N/A Ultrastructurai Study of Fixed Drug Eruption Ai Young Lee, M.D., Yoo Shin Lee, M.D. Department of Dermatology, College of Medicine, Seoul National University Hospital Seoul, Korea Although patch test responses and histopathology of fixed drug eruption and allergic contact dermatitis are similar, the former recurrs persistently at the same site. The pathogenesis of this site predilection remains to be clarified. The purpose of this study was to examine ultrastructurally healed and active sites of fixed drug eruption. Degenerative changes of keratinocytes, which showed aggregation of tonofilaments, increment of electron density, decrement of nuclear size and irregular condensation of chromatin, were observed in asymptomatic lesions above 1 month after attacks as well as in active lesions. The changed keratinocytes usually attached to lymphocytes and Langerhans cells in both active and healed lesions. The ultrastructrual difference between active and healed lesions lay mainly in the extent and degree to be changed. In the provocation, changed keratinocytes with lymphocytes and Langerhans cells appeared earlier than macrophages. Langerhans cells and lymphocytes seemed to be pulled from dermis to epidermis in active and provoked lesions, and to play a role in the pathogenesis. (Kor J Dermatol 29(4) : 466-470, 1991)

Pseudoxanthoma elasticum-like papillary dermal elastolysis (PXE-like PDE) is a rare acquired elastolytic disorder that clinically resembles pseudoxanthoma elasticum. The pathogenesis of this disease is uncertain, but it seems to be related to intrinsic aging, abnormal elastogenesis, a genetic predisposition and exposure to ultraviolet radiation. It generally develops on the sides of the neck and supraclavicular regions in elderly persons. Histologically, the characteristic features are the loss of elastic fibers in the papillary dermis and no solar elastosis. We report here on a case of PXE-like PDE in a 26-year-old female whose skin lesion developed 9 years previously. Despite the preferential occurrence at an old age, our patient was young. Suction is a useful tool for separation of epidermis from dermis in vivo. With appropriate suction pressures and durations applied, cellular damage of the separated epidermis can reduce, This study was performed to examine the appropriste suction pressures and durations in the lhsional and normal skin of four vitiligo patients who wanted to take epidermal autograft. The suction pressures and durations for making the blisters of similar morphology were different according to person. Microscopic changes in the blisters of similar morphology were not very different, and suction of 200 to 250 mmHg for 2 to 3 hours with a 2cm diametered suction cup was considered to be acceptable. Obvious damage to keratinocytes and diminish of melanacyte number were observed with much more powerful and longer suction such as suction of 350 to 400 mmHg for 3 to 4 hours. The level of dermo-epidermal separation was between basal cell membrane.

Contact allergens themselves are not enough to predict the development of allergic contact dermatitis(ACD). Recommendation of repeated insult patch tests(RIPTs) as methods of safety prediction for topical products suggests that repeated exposure to sufficient allergens could be appropriate for sensitivity induction and ACD development. A significant association between hand eczema and nickel allergy, and between polysensitization and leg ulcer duration more than 5 years could reflect the role of repeated exposure in ACD development. Repeated exposure to allergens may involve in high incidence of ACD in atopic dermatitis. Identification of risk factors for ACD development is needed for its prevention. Taking into consideration of the above mentioned results, repeated exposure to allergens should avoid prevention and/or control of ACD development.

In allergic skin diseases, cutaneous adverse drug reactions, atopic dermatitis, urticaria, and allergic contact dermtitis are included. Although urticaria and allergic contact dermaitis is less common in incidence compared to atopic dermaitis, both have been considered as representative allergic skin diseases of IgE-mediated and cell-mediated immune responses, respectively. The common causes of allergic contact dermatitis and urticaria in children could be different from those in adult. If allergic contact dermaitis were suspected, patch test would be useful for not only diagnosis but also identification of causative antigen(s). [Pediatr Allergy Respir Dis(Korea) 2010;20:4-9]

Sulfites are in widespread use as preservatives and antioxidants in food, pharmaceutical and cosmetic industries. Although consumption of foods and drinks that contain sulphites is main source of exposure, exposure may occur through the use of pharmaceutical products and in occupational settings such as hairdressing and the food industry (bakers or caterers). Therefore, physicians should know the potential sources of exposure. In addition, exposure to sulphites has induced a range of adverse reactions, such as dermatitis, urticarial, anaphylactic, and asthmatic reactions. Therefore, physicians should also be aware of the range of clinical manifestations of sulphite sensitivity. Here, we are going to deal with atopic dermatitis, allergic contact dermatitis, and urticarial caused by sodium metabisulphites.
Cytochrome p450 동위효소를 통한 고정약진의 원인규명에 관한 연구

N/A The reasons of same site recurrence in fixed drug eruptions (FDEs) remain to be clarified. Although the nature of antigen in FDE is unknown, drug metabolites could play a role for antigen formation. Cytochrome p450 isozymes (CYPs) are important enzymes for drug metabolism. This study was done to examine the role of CYPs in FDEs. Provoked lesion was compared with non-provoked lesion by the same drug on the same patient to overcome inter-individual variations of CYPs. The reverse transcriptase-polymerase chain reaction (RT-PCR) with primers for CYPs and the immunohistochemistry (IHC) with anti-CYPs, pancytokeratin, and leukocyte common antigen (LCA) antibodies were conducted. The causative drugs were different in 13 patients who conducted RT-PCR, and the result could not be analyzed by the cause. The levels of CYP2C8/19 and CYP2E1 mRNAs increased significantly in provoked lesions. The keratinocytes in cases of mefenamic acid-induced FDEs stained strongly with anti-CYP2C9 antibody not with the other three antibodies (CYP1A1, CYP2E1, and CYP3A4), The FDE cases from doxycycline, which is not metabolized by CYP2C9 enzyme, and those from chlormezanone did not react to anti-CYP2C9 antibody, The cells stained with CYP antibodies did not react with anti-LCA antibody but with anti-pancytokeratin antibody. The number of cells which reacted to anti-LCA antibody clearly increased in the provoked lesions, regardless of the cause. The above results suggest that CYPs may contribute the drug antigen formation and different levels of CYPs between provoked and non-provoked lesions can play a role for the same site recurrence of lesions in FDEs.

Arteriovenous hemangioma is a type of hemangiomas which are derived from or are composed of vessels. Recently, authors experienced a case of arteriovenous hemangioma, which clinically suspected a granuloma pyogenicurn but could be confirmed histopathologically.
