Following chemotherapy for acute myelomoncytic leukemia, an 3-year-old boy developed several painful erythematous indurated patches at previous sampling sites and at the site where an arm board and adhesive tape were used in securing an intravenous infusion set. The lesions rapidly progressed to ulcers with central black eschars. Biopsies and cultures demonstrated a fungus, Aspergillus flavs, as the etiologic agent, without evidence of systemic dissemination. Local care, including s irgical debridement, were performed. The patient also received treatment with intravenous; amphotericin B followed by oral itraconazole. Six months later, the skin lesions had healed, leaving some scar and deformities. Partial Discharge (PD) phenomena occurred by different nature of insulating defects has been regarded as a random process by which Phase Resolved Partial Discharge Analysis(PRPDA) has been proposed and then commercially accepted for the diagnosis of the power apparatus since more than three decades. Moreover, for the same purpose, a novel approach based on the Chaotic Analysis(CAPD) has been proposed since 2000, in which Partial Discharge(PD) phenomena is suggested to be considered as a deterministic dynamical process. In this work, for the diagnosis of Gas Insulated Transformer(GITr), four different types of specimen were fabricated as a model of the possible defects that might possibly cause its sudden failures such as turn to tum insulation, inter coil insulation, free moving particle and protrusion. For this purpose, these defects are introduced into the Gas Insulated Transformer(GITr) mock-up and experimental investigations have been carried out in order to analyze the related Partial Discharge(PD) patterns by means of both Phase Resolved Partial Discharge Analysis(PRPDA) and Chaotic Analysis(CAPD) respectively and then their comparisons are made systematically.
철도 시설물 자동화 계측을 활용한 경보 전파 시스템 개발

현재 국토해양부에서는 건설교통재난ㆍ재해대책편람을 발간하여 철도를 포함한 국가 주요 시설물에 대한 재난 대응지침을 수립한바 있으나, 지령 및 상황전파를 전화나 팩스등에 의존하고 있어 업무 전달 속도가 현저히 느리고 인력중심의 안전관리 업무처리로 인해 상황발생 시 신속한 대응이 어려운 실정에 있다. 또한 이러한 실정과 유사하게 철도관련 토목공사 및 유지관리 부문에서도 센서기반 자동화 계측 시스템을 상당수 도입하고 있으나, 운영되고 있는 대부분의 시스템은 계측데이터를 분석하고 시설물 내부 위험을 평가하여 관리주체에 보고 및 전달시간이 상당한 소요되고, 긴급하게 시설물 이상여부를 보고해야 하는 상황에 자동화 계측시스템이 경보 전파를 적절하게 발휘하지 못하는 경우가 비번하게 발생한다. 따라서, 본 논문에서는 기존 철도 토목관련 자동화 계측 경보전파체계에 관한 문제점을 파악하여, 국부단일 토목공사의 단위 계측시스템 뿐만 아니라, 철도에서 활용되기에 적합한 체계로 경보 전파 자동화 시스템을 제안하고, 나아가 국가 기반시설물 안전관리에 연계ㆍ활용 될 수 있도록 상황전파 표준 프로토콜 및 전파시스템을 제안하고자 한다. The key object is the progress report & propagation automation system based on sensors of railway, which is major for providing objective data to realize and support decision makings and delivering decision to a certain area, department, manager and other people rapidly. Collecting, reviewing and database of existing progress report & propagation manual in order to achieve networking of safety management on major social infrastructure of the nation, materialization of field-oriented intelligent business process by developing mobile safety management command transmission device and integrating it into facility safety management network.
충수돌기에서 기인한 고등급 복막가성점액종의 반복적인 용적축소수술의 임상적 고찰

Purpose: To evaluate the effect of repeated debulking surgery for high-grade pseudomyxoma peritonei (PMP) originating from the appendix. Methods: Between January 1998 and December 2014, fifty patients, who underwent debulking surgery for high-grade PMP originating from the appendix, were obtained from a prospectively collected database and retrospectively analyzed. Two groups according to the number of operations were divided and analyzed. Results: A total of 118 operations were performed. Thirty-one patients received more than two operations. The median interval between operations was 18.2 months (range, 2–170 months). Complications developed after 26 operations (22.0%), including ileus (n=10), intra-abdominal fluid collection (n=7), surgical site infection (n=5), and others. There were two mortalities within 30 days after operation. Between two groups of patients who received one operation only and patients who received more than two operations, transfusion, diversion operation, and postoperative complication rate showed statistically significant differences. Two groups of patients had no differences in overall survival rates. Conclusion: Our results indicate that the number of operations does not affect the survival rate of high-grade appendiceal PMP, in which repeated debulking surgery is vital to relieve symptoms of the tumor burden.
