        Kikuchi-Fujimoto Disease의 치험례

        본 과에서는 우측 악하부의 부종 및 동통을 주소로 내원한 환자를 초기에 우측 악하부 및 협부 농양으로 진단하고 절개 및 배농술을 시행하였으나 증상의 호전이 없고 고열과 림프절 비대가 지속적으로 관찰되었다. 이에 저자 등은 비대된 임프절의 적출술과 다양한 검사를 통하여 괴사성 림프절염을 조직학적으로 확인하고 최종적으로 Kikuchi Fujimoto Disease로 확진하였다. Kikuchi-Fujimoto Disease는 임파선 비대와 고열이 동반되는 원인 불명의 괴사성 임파선염으로 특징지워지는 질환으로서, 적출생검을 통해 증상이 사라지는 경우가 많으므로 상기와 같은 증상이 동반된 환자의 경우 악성 림프종이나 전신성 홍반성 루푸스등의 여러 질환과의 감별진단을 통하여 정확한 치료를 시행하는 것이 추천된다. Kikuchi-Fujimoto disease is a rare disease first described in 1972 by Kikuchi and Fujimoto et al. The disease is described as a benign and unusual self-limiting histiocytic necrotizing lymphadenitis of unknown origin, which is characterized histologically by necrotic foci surrounded by histiocytic aggregates. is usually manifested with lymphadenopathy and high fever. This disease mostly affects young Asian women between 20 and 30 years of age and has rarely been reported in children. Main symptoms are indolent or light tender, enlarged lymph nodes in the neck area. The correct diagnosis requires the histologic examination of the lymph node. Kikuchi-Fujimoto disease is easily confused histologically and clinically with lymphoma and systemic lupus erythematosis histologically and clinically. Although it is an uncommon cause of fever of unknown origin, early recognition of KFD is very important and will minimize potentially harmful and unnecessary evaluations and treatments. We reported a case, a 23-year old man who had Kikuchi-Fujimoto disease with a literature review.

        탈단백 소뼈 기질과 다양한 종류의 흡수성 막을 이용한 상악동 거상술 후의 조직-형태학적 분석

        Purpose: The aim of the present study was to evaluate the effect of Bio-Oss on bone formation in terms of healing period and type of membrane so that determine the most suitable condition for implant fixation in grafted maxilla. Material & Method: Forty-five biopsy specimens from graft site were evaluated. Sinus lift was performed in the patients with reduced alveolar bone height(less than 5mm). The specimen was taken at the time of implant fixation, which was performed at least 5 months after the sinus lift procedure. All specimens were stained with H&E and Trichrome staining and evaluated histomorphometrically. Result: The results showed that Bio-Oss particle was in direct contact with newly formed bone in all cases. In the present study, the amount of newly formed bone and the residual bone substitute material were not statistically different according to various membrane and different healing period. There was no difference between the histological feature of the specimen of 5 and 31 months. No statistical significance was detected between male and female. Conclusion: The result implies that Bio-Oss does not seem to be resorbed over time regardless of the type of the membranes. The further investigation is needed to clarify this issue with the extended period of follow-up.

        글리옥심 리간드를 갖는 가교 폴리스티렌계 킬레이트 수지의 합성 및 특성

        Chelating resins of crosslinked polystyrene (PS) beads have been prepared by introduction of glyoxime group by subsequent reactions of acylation, oxidation and oximation with hydroxylamine. Ethylene glycol dimethacrylate (EGD) or tetraethylene glycol diacrylate (TGD) was used in the suspension polymerization system of styrene and divinylbenzene (DVB) in order to give hydrophilicity to the beads and 2, 2, 4-trimethylpentane was used as a diluent. It has been found that porous or reticular type PS beads having large surface area is obtainable from this system and they have high capacity of adsorbing metal ions such as $UO^{2+}_2, \;Cu^{2+}, \;Ni^{2+}$dissolved in water due to their high ability of chelation of glyoxime group and enhanced hydrophilicity of the resin due to the presence of hydrophilic crosslinking agent unit. It was also recognized that the largest capacity of adsorption was exibited at pH 4~5 and a high selective adsorption to $UO^{2+}_2$.

      • 정맥하 진정요법에서 Midazolam 단독 사용과 Midazolam, Fentanyl 또는 Propofol 병용 요법의 비교

        Background: The purpose of this study was to compare the use of midazolam only with midazolam with fentanyl or propofol in IV sedation. Methods: 24 cases were divided to midazolam group (M group), midazolam + fentanyl group (MF group), midazolam + propofol group (MP group) and midazolam + fentanyl + propofol group (MFP group). In M group, 2 ml midazolam was injected at first, than at 2 minutes interval 1-2 ml injected continuously depending on the level of sedation. In MP, MFP groups, propofol was injected at the speed of 15-20 ml/hr by infusion pump. In this study, the sedation level was evaluated by using OAA/S scale. In each groups, the recovery time was measured until OAA/S scale score level was 5, and pre and postoperative blood pressure change was measured. Each group's data was statistically analyzed using one-way ANOVA. If significant statistical difference were observed, Dunnet test was performed, and control group was M group. Results: Pre and postoperative blood pressure change were not represent significant statistical difference in 4 groups (P value = 0.679 [systolic], P value = 0.206 [diastolic]). But recovery time were represent significant statistical difference (M group: 35.6, MF group: 32.5, MP group: 17.9, MFP group: 19.6 [P value = 0.002]). The result of Dunnet test on recovery time showed significant statistical difference on MF, MFP group when M group was control group. In MFP group, sedation was increased by using supplemental fentanyl, and postoperative pain control was dominant. Conclusion: To achieve the effect of anxiolysis, analgesia, amnesia effectively, and short recovery time, MFP group is mostly recommended.

      • VXIbus 시스템 모듈화의 試作에 관한 연구

        본 논문에서는 새로운 방식의 통합계측 시스템으로 각광받고 있는 VXIbus(VMEbus Extension for Instrumentation)시스템에 적합한 신호발생기의 모듈화방안에 관하여 연구하였다. 신호발생기의 모듈화를 위하여 우선적으로 기본적인 시스템의 사양을 결정하였으며, 이러한 사양에 따라서 신호발생기를 설계 및 제작, 실험하였다. 설계 및 제작된 신호발생기를 기본으로 신호발생기와 VXIbus controller와의 통신을 위해서 인터페이스회로를 설계하였으며, 인터페이스를 통한 신호발생기의 제어를 위하여 모듈형신호발생기의 내부동작을 규정하였다. In this paper, Modulization method of signal generator suitable for VXIbus system which make appearance before the footlights as new form of total instrumentation system is studied. For modulization of signal generator, First, Basic specification of system had been determined, and Next, according to specification, Signal generator was designed, tested and realized. Interface circuits which are based on this signal generator are designed for communication between signal generator and VXIbus controller. As signal generator is controlled by interface, internal behavior of module type signal generator is specified.

      • 주파수 도약 대역 확산 통신 방식의 동기 시스템 구현에 관한 연구

        주파수 도약 대역 확산 통신 방식에서 기존의 동기 시스템은 VCO의 비선형 특성, 루프의 온도보상문제, 소형화. 경량화 문제 등으로 시스템 구현에 많은 어려움을 겪고 있었다. 본 연구에서는 정합필터(matched filter)를 이용한 초기동기와 ADPLL(All Digital Phase Locked Loop)을 이용한 동기 추적 방식의 동기 시스템을 구현하였고, 초기동기시간 약 600μS, 동기추적에 약46mS의 시간을 얻어 전체 동기 시스템은 약 65mS이내를 얻었다. 일반적으로 동기시간이 100mS 이내에 이루어지면 완전한 동기를 이룬것으로 보며, 따라서 구현된 동기 시스템이 매우 안정되고 정확한 동기를 이루고 있음을 알 수 있었다. In the frequency hopping spread spectrum communication, the conventional synchronization systems have many problems in their implementation. It is nonlinear characteristics of VCO, temperature compensation of loop and implementing compactly. In this paper, we implemented the synchronization system which consists of the ACQUISITION by matched filter and the TRACKING by ADPLL(All Digital PLL). In this synchronization system, acquisition time and tracking time were about 600㎲, 64ms respectively, so that total synchronization time could be within 65ms. Generally, as synchronization time is within 100ms we can regard it as complete synchronization. therefore, we could prove that the implemented synronization system has a stable and accurate synchronization.
