Renovascular hypertension is the most common cause of curable hypertension. The exact prevalence of renovascular hypertension is not known, and the diagnosis is probably missed in many patients. It is important to recognize this condition in clinical practice, first, because it is a correctable form of secondary hypertension, and second, it is a reversible cause of renal failure in some patients. The Basic lesion of renovascular hypertension is stenosis of the renal artery caused usually by either one of the two most common etiologies, atherosclerosis or fibromuscular dysplasia. Other known causes of renovascular hypertension include aneurysm, embolism, arterio venous fistula, neurofibromatosis, tumor, hematoma, foreign body induced compression, and trauma. Its treatment can be divided into surgical intervention, percutaneous transluminal balloon angioplasty, and medical therapy. Recently, selective embolization has been introduced as a useful alter native therapeutic modality in the management of arterio venous fistula and aneurysm. We report a case of delayed renovascular hyper-tension in stab injury induced renal artery-IVC fistula and its renal artery aneurysm in a 20 year-old man. Embolization of the renal arterio venous fistula was achieved by inserting a detachable balloon, and embolization of the renal aneurysm and lumbar artery pseudoaneurysm was performed using micro-eo-ils. Arteriogram immediately after embolization con- firmed complete occlusion of the ancurysm and closure of the arterio venous fistula. After the procedure, improvement of blood pressure was noted in the patient who is currently being followed-up on an out-patient basis.

초정밀 위치 제어 시스템에서 미지의 상태변수를 관측하는 경우, 파라미터의 불확실성은 관측기 설계에 많은 어려움을 주게 된다. 슬라이딩 모드 관측기는 불확실성과 외란이 존재하는 시스템의 상태변수를 관측가능하게 하며 강인한 성능을 보여준다. 본 논문에서는 불확실성이 존재 하는 시스템의 관측기를 설계하고 최적제어를 통하여 정밀 스테이지의 위치를 제어하며, 시뮬레이션을 통해 개선된 성능을 입증한다.
이산 시간 시스템에서 채터링 감소를 위한 연속 슬라이딩 모드 제어

고전적인 슬라이딩 모드 제어는 시스템의 동특성에 관계없이 시스템의 상태를 설계자의 의도대로 목적에 맞게 제어할 수 있지만 시스템의 본질적인 한계로 인해 제어입력에서 채터링이라는 불가피한 요소를 포함하게 된다. 본 논문에서는 이산 시간 시스템에서 고주파 성분으로 인한 스위칭 입력의 부정적인 영향인 채터링을 감소시키기 위해 연속 슬라이딩 모드 제어기법을 제안한다. 제안된 기엽은 PZT 나노 스테이지에서 슬라이딩 모드의 강인성과 만족스러운 시스템 제어성능을 보여준다.

Nocardia is an opportunistic pathogen that can cause disseminated disease in serious immunosuppressive patients with organ transplantation, advanced HIV infection, malignancy or long-term corticosteroid use. Cerebral nocardiosis constitutes the most severe form of Nocardial infection. Early detection and treatment of cerebral abscess of Nocardia is important because the mortality is three times higher than that of other bacterial cerebral abscesses. We report a case of N. farcinica brain abscess in a focal segmental glomerulosclerosis (FSGS) patient after steroid treatment.

N/A A generalized Shwartzman reaction(gSr) is characterized by vascular injury with microvascular fibrin deposition, leukocyte aggregation and hemorrhagic lesions in various organs and elicited by endotoxin from gram-negative bacteria. Leukocyte adherence to endothelial cells has been regarded as a pivotal event in this type of neutrophil mediated inflammatory response. Recently, it was described that leukocyte adhesion to endothelium is facilitated by the adhesion molecules, such as LFA-1(CDIIa/CD18) or Mac-1 (CDllb/CD18) binding to intercellular adhesion mole-cule-1(ICAM-1). To investigate the role of cell adhesion molecules in inflammatory vascular injury, the authors examined the expression of cell adhesion molecules, ICAM-1 and CD18, in gSr. Group A(n=5) received two injections of lipopolysaccharide(LPS) from E. coli 055:B5; 200μg/kg and 200μg/kg 24hours apart. Group B(n=5) received one dose of LPS(200μg/l) after glucocorticoid treatment(methylprednisolone, 25mg/kg/ day). Group C(n=5) received single dose of LPS(200μg/kg). The control group(n=3) received two injec-tions saline instead of LPS. Animals were sacrificed four hours after the last dose of LPS or saline, The results were as follows ' 1) Group A and B showed numerous fibrin micro-thrombi, inflammatory cell infiltration and some he-morrhagic change in kidney and lung, but group C and the control group did not show any lesions. In kidney the degree of fibrin deposition by IF was significantly increased in group A and B when compared to group C(p<0.05). 2) ICAM-1 was expressed by vascular endothe-lium including glomerular capillaries and some lym-phocytes in the control group, This expression was increased in group C. But, group A and B was significantly decreased in ICAM-1 expression, especially in kidney comparing with group C and the control group(p<0.05). 3) Group A and B were significantly increased in CD18 expressing cells in glomerulus when compared with the control group(p<0.05). The number of CD18 expressing cells in glomerulus correlated significantly with a degree of fibrin deposition. In conclusion, these results suggest that the development of gSr requires the adhesion of activated inflammatory cells especially neutrophils to vascular endothelium and these are achieved by the interaction of adhesion molecules like ICAM-1 and CD18. Re- placing the LPS with a glucocorticoid also expressing ICAM-1 on endothelium and prepares for leukocyte adhesion to endothelium.
나노 스테이지에 대한 슬라이딩-모드 제어 기반의 강인 최적 제어기 설계

In this paper, we design a robust optimal controller for ultra-precision positioning system. Generally, it is hard to control the nanometric scale positioning system because of the parameter uncertainties and external disturbances. To solve this problem, we suggest a control algorithm based on the modified sliding-mode control and the LQ control in an augmented system. The augmented system is composed of additional state variables: state estimates and control input in the nominal system. Through comparison with LQ optimal control, it is verified that the proposed control algorithm is more robust to the unexpected parameter variations and external noises.
적응형 퍼지-칼만 필터 기반의 초정밀 위치 결정 시스템 제어

본 논문에서는 초정밀 위치 결정 시스템에서 보다 정확한 제어를 위한 새로운 제어기를 설계한다. 외란을 고려한 시스템의 경우, 환경이 달라질 때마다 측정 노이즈를 정확하게 알아내기란 쉽지 않다. 따라서 측정 장치의 정확성을 나타낼 수 있도록 칼만 필터 추정기와 퍼지 이론을 이용하여 정확한 측정 오차값을 구한다. 이때, 파라미터 불확실성과 외란에 강인한 제어를 위해 슬라이딩 모드 제어기와 LQ 최적 제어기가 적용된다. 최종적으로, 제안된 제어기와 시간 최적 제어기의 성능비교를 통하여 보다 강인하고 안정된 성능개선을 증명한다.
