relatively low revision rate.
