      • 韓國人의 Residual Accommodation에 關한 硏究 : Ⅲ. 1% Cyclogyl 毛樣筋弛緩劑 使用時의 單眼 및 兩眼의 Residual Accommodation

        This study following the first and second similar reports, was performed to find the residual accommodation remaining after instillation of 1% cyclogyl solution in 402 eyes of young Koreans ranging f roin 6. to 25 years of age. The amounts of the monocular and binocular residual accommodation were measured by the same method as in the second report, and compared with those results. The results obtained were summarized as follows. 1) The average value of the monocular, residual accommodation was 1.45±0.29D which shoved no significant differences in comparigon with the second report. 2) The residual accommodation showed gradual decrease with increasing age. 3) The cycloplegic effect under 2.00 D was 97.26% which was slightly greater than the second report. 4) The average value of the binocular residual accommodation was 1.54±0.34D which was slightly greater than the monocular residual accommodation.

        Since the cornea is the organ with the greatest refractive power in the eye and is the most accessible organ of the eye, the corneal surgery seems to be logical in modifying the ocular refraction by means of surgery. Recently many studies of the refractive corneal surgery in experimental animal have been presented by Barraquer, Sato, Krwawicz, Onuma, Viikarik, Martinez and Katzin etc. However, clinical applications of these surgeries in human have not been actually used as yet because of poor basis of the experimental study. In this experiment the author attempted to study the refractive changes by six different surgical techniques to modify the curvature of the anterior surface of the cornea without compromising its transparency. Materials and Method The animals used in this experiment were 70 mature rabbits weighing approximately 2.0 to 2.5㎏ and of both sexes. Before the operation, the corneal curvature and the refractive power of the eye were measured with keratometer and retinoscope under cycloplegic state produced by instillation of 5 percent homatropine solution. They were anesthetized generally with Nembutal (30㎎/㎏) injection into the marginal ear vein and topically with 0.5 percent pontocaine solution. 1. Group Ⅰ: In order to correct myopia, the stromal corneal tissue was taken out. A circular keratectomy, of 5.5㎜ in diameter and 0.15㎜ thick, was done in the optical center of the corneal stroma with small electrokeratome. 2. Group Ⅱ: In order to correct hyperopia, the stromal corneal tissue was embedded. An interlamellar pocket was dissected with spatula and a stromal tissue of the donor cornea, of 5.5㎜ in diameter and 0.15㎜ thick, was introduced in the pocket and centered in the pupillary zone. 3. Group Ⅲ: In order to correct myopia, a corneal tissue ring was resected and the wound was sutured. A corneal tissue ring of 0.5㎜ width, 0.2 to 0.25㎜ depth and 7mm diameter was resected in the periphery of the optical zone of the anterior corneal surface and the wound was sutured. 4. Group Ⅳ: In order to correct hyperopia, a corneal tissue ring was resected and the wound was not sutured. A corneal tissue ring of 0.5㎜ width, 0.2 to 0.25㎜ depth and 7mm diameter was resected. 5. Group Ⅴ: In order to correct myopic astigmatism, the arciform corneal tissues were resected only in the cornea of the greatest curvature meridian and the wounds were sutured. The arciform corneal tissues of 0.5㎜ width, 0.2 to 0.25㎜ depth and 5㎜ length were resected, parallel with limbus, in the upper and lower part of the peripheral cornea, and the wounds were sutured. 6. Group Ⅵ: In order to correct hyperopic astigmatism, the arciform corneal tissues were resected only in the cornea of the least curvature meridian and the wounds were not sutured. The arciform corneal tissues of 0.5㎜ width, 0.2 to 0.25㎜ depth and 5㎜ length were resected, parallel with limbus, in the upper and lower part of the peripheral cornea. The corneal curvature and the refractive power of the operated eye were measured with keratometer ana retinoscope at intervals of one month for six months, and the refractive changes were statistically observed. The results obtained were summarized as follows. Results and Conclusion 1. In group of the keratectomy of the stromal tissue, the average value of the corneal curvature decreased by operation was -4.06±0.46 D. and the average value of the hyperopic change of the refraction was +6.24±0.73 D. 2. In group of the the intralamellar transplantation of the donor stromal disc, average value of the corneal curvature increased by operation was +6.61 ±0.53 D. and the average value of the myopic change of the refraction was -12.21±0.56 D. 3. In group of resection of the corneal tissue ring with subsequent sutures of the wound, the average value of the corneal curvature decreased by operation was -2.18±0.37 D. and the average value of the hyperopic change of the refraction was +4.80±0.94 D. 4. In group of resection of the corneal tissue ring without suture, the average value of the corneal curvature increased by operation was +1.11±0.25 D. and the average value of the myopic change of the refraction was -1.22±0.37 D. 5. In group of resections of the upper and lower arciform corneal tissues with subsequent sutures of the wound, the average value of the corneal curvature in vertical principal meridian decreased by operation was -l.69±0.58 D. and the average value of the hyperopic change of the refraction in vertical principal meridian was +1.71±0.37 D. 6. In group of resections of the upper and lower arciform corneal tissues without sutures, the average value of the corneal curvature in vertical principal meridian increased by operation was +1.09±0.24 D. and the average value of the myopic change of the refraction in vertical principal meridian was -0.66±0.14 D. 7. In comparing effects of six refractive corneal surgeries, the method of the intralamellar transplantation showed the largest refractive change and seems to be effective especially for the refractive correction of the monocular aphakic eye.
