        한약으로 관해가 유도된 극심한 설사의 궤양성대장염 환자 증례보고

        Ulcerative colitis (UC) is one of inflammatory bowel diseases (IBD) with unknown etiopathogenesis. This case report details remission of UC induced by Korean herbal medicine treatment. A 31-year-old male patient diagnosed as UC after a series of examinations including endoscopy and fecal calprotectin (FC) test. He had severe bloody diarrhea over 10-20 times a day despite taking 5-aminosalicylic acid and steroid. He was given individualized Korean herbal medicine Dowha-tang according to pattern identification. After taking Dowha-tang for 7 months, clinical remission was achieved. The symptoms disappeared and FC level went down to normal level. No adverse events were reported. This case report shows that Korean herbal medicine treatment may have the potential for clinical remission of UC. Further investigation is warranted.

        침 치료 임상연구 동향에 대한 계량서지학적 분석

        Objectives : As acupuncture treatment has been widely practiced in many countries around the world, clinical trials of acupuncture treatments also have become popular. The objective of the study was to explore the trends of research investigating the effect of acupuncture treatment in clinical trials using a bibliometric approach, a quantitative analytical methods. Methods : Publications related to clinical trials using acupuncture from 2000 to 2019 were retrieved from the Web of Science database. Extracted articles were analyzed in terms of publication year, country, journal, research area, organizations and authors. Trends in research on acupuncture in clinical trials were visualized using the VOSviewer program. Results : A total of 3,166 articles of acupuncture clinical trials published from 2000 to 2019 were identified and analyzed. The country producing the most articles in this field was USA followed by China, England, South Korea, and Germany. A network analysis based on the co-occurrence of keywords showed following three clusters: clinical studies, pain management studies, and methodology studies. Conclusions : This study provided a macroscopic overview of research in acupuncture clinical trials. These findings provide an expansive strategy for researchers in this field to cooperate with other researchers or organizations.

        지역물류단지 최적 입지선정을 위한 방법론 연구

        물류단지란 물류단지시설과 지원시설을 집단적으로 유치, 육성하여 화물이동의 효율성을 높이기 위한 시설이다. 정부에서는 매 5년마다 물류시설개발종합계획을 통해 물류단지를 포함한 주요 물류시설의 배치계획을 수립함으로써 국가의 합리적, 균형적 물류시스템 구축을 도모하고 있다. 그러나 거시적인 관점에서 지역별 화물처리 수요, 공급 우선순위에 대해 언급할 뿐 가능입지, 추진방법 등 구체적 추진계획은 포함하지 않고 있다. 따라서 본 연구에서는 ‘물류단지 최적 입지선정을 위한 프로세스’를 제시하고자 한다. 우선 ArcGIS를 활용한 토지분석을 통해 물류단지로서 적합한 대상지를 파악하고, 접근성, 주변환경 등을 종합적으로 고려하여 후보지를 선별한다. 이어서 물류시설 위치 및 처리 물동량을 입력변수로 하는 p-median모형을 적용하여 가장 적합한 후보지를 최종 선정한다. 본 연구에서 개발한 프로세스를 충청남도 사례에 적용함으로써 방법론의 적용 가능성을 검증해 본다. 본 연구의 결과는 물류시설개발종합계획, 지역물류기본계획 등 관련 계획의 효율적 추진 및 물류기업체의 시설 투자를 위한 참고자료로 활용될 수 있다. The government establishes 'logistics facilities development plan' every 5 year to promote reasonable and efficient development of the national logistics system. However, the plan focuses on freight demand and supply priority excluding detailed action plans like possible locations, construction strategies, etc. This research proposes 'optimal location selection process of logistics complex'. First, land analysis is conducted using ArcGIS to find possible locations and then accessibility and surrounding environment are analyzed to choose several potential sites. Next, the p-median model considering logistics facility locations and their freight movement is appled to select the optimal locations. A case study is shown to demonstrate application possibility of the proposed methodologies. The results of the research can be used as a good guideline and reference for making logistics relevant plans and polices.

        전기 화물차 도입에 따른 도로운송분야의 이산화탄소 배출량 및 환경적 비용편익 분석

        본 연구는 도로운송 분야의 온실가스 배출량 저감을 위한 방안 중 전기 화물차 도입에 대한 효과를 분석하였다. 이를 위해 화물 운송, 전기 자동차, 환경정책 등의 자료를 수집하고, 도로교통 분야의 전기 화물차 도입에 관한 주요 변수를 정의하였으며, 이 변수들을 예측하기 위한 시뮬레이션 모형은 시스템 다이내믹스 방법론으로 구축하였다. 우선 전기 화물차 도입에 대하여 인과지도를 작성한 후 주요 변수들을 포함한 세부모듈을 경제모듈, 화물차 등록모듈, 환경모듈로 구성하여 작성하였으며, 시뮬레이션 수행 후 핵심변수의 평가를 통하여 시뮬레이션 모형을 검증하하였다. 그 결과, 이산화탄소 배출량은 시뮬레이션 종료 기간인 2050년에 2020년 대비 20.6% 증가할 것으로 예측되었다. 이에 본 연구는 이산화탄소 배출량 저감을 위한 방안으로 전기 화물차 도입 정책을 구성하고, 정책을 반영하기 위한 시뮬레이션 모형을 구축하였다. 이어서 전기 화물차 등록대수의 변화에 따른 이산화탄소 배출의 감축량을 산정 하고, 환경 비용편익 분석을 실시하였다. 전기 화물차가 도입되었을 경우 도입되지 않았을 때보다 이산화탄소 배출량이 약 71.4-79.6% 감소할 것이라 분석되었으며, 총 시뮬레이션 기간 동안의 배출 감소량은 시나리오 별로 81,244-121,345 천tCO₂가 감소될 것이라 전망되었다. 또한 전기 화물차 1대당 시나리오 별로 최소 2,176,416원에서 최대 2,255,214원의 환경비용 절감을 가져올 것이라 예상되었다. 본 연구를 통하여 전기 화물차 도입이 도로운송 분야에서 조속히 이루어져야 하는 방안이라는 것을 판단할 수 있으며, 앞으로 화물차의 이산화탄소 배출량 저감에 전기 화물차 도입이 크게 기여할 것으로 기대해 볼 수 있다. This study analyzed the effects of introducing electric trucks among measures to reduce greenhouse gas emissions in the transport sector. For this study, data such as freight transport, electric vehicles, and environmental policies were collected, and key variables were defined for the introduction of electric trucks in the road transport sector, and simulation models were constructed with system dynamics methodology. First of all, a causal map was prepared for the introduction of electric trucks, and detailed modules including major variables were composed of economic module, truck registration module, and environmental module. After simulation, simulation models were verified and results were analyzed. Carbon dioxide emissions are forecast to increase by 20.6% in 2050, the end of the simulation period, compared to 2020. As a way to reduce carbon dioxide emissions, this study constructed a simulation model to form an electric truck introduction policy and reflect policies, calculated the amount of carbon dioxide emissions due to changes in the number of electric trucks registered, and conducted an environmental cost benefit analysis. As a result, if electric trucks were introduced, carbon dioxide emissions would be reduced to about 71.4-79.6% compared to when electric trucks were not introduced, and emissions reductions over the total simulation period would be reduced by 81,244-121,345,000 tCO₂ per scenario. It was also analyzed that each scenario would result in a reduction in environmental costs of at least 2,176,416 won to a maximum of 2,255,214 won. Through this study, it can be determined that the introduction of electric trucks should be carried out as soon as possible in the transportation sector, and how much contribution can be made to the reduction of carbon dioxide emissions from trucks.

        Association of Trial Registration with Reporting Biases in Randomized Controlled Trials of Acupuncture

        Objectives : To investigate the association of trial registration status with presence of reporting bias including publication bias and outcome reporting bias in recently published randomized controlled trials (RCTs) of acupuncture. Methods : A PubMed search for RCTs of acupuncture published from March 2016 to February 2017 was conducted. Primary outcomes were identified and the direction of the results was judged as positive (i.e., statistically significant) or negative. The trial registration was identified by manually screening the trial registration number in the main text of the published article and classified into 1) prospective registration; 2) retrospective registration based on the registration date or; 3) no registration. Results : Of the 125 included RCTs, only 40 studies (32.0%) prospectively registered the study protocols. Among 65 RCTs that adequately reported the primary outcome, unregistered trials were more likely to report positive results than the registered ones (p=0.013). Of the 40 prospectively registered studies, 19 trials (47.5%) had the discrepancies between the registered and published primary outcomes and furthermore, 40% of them reported the positive findings. Conclusions : Unregistered trials were more likely to report positive results and the discrepancies between the registered and published primary outcomes were detected in about a half of the prospectively registered studies, 42.1% of which tended to report positive findings. Journal editors and researchers in this field should be alerted to various reporting biases.

        진도항 해양관광 발전전략에 관한 연구

        진도항은 2021년 해양수산부에서 발표한 제4차(2021~2030) 전국항만기본계획에 따라 여객 수송 기능을 강화하여 해양관광 중심으로 발전하겠다는 비전이 수립되었다. 진도군은 풍부한 관광자원을 보유하며, 진도항의 해양관광 기능을 활성화함으로써 진도군의 관광산업을 발전시켜 지역 경제성장 동력을 확보할 계획이다. 이에, 본 연구는 진도항의 해양관광 발전전략을 제시하기 위해 해양관광 선행연구와 전문가 인터뷰를 통해 진도항 해양관광 발전전략 평가항목을 4개 대분류와 12개 소분류로 제시하였다. 이어서 AHP(Analytic Hierarchy Process) 분석으로 평가항목 간의 상대 중요도 결과를 도출하였다. 대분류 중 ‘카페리 운영 활성화’가 중요한 항목으로 평가되며, ‘인프라 확충’이 그 뒤로 나타났다. ‘지원체계 마련’과 ‘크루즈 관광 활성화’는 중요도가 상대적으로 낮았다. 소분류 중에 ‘카페리 노선 확대’와 ‘카페리 수송 자원 확보’, ‘여객터미널 편의시설 확충’은 상위 3개 항목으로 평가되었다. 본 연구는 AHP 분석결과를 토대로 진도항 해양관광 발전전략을 카페리 노선 확대, 카페리 여객 및 화물 수송 자원 확보, 여객터미널 편의시설과 배후지 관광산업 시설 구축 등으로 제시하였다. 또한, 장래 진도항의 해양관광 고부가가치 산업을 위한 크루즈 활성화 정책을 제언하였다. 본 연구의 분석결과는 향후 진도항 해양관광 발전전략을 수립하기 위한 기초 정책 자료로 활용될 것으로 기대된다. Purpose : The purpose of this study is to suggest strategies to develop the Jindo port’s ocean tourism by the AHP (Analytic Hierarchy Process) analysis. Research design, data and methodology : This study derived the development strategies of ocean tourism for Jindo port through literature and experts’ opinions and conducted AHP analysis by investigating 35 experts. Results : According to the AHP survey results, ‘Vitalization of car ferry operation’ and ‘Infrastructure expansion’ were evaluated as more important than the other 2 major categories. The important sub-categories were 'Expansion of car ferry routes', 'Securing car ferry transport resources', and 'Expansion of passenger terminal convenience facilities'. Conclusions : In order to develop Jindo port’s ocean tourism industry, activating the car ferry operation and expanding the port’s infrastructure including the passenger terminal and hinterland’s tourism facilities should be executed first.

        여수·광양항의 운영 효율성 평가에 관한 연구

        Yeosu·Gwangyang port is considered as the 5th major port in Korea with Busan port, Incheon port, Ulsan port, and Pyeongtaek·Dangjin port. With handling oil, ore, coal, and containers as the main items, the overall throughput of Yeosu·Gwangyang port has increased steadily. Yeosu·Gwangyang port has been propelling various strategies such as the renovation of old terminals, rearrangement of container berths, and establishment of transportation network to strengthen the port competitiveness. However, in the past 5 years, Yeosu·Gwangyang port has the highest ship congestion rate among the 5 major ports in Korea. It indicated that there is an inefficient operation due to the facility or technology inadequate in Yeosu·Gwangyang port. Previous studies have mainly focused on deriving development strategies for Yeosu·Gwangyang port. The studies that have analyzed the operational efficiency of Yeosu·Gwangyang port are insufficient. Therefore, in this study, to evaluate the port operation efficiency of Yeosu·Gwangyang port, a group of competing ports was compared and analyzed by Entropy-DEA(Data Envelopment Analysis) model. The competing ports were selected based on four major handling items (oil, ore, coal, and container) at Yeosu·Gwangyang port. According to the results, Yeosu·Gwangyang port had the best efficiency on oil terminal operating. The ore terminal, coal terminal, and container terminal showed inefficient results. In particular, it was analyzed that the efficiency of the container terminal was the worst. It is necessary to establish strategies such as promotion of automation for container terminals and marketing in consideration of regional characteristics. This study diagnosed the operation status of Yeosu·Gwangyang port and could be utilized as a reference for establishing the development strategies for Yeosu·Gwangyang port.

      • A Study on Port (Terminals) Operational Efficiency Analysis Using PCA-DEA Approach: Focusing on Container Terminals on Busan Port

        Previous literature has recognised the importance of port efficiency to port performance, and two popular methodologies of DEA (Data Envelopment Analysis) and SFA (Stochastic Frontier Analysis) have been widely applied to efficiency evaluation. However, there seems a limitation of lacking objectivity when selecting the variables of inputs and outputs. Meanwhile, as the largest port in Korea, although many related efficiency studies already exist, most of them are founded on a whole port level basis while not on the container terminal level. Therefore, to fill the gap, this paper introduces a PCA (Principal Component Analysis)-DEA approach to research the operational efficiency of nine container terminals of Busan Port with the latest data of 2021. Container throughput is set as the output, and 13 variables from three scopes of land, facilities and labour are identified as inputs, followed by PCA which eventually detects three indictors of “No. transfer crane (RMGC) in yard”, “Staff average salary” and “Container freight station (CFS) space” as the core inputs representing the above three dimensions. Then, the DEA(CCR) and DEA(BCC) models are used to gain the assessment results: “PNIT”, “PNC” and “HJNC” are DEA efficiency, while others of “HPNT”, “BNCT”, “Jaseongdae”, “Shinseondae”, and “Gamman” and “Shin Gamman” are non-DEA efficiency. In addition, to achieve the target efficiency for the non-effective terminals, this paper confirms improvements through Return to Scale (ROS) and input redundancy discussion. The PCA-DEA port (terminal) evaluation approach is practical and unbiased, and it may have some special practical value for the port industry.
