        실험적 녹농균각막염 모델에서 인터페론 조절인자와 산화질소 합성요소의 발현

        Purpose: To investigate the expression of IRF-1, IRF-7 and iNOS in the mice model of Pseudomonas aeruginosa keratitis. The enzymatic activity of iNOS and its expression were also investigated. Methods: With western blot analysis, the protein expression of IRF-1, IRF-7 (at 24 hours), and iNOS (at 12 hours and 24 hours) were evaluated in the mouse model of P. aeruginosa keratitis. iNOS enzymatic activity was determined with a scintillation counter. IRF-1 and IRF-7 expression were localized with immunofluorescent labeling. The wounded control group was given the same corneal wound without bacterial inoculation, and the fellow eyes served as normal controls. Results: Expression of IRF-1, IRF-7 and iNOS was highly upregulated in corneas with P. aeruginosa keratitis compared to normal or wounded corneas. iNOS enzymatic activity also was higher in infected than normal corneas. In wounded corneas, NOS2 expression and activity slightly increased at 12 hours after the infection. Intense IRF-1 immunopositivity was seen in the epithelial layer of infected corneas. Some corneal stromal cells and endothelial cells showed moderate positive labeling in infected corneas. IRF-7 showed intense labeling in the epithelial layer and endothelial cells of normal as well as infected corneas. Increased IRF-7 labeling was observed in epithelial cells in the ulcerated region of infected corneas. Conclusions: These results suggest that IRF-1, IRF-7 and iNOS may play a regulatory role in the immune responses and wound healing process in P. aeruginosa keratitis.

        말초혈액단핵구와 화학적으로 손상된 각막세포의 혼합반응에 대한 시스테아민의 효과

        목적: 말초혈액단핵구와 화학적으로 손상된 각막세포의 혼합반응에 대한 시스테아민의 효과를 알아보고자 하였다. 대상과 방법: 사람각막간질세포를 0.05 N NaOH로 60초간 처리하여 화학적으로 손상시킨 후 말초혈액단핵구자극 검사를 시행하였다. 혼합 말초혈액단핵구-각막세포반응은 이후 다양한 농도의 시스테아민(0-10 mM)으로 처리되었다. 세포 내 활성산소물질 형성 정도는 산화에 민감한 형광탐침자인 2′,7′-dichlorofluorescein diacetate (DCF-DA)를 사용하여 측정되었다. NaOH로 처리된 각막세포에 의해 촉진된 말초혈액단핵구의 증식 속도와 염증성 사이토카인 분비는 각각 bromodeoxyuridine 증식분석검사와 효소결합 면역흡수분석법으로 측정하였다. 결과: 말초혈액단핵구 증식은 시스테아민의 농도에 비례하여 억제되었다(p=0.019). DCF-DA 형광은 시스테아민 농도에 비례하여 감소하였다(p<0.001). 시스테아민의 농도에 비례하여 기질분해효소-9 (matrix metalloprotease-9, MMP-9), 인터류킨-6 (interleukin-6, IL-6), 형질전환성장인자-베타1 (transforming growth factor-beta 1, TGF-β1)은 억제되었고(p<0.05), 대식세포이동저해인자(macrophage migration inhibitory factor, MIF)는 증가하였다(p=0.008). 결론: 시스테아민이 활성산소물질의 억제를 통하여 화학적으로 손상된 각막세포에 의해 유발된 말초혈액단핵구의 증식과 염증 사이토카인의 분비를 억제하였다. 따라서 시스테아민이 화학적 각막손상 환자에서 치료제로 사용될 수 있을 것으로 기대된다. Purpose: To investigate the effect of cysteamine on mixed peripheral blood mononuclear cells (PBMCs)-chemically injured keratocytes reaction (mixed lymphocyte-keratocyte reaction; MLKR). Methods: PBMC stimulation assay was performed after keratocytes were chemically injured with 0.05 N NaOH for 60 seconds. MLKR was treated with various concentrations of cysteamine (0-10 mM). Intracellular reactive oxygen species (ROS) formation was measured using the oxidation-sensitive fluorescent probe, 2′7′-dichlorofluorescein diacetate (DCF-DA). Proliferation rate of PBMCs stimulated by NaOH-treated keratocytes and secretion profiles of matrix metalloprotease-9 (MMP-9), transforming growth factor-beta1 (TGF-β1), interleukin-6 (IL-6), and macrophage migration inhibitory factor (MIF) were determined using the bromodeoxyuridine proliferation assay and enzyme-linked immunosorbent assay, respectively. Results: Proliferation rate of PMBCs was suppressed by cysteamine in a dose-dependent manner (p = 0.019). Fluorescence of DCF-DA decreased depending on cysteamine concentration (p < 0.001). MMP-9, IL-6 and TGF-β1 levels were suppressed by cysteamine in a dose-dependent manner (p < 0.05), whereas MIF levels increased with cysteamine concentration of 0.5-10 mM (p = 0.008). Conclusions: These study results indicate that cysteamine induced the ROS-mediated inhibition of inflammatory cytokine release and proliferation of PBMCs stimulated by chemically injured keratocytes. Thus, cysteamine can be used in the treatment of chemical corneal burns.

        이중샤임플러그 전안부분석기를 이용한 백내장 수술 시 인공수정체 도수 계산

        목적: 백내장 수술 환자에서 이중샤임플러그 전안부분석기(dual Scheimpflug analyzer, DSA; Galilei G2)로 측정한 생체계측값과 예측굴절력을 기존 방식과 비교하고 DSA의 simulated keratometry (simK)를 이용한 인공수정체 계산공식들의 예측오차를 알아보고자 하였다. 대상과 방법: 백내장 환자 총 39명을 대상으로 초음파계측기, 자동각막굴절계, 부분결합간섭계(IOL master??), DSA로 생체계측을 하였다. 수술 전 자동각막굴절계의 auto-keratometry (Auto-K), 부분결합간섭계의 mean K, DSA의 simK와 총각막굴절력(total corneal power, TCP)을 측정하고 4가지 각막굴절값을 이용하여 예상굴절력을 계산하였다. 또한 DSA의 simK를 Sanders-Retzlaff-Kraff (SRK)-II, SRK-T, Holladay II, Haigis, Hoffer-Q 공식에 대입하여 얻은 예상굴절력을 비교하였다. 수술 후 1, 3, 6개월에 측정한 구면렌즈대응치와 예상굴절력의 차이를 평균예측오차로, 그 절대값을 절대예측오차로 정의하여 그 값을 비교하였다. 결과: DSA의 TCP는 simK, mean K, Auto-K에 비해 유의하게 낮았다(p〈0.05). 수술 후 1개월에 평균예측오차는 simK, mean K와 Auto-K 사이에 차이가 없었고, TCP를 사용한 경우에는 다른 각막굴절력을 사용한 경우보다 평균예측오차가 컸다(p〈0.05). 수술 후 6개월에 simK와 TCP를 사용하여 계산된 평균예측오차는 유의한 차이를 보였다(p〈0.05). DSA의 simK를 5가지 인공수정체 계산공식에 대입하여 계산된 평균예측오차는 SRK-T 공식에서 가장 작았다. 결론: 정상각막을 가진 백내장 환자에서 인공수정체 도수 계산 시, DSA의 TCP보다 simK와 SRK-T 공식을 사용하는 것이 유용할 것으로 생각된다. Purpose: To investigate the accuracy of intraocular lens power calculations using simulated keratometry (simK) of dual Scheimpflug analyzer and 5 types of formulas in cataract patients. Methods: The keratometry (K), axial length (AXL) and anterior chamber depth (ACD) were measured using ultrasound biometry (USB) combined with auto-keratometry (Auto-K), parital coherence interferometry (PCI; IOL master??) and dual Scheimpflug analyzer (DSA; Galilei??) in 39 eyes of 39 patients. Predicted refraction was calculated using Auto-K, mean K of PCI, and simK and total corneal power (TCP) of DSA in the Sanders-Retzlaff-Kraff (SRK-T) formula. The SRK-II, SRK-T, Holladay II, Haigis, and Hoffer-Q formula were used to calculate predicted refraction with the simK of DSA and AXL of USB. Manifest refraction, mean numerical error (MNE) and mean absolute error were evaluated 1, 3 and 6 months after cataract surgery. Results: TCP of DSA was lower compared with other keratometric values (p 〈 0.05). The MNE was not different among Auto-K, mean K and simK. The MNE using TCP was larger compared with Auto-K, mean K and simK at 1 month after surgery (p 〈 0.05). There was a difference in MNE between simK and TCP of DSA at 6 months after surgery (p 〈 0.05). The MNE of SRK-T formula was the smallest in the intraocular lens (IOL) power calculation using the simK of DSA. Conclusions: We suggest using IOL power calculations with simK of DSA and SRK-T formula rather than TCP of DSA in cataract patients with normal corneas.

        국내 콘택트렌즈 관련 합병증 양상에 대한 설문 조사: 한국콘택트렌즈학회

        Purpose: To investigate the epidemiology of contact lens (CL)-related complications in Korea. Methods: A questionnaire regarding CL-related complications including patient’s gender, age, causative factors, and signs and symptoms was distributed to members of the Korean Ophthalmological Society by The Korean Contact Lens Study Society and the results of the questionnaire analyzed. Results: Responses to the questionnaire written by ophthalmologists from 22 institutes and clinics were collected from 499 subjects over a 20-month period starting in October 2008 and analyzed. The mean age of respondents was 22.9 years and the male-to-female ratio was 1:8.1. The soft CL and cosmetic colored lens comprised the majority (46.6% and 42.1%, respectively) of the reported cases, followed by the rigid gas permeable lens (RGP lens; 10.6%) and orthokeratology lens (0.8%). In subjects using a cosmetic colored lens, 62.2% showed emmetropia and 89.1% of the lenses were prescribed by opticians. The main complications included corneal erosion, sterile corneal infiltrate, allergic disease, conjunctival injection, corneal ulcer, and dry eye syndrome. The most common causative factor of complications was excessive lens wear. Comparing main causative factors according to the RGP lens prescriber, the most common factor was poor lens fit. Conclusions: The number of cosmetic colored lens-related complications in the emmetropic eyes of young patients is increasing rapidly. Considering opticians are the main CL prescribers in CL-related complications, Korean ophthalmologists need to pay more attention to CL fitting and constant education of patients regarding proper CL wear and care.         각막윤부 유피종의 미용적 치료 2예

        Purpose: To report two cases of cosmetic treatment of limbal dermoid, which consist of local excision of the limbal dermoid, corneal tattooing, and amniotic membrane transplantation. Methods: Dermoid excision, corneal tattooing, and amniotic membrane transplantation were carried out in both a girl and a woman who each had a limbal dermoid in their left eyes. Postoperatively, they were followed up for more than six months. Results: No specific complications occurred. The patients were satisfied with the surgical results. After a few months the conjunctiva was found to grow into the tattooed cornea. Conclusions: In the treatment of limbal dermoid, local excision combined with corneal tattooing and amniotic membrane transplantation can be a cosmetically successful alternative choice.

        안구결막질환에서 고주파 결막전기소작술의 효과

        Purpose: To investigate the efficacy and safety of radiofrequency electrocautery for conjunctival diseases combined with dry eye syndrome. Methods: Twenty eyes of 16 patients with conjunctival diseases underwent the high frequency electrocautery for conjunctival diseases. Conjunctival evaluation, tear break-up time (TBUT), Schirmer tear production test, ocular surface disease index (OSDI) and visual analogue pain score (VAS) were performed preoperatively, 1 week, 1 month and 3 months after surgery. Results: Mean age was 53.9 ± 19.4 years. There were 2 males and 14 females. Nine eyes with conjunctivochalasis, 6 eyes with chemosis, 1 eye with conjunctival inclusion cyst, 2 eyes with conjunctival nevi and 2 eyes with lymphangiectasis were included. Conjunctival diseases were improved and no severe complication occurred. TBUT increased and OSDI decreased 3 months after surgery. There were no difference in Schirmer test and VAS. Conclusions: High frequency electrocautery is an effective, safe and simple treatment for conjunctival diseases combined with dry eye syndrome.

        백내장 수술 전후 이중샤임플러그 전안부 분석기로 측정한 수정체 내부 신호강도

        목적: 백내장 수술 환자에서 이중샤임플러그카메라를 사용하여 측정한 수정체 내부 신호강도의 의의를 알아보고자 하였다. 대상과 방법: 2012년 2월부터 2013년 1월까지 백내장수술을 받은 148명 151안을 대상으로 하여 수술 전 샤임플러그카메라로 수정체 내부신호강도를 측정하였고 수술 전과 수술 후 1개월에 최대교정시력과 이중샤임플러그카메라로 상측, 하측, 이측, 비측 전방각의 크기와 전방의 깊이를 측정하였다. 수술 전후의 시력을 측정하였고 상측, 하측, 이측, 비측 전방각의 수술 전후의 변화와 수정체 내부신호강도와의 연관성을 조사하였다. 결과: 수정체내부 신호강도는 수술 전 시력, 수술 전 전방 깊이와는 유의한 상관관계를 보였다(r=0.287, p=0.001 and r=-0.271, p=0.004). 수술 전후 전방각은 수술 후 유의하게 증가하였고(p<0.001) 수술 전후의 전방각의 변화량은 수술 전 전방각의 크기와 상관관계를 보였다(p<0.001). 그러나 수정체 내부신호강도는 수술 전 전방각의 크기와 연관이 없었고 수술 전후 전방각의 변화량과 상관관계를 보였다(p<0.001). 결론: 이중샤임플러그전안부촬영기로 측정한 수정체 내부신호강도는 수정체의 혼탁 정도를 객관적으로 측정하고 수정체의 부피를 예측하여 전안부의 구조를 이해하고 백내장수술 후 예후를 예측하는 데 도움이 될 것으로 생각한다. <대한안과학회지 2015;56(5):702-708> Purpose: To investigate the clinical significance of the internal lens signal measured using dual Scheimpflug anterior segment analyzer (GalileiTM, Ziemer, Switzerland) in patients receiving cataract surgery. Methods: The present study included 151 eyes of 148 patients who received surgery for senile cataracts from February 2012 to January 2013. Preoperative internal lens signals were measured preoperatively. The depth of anterior chamber and anterior an-gles were measured using dual Scheimpflug anterior segment analyzer preoperatively and 1 month postoperatively. Preoperative and postoperative best-corrected visual acuities (BCVAs) were measured. The relationships between preoperative internal lens signal and the changes in BCVA or anterior angles were evaluated. Results: Internal lens signal and preoperative BCVA (log MAR) or preoperative anterior chamber depth were highly correlated (r = 0.287, p = 0.001 and r = -0.271, p = 0.004, respectively). Anterior angles increased 1 month after surgery compared with the preoperative values (p < 0.001). The amount of change between preoperative and postoperative anterior angles correlated with preoperative anterior angles (p < 0.001). However, no statistically significant correlation was observed between internal lens sig-nal and preoperative anterior angles or postoperative BCVA. Internal lens signal correlated with changes in postoperative ante-rior angles (p < 0.001). Conclusions: Internal lens signal correlated with preoperative visual acuity and may help evaluate the cataract severity quantita-tively and objectively. Internal lens signal may aid in understanding the structure of anterior segments by predicting the lens volume. Knowing the effect of visual impairment due to cataracts and predicting visual improvement after cataract surgery is necessary. J Korean Ophthalmol Soc 2015;56(5):702-708
