증례 : 감염 ; rpoB 유전자 RFLP에 의한 Mycobacterium Marinum 힘줄윤활막염 진단 1예

Mycobacterium marinum is an uncommon cause of skin and soft-tissue infection. The diagnosis of M. marinum infection is often delayed when only a conventional tissue culture method is used. PCR-restriction fragment length polymorphism (RFLP) analysis using the novel region of the rpoB gene is now available for the rapid identification of Mycobacteria. We report a case of hand infection caused by M. marinum that was identified by PCR-RFLP analysis. The PCR-RFLP assay is a specific and rapid method for the identification of Mycobacteria that facilitates the early diagnosis of non-tuberculous Mycobacterium infection. This study decomposes price fluctuation factors of chinese cabbage into three factors: change in acreage, yield response factors, and other factors affecting market sales volume. We estimate the parameters of yield response model and wholesale price model for each of the three types of cabbage: spring, highland, and autumn cabbage. Based on the estimation results, we conduct a scenario analysis to prepare an action plan to respond to sharp price fluctuations. The results of scenario analysis show that cabbage price stability in the harvest season depends on modest temperature and precipitation during growing season, minimized typhoon damage, and maintenance of stable crop acreage. If one of the factors affecting the harvest price is forecasted to change sharply, we need to take immediate measures on supply-demand stabilization.

This study analyzes how the direct rice income compensation payment program affects on farm income for various types of farm household with two different data sources, a farm household survey database and a database on agricultural business entities. We analyze farm income effects by reviewing farm income increase and stabilization, income equality, business stability, and economic safety net for farm households. The results show that the program contributed significantly to increasing and stabilizing farm income, income equality, and business stability. In particular, variable direct payments have a bigger effect on the groups of expert farmers and preliminary expert farmers, while fixed direct payments on senior and retired farmers. Without direct payments, 16~28% of ordinary farmers would face serious management risk and about 3% of senior and retired farmers could not cover the minimum cost of living or household consumption expenditure. Therefore, it is needed to maintain fixed direct payments for senior and ordinary farmers in order to support their farm management and income stabilization.

Objectives:The purpose of this study is to evaluate the clinical applicability of cognitive evoked potentials (CEPs) to identify the cognitive disorder in cerebrovascular diseases (CVD). Methods:The P3 latencies, amplitudes and latency to amplitude ratios (LARs) of CEPs were measured in 25 healthy controls and 35 patients with CVD. The association of CEPs with variables including age, sex, Mini-Mental State Examination (MMSE), CVD types, loci of hemiplegic limbs, duration, education, Brief Psychiatric Rating Scale (BPRS), and Instrumental Activities of Daily Living (IADL) were also analyzed. Results:1) The P3 latencies (447.87±113.06 msec) and LARs (65.83±43.25) were prolonged in CVD (p<0.01), while the amplitudes (8.18±2.51 μV) were not changed. 2) The P3 latencies (537.31±101.14 msec) and LARs (94.89±46.44) in CVD with MMSE <24 were prolonged, and the amplitudes (6.45±1.98 μV) were reduced significantly (p<0.01, respectively), as compared with those in CVD with MMSE≥24 (363.39±19.67 msec, 38.40± 8.37 and 9.81±1.76 μV, respectively). 3) The BPRS and IADL in CVD with MMSE<24 were significantly (p<0.01) different from those in CVD with MMSE≥24. 4) On analyzing the association of CEPs with variables in CVD, the P3 latencies were correlated positively with age, BPRS and IADL, while negatively with MMSE and DLFA. The amplitudes were correlated positively with MMSE, while negatively with age, BPRS and IADL. The LARs were correlated positively with age, BPRS and IADL, while negatively with MMSE. 5) On analyzing the association of CEPs with variables in CVD with MMSE≥24, the P3 latencies were correlated positively with age, while negatively with MMSE. The amplitudes and LARs were not correlated with variables. 6) On analyzing the association of CEPs with variables in CVD with MMSE<24, the P3 latencies were correlated positively with BPRS, while negatively with MMSE. The amplitudes were positively correlated with age. The LARs were positively correlated with IADL. Conclusions:The P3 latencies and LARs of cognitive evoked potentials seemed to a useful clinical measures to assess cognitive disorders in CVD.

증례 : 순환기 ; 유전성 항트롬빈 결핍증이 동반된 만성 폐혈전색전성 폐동맥 고혈압 1예

저자들은 유전자 검사로 확진된 유전성 항트롬빈 결핍증을 가진 39세 남자 환자에서 만성 폐혈전색전성 폐동맥고혈압이 발생하여 혈전내막제거술을 시행한 증례를 국내 최초로 보고하는 바이다. 환자는 내원 시 NYHA class III의 심한 호흡곤란을 보였다. 검사 결과 환자는 폐혈전색전증 및 우심실 부전을 동반한 심한 폐동맥 고혈압을 가진 것으로 진단되었다. 유전자 검사로 SERPINC1 유전자에 새로운 돌연변이가 있는 것이 확인되었다. 환자는 하대정맥 필터 삽입과 폐동맥 혈전내막제거술을 받았다. 이후 1년의 경과관찰 동안 환자는 NYHA class I의 상태를 유지하였고, 우심실 기능의 정상화를 보여주었다. Presented is a case study of a 39-year-old man with chronic thromboembolic pulmonary hypertension (CTEPH) and an underlying antithrombin III (AT III) deficiency. The subject presented with severe dyspnea (NYHA functional class III). A diagnostic workup led to a diagnosis of pulmonary thromboembolism and severe pulmonary hypertension with right ventricular failure. Genetic analysis revealed a novel nonsense mutation (c.243G>A) in SERPINC1. Pulmonary thromboendarterectomy was performed following the insertion of an inferior vena cava filter. After one year, the subject remained in NYHA functional class I and exhibited normal right ventricular function. This is the first report of a genetically confirmed AT III deficiency complicated by CTEPH in Korea. (Korean J Med 79:691-696, 2010)
