가토에서 유발시킨 음경지속발기증에 있어서 발기조직의 형태학적 변화

The introduction of intracorporeal injection of vasoactive drugs for the diagnosis and treatment of impotence has resulted in increased incidence of priapism recently. To get information of maintaining erectile function in prolonged priapism and get more detail of ultrastructural changes of erectile tissue of induced priapism in rabbits by changing the time limitation (at 4, 8, 12 and 24 hours), we studied the tissue with the light and electron microscopy and the analysis was made through computer assisted image analysis system. The trabecular interstitial edema was the first reaction of the cavernosal tissue in prolonged erection and the edema was aggravated with the duration of priapism in analyis. Under the light microscope, at 8 and 12 hours duration of erection tissue alterations were insignificant except trabecular interstitial edema and extravasation of red blood cells. From 18 hours duration of erection mild focal necrosis of the smooth muscle fibers with Neutrophilic infiltration appeared and necrosis became more diffuse at 24 hours duration of erection. Under the electron microscope, focal necrotic smooth muscle cells and interweaving chain formation of endothelial cells were seen from 18 hours duration of erection and at 24 hours duration of erection the smooth muscle cells exhibited fragmentation, dissociation and clumping of cytoplasmic filaments and indistinct cytoplasmic organelles. This animal study suggests that the risk of irreversible changes of erectile tissue may be occured before 18 hours duration of erection in the rabbit and proper treatment should be considered before the time showing irreversible change of erectile tissue for prevention of impotence.
