The types of Alpine ski injuries have changed through the years in relationship to the development of skiing equipment. Modern skis, boots and bindings are better at protecting the tibia, which previously was almost as commonly injured as the knee. Since the 1980s, severe knee sprains, most of them involving the anterior cruciate ligament have tripled while injuries of the lower extremity below the knee diminished significantly. However, recent studies show no further improvements in either lower leg fractures or increase in the rates of ACL sprains has occurred. The use of carving skis presents an increased risk for sustaining isolated ACL injuries in more skilled skiers and less skilled skiers are more likely to sustain an ankle fracture than skiers using conventional skis. To restore the trend of diminishing lower leg injury rates, efforts will be needed to motivate skiers to have their equipment serviced by ski shop professionals following ASTM (American Society for Testing and Material) standard procedures. As of now, there are no boots, bindings or skis on the market designed to protect skiers from the ACL injury. The only method proven to reduce ACL injury Is a training program based on recognizing the circumstances that lead to ACL injury in skiing and to avoid these events. 역사적으로알파인스키부상은스키장비의발달과밀접한관련을가지고변화하여왔다. 근래에일어난스키장비의혁명으로인하여스키부상은또다른양상으로바뀌어가고있는추세이다. 현대적인스키, 스키화, 바인딩의개발로과거에빈발하던경골간부골절이나족관절손상은1970년대말에이르러90$\%$ 가까이감소하였다. 하지만1980년대에들어서전방십자인대손상을포함한심한슬관절손상은 약3배가까이증가하였으며,장비의개발에도불구하고전방십자인대손상을줄이지못하고있다. 최근에는슬관절이하의하지손상및전방삽자인대손상발생율모두변화가중단된상태이다. 최근스키디자인을완전히바꾸어놓은카빙스키는전통스키에비하여상급자에게전방십자인대단독손상의위험성을증가시키는것으로보인다. 대신초급자에게는족관절손상의위험성이높다. 스키로인한하지손상율을더욱낮추기위해서는스키전문점의스키기술자가ASTM (American Society for TestingMaterial)의표준에따라장비를관리하도록유도하여야한다. 현재까지개발된스키장비중전방십자인대손상을줄일수있는것으로검증된것은없다. 효과가입증된유일한방법은,위험한상황을인식하여그것을피하는방법을익히는트레이닝프로그램뿐이다.

The purpose of this study was to analyzed whether the genetic polymorphisms in the candidate genes including COL1A1, COL3A1 and MMP3 genes are significantly associated with the vulnerability to anterior cruciate ligament(ACL) rupture with surgery experience. A total of 153 subjects were recruited in this study, and they were divided into both ACL rupture group(n=80) and control group(n=73) by clinical status. In the case of ACL rupture patients, they all experienced the surgery. Automated DNA isolation kit isolated total genomic DNA, and the genetic analyses of these candidate genes were performed by using SNaPshot method. Among three candidate genes analyzed, the rs#1800012 polymorphism in COL1A1 gene indicated the monomorphic pattern in this study. Also, with respect to the rs#1800255 polymorphisms in COL3A1 gene, there were no significant differences in the genotype and allele frequencies of this genetic polymorphism between both groups, respectively. In the case of the rs#679620 polymorphism in MMP3 gene, this genetic polymorphism was significantly associated with the susceptibility to ACL rupture in this study(p<.05), and especially, the GG genotype and G allele of this genetic polymorphism indicated the significant association with ACL rupture risk. Further studies will be needed to define the precise role of the rs#679620 polymorphism in MMP3 gene on the occurrence of ACL rupture.
알파인 스키 부상(1) : 역사적 고찰 및 최근 경향 Historical Review and Current Findings

Modern skis, boots and bindings are better at protecting the tibia, which previously was almost as commonly injured as the knee. Since the 1980s, severe knee sprains, most of them involving the anterior cruciate ligament have tripled while injures of the lower extremity below the knee diminished significantly. However, recent studies show no changes in rates of lower leg fractures or ACL sprains has occurred. In the incidence of skiers thumb has decreased by 62% since the 1970s. Compared with Alpine skiers, snowboarders have relatively high number of upper extremity injuries(distal redius fracture in particular) but significant fewer thumb injuries. Ankle injuries and overall lower leg injuries including tibia shaft fracture during snowboarding have delcined consistently over recent years without a specific reason indentified. The use of carving skis presents an increased risk of rsustaining isolated ACL injuries in more skilled skiers and less skilled skiers are more likely to sustain an ankle fracture than skiers using conventional skis. The overall rate of skiboard injuries is 2.2 times higher than that of conventional skis and in terms of lower extremity injury, the injury rate is 3.9 times higher than that of conventional skis. For diminishing lower leg injury rates, efforts will be ASTM(American Society for Testing and Material) standard procedures. The only method proven to reduce ACI injury is training program based on recognizing the circumstances that lead to ACL injury in skiing and to avoid these events.

Supracondylar fracture of the humerus is the most common fracture around the elbow joint in children, especially in the age from 4 to 9. 97% of The fractures are extension type and there are many problems in management such as the method of reduction and maintenance of reduction, Volkmann's ischemia, neurovascular injuries, cubitus varus or valgus deformity, and myositis ossificans etc. Currently the methods of treatment of supracondylar fracture include open reduction and pin fixation, closed reduction and percutaneous pin fixation, and closed reduction and immobilization by splint. 134 children with supracondylar fracture of humerus, conservatively 49 cases and operative 85 cases, were treated from January 1991 to October 1995 and were followed u p for at least 6 months. We analyzed the type of fracture, method of treatment and results and concluded that accurate reduction, minimizing soft tissue injury and maintenance of reduction are important factors for gaining good results.
