        동적 QoS 속성을 고려한 단일 웹서비스의 교환을 지원하는 프레임워크

        현재 웹 어플리케이션을 구축하는데 있어 웹서비스는 중요한 역할을 담당하게 되었다. 웹서비스의 수와 종류가 많아짐에 따라, QoS는 웹서비스의 선택과 조합에서 중요한 기준이 되었다. 하지만 웹서비스의 QoS요소는 실행환경에 따라 동적으로 변하고, 이로 인해 선택된 웹서비스가 요청자의 QoS요구사항에 부합하지 않는 웹서비스가 되기도 한다. 본 논문에서는 요청자의 QoS요구사항에 적합한 웹서비스를 찾고 웹서비스의 QoS에 기반하여 웹서비스를 동적으로 변경하는 프레임워크를 제안하였다. 그리고 웹서비스 변경 시 발생하는 웹서비스의 인터페이스적응문제와 수행중인 작업의 보장문제에 대한 해결방법도 함께 제시한다. At present, Web services become main means to construct web applications. As the number of web services grows and various kind of web services have been produced, QoS properties of web services are one of important criteria to select more appropriate web services. However, the selected web services may become inappropriate for the Requester's requirements, because QoS is kind of dynamic properties. this paper proposes a framework that proposed that finds an appropriate web service for Requester's QoS requirements, and dynamically replaces an inappropriate web service with right one. The problems which happen in substitution step are also identified and the solutions are described with an illustrative example.

        NF-κB와 MAPK억제를 통한 시호소간탕(柴胡疏肝湯)의 항염증효과

        Objectives : Sihosogan-tang (SST) is one of the traditional herbal formula and also one of the Korean medical insurance medicines. It commonly used in the treatment of hepatitis, chronic gastritis, intercostal neuralgia, pleurisy, and depression in East Asia. In the present study, we have demonstrated the anti-inflammatory effects of SST in macrophage cell line. Methods : To investigate mechanism of the anti-inflammatory effect of SST, we examined the productions of nitric oxide (NO) and pro-inflammatory cytokines, and the expressions of inducible NO synthase (iNOS), nuclear factor-κ B (NF-κB) and mitogen-activated protein kinase (MAPK) on RAW 264.7 cells activated by LPS. Results : SST significantly inhibited the expression of iNOS increased by LPS, and also significantly inhibited the production of NO. In addition, SST significantly inhibited pro-inflammatory cytokines such as TNF- α and interleukines. SST inhibited the expression of NF-κB and MAPK activation. Conclusions : These results suggest that SST ameliorates inflammatory response in LPS-activated RAW 264.7 cells through the inhibition of the NF-κB and MAPK pathway. Therefore, this study supplies objective evidence for the anti-inflammatory effect of SST.

        다발성 벌 자상에 의한 길랑 바레 증후군 1례

        Severe systemic responses including neurologic complications such as myasthenia gravis, myeloradiculopathy, optic neuropathy, parkinsonism, stroke and Guillain-$barr{\acute{e}}$ syndrome can occur after bee stings. This case describes a 78-year-old female who presented with symptoms of acute progressive bilateral symmetrical weakness in both lower legs after multiple bee stings. Nerve conduction study findings were consistent with acute sensorimotor axonal neuropathy and recovered by treatment with intravenous immunoglobulin. This case highlights that bee stings can result in acute onset Guillain-$barr{\acute{e}}$ syndrome, although the pathophysiologies of bee venoms need to be investigated accurately.

        Nrf2 활성화를 통한 익위승양탕(益胃升陽湯)의 간세포 보호 효과

        Objectives : Oxidative stress is a important cause of liver disease, and regulation of oxidative stress is essential to maintain the normal metabolic function of the liver. Until a recent date, there has been no studies on the hepatoprotective effect of Ikwiseungyang-tang (IWSYT). Therefore, this study aims to demonstrate the hepatoprotective effect of IWSYT and its related molecular mechanisms on arachidonic acid (AA) + iron induced oxidative stress model in HepG2 cells. Methods : To determine the cytoprotective effect of IWSYT against AA + iron-induced oxidative stress, cell viability, apoptosis-related proteins, intracellular reactive oxygen species (ROS), GSH, and mitochondrial membrane potential (MMP) were measured. Nuclear factor erythroid 2-related factor 2 (Nrf2) activation was analyzed by immunoblot analysis. In addition, Nrf2 transcription activation through ARE binding was measured by reporter gene assays, and the expression of the Nrf2 target antioxidant genes were confirmed by immunoblot analysis. Results : IWSYT increased cell viability from cell death induced by AA + Iron, and inhibited apoptosis by regulating apoptosis-related proteins. Furthermore, IWSYT protected cells by inhibiting intracellular ROS production, GSH depletion, and MMP degradation. Nrf2 activation was increased by IWSYT, and Nrf2 target genes were activated by IWSYT too. Conclusions : These results suggest that IWSYT can protect hepatocytes from oxidative stress through Nrf2 activation and can be potentially applied in the prevention and treatment of liver damage.

        Nrf2활성화를 통한 삼출건비탕(蔘朮健脾湯)의 간세포보호효과

        Objectives : Oxidative stress is an important cause of many diseases including liver injury. Therefore, adequate regulation of oxidative stress plays a pivotal role in maintaining liver function. Until recently, there has been no studies on the hepatoprotective effect of Samchulgeonbi-tang (SCGBT). Therefore, the hepatoprotective effect of SCGBT was investigated in HepG2 cells. In this study, oxidative stress was induced by arachidonic acid (AA) and iron. Methods : To analyze the hepatoprotective effects of SCGBT against oxidative stress induced by AA + iron, the cell viability, apoptosis-related proteins and intracellular ROS, glutathione (GSH), and mitochondrial membrane permeability (MMP) were measured. In addition, nuclear factor erythroid 2-related factor 2 (Nrf2) transcription activation and expressions of Nrf2 target gene were analyzed through immunoblot analysis. Results : SCGBT increased the cell viability from AA + iron - induced cell death and inhibited apoptosis by regulating apoptosis related proteins. SCGBT protected cells by inhibiting ROS production, GSH depletion, and MMP degradation against AA + iron induced oxidative stress. Furthermore, Nrf2 activation was increased by SCGBT, and the Nrf2 target genes were also activated by SCGBT. Conclusions : These results suggest that the SCGBT has a hepatocyte protection effect and antioxidant effect from AA + iron induced oxidative stress.

        부식제 음독 환자에서 초기 내시경 소견의 중증도에 따른 임상 소견 및 예후 비교

        Purpose: We investigated comparison of clinical characteristics and prognosis by initial endoscopic severity in caustic injury and then discussed predisposing factors which can be helpful in predicting the prognosis and determining the treatment. Methods: This study was a retrospective review of medical records from patients over the age of 15, who underwent initial endoscopy for caustic injury from April 2007 through November 2014. Patients were classified according to two groups based on the initial endoscopic finding by Zargar’s classification: patients with grade 0, I, IIa at esophagus (low risk group) and patients with grade IIb, IIIa, IIIb at esophagus (high risk group). The two groups were then compared. Results: A total of 55 patients were included (low risk group [n=44] vs. high risk group [n=11]). Old age (p<0.001), large amount of ingestion (p<0.05), oropharyngeal symptoms (p<0.01), high SOFA score (p<0.001), high WBC count (p<0.05), low base excess (p<0.01), and HCO3 (p<0.05) were statistically significant factors in the high risk group. A poor prognosis was observed for hospital stay (p<0.001), ICU admission (p<0.001), mortality (p<0.01), and stricture (p<0.001) in the high risk group. Conclusion: Clinical characteristics including age, amount of ingestion, oropharyngeal symptoms, SOFA score, WBC count, base excess, and HCO3 can be helpful in the decision to undergo initial endoscopy and risk assessment by initial endoscopic severity can be helpful in predicting prognosis and determining the treatment plan.

        자매염색분체교환을 통한 글라이포세이트 유전독성

        Purpose: Glyphosate (N-phosphonomethyl glycine) is widely used as an herbicide for weed control in rural areas. It is also readily available for suicide attempts. Glyphosate has high toxicity and negatively affects the human body. The aim of this investigation was to study the genotoxicity of a low-concentration of glyphosate through sister chromatid exchange (SCE) in human blood lymphocytes in vitro. Methods: Primary lymphocyte cultures were obtained from blood samples of 11 males and seven females who had been exposed to glyphosate (0, 100, 200, and 300 ng/mL). The frequency of SCEs was examined and statistical analysis was performed. Results: All doses of glyphosate induced a significant dose-dependent increase in SCE frequency compared with the control group (P<0.001). In particular, the SCE frequency for exposure to low-dose glyphosate was significantly higher in females than in males. Conclusion: According to the result of this study, even a low-dose of glyphosate may damage DNA and females are more vulnerable to glyphosate.

        Trichloroethylene (TCE) is an unsaturated chlorinated hydrocarbon in the form of a colorless, volatile liquid, which is used as an industrial organic solvent for spot removal and for metal degreasing. In general, the primary complications of TCE poisoning result from involvement of the central nervous and respiratory systems, including aspiration pneumonia. A case is reported of a 54-year-old man who presented in a comatose state after accidental ingestion of 100 ml of TCE, and who recovered after conservative treatment and mechanical ventilation. We discuss this case and present a literature review.

        미만성 축삭 손상에서 전산화단층촬영과 경사에코 자기공명영상을 이용한 예후의 평가

        Purpose: Diffuse axonal injury (DAI) is clinically defined as a coma of over six hours in a head trauma victim without a focal mass lesion. The emergency physician usually resuscitates and stabilizes a comatose head trauma victim in the emergency Department. After assessment and treatment, the prognosis is very important to both the victim and the physician. The prognosis for DAI is based on Glasgow Coma Scale (GCS) and other imaging data. We investigated the prognostic value of computed tomography (CT) and gradient-echo magnetic resonance imaging (GRI) for head trauma victims with DAI. Methods: Fifty-three(53) head trauma victims of DAI were enrolled in this study from 2007 to 2012. During the study period of six years, data on trauma victims were collected retrospectively. We analyzed the differences in the Glasgow Outcome Scale (GOS) result between the CT and the GRI modalities. Results: We classified the study group by using GOS. Between the good outcome subgroup (GOS scores of 4 and 5) and the poor outcome subgroup (GOS score of 1-3), there were no statistical difference in sex, age, initial vital signs and initial GCS score. The good outcome subgroup had non-hemorrhage on CT(52%), which was correlated with good outcome and a shorter awakening time, while a larger number and a deeper location of hemorrhagic lesions on in GRI were correlated with poor outcome in DAI. Conclusion: We conclude that the existence of hemorrhagic lesions on CT, and the number and location of those lesions on GRI had good prognostic value for head trauma victims with DAI.         해부학 교육을 위한 아나토그래픽 (anatographic)의 효과와 이용가능성

        해부학 학습을 위해 다양한 방법 중, 많은 양의 데이터와 복잡한 정보를 효과적으로 전달할 수 있는 인포그래픽 (infographics)이 있다. 이에 의과대학 학생들이 직접 해부학 자료를 아나토그래픽(anatographic)으로 만들고 공유하며 이용하였다. 본 연구는 60명의 의예과 학생, 111명의 의학과 학생, 그리고 34명의 의사를 포함한 총 205명에게 아나토그래픽의 예시를 보여주고, 이것의 활용가능성과 해부학 학습방법별 만족도 및 장단점에 대해 설문조사를 해보았다. 해부학 학습을 위해 대부분 교과서나 구글, 기타 검색사이트, 어플 등을 이용하였으며, 5점 척도에 따른 유용성은 교과서가 3.75로 가장 높았으며, 구글(3.60), 어플(2.53), 검색사이트(2.08)가 뒤를 이었다. 아나토그래픽에 대한 유용성은 3.59로 어플이나 기타 검색사이트보다 유의하게 높게 나타났다(p<0.05). 아나토그래픽이 유용한 이유는 태그로 쉽게 검색이 가능한 점과 질문을 할 수 있기 때문에 정보공유가 가능하고 스마트폰을 이용하여 어디서든 학습을 할 수 있는 것이었다. 반면에, 온라인접속의 번거로움이 있고 의사들이 활용하기에는 자료의 신뢰도가 떨어질 수 있다는 점이 있었다. 학생들은 스마트폰을 쓰다 보니 학습 외 다른 용도로 사용을 하게 된다는 문제가 있었다. 아나토그래픽이 가장 도움이 될 것으로 생각되는 인체부위는 머리와 목(21.4%) 그리고 신경(21.0%)이었고, 복잡한 구조를 보다 쉽게 이해할 수 있다고 하였다. 아나토그래픽은 방대한 양의 해부학을 효과적으로 전달할 수 있는 교육자료로 가능성을 보였고, 이것의 장단점을 잘 파악하여 활용한다면 향후 해부학 교육을 위해 유용하게 활용될 수 있을 것이다. There are various methods for anatomy learning. Recently, there is infographics as a way to effectively transmit large amounts of data and complex information. In this way, medical students made anatomical data as ‘anatographic’. A total of 205 participants including 60 pre-medical students, 111 medical students, and 34 doctors participated in the study. We have shown an example of anatographic, and we surveyed its applicability and the satisfaction of each method. They use textbooks, Google, other searching sites, and applications for anatomy learning. The usefulness according to the 5-point scale was 3.75 for textbooks and 3.60 for Google, 2.53 for applications and 2.08 for searching sites, respectively. The utility of anatographic was 3.59, which was significantly higher than that of the application or other searching sites (p<0.05). Anatographic is useful because of easy search for tags and questions, information share and learning from anywhere using your smartphone. On the other hand, there was the hassle of online access and the reliability of the data could be reduced for doctors to use. There was a problem that students used the smartphone outside of learning. Anatographic is the most beneficial system for the head, neck, and nerves, and it is easier to understand complex structures. Anatographic have shown potential as an educational source that can effectively transmit vast amounts of anatomical knowledge, and they will be useful for future anatomy education.
