        합리적 관개용수량 산정에 관한 연구

        In this study, the actual water management were investigated and analyzed on site for effective use of the paddy irrigation water. And this study is tried to analyze the difference between simulated irrigation water according to current design method and actual water supply in experimental reservoirs. The reservoirs in Idong, Yongdeok and Misan, which are managed by Korea Rural Community & Agriculture Corporation, were selected fur field investigation. The purpose of this study is to suggest an improved method for irrigation water calculation through quantitative and periodical analysis considering the difference between design and actual field condition.

        현장 측정을 통한 관개용수로의 손실량 추정

        Water losses in irrigation canals are mainly estimated as the sum of conveyance and delivery water loss. The losses occur via the evaporation, infiltration, gate operation and water distribution processing. Recently, the study regarding these water losses are not satisfactory enough, also delivery water loss has not been mainly considered on field design. The objective of this study is to investigate and analyze the volume of water loss in irrigation canals considering condition of actual farm land. A field measurement was performed at four research sites, which are managed by Korea Rural Community & Agriculture Corporation, to evaluate conveyance and delivery water loss for 2 years. The measurement was performed by canal type, size and designed flow using the inflow-outflow method at a major points such as start and end of each canal, derivation point of canal and inlet of paddy fields. Results of this study showed that water loss ratio in lateral canals was bigger than that of main canal unlike current design standard and the loss decrease as flow increase. The total of water loss ratio including conveyance and delivery water loss in several irrigation canals ranged between 33.25 and 45.0%.

        안면신경 봉합 후 지방조직으로 둘러싼 부목의 임상적 적용

        Facial deformity after nerve injury changes ones' social life. We experienced a few patients with healthy early recovery of muscle contraction after the operation with soft tissue wraparound splint. Under general anesthesia, exploration to find as many injured nerve stumps with ${\times}2.5$ loopes was undertaken at first. Interfascicular repair was done with minimal tension by 10-0 nylon under a microscope, and the suture site was sealed by approximating the surrounding fat flaps. This conjoined adipose tissue flap was a splint as a wraparound environment to reduce the tension in the coaptation site, and to increase the relative concentration of releasing neurotrophic factors by surrounding it. A 45-year-old man fell down in a drunken state and had deep laceration by broken flowerpot fragments with facial muscle weakness on the right cheek. His injured mandibular branches of the facial nerve were found. A 31-year-old female suffered from motionlessnesss of frontalis muscle after a traffic accident. She had four frontal branches injured. The man had his cheek with motion after seven days, and the woman two months after the operation. The nerve conduction test of the woman showed normalized values. Facial nerve repair surrounded by adipose tissue wraparound splint can make the recovery time relatively short.

      • 수치해석모형(數値解析模型)에 의(依)한 홍수추적(洪水追跡)

        In this study, an implicit one-dimensional model, DWRM(Dynamic Wave Routing Model) was developed by using the four-point weighted difference method. By applying the developed model to the Keum River, the parameters were calibrated and the model applicability was tested through the comparison between observed and computed water levels. In addition, the effects of the construction of an estuary dam to the flood wave were estimated as a result of the model application. The results of the study can be summarized as follows; 1. The roughness coefficients were evaluated by comparison between observed and computed water level at Jindu, Gyuam and Ganggyeung station in 1985. The Root Mean Squares for water level differences between observed and computed values were 0.10, 0.11, 0.29m and the differences of peak flood levels were 0.07, 0.02, 0. 07m at each station. Since the evaluated roughness coefficients were within the range of 0.029-0. 041 showing the realistic value for the general condition of rivers, it can be concluded that the calibration has been completed. 2. By the application of model using the calibrated roughness coefficients, the R. M. S. for water level differences were 0.16, 0.24, 0. 24m and the differences of peak flood level were 0.17, 0.13, 0.08 m at each station. The arrival time of peak flood at each station and the stage-discharge relationship at Gongju station agreed well with the observed values. Therefore, it was concluded that the model could be applied to the Keum River. 3. The model was applied under conditions before and after the construction of the estuary dam. The 50-year frequency flood which had 7,800m<sup>3</sup>/sec of peak flood was used as the upstream condition, and the spring tide and the neap tide were used as the downstream condition. As the results of the application, no change of the peak flood level was showed in the upper reaches of 19.2km upstream from the estuary dam. For areas near 9.6km upstream from the estuary dam, the change of the peak flood level under the condition before and after the construction was 0.2m. However considering the assumptions for the boundary conditions of downstream, the change of peak flood level would be decreased.

        We experienced a case of 49-year-old male patient with amputated two fingers preserved in Soju (Korean traditional liquor, a kind of alcoholic beverages). The amputation level of the two fingers was at the distal interphalangeal joint. The Soju was not an adequate physiologic solution for preserving the amputated tissues. Even though arterial anastomosis was successful, there was no venous drainage visible in the operative field. On the first day after the initial operation, we succeeded in the anastomosis of one vein in one of the two amputated fingers. This was 12 hours after arterial anastomosis was carried out. But no venous dranage was visible in the other finger. In spite of a salvage procedure sustained with external bleeding for 7 days, this replanted fingertip eventually fell into necrosis.

        헤링씨 법칙 비의존성 단안성 윗눈꺼풀처짐증 환자에서 수술 후 생긴 건측의 눈꺼풀처짐증

        Background: Correction of unilateral blepharoptosis is unexpectedly difficult because healthy eye is often affected by Hering's law. Methods: We measured changes of marginal reflex distance (MRD1) on the unaffected eyelids between preoperative and 3-month postoperative photographs after ptosis correction. This study analyzed 134 unilateral blepharoptosis patients with ptosis correction from February 2002 to February 2011. Fifty patients among them were negative in Hering's law dependence test. From the preoperative and postoperative photographs the MRD1 of unaffected upper eyelids were measured and adjusted with the average pupil diameter of Koreans. Mean age was 34.4 and male was 30 and female was 20. Average follow-up periods were 14 months. Results: Thirteenth unaffected eyes (26%) showed decreased MRD1, and 3 patients (6%) showed decreased MRD1 value over 1.0 mm. Then 3 patients needed additional operations for correction of preoperatively unaffected but ptotic eyelids. Conclusion: There were no meaningful data statistically in the value of MRD1 in every unaffected eye of the patients and in the difference between preoperative and postoperative MRD1 of groups divided according to severity, causes, and types of operation.

      • 급성 수부 손상에서 간과된 혈관성 손상을 가진 수부의 운명

        In acute hand injury, there are sometimes hattened neglected or overlooked vascular injury by primary operators. The authors evaluated the final results and prognosis after secondary revascularization. In eight cases, the authors performed secondary revascularization after prolonged warm ischemia. Five fingers in five cases among them were successfully survived and three cases finally necrosed. The mean warm ischemic time was 56.1 hours. In revascularization procedures, end-to-end artery anastomosis was possible in six cases. In two cases, vein graft was needed to anastomose digital artery, which resulted in complete survival of the fingers. In all three cases, revision amputation of the fingers was done. In acute complex hand injury, the importance of evaluation of the vascular injury can not be overemphasized. The necessity of the early secondary revascularization as well as serious consequence caused by misdiagnosis of vascular injury should be aware.

        실라스틱(Silastic$^{(R)}$) 배액관을 이용한 간편한 봉합고정 드레싱

        Purpose: Tie-over dressing is widely used to secure skin grafting on face, body, or extremities. It can be a rather complicated task and is not easy to make compressive dressing again if performed in a conventional method. So, we hereby introduce an easy reproducible tie over dressing method. Methods: After completing the skin graft, Cut the silastic drainage longitudinally in half and spread to the grafted skin margin. Drainage is fixed by using the stapes or sutures. A fluffy gauze bolus dressing is placed over a furacin impregnated gauze and wrapped around. After suturing the distal margin of silastics with opposite side using the silk thread either 5 - 0 or 3 - 0, knot of suturing, which is pressed down against the dressing while the threads are tightened, is made into center of each sides. Results: It can make dressing again after observing the grafted skin, and it can also make pressure on the grafted area evenly until the grafted skin is taken. Conclusion: This dressing method makes the surgeons and patients comfortable. To surgeons, it provides more rapid and easier way to do dressing, and to patients, it eliminates pain caused by redressing.
