        외상성(外傷性) 기관지(氣管支) 파열(破裂) - 1례(例) 보고(報告) -

        Rupture of a bronchus is an unusual result of nonpenetrating trauma to the chest. This case was a 19 year old male who was a worker in the mine. The patient had sustained a compression chest injury without external wound or rib fracture. At five days after trauma, he was suffered from dyspnea and the symptom was progressively increased. Two weeks after the chest trauma, a diagnosis of left main bronchial obstruction due to traumatic bronchial rupture was made by means of bronchoscopy and bronchography. He was operated upon. The left lung was completely atelectatic and the left main bronchus was found to be transected at the level of 3cm from carina. End-to-end anastomosis of the bronchus was performed and the left lung was aerated very well. postoperative course was uneventful.

        기관(氣管) 섬유종(纖維腫)의 1례(例)

        Primary tumors of trachea are rather uncommon, and few cases of direct surgical excision were reported in the literature. Recently we had the opportunity to see a patient with a benign obstructing tumor of the trachea which was confirmed as fibroma. The patient has complained of intermittent dyspnea, especially during inspiratory phase, dry cough and wheezing of a strident character for last 8 years. Bronchoscopy or bronchography were not attempted because of severe dyspnea. Trachea tomogram revealed oval mass at the terminal trachea. The right posterolateral thoracotomy was performed. Tumor, $2.5{\times}1.7cm$ in size, was located at terminal trachea and removed through right lateral tracheotomy without difficulty. Postoperatively all the symptoms and signs disappeared.

        흰쥐의 심장을 이용한 Modified Isolated Working Heart Perfusion Technique

        We have modified an isolated perfusion rat heart model of cardiopulmonary bypass, with which we are able to screen the effects of various cardioplegic solutions and hypothermia upon the ability of the heart to survivie during and recover from period of ischemic arrest. The modified experimental model was differed from the original as follow : a heat coil chamber of atrial and aortic reservoir provided temperature control, and the perfusate was gassed with each pure oxygen and pure carbon dioxide in 95:5 ratio. The Langendorff perfusion was initiated for a 10 minute period by introducing perfusate at $37^{\circ}C.$ into the aorta from the aortic reservoir located 100 cm above the heart. The isolated perfused working rat heart model was a left heart preparation in which oxygenated perfusion medium (at $37^{\circ}C.$) entered the cannulated left atrium at a pressure of 20 cm $H_{2}O$ and was passed to the ventricle, from which it was sponeously elected(no electrical pacing) via an aortic cannula, against a hydrostatic pressure of 100cm $H_{2}O$. during this working period various indices of cardiac functin were measured. The cardiac functions were stable for over 3 hour with perfusion of Krebs-Henseleit bicarbonate buffer solution containing only glucose (11.1 mM/L). The percentage of cardiac functins were maintained about 94% on heart rate, 80.6% on peak aortic pressure, 87.7% on coronary flow and 76.3% on aortic flow rate after 3 hour of working heart perfusion at a pressure of 20 cm $H_{2}O$. We believe this preparation to be a good biochemical model for the human heart which offers many advantages including economic, speed of preparation, reproducibility, and the ability to handle large numbers.

        만성 폐전색증의 폐동맥 색전제거및 내막 절제술;치험 2례

        Two successful pulmonary thromboendartectomies were performed. In the first case, it was performed under cardiopulmonary bypass with moderate hypothermia and ventricular fibrillation In the second case, it was done under deep hypothermia and intermittent circulatory arrest. The patients are recovered uneventfully without complication and discharged from the hospital on warfarin. Their symptoms were improved and the follow-up pulmonary perfusion scans revealed no evidence of residual pulmonary embolus in both cases.

        가토 적출 폐장의 장시간 보존에 관한 실험적 연구

        We have modified the isolated perfused working rabbit lung model [IPWL] by perfusing the isolated lung with a hollow fiber membrane deoxygenator.For assessment the stored lung was ventilated with FIO2 0.4 and perfused with 37$^{\circ}$C deoxygenated circulating blood at a rate 5ml/kg/min for several hours until lung failure.We chose to compare our developing solution which contained low potassium and pentastarch with the modified Euro-Collins solution .Experiments were divided into four groups[n=6] based on the type of flushing preservation solution and preservation time.The flushed lungs were then preserved into same solution at 8~10$^{\circ}$C with 100% O2 inflated condition for 1 or 20 hours.These following results were obtained.The IPWL model requires only one animal per experiment and allows for the continuous assessment of aerodynamic performance. This should therefore be used as screening test in lung preservation.One hour preservation groups, there were no significant difference in recovery rates of PaO2, PAP and Paw. Survival time in the one hour preservation groups were very significant long in the Group II[LPPS, p<0.01]. Twenty hours preservation groups, there were no significant difference in the recovery rates of PAP and Paw between Group III[m-ECS] and Group IV[NS], but PaO2 was significantly worse at onset of reperfusion in Group III when compared with Group IV [p<0.05]. And also survival time in the 20 hours preservation groups were significant long in the Group IV [p<0.05].

        가수분해법에 의한 알루미나 코팅 지르코니아 분말의 제조

        Zirconia powder coated with alumina was prepared by hydrolysis of alumina butoxide. The coated powder was obtained by a hydrolysis reaction between the adsorbed water on the surface of zirconia particles and aluminum sec-butoxide. Amorphous aluminum hydroxide was uniformly coated on the surface of zirconia particles with the thickness of about 30 nm. The shape and distribution of aluminum hydroxide was varied with an existence of surfactant. The coated layer of aluminum hydroxide consists of the fine particle size, and the zirconia powder coated by alumina hydroxide have the large specific surface area of 120 $m^2$/g, compared with that of starting zirconia powder.

        Ebstein 기형에 인공판윤을 이용한 금속형 St. Jude Medical 인공판막 대치술

        Ebstein`s anomaly is characterized by a downward displacement of a malformed tricuspid valve, The ideal surgical management of Ebstein`s anomaly is not yet established. Recently we experience one case of Ebstein`s anomaly, which was treated sussessfully by partial artificial annulus formation, and tricuspid valve replacement with St. Jude Medical valve. We have achieved excellent results with mechanical valve replacement and partial artificial annulus formation using wessex pericardial patch. On follow up for 4 years, the patient is well and in functional class I.

        소결분위기가 이트리아 안정화 지르코니아 세라믹스의 미세구조 및 등온상전이에 미치는 효과

        수소 및 산소분위기 하에서 2Y-TZP 및 3Y-PZP 세라믹스를 소결하여 각각 제조한 다음, 각 시편의 미세구조 및 25$0^{\circ}C$에서의 등온상전이 거동을 비교하여 고찰하였다. 그 결과 수소분위기에서 소결한 모든 시편의 표면에서는 Zr$_{4+}$ 이온의 환원에 의해 산화분위기에서 소결한 시편에 비하여 색이 검게 변하였으며, 장시간 소결시 시편 표면에 조대 입자의 생성 및 입계 이완이 관찰되었다. 이에따라 수소분위기 하에서 장시간 소결된 시편의 경우 냉각 시 상전이가 일어나 다량의 단사정상이 상온에서 존재하였다. 저온 aging동안 등온상전이 속도는 산소분위기에서 소결한 시편보다 수소분위기에서 소결한 시편에서 낮게 나타났으며, 이러한 경향은 $Y_{2}$O$_{3}$의 고용량이 크고, 입자크기가 적은 시편일수록 증가하였다. 또한 2Y-TZP 시편에서는 입자크기에 상관없이 대부분의 정방정상이 단사정상으로 빨리 등온상전이를 일으켰으나, 3Y-TZP 시편의 경우, 등온상전이는 입자크기가 증가함에 따라 상전이 속도도 증가하였다.

        Yttria를 도핑한 세리아 안정화 지르코니아 세라믹스의 미세구조

        세리아 안정화 지르코니아 성형체내에 이트리아를 침적법으로 도핑시켜 미세구조를 관철하였다. 도핑된 이트리아의 양과 열처리 조건을 제어하여 입계가 변형된 불규칙 입자형상의 미세구조를 형성시켰으며, 이때 입자형상의 불규칙 정도는 첨가된 이트리아의 양에 비례하였다. 0.2M 농도의 Y(NO$_3$)$_3$.6$H_2O$의 수용액에 재침적하여 1$650^{\circ}C$에서 2시간 열처리한 시편의 미세구조는 표면부와 내부 모두에서 불규칙 입자형상을 나타내었으며, 입자의 불규칙도도 가장 큰 것으로 나타났다. 가압소결을 행한 결과 입계가 불규칙하게 변화된 미세구조가 창출됨과 동시에 평균입자 크기가 0.3$\mu\textrm{m}$ 가량인 미세한 소결체를 제조할 수 있었으며, 상분석 결과 대부분 정방정상을 유지하고 있었다. 불규칙 입자형상의 미세구조를 가진 시편의 파괴인성값은, 정상적인 입자형상을 갖는 시편에서의 압흔 균열 크기로부터 비교 유추한 결과 17.6MPa.m(sup)1/2 이상인 것으로 나타났다. Yttia-doped ceria-stabilized ziconia polycrystals(Ce-TZP) was prepared by dipping method and its microstructure was investigated. By controlling doped-yttria content and annealing condition, yttria-doped Ce-TZP showed the microstructure with irregular grain shape and undulated grain boundary. Irregularity of grain shape increased with the amount of yttria doped, and severe undulated grain boundary was observed mainly at the surface region. In the case of yttria-doped Ce-TZP annealed at 1$650^{\circ}C$ for 2h after two dipping times into yttrium nitrate solution of 0.2M, it showed irregular grain shape both at the surface and at the interior region as well as the most severe irregularity. Hot pressed specimen had mean grain size of 0.3$\mu\textrm{m}$ and undulated grain boundary. All specimens with irregular grain shape were retained the tetragonal phase. The fracture toughness of yttria-doped Ce-TZP with irregular grain shape was over the value of 17.6MPa.m(sup)1/2.
