        사용 방법에 따른 임플란트 토크 조절기의 정확성 비교 분석 연구

        Purpose: This study was to evaluate the accuracy of how to use the Mechanical Torque-Limiting Devices. Materials and methods: Three different implant manufacturers (Dentium, Osstem, Shinhung) were prepared for each two implant torque controllers. Divided into two groups depending on the method used for removal torque was measured. Repeated measures of ANOVA test (${\alpha}=.05$) was used as statistics to evaluate the effect of repeated loading number on the removal torque. Independent t-test was used to evaluate the difference in removal torque of two groups. Results: The removal torque significantly decreased as the number of loading repetition increased (P<.05). There was significant difference between two groups. Conclusion: Loosen the handle part of the implant torque controllers spring resilience to recover one group compared to the group that did not, showed a little more closed to the reference value. 연구 목적: 임상에서 가장 보편적으로 사용되는 기계식 토크 조절기를 선택하여 토크 조절기를 사용하는 방법에 따라서 제조사에서 권장하는 토크를 적용하고 유지하는데 정확도의 차이가 있는지를 비교, 분석하고자 하였다. 연구 재료 및 방법: 36개월을 사용한 3개 제조회사 제품의 토크 최대값 제한 장치 형태의 기계식 토크 조절기를 대상으로 하였다. 토크 조절기를 사용할 때마다 핸들 부위를 조이고 사용 후에는 핸들 부위를 풀어서 토크 조절기 내부의 스프링이 압축력에 의해 변형되는 것을 막고자 한 그룹과 토크 조절기를 사용 후에도 핸들 부위를 풀어내지 않고 토크 조절기 내부의 스프링이 압축력 하에 유지되게 사용한 그룹으로 나누어, 총 6개의 토크 조절기를 이용하여 풀림 토크를 측정하였다. 반복 측정에 따른 풀림 토크값의 차이를 비교하기 위해 repeated measures of ANOVA test (${\alpha}=.05$)를 사용하였고, 사용 방법의 차이에 따른 두 그룹간의 비교를 위해 독립표본 t-검정(Independent sample t-test)을 시행하였다. 결과:모든 실험군에서 반복 측정에 따라 풀림 토크값이 유의성 있게 감소하는 것으로 나타났고(P<.05), 사용 방법에 따른 두 그룹 간에 풀림 토크값은 유의한 차이가 있었다. 결론: 토크 조절기를 사용한 뒤 다시 핸들부분을 풀어서 토크 조절기 내부의 스프링이 압축력에 의해 변형되는 것을 막고 스프링의 복원력을 회복한 그룹은 그렇게 사용하지 않은 그룹과 통계적으로 유의한 차이가 관찰됐고, 기준 토크값(30 Ncm)에 좀 더 가까운 풀림 토크값을 보였다.

        식품의 물성이 제작근의 활성도 및 저작 양태에 미치는 영향

        To elucidate the effects of food texture such as hardness, cohesiveness, adhesiveness on mastication procedure, the electromyographic activity (EMG) of the masticatory muscles and the chewing movements were analyzed in eighteen young subjects during ordinary chewing. Seven different foods were selected by sensory texture profiling and mechanical test with texturometer; egg white, sausage, pizza cheese, yang-geng, biscuit, peanut and almond. The indexes of jaw movements used were the chewing number, chewing time, maximum opening, average opening, closing velocity and opening velocity. The EMG indexes analyzed were maximum and average integrated amplitude of the anterior temporal and masseter muscles. The results obtained were as follows. 1. The chewing time and chewing number, mandibular displacement and mandibular movement velocity were not changed by the hardness, cohesiveness and adhesiveness of the food (p>0.05). 2. The harder food materials showed a higher amplitude of the anterior temporal muscle integrated EMGs than the softer ones (p<0.05). 3. The maximum and average integrated EMGs of the masseter muscle increased with the increase of hardness of the food (p<0.05). 4. The integrated EMGs of preferred side and non-preferred side masticatory muscles increased with the increase of hardness of the food (p<0.05). 5. The adhesiveness and cohesiveness of the food were not the determining factor to the integrated EMGs of masticatory muscles (p<0.05).

        수종 치과용 석고의 접촉각 및 젖음성에 관한 비교 연구

        The purpose of this study was to investigate the contact angles and wettability of conventional dental stones and improved dental stones and newly developed dental stones on several impression materials. Materials included in this study were several dental stones and newly developed dental stone ; 2 type III stones (Snow Rock, New Diastone), 6 type IV stones(Crystal Rock, Vel Mix, Fuji Rock, Tuff Rock, Resin Rock and newly developed dental stone) and 1 type V stone (Die Keen). Contact angles on the impression materials were measured with contact angle measuring device. Ten specimens for each material, total 180 specimens were made on void entrapment model. The two impression materials (Handae, GC) were used to produce 9 groups of die stone casts form void entrapment model. Voids in the stone casts were counted under a stereoscopic microscope. The grad for the reproduction ability of each materials on the void entrapment model was calculated from the casts by one examiner. From the experiment, the following results were obtained : 1. The newly developed stones showed smallest contact angle. Type III dental stone had larger contact angles than type IV and V stones. Contact angle was much affected by the impression materials. 2. Resin containing die materials such as Tuff Rock and Resin Rock had smallest void number than any other groups. 3. In comparing reproduction parameters, Tuff Rock and Resin Rock presented superior results, while Vel Mix showed lowest reproduction ability.

      • 임플란트 보철 후의 식편압입을 최소화하기 위한 고려사항: 인접치와 대합치

        Food impaction due to proximal space opening after implant restoration is a very common phenomenon in patients who have implant prosthesis. This occurs because the movement mechanism between the implant and the tooth is different, and it occurs about 30-60% over time. Contributing factors include the arch (mandible), region (posterior teeth), adjacent teeth (non-vital teeth), and antagonist teeth (natural teeth or implants), etc. While this phenomenon cannot be prevented, efforts should be made to minimize it. In order to have an ideal proximal contact as much as possible, the concave shape or the prominent lower proximal shape should be modified to create a symmetrical proximal shape. with the buccal dentate in the upper third height should be adjusted. Other conditions should be modified so that the heights of the marginal ridges are similar. Since an irregular occlusal plane is a cause of poor prognosis, food impaction should be minimized by restoring the ideal occlusal plane by correcting the extruded antagonist and reduction of the disto-buccal cusp.

        임플란트 보철물의 식편압입 : 교합적 원인분석과 대처

        Food impaction between the implant prostheses and adjacent teeth is the most frequently observed problem. It may be caused by the migration of the adjacent teeth. This symptom may be observed at the mesial aspect of implant prostheses especially, and related with the multiple contributing factors including teeth vitality and antagonist. Idal proximal contact with optimal strength and shape should be made for preventing the food impaction. Shape of customized abutment and prosthesis should have optimal emergence profile. Long duration from the extraction to the delivery of implant prostheses, the adjacent teeth and antagonist teeth may have possibility of occlusal interferences. Remained teeth mobility can induce the food impaction regardless of interproximal contact strength. Occlusal adjustment to remove occlusal interferences can be a method for enhancing the stability of interproximal contact.

      • Resin-bonded Prosthesis에 대한 문헌 고찰과 성공을 위한 제언

        The use of resin-bonded fixed partial dentures described in the early 1980s caused an conservative way to preserve tooth structure in the restorative dental community. The treatment of patients with requires long term analysis of clinical application and basic research. Failure rates of these prosthesis ranged from 3% to 55%. These varieties were orginated by different techniques, materials, tooth preparation methods and diverse clinical situations. This article review was focused on the standard long term results and in vitro studies on bond strength between metal and teeth. From this, many useful clinical guidelines to RBFPD could be adopted to clinical dentistry. For successful results, careful case selection and good clinical skills are needed. And appropriate techniques for each situations should be adopted. Also, RBFPD using new materials like all-ceramics, FRC/Ceromer was introduced.

        부가중합 실리콘 인상재 및 모형재의 젖음성에 관한 비교 연구

        Wettability of addition silicone impression material is very important property for making an accurate restoration. This study examined the impression quality in clinical condition and the wettability of impression and die material. Four commercially available addition silicone impression material (Express, Examix, Contrast, Perfect) and three die materials (Die-Keen, Vel-Mix, Fuji-Rock) were studied. A total of 50 putty/wash and heavy body/wash impressions of wet intact permanent molar teeth were examined for definition of the gingival sulcus reproduction and then classified in quality ranking. The percentage of the sulcus reproduction ability of each material was calculated from the sulcus depths of cross-sectioned epoxy resin casts from the impressions and clinically measured sulcus depths, The same impression materials were used to produce 3 groups of die stone casts form void entrapment die had been exposed to milk. Voids in the impression body and stone casts ere counted under a stereoscopic microscope. From the experiment, the following results were obtained : 1. Sulcus reproduction ability of additional silicone impression material were diminished in order of Express, Examix, Perfect, Contrast. The significant difference was found between Perfect and other material. Heavy body/wash combination was superior In putty/wash method n Perfect impression material. 2. In direct observation, Contrast showed least void in impression body but correlations ere not found between sulcus reproduction and void production. 3. In void entrapment laboratory test, wettability were diminished in order of Examix, Contrast, Express, Perfect. Clinical impression recording seems not to correlate with laboratory test. 4. The wettability of die material to impression material was not different in Express, Examix, Contrast. But, in Perfect, Die-Keen had superior wettability to others.
