Purpose: We analysis the outcome and complications of treatment of lesser toe brachymetatarsia. Materials and Methods: We analysed 28 patients 35 cases of lesser toe brachymetatarsia. Mean post operative follow up period was 2 years 8 months. All of the patients were female and mean age at operation was 21 years old. 2 cases of third metatasal bone and 33 cases of fourth metatarsal bone were operated. 8 metatarsal bones were treated using one staged lengthening with tricortical bone graft and 27 metatarsal bones were treated using callotasis with monofixator. Results: The average amount of lengthening was 13.3mm(12mm-15mm) in one staged lengthening, while 14.4mm(4mm-23mm) in callotasis. Average percentile increase was 28.9%(26%-34%) in one staged lengthening and 32%(18%-46%) in callotasis. The average healing index of callotasis was 76 days/cm (41 days/cm-166 days/cm). Satisfied outcomes in 4 cases of 8 cases (50%) after one staged lengthening and 17 cases of 27 cases (63%) after callotasis. 6 complications in 4 cases were occurred after one staged lengthening; insufficient length gain in 3 cases, fracture on the junction of graft bone and metatarsal bone in 1 case, plantar bowing deformity in 1 case and bony fusion of metatarsophalangeal joint in 1 case. 17 complications in 10 patients were occurred after callotasis ; metatarsophalangeal joint stiffness in 8 cases, metatarsophalangeal joint subluxation in 2 cases, overlengthened metatarsal bone in 2 cases, tapering of callus in 1 case, fracture of callus in 1 case, premature consolidation of callus in 1 case, osteomyelitis of metataral head in 1 case and plantar bowing deformity in 1 case. Conclusion: Although one staged lengthening and gradual lengthening using callotasis are effective treatment for lesser toe brachymetatarsia, complications not rarely occured after lengthening. Insufficient lengthening are most common complication after one staged lengthening while metatarsophalangeal joint stiffness lire most common complication after callotasis.

Brachymetatarsia, a relatively uncommon foot deformity, is defined as abnormaly short or hypoplastic metatarsal. It is thought to be produced due to premature closure of the epiphyseal plate at the distal aspect of the metatarsal. The exact etiology of the premature closure is unknown but it may be congenital, traumatic, iatrogenic or environmental. Though the signs and symptoms vary according to age, the presenting probslem is mostly aesthetic. So the lengthening of the abnormaly short metatarsal is the treatment of choice. We experienced two different methods for the surgical correction of the brachymetatarsia. Procedures were performed for the metatarsal lengthening. For 7 cases, we osteotomized the short metatarsal horizontally and interposed autogenous iliac bone graft. In the next 3 cases, we applied llizarov callus distraction technique for the simultaneous metatrsal and soft tissue lengthening, the device was Orthofix dynamic axial fixation system. Postoperative result showed excellent outcome in aesthetic as well as functional aspect in both techniques. Each procedure has their own advantages and disadvantages, so we would like to present the actual case history, surgical technique, perioperative technique in detail.

Baseball is rapidly getting popularity in Korea as it has been in U.S.A and Japan. During the last decade, threre were several studies about baseball injuries which oriented mainly to the overuse injuries of growing baseball players. But there was no study the professional baseball players systematically. This study was surposed to see the general characteristics of baseball injuries and to provide the prevention and rehabilitation programs if recommended. The number of 83 injuries of 60 baseball players was analysed according to the position, injury site, diagnosis, injury mechanism and duration of injury. The mean age and height of 60 subjects were 24.6 years old and 180.65㎝, respectively. The baseball position of 82 injured cases were composed of 27 pitchers, 30 infielder, 19 outfielder and 6 catchers. The characteristics of injury were as follows: 1.The number of chronic injury was 56 cases(68.3%) and the percentage of chronic injury of pitchers was up to 81.5% compared 73.3% of infielder and 47.4% of outfielder. 2. The injury rate of both lower and upper extremitis were equal and the rate of lower back problems was the second, which should be concerned. 3. The number of cases injured by direct collison is as low as 15(18.3%). The most common secondary cause of injury was aggressive throwing itself and 26 out of 49 cases injured not by direct collision. 4. More concern should be payed about the low back problems and about the catcher and the field players as much as pitchers from now one.
