        선천성 코눈물관 막힘 환자에서 내시경적 실리콘관 삽입술의 치료효과

        Purpose: To evaluate the surgical results and clinical effectiveness of silicone tube intubation through Hasner’s valve under intranasal endoscopic guidance in children with congenital nasolacrimal duct obstruction (CNDO). Methods: The present study enrolled a total of 58 eyes of 46 children over 12 months of age diagnosed with CNDO in the Department of Ophthalmology, Inje University College of Medicine from 2003 to 2010. Patients who had previously undergone ineffective probing or had a combination of an anatomical abnormality such as secondary nasolacrimal duct obstruction were also included. The silicone tube was intubated through the Hasner’s valve and observed with intranasal endoscopy; tube retention time was planned for at least 12 weeks. Results: A total of 20 eyes were previously probed preoperatively (mean 1.7 times). After the average follow-up period of 13.5 months, the overall success of silicone intubation was 57 of 58 eyes (98%). In one failed eye, nasolacrimal obstruction was already combined preoperatively, and unintentional tube removal by the patient occurred within one week. Silicone tube reintubation was performed after four months, and CNDO symptoms improved. Conclusions: Intranasal endoscopic observation to ensure the passage of a silicone tube through Hasner’s valve decreases anatomical injury. When considering patient age and former treatment, silicone tube intubation with intranasal endoscopic observation could produce desirable results with a higher success rate.         Choline Acetyltransferase 유전자 다형성이 경도인지손상 발현에 미치는 영향

        Objectives : The potential association between choline acetyltransferase(CHAT) polymorphism and the risk of mild cognitive impairment(MCI) has not been investigated in Korea. We examined the main effect of CHAT polymorphism and its interaction with apolipoprotein E(APOE) polymorphism in the development of MCI in elderly Korean sample. Methods : We analyzed CHAT 2384G > A polymorphism and APOE polymorphism among 149 MCI subjects with MCI and 298 normal controls. We tested the association between MCI and CHAT A allele status using a logistic regression model. In addition, we employed generalized multifactor dimensionality reduction(GMDR) to investigate the interaction between CHAT and APOE with regard to the risk of MCI. Results : The CHAT A allele was associated with AD risk(OR = 1.59, 95% CI = 1.02-2.48, p = 0.042). No significant gene-gene interaction between CHAT and APOE was found in GMDR method(testing balanced accuracy = 0.540, p = 0.055). Conclusion : The CHAT A allele was associated with MCI risk in the Korean elderly. Its interaction with the APOE ${\varepsilon}4$ allele was not significant with regard to the development of MCI.

        의료 WBAN 환경을 위한 IEEE 802.15.4 MAC 성능 개선

        WBAN(Wireless Body Area Network)는 인체주변 2-3m 영역에서 의료 및 비의료 디바이스들로 구성된 다양한 환자 모니터링 분야를 지원하기 위한 무선센서네트워크이다. WBAN 환경을 위해서는 저전력 소비, QoS, 듀티사이클등의 요구사항을 만족하고 주파수 대역을 효율적으로 분배하며 트래픽 로드에 강하면서 에너지를 절약하는 MAC(Medium Access Control)이 설계되어야 한다. 본 논문에서는 트래픽 로드가 증가할 때를 고려해 에너지에 효율적인 AQ(Adaptive Queuing) MAC 슈퍼프레임 구조를 제안한다. 또한 시뮬레이션 결과 제안하는 AQ(Adaptive Quenuing)MAC를 IEEE 802.15.4 MAC과 비교 하였을 때 전송처리율, 평균MAC 지연율 측면에서 향상된 결과를 얻을 수 있었다. WBAN(Wireless Body Area Network) is a Wireless Sensor Network for supporting various applications around body within 2~3m which consists of medical and non-medical device. MAC in WBAN environment should satisfy requirements such as low power consumption, various transmission rate, QoS, and duty-cycle, efficiently distribute frequency band, be strong at traffic load and save energy. This paper proposes AQ(Adaptive Queuing) MAC superframe structure for efficient energy use, considering the increase of traffic load. The simulation result also show that transmission rate and average MAC delay rate is improved comparing IEEE 802.15.4 MAC with AQ MAC.

        IEEE 802.15.6 기반 CSMA/CA 성능 향상에 관한 연구

        WBAN을 위한 MAC 프로토콜은 의료용 센서 노드의 가변적인 데이터를 효율적으로 처리하기 위해서 응급상황에서 발생하는 트래픽을 가장 높은 우선순위로 처리하는 CSMA/CA(: Carrier Sense Multiple Access with Collision Avoidance) 알고리즘을 수행하게 된다. 노드가 전송하는 응급 메시지는 충돌이 발생하게 되고 응급 메시지 재전송으로 인한 전송 지연이 발생할 수 있으며 재전송으로 인한 에너지 낭비를 가져올 수 있다. 본 논문에서는 이러한 문제점들을 개선하기 위해서 우선순위 큐잉을 적용한 CSMA/CA 알고리즘의 경우 MAC 명령어 프레임과 데이터 프레임의 우선순위를 정하여 서로 다른 백오프 파라미터를 적용하고 충돌로 인한 프레임 손실을 최소화하는 알고리즘을 제안하였다. 성능평가 결과 제안한 MAC 프로토콜을 사용했을 때가 IEEE 802.15.6을 사용했을때 보다 충돌 확률이 감소하여 패킷 전송 처리율이 증가하고 패킷 손실량이 감소함을 보였다. MAC protocol for WBAN performs CSMA/CA(:Carrier Sense Multiple Access with Collision Avoidance)algorithm that handles traffic which occurs in emergency with top priority in order to deal with variable data of sensor node for medicine. Emergency message that node sends undergoes collision and delay of transmission by retransmission of emergency message and waste of energy by retransmission. This paper suggests algorithm that minimizes loss of frame caused by collision and applies different backoff parameters by setting order of priority between MAC instruction frame and data frame in CSMA/CA algorithm case which applies order of priority queuing to improve aforementioned problem. According to performance evaluation, it has been found that using suggested MAC protocol showed lower collision probability, higher packet transmission processing ratio and lower packet loss compared with using IEEE 802.15.6.

        알츠하이머병의 최신지견

        Alzheimer's disease (AD) is a neurodegenerative disorder in which neuronal loss causes cognitive decline and other neuropsychiatric problems. It can be diagnosed based on history, examination, and appropriate objective assessments, using standard criteria such as the Diagnostic and Statistical Manual of Mental Disorders or the National Institute of Neurological and Communicative Disorders and Stroke and the Alzheimer's Disease and Related Disorders Association (NINCDS-ADRDA). Brain imaging and biomarkers are making progress in the differential diagnoses among the different disorders. The cholinesterase inhibitors, donepezil, rivastigmine and galantamine and N-methyl-D-aspartate receptors antagonist memantine are approved by the US Food and Drug Administration for AD. Recently some acetylcholinesterase inhibitors gained approval for the treatment of severe AD and became available in a higher dose formulation or a patch formulation. Optimal care in AD is multifactorial and it should include early diagnosis and multidisciplinary care with pharmacological and nonpharmacological interventions including exercise interventions, cognitive interventions and maintenance of social networks.

        웨이블렛 변환과 HMM을 이용한 고유공간 기반 얼굴인식에 관한 연구

        본 논문은 Wavelet 변환을 이용한 실시간 얼굴 영역 검출을 제안하였으며, 계산의 효율성과 검출 성능을 동시에 만족시키는 강인한 검출 알고리즘을 제안하였다. 검출된 얼굴 영상은 주성분 분석을 통해 저차원 얼굴 심볼로 구성하여 얼굴을 인식한다. 제안된 방법은 기존의 기하학적인 특징 기반 방법이나 외관기반 방법의 비해 많은 계산 량이 요구 되지 않고 최소한의 정보를 사용하고도 높은 인식률을 유지 할 수 있기에 실시간 시스템 구축에 매우 적합하다. 또한 얼굴 인식 시 발생하는 잘못된 인식이나 인식 오차를 줄이기 위해 고유 공간상에 투영된 모델 특징 값을 군집화 알고리즘을 통해 특정한 기호로 구성하여 은닉마르코프 모델의 입력 기호로 사용하였다. 이렇게 함으로써 임의의 입력 얼굴은 확률 값이 가장 높은 해당 얼굴 모델로 인식하게 된다. 실험 결과 기존의 방식인 Euclidean과 Mahananobis방법 보다 제안한 방법이 잘못된 매칭이나 매칭 실패에서 우수한 인식 성능을 보였다. This paper proposed the real time face area detection using Wavelet transform and the strong detection algorithm that satisfies the efficiency of computation and detection performance at the same time was proposed. The detected face image recognizes the face by configuring the low-dimensional face symbol through the principal component analysis. The proposed method is well suited for real-time system construction because it doesn't require a lot of computation compared to the existing geometric feature-based method or appearance-based method and it can maintain high recognition rate using the minimum amount of information. In addition, in order to reduce the wrong recognition or recognition error occurred during face recognition, the input symbol of Hidden Markov Model is used by configuring the feature values projected to the unique space as a certain symbol through clustering algorithm. By doing so, any input face will be recognized as a face model that has the highest probability. As a result of experiment, when comparing the existing method Euclidean and Mahananobis, the proposed method showed superior recognition performance in incorrect matching or matching error.

        쯔쯔가무시 감염에서 발생한 양안성 시신경염 1예

        Purpose: To report a case of bilateral optic neuritis in a child after tsutsugamushi infection. Case summary: An eight-year-old boy diagnosed with tsutsugamushi infection was referred to the ophthalmology clinic at our institution. A typical eschar was detected on his right clavicle. The patient complained of severely decreased visual acuity. The initial best corrected visual acuity (BCVA) in his right eye was 0.08, and in his left eye was negative light perception. Fundus exam showed bilateral optic disc swelling and hyperemia. Optic neuritis was diagnosed after tsutsugamushi infection, and high-dose intravenous steroid therapy was initiated according to the ONTT study protocol. Twelve months after treatment, the right eye BCVA improved to 1.0, but the left eye BCVA was only 0.06. Conclusions: The authors of the present study experienced a case of bilateral optic neuritis after tsutsugamushi infection with a relatively positive response to the steroid therapy.       • 임베이디드 리눅스에서의 확장된 NAT를 이용한 VPN 구현

        최근 인터넷의 성장과 함께 인터넷을 이용한 사설망인 VPN의 필요성이 커지고 있다. 본 논문에서는 확장된 NAT 기능을 이용한 VPN 기능을 임베이드 리눅스 환경에서 구현하여 기존 VPN의 단점을 보완하고, VPN 장비의 크기를 최소화 할 수 있도록 한다. 제안된 VPN 기능은 실제 인터넷 환경에 적용하기 쉬우며 보안 프로토콜이나 암호화 알고리즘을 생략하고도 강한 보안성을 제공하고, 기존의 공개망을 최대한 활용할 수 있어 그 활용가치가 높을 것으로 생각된다. 또한, 임베이드 시스템에서 VPN을 구현함으로써 장비의 소형화, 안정성 및 경제성을 실현하여 그 활용가치가 높을 것으로 생각된다.
