N/A The Sttudy of Skin Disorders in Alcoholic Pationts Jae Kyoung Park, M.D., Sang Won Kim, M.D. Department of dermatology, Catholic Hospital, Taegu, Korea We performed the comparative dermatologic study in 225 alcoholic patients(217 male, 8 female) diagnosed upon psychiatric grounds and in 227 normal heathy controls to evaluate the cutaneous effect of alcohol during a 4-month period from April to july, 1988 and the same month period in. 1989. The mean age of alcoholic patients was 46.5 years(male : 46.7, female : 40.8), being most prevalent in the 4th to 6th decades. The number of skin disorders was 1.7 per person in patients and 1.4 in controls. There were infections with fungal, bacterial and viral agents in 180, dermatitis and eczema in 69 spider angioma in 29, and nail changes in 22, revealing slightly more prevalent than those in controls(80.0% vs 77.5%, 30.7% vs 25.1%, 12.9% vs 7.9%, 98% vs 4.4%). Rosacea showed no difference between two groups(10.0% vs 10.1%). The prevalence of oral manifestations, tattoo, penile foreign body and melanoderma were low, but showed highly meaningful results. Tinea infection and seborrheic dermatitis were much commonly observed, and chronic gingival inflammation with tongue changes and clubbing fingers were remarkable. It was suggested that skin disorders in alcoholic patients may be the secondary events to failure of self-management, poor personal hygiene, nutritional deficiency and/or internal disorders associated with alcohol abuse than the direct cutaneous effect of alcohol.

운동유발성 천식환자에서 운동후 요중 메틸히스타민 및 설피도펩티드 류코트리엔의 농도 변화

Background: Exercise can aggravate asthmatic symptoms in many patients with bronchial asthma. It is caused by that inhaled air bypasses nasal cavity and goes directly to the lower airways through open mouth dring exercise. Although the pathogenetic mechanisms of exercise -induced asthma(EIA) have not been clarified yet, there is evidence that chemical mediators, released from the inflammatory cells triggered by airway cooling or drying, might be responsible for induction of bronchoconstriction. However, it has been controversial which chemical mediators or cells are involved in such process. Objectiye . The aim of this study was to evaluate t.he role of activated mast cells in the pathogenesis of EIA and find out whether or not sulfidopeptide leukotrienes (LTC4/d4/E4) are involved in the exercise-induced bronchoconstriction. Material and Method: Eleven asthmatics with documented exercise-induced bronchoconstriction and 10 control subjects were studied. Before and 6 hours after free running for 6 minutes, forced expiratory volume in 1 second (FEV,) and the concentrations of N- methylhistamine, LTE4, and creatinine in unine collected for 6 hours after exercise were determined. Result: Urinary concentrations of N-methylhistamine(mean+SE, ng/mg creatinine) of EIA patients before and after exercise were 159+40 and 450+75, respectively. Those of control subjects were 208+ 54 and 275+ 62, respectively. Uninary N-methylhistamine levels of EIA group increased significantly after exercise, while those of control group did not change. Urinary concentrations of LTE,(mean+SE, pg/mg creatinine) of EIA patients before and after exercise were 15.6 k2.6 and 22.2+5.8, respectively. Those of control subjects were 10.4+ 4.0, 18.2 +7.0, respectively. The concentrations of LTE4 in the urine samples collected before exerise revealed no difference between EIA and control subjects (p=0.07). There was no change after exercise in both groups. Percent fall of FEV, was 29.1+8.0% (mean+SD) in EIA group and 3.4 + 4.0% in control group, respectively. There was no correlation between reduction of FEV, and change in urinary concentrations of N-methyl-histmine after exercise. Conclusion'. Chemical mediators of activated mast cells may be involved in exercise-induced bronchoconstriction, but there is little evidence for enhanced sulfidopeptide leukotriene generation as assessed by urinary LTE4.

『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 준거하여 한약 투여 후 호전된 갱년기 증후군 1례 임상 보고

Objective : This case report aims to demonstrate the effect of Herbal medicine on menopausal syndrome. Methods : A 54 years woman suffered from menopausal syndrome with hot flush, insomnia and dyspepsia. According to Diagnostic system based on Shanghanlun provisions, the patient was diagnosed with Taeyang-byung and treated by Sosiho-tang decoction at first. But the next time the patient visited, she was treated by Oryeong-san decoction. The result of administrations were evaluated by Kupperman's index(KI), Menopause rating scale(MRS). Results : After administration of Sosiho-tang decoction for 35 days, KI was decreased from 50 to 19, MRS was decreased from 9 to 4. But her chronic dyspepsia wasn't getting better. So the KMD prescribed Oryeong-san decoction. After administration of Oryeong-san decoction for 14 days, KI was decreased from 19 to 0, MRS was decreased from 4 to 1. And her chronic dyspepsia was functionally improved. Conclusions : The patient completely recovered from menopausal syndrome and dyspepsia as treated by Sosiho-tang decoction and Oryeong-san decoction according to Diagnostic system based on Shanghanlun provisions.

ESP : 녹화 및 검색 기능 향상을 위한 DVR전용 파일 포맷

DVR(Digital Video Recorder)시스템은 기존의 아날로그 테이프 저장 시스템과는 달리 영상 입력을 디지털 신호로 변환하여 압축 저장하는 장치로서 기존의 실시간 감시, 녹화, 검색 기능의 개선은 물론 워터마킹, 무선 감시 등의 부가기능 추가 및 응용이 가능하게 되어 아날로그 테이프 저장 장치를 대체해 가고 있다. DVR 시스템에서 녹화 및 검색용 동영상 파일 포맷으로는 AVI 포맷이 가장 널리 쓰이고 있다. 그러나 AVI 포맷은 구조적인 문제로 DVR의 녹화와 검색을 위해 최적이라 할 수 없다. 일부 DVR 업체에서는 AVI 포맷을 대체하기 위한 자체 포맷을 개발하여 사용하고 있지만 공개된 파일 포맷은 없는 실정이다. 본 논문에서는 DVR 전용 파일 포맷인 ESP 포맷을 제안한다. ESP 파일 포맷은 AVI 포맷의 문제점을 해결하면서 AVI 포맷의 장점들은 최대한 적용하였다. ESP 포맷은 다중 스트림 녹화 및 이벤트 녹화도 지원하여 기존 DVR 시스템의 녹화 및 검색 기능을 개선시킨다. DVR(Digital Video Recorder) system stores video inputs in compressed digital formats and retrieve them. DVR system has several advantages over traditional analog tape recorder system which are (1) improved real-time monitoring, recording and searching capability, and (2) other capabilities such as watermarking and remote monitoring through network. AVI format is the most popular format used for DVR systems. However, AVI format has drawbacks in recording and searching due to structural problem. Some vendors develop and use their own format, do not open the format to the public. In this paper, ESP format is proposed. ESP format solves the drawbacks of AVI format, and the advantages of AVI format apply to ESP format. Moreover, ESP format provides multistream recording/replay and event-recording. In result, ESP format enchances functionality of recording and searching in DVR system.

대한상한금궤의학회의 학생우수인재교육과정 개선을 위한 설문조사 연구

Objective : This Study aims to survey the current situation of the 'Future Leaders of Korean Medicine' curriculum and thereby improve the quality of the curriculum. Methods : 10 students in the curriculum answered 3 items of the NSSE questionnaires; 'Active and Collaborative Learning', 'Student-Faculty Interaction', 'Satisfaction'. Results : 'Worked with other students on projects', 'Tutored or taught other students (paid or voluntary)' average frequencies are relatively low in the 'Active and Collaborative Learning' item. 'Worked with faculty members on activities other than course-work' average frequency is relatively low in the 'Student-Faculty Interaction' item. Conclusions : The curriculum should be improved to encourage the students to take the leading roles at Collaborative Learning and to interact with the advisors.

Cervical Spondylotic Myelopathy 환자에게 소시호탕(小柴胡湯) 투여 후 발생한 약물유해반응에 관한 임상 보고

Objective : The purpose of this paper is to report the adverse drug reactions of patient with cervical spondylotic myelopathy(CSM) treated by herb medicine, Soshiho-tang. Methods : The patient was prescribed Soshiho-tang herbal medicine for 9days. The patient complained of dizziness, difficulty of concentration and elevation of blood pressure on the 6th day. The doctor made the patient stop to take the herbal medicine and every symptoms and abnormal blood pressure are normalized after 6 days. The abnormal responses were assessed by WHO-UMC Causality Categories and LDS scale. Results : The WHO-UMC Causality is 'probable/likely' and LDS scale is 5, it means 'moderate' severity. Conclusions : The herbal medicine, Soshiho-tang is relevant to abnormal responses of the patient with CSM. This is a first case report of hypertension induced by herbal medicine, Soshiho-tang in Korea.

오령산(五?散)으로 호전된 일차성 불면증 1례 임상 보고

Objective : This case report aims to demonstrate the effect of Oryeong-san on Insomnia. Methods : A 26 year old woman suffered from insomnia with mental and physical exhaustion, reduced stamina, lack of concentration in daytime. According to Diagnostic system based on Shanghanlun provisions, the patient was diagnosed with Taeyang-byung and treated by Oryeong-san decoction. The result of administration was evaluated by Insomnia Severity Index. Results : After administration of Oryeong-san decoction for 1 month, Insomnia Severity Index was decreased from 24(clinical insomnia, severe) to 1(no clinically significant insomnia). Conclusions : The patient completely recovered from insomnia and daytime symptoms as treated by Oryeong-san decoction according to Diagnostic system based on Shanghanlun provisions.

We reported herein a case of median raphe cysts and canal of the scrotum. The 7-year-old patient had had the beaded cysts of 0.1 to 0.5cm in size slong a cord-like canal measuring about 8cm in the median raphe of the scrotum since the birth on. The cystic wall was unilocular and lined by pseudostratified columnar epitheliurn of one to four cells in thickness.

표재성 이주성 혈전성 정맥염 3예

We report herein three cases of superficial migratory thrornbophlebitis, who were all male, aged 33(Case 1), 49(Case 2), and 41(Case 3), respectively. Common to all three cases were the lesions which were multiple tender superfieial erythematous nodules, about 0.5 to 1.5cm in diameter, in cord-like indurations slong the superficial veins over the legs and feet. Histopathologically, the deep dermal veins commonly showed luminal occlusions by thrombi, and inflarnmstory infiltrates of a fairly large number of lyrnphohistiocytes and a few giant cells in their walls and surrounding tissues. Cases 1 and 2 were with idiopathic type, and Cage 3was associated with Buergers disease. The systemic therapy with dipyridamole, nicotinamide, aspirin and piraicam had been tried with unsatisfactory results.
