        응급실 간호사의 환자안전 위험요인에 대한 위험성 인식과 안전 간호활동

        Purpose: This was a descriptive research study to examine the patient safety risk factors and the level of safety management of nurses in emergency service, hospitals and to analyze the relationship between the two factors. Method: Data for analysis were collected from 232 nurses in emergency service, hospitals in Busan and Gyeongnam from July 30 to September 7, 2013. Data were analyzed using descriptive statistics, t-test, one-way ANOVA, and Pearson correlation coefficients. Results: Therapeutic agents showed the highest risk level. The prevention of transfusion errors showed the highest performance. As the nurses were working in regional emergency medical centers and received education more than 7 sessions on patient safety, they readily recognized the riskiness of the safety risk factors. In addition, as the nurses were older than 40, married, having more education about safety and understood the incident report registration system well, they performed safety management better. There were significant correlations between perception of the patient safety risk factors and performance for safety management. Conclusion: Nurses in emergency service, hospitals should try to improve safety management to reduce the risk factors shown to be higher based on the results and ensure the patient safety.

        고혈당으로 유도된 염증반응 모델에서 유황양파즙의 항염증 효능 평가

        고혈당으로 유도된 염증반응 모델에서 유황양파즙(onion juice)의 항염증 효능을 평가하였다. 고혈당(HG, 25 mM glucose) 조건하에서 48시간 배양된 THP-1 세포들은 정상혈당(NG, 5.5mM glucose) 조건의 세포들에 비해 유의적으로 낮은 세포 생존율을 보였으나, 50-150 μL 유황양파즙을 함께 처리하자 NG군 수준으로 회복되었다. 이는, 유황양파즙이 고농도 당에 의한 세포독성을 완화시켜 주었음을 시사한다. 고혈당으로 유도된 HG군에서 염증성 사이토카인 TNF-α는 NG군의 290% 수준까지 분비되어 유의적으로 증가하였으나, 50 μL 유황양파즙 처리로 TNF-α의 분비 수준이 전반적으로 저해되었으며, TNF-α의 발현 수준 역시 낮아져, 유황양파즙이 고혈당에 의한 염증반응을 저해하였음을 시사해 주었다. 이는 유황양파즙에서 보고된 높은 수준의 플라보노이드에 의한 효과로 일부 해석되었다. 본 연구는 식품 섭취를 통한 당뇨 및 당뇨합병증 예방의 가능성을 보여줌으로써, 식이인자의 항염증 효능에 관한 향후 연구들의 기초적 근거 자료로 활용될 수 있다. The anti-inflammatory activity of onion juices prepared from sulfur-fertilized onions was investigated by measuring the secretion of proinflammatory cytokines from human monocytes cultured under hyperglycemic condition. Human monocytic (THP-1) cells were cultured under normoglycemic (NG, 5.5 mM glucose) or hyperglycemic (HG, 25 mM glucose) conditions, with or without onion juice. Without onion juice, cell viability decreased significantly in the HG state for 48 h, compared to that in the NG state. With onion juice (50-150 μL) treatment, the cell viability was not different from that under the NG condition, suggesting that onion juice prevented HG-induced monocytes cytotoxicity. While the HG condition in vitro significantly induced TNF-α release from THP-1 cells and its gene expression, onion juice (50 μL) significantly suppressed them. This indicates that onion juice inhibited HG-induced cytokine production in monocytes. These results suggest that onion juice from sulfur-fertilized onions can be used for the prevention of diabetes and related diseases.

        스트렙토조토신으로 유도한 당뇨마우스에서 Sodium Butyrate의 혈당, 혈청 지질 성상 및 염증 억제에 미치는 영향

        본 동물실험은 STZ로 유도한 C57BL/6에게 5% sodium butyrate를 급여했을 때 항당뇨 및 항염증 효과를 연구하고자 하였다. 본 연구에서 STZ로 당뇨를 유발한 마우스에게 5% sodium butyrate를 급여했을 때 체중과 식이섭취량에서는 크게 유의적 차이가 없음을 확인하였다(p<0.05). STZ에 의한 당뇨 쥐는 인슐린의 분비가 감소되면서 당대사의 불균형을 초래하며 간 등이 비대해진다고 알려져 있으나, 본 연구에서는 간의 장기 무게에서는 크게 실험군 간에 유의적인 차이가 없었다(p<0.05). 또한 비장과 흉선의 무게는 0.5% sodium butyrate 첨가 식이군에서 유의적으로 낮아짐을 알 수 있었다(p<0.05). 당뇨병은 염증 상태로서 고혈당으로 인하여 monocyte에서는 여러 염증성 사이토카인이 분비가 활성화된다. TNF-${\alpha}$, IL-6 등은 염증성 사이토카인으로서 혈관염증의 중요한 마커로 인식되고 있고, 당뇨병 환자들은 이러한 염증성 사이토카인이 높은 수준으로 활성화 된다. STZ 처리 시 마우스 혈청에서의 염증성 사이토카인의 분비 및 발현이 증가되었으나, 5% sodium butyrate를 급여했을 때 염증성 사이토카인의 분비 및 발현이 저해됨을 확인할 수 있었다. 본 연구는 sodium butyrate 보충은 당뇨병이 유발된 동물모델에서 혈청지질 농도 및 혈당 조절, 염증 상태를 개선에 다소간의 효과가 있는 것으로 나타났다. 이에 따라 당뇨병과 같은 만성적인 대사질환 개선에 sodium butyrate가 효과적인 식이인자가 될 것으로 생각된다. 그러나 앞으로 더 명확한 효능을 탐색하기 위해서 시료 첨가수준의 다각화 및 여러 가지 보완연구가 필요할 것으로 생각 된다. Sodium butyrate is a short-chain fatty acid derivative found in foods, such as Parmesan cheese and butter and is produced by anaerobic bacteria fermentation of dietary fibers in the large intestine. There have been reports that butyrate prevented obesity, protected insulin sensitivity, and ameliorated dyslipidemia in dietary obese mice. This study investigated the effects of sodium butyrate on fasting blood glucose level and serum lipid profile in streptozotocin(STZ)-induced diabetic mice. Male C57BL/6 mice were fed AIN-93G for four weeks prior to intraperitoneal injections with STZ (100 mg/kg body weight). Diabetic mice had supplements of 5% sodium butyrate for four weeks. The 5% sodium butyrate diet significantly improved fasting blood glucose level and lipid profile in STZ-induced diabetic mice. Inflammation has been recognized to decrease beta cell insulin secretion and increase insulin resistance. Circulating cytokines can directly affect beta cell function, leading to secretory dysfunction and increased apoptosis. Thus, anti-inflammatory therapies represented a potential approach for the therapy of diabetes and its complications. In this animal study, the 5% sodium butyrate supplementation also inhibited inflammatory cytokine production in STZ-induced diabetic mice. These results suggested that sodium butyrate can be a potential candidate for the prevention of diabetes and its complications.

        모싯잎 가루를 첨가한 찰보리 찐빵의 항산화 활성, 이화학적 특성 및 관능적 품질 특성에 관한 연구

        본 연구에 의하면 보리와 모싯잎을 첨가하여 찐빵을 제조하여 관능검사와 기계적 측정을 확인하여 기능성 식품개발과 보리 이용의 효율성 증대를 모색하였다. 기능성 식품인 찰보리와 모싯잎 가루의 항산화성을 알아보기 위하여 모싯잎 가루를 첨가한 보리찐빵의 DPPH 라디컬 소거능, 총 폴리페놀 함량을 측정하여 항산화 활성을 알아보았다. 찐빵의 총 폴리페놀 함량의 결과는 모시 찰보리 찐빵은 찰보리 찐빵보다 높은 수치를 나타내었다. 이는 보리와 찰보리의 시너지 효과로 보인다. 색도 측정을 한 결과, L값은 밀찐빵이 가장 높았고, 찰보리 찐빵이 가장 낮았다. a값은 모시 찰보리 찐빵이 유의적으로 그룹간에서 가장 낮았고(p<0.05), 찰보리 찐빵이 가장 높았다. b값은 모시 찰보리 찐빵이 16.18로 가장 높았고, 찰보리 찐빵이 가장 낮았다. 물리적 특성을 측정한 결과, 밀찐빵에 비해 찰보리 찐빵과 모시 찰보리 찐빵 간에는 경도가 높고, 찰보리 찐빵과 모시 찰보리 찐빵 간에는 경도 차이는 유의적으로 나타나지 않았으며, 탄성력이 모시 찰보리 진빵이 높았고, 부착성이 월등이 낮음을 관찰할 수 있었다. 모싯잎을 첨가한 찰보리 찐빵의 관능검사 결과, 색, 향미, 맛, 촉촉함, 씹힘성, 전반적인 기호도를 알아보았다. 전반적인 기호도는 모시 찰보리 찐빵이 유의적으로 가장 높았으며, 찰보리 찐빵과 대조군(밀찐빵) 간에는 유의성이 없었다. 이상의 관능검사 결과를 보면 모싯잎을 첨가한 밀가루와 보리가루의 비율이 7:3인 찰보리 찐빵이 가장 높은 관능평가를 받은 것으로 나타났다. 결과적으로 모싯잎을 첨가한 찰보리 찐빵은 높은 항산화 활성뿐만 아니라, 품질 특성을 향상시켰고, 기호도를 증가시켰다. 본 연구는 이러한 기초자료를 바탕으로 찰보리와 모싯잎을 함께 첨가하여 사용하는 것이 식품의 품질 특성 및 기능성을 높이는데 공헌을 할 것으로 생각된다. Increased consumption of a polysaccharide, ${\beta}$-glucans, in foods may prevent health related problems such as cancer, cardiovascular diseases and diabetes. ${\beta}$-glucans is a fibrous polysaccharide having proven both functional and medicinal properties. Recently, the FDA recommends the consumption of oat or oat products containing a total of at least 3 grams of bea-glucans per day for health improvement. The content of ${\beta}$-glucans in barley was almost four times higher than that in oat. In this study, the physicochemical and sensory characteristics and biological properties of steamed barley bread added with ramie leaf powder was investigated. The study of sensory characteristics was performed using the Quantitative Descriptive Analysis (QDA). 30 panelists were selected among our university students. They then evaluated the different sensory characteristics, such as overall preference, color, flavor, chewiness, moistness and taste. The color and texture analyses were determined using a colorimeter and texture analyser, respectively. In the sensory, color and texture evaluation, barley bread with ramie leaf showed higher values than barley bread and wheat bread did. The physiological activities were investigated through the total phenol content and 1,1-diphenyl-2-picrylhydrazyl(DPPH) radical scavenging activity. Hence, barley appears to be a suitable food resource for making bread. This study suggests that barley bread added with ramie leaf can be used as one of the processing methods in promoting the consumption of barley. It might also help with the improvement of barley food industry.

        광주 지역 건강검진센터를 내원한 수진자의 인스턴트 커피믹스 섭취와 대사적 위험요인과의 연관성

        본 연구는 2015년 7월부터 2016년 6월까지의 연구기간 동안 내원한 광주선한병원의 건강검진 수진자들의 인스턴트 커피믹스 섭취량을 조사하여 대사증후군 위험인자와의 연관성을 분석하고자 하였다. 그 결과 인스턴트 커피믹스 섭취와 대사증후군의 위험인자 중 특히 허리둘레와 혈중 중성지방 농도는 양의 상관성을 보였고, 인스턴트 커피믹스 섭취빈도가 증가할수록 공복혈당과 혈중 중성지방 농도가 증가하는 유의적인 상관성을 보였다. 또한, 다중회귀분석에서도 1 cup/d 이상 인스턴트 커피믹스를 섭취하는 대상자에서 보정 여부와 상관없이 대사증후군 위험인자인 혈중 중성지방 농도가 상승하는 유의적인 연관성을 나타냈다. 이와 같은 인스턴트 커피믹스와 관련한 연구 조사와 선행 연구 결과를 통하여 볼 때 건강인은 물론 지방 섭취에 주의해야 할 대상자는 섭취 커피 종류에 따른 섭취 빈도와 섭취량 조절 교육이 중요하다. Metabolic syndrome is associated with risk of developing cardiovascular disease and type 2 diabetes, and incidence of metabolic syndrome is showing an increasing trend. Recently, coffee intake is argued to have an effect on the gut-brain axis, and it is widely recognized has being associated with many beneficial effects, including those related to metabolic imbalance. However, it is essential to understand how the dietary aspects of coffee intake and type of coffee intake affect diet and overall health. The association between coffee consumption and health is still controversial, and the average body weight of the Korean population is increasing. Therefore, this study focused on analyzing the effects of instant coffee (particularly instant coffee mix) in Korea, especially with respect to metabolic syndrome. The study was conducted on participating subject’s that visited Sun-Han hospital, and instant coffee mix intakes and metabolic risk factors during the study period were examined from July 2015 to June 2016. The results showed that waist circumference (r=0.405, P=0.036) and serum triglyceride level (r=0.435, P=0.023) were positively correlated with instant coffee mix intakes. In addition, instant coffee mix intakes were significantly associated with serum triglyceride level in subjects who consumed more than 1 cup/d of coffee (P<0.05). Therefore, subjects should be careful about fat intake and be educated about the type and frequency of coffee intakes.

        광주광역시에 거주하는 재한 중국유학생의 식생활 분석

        With the globalization of Korean education and the rise in the average level of education, the number of international students in Korea, especially those from China, is rapidly increasing every year. This study aimed to provide the basic information for healthy dietary education by evaluating the dietary and nutritional status of Chinese students living in Gwangju province of Korea. Data from 468 subjects who met the study participation criteria and who had given their written informed consent were used for analysis. Of the total subjects, 52.4 and 80.1% were non-drinkers and non-smokers, respectively (p<0.01). It was observed that women were more stressed than men when they did not have convenient access to a Chinese food market near their place of residence (p=0.032). The subjects with a high cultural adaptation consumed kimbap (p<0.001) and pizza (p=0.017) more frequently than the subjects with a low cultural adaptation. An analysis of the nutrient quotient revealed that male scores were higher than those of the females with regard to water intake (p=0.035) and exercise level (p=0.021). For Chinese students living in Korea to maintain proper dietary habits, it is necessary to educate them on how to purchase convenience foods and improve their lifestyle, including limiting drinking and smoking. It is also necessary to develop emotional support programs for stress management and cultural adaptation.

        어린이 영양지수(NQ)를 이용한 광주 지역 일부 고학년 초등학생의 영양상태 평가

        This study was conducted to evaluate the overall dietary habits and nutritional status of 294 elementary school students located in Gwangju using the NUTRILITE nutrition quotient (NQ). The NQ score of the subjects was 59.5 out of 100, which corresponds to the level requiring "monitoring". Compared to the 64.4 score of students in the 5<sup>th</sup> and 6<sup>th</sup> grades of six metropolitan cities, the scores of the present subjects were lower. The NQ components were 57.4 for balance factors, 52.4 for moderation factors, 55.0 for diversification factors, 57.6 for practice factors, and 80.1 for environmental factors. Regarding the characteristics according to the NQ level, the score (61.5 in 7 times/wk vs. 55.3 in 1-2 or none times/wk) tended to be higher in subjects who frequently eat with their families (p<0.001). In conclusion, nutrition education should be provided to students to increase the frequency of meals with their families, to develop a habit of exercising regularly, and reduce the consumption of ramen, cookies, and processed drinks that are frequently consumed. These results can be used as basic data to develop nutrition education programs to improve the dietary habits and nutritional status of students in Gwangju.

        광주 지역 건강검진센터를 내원한 수진자의 유제품 섭취와 전당뇨병 위험 인자와의 연관성

        본 연구는 2015년 7월부터 2016년 3월까지 내원한 대상자들을 공복혈당에 따라 정상군과 전당뇨병군으로 구분하고 각 대상자의 유제품 섭취량과 당뇨병 위험인자 및 전당뇨병 발병 유무와의 연관성을 분석하고자 하였다. 분석한 당뇨병 위험인자 중 특히 수축기 혈압과 이완기 혈압이 전당뇨병 발병 위험과 유제품 섭취량과 유의적인 연관성을 보였다. 또한, 유제품 섭취 수준에 따른 당뇨병 위험 인자의 특성에 있어 유의적인 차이는 없었지만 유제품 섭취량과 전당뇨병 발병 위험과 유의적인 음의 연관성을 보여주어 대규모 연구수행의 필요성을 제안하였다. 그리고 당뇨병 관리의 대상을 전당뇨병 대상자까지 확대시켜 지속적인 공복혈당 모니터링을 실시하고, 당뇨병 예방 영양중재와 유제품 식사 가이드라인을 개발하는 것이 중요하다. The prevalence of diabetes is increasing worldwide, particularly in Asian regions such as Korea. It is estimated that the number of diabetic patients will exceed 5 million by 2030 and that about 30% of people with diabetes are unaware of their condition. A comparative analysis was conducted to determine the relationship between dairy food intake and pre-diabetes risk factors in subjects that visited the health examination center of Sunhan Hospital, Republic of Korea. Subjects were classified into two groups according to fasting blood glucose concentrations: 1) normal (fasting blood glucose <100 mg/dL, n=57) and 2) pre-diabetes (100 mg/dL≤ fasting blood glucose ≤125 mg/dL, n=56). Total dairy food intake tended to be lower in females and subjects with pre-diabetes compared with males and normal subjects, respectively; however, differences between groups were not significant due to a wide range of variations. HbA1c, systolic and diastolic blood pressures were significantly associated with the prevalence of pre-diabetes. Odds ratios for pre-diabetes were significantly reduced per incremental increases in dairy food intakes after adjusting for confounding factors. Findings in the current study suggest that appropriate intake of dairy foods may need to be considered for the prevention of pre-diabetes.

        제6기 국민건강영양조사 자료를 이용한 한국 노인의 골관절염 유병 여부와 관련 위험 요인(2013~2014 KNHANES VI)

        This study was performed to determine the prevalence and associated risk factors of osteoarthritis in the elderly using the Korea National Health and Nutrition Examination Survey (2013~2014 KNHANES VI). A total of 2,148 (936 males, 1212 females) elderly participants aged 65 years or older were selected. Prevalence of osteoarthritis was higher in 460 female (80.7%) and 167 rural (29.1%) groups, than in 110 male (19.3%) and 404 urban (70.9%) groups. While the mean values of LDL-cholesterol, triglyceride level, and waist circumference were significantly higher in the osteoarthritis group than the normal group, other parameters including height, HDL-cholesterol, fasting blood glucose, hemoglobin and hematocrit values were not. Quality of life (EQ-5D) was also significantly lower in the osteoarthritis group. The results of logistic regression analysis showed that smoking, alcohol intake, obesity, hypercholesterolemia and energy intakes were significantly different in regards to the prevalence of osteoarthritis. This study suggests that obesity and energy intakes were associated with osteoarthritis, whereas risk factors of chronic disease, such as hypertension, Hypo-high density lipoprotein, hypertriglyceridemia, anemia, diabetes, vigorous physical activity, moderate physical activity and walking were not. Long-term prospective study is necessary to investigate the effects of these factors on osteoarthritis.

        B16F10 Melanoma 세포에서 삼채 뿌리 추출물의 멜라닌 생성 저해 효과

        본 연구는 AHWE와 AHEE의 멜라닌 생성 억제 효과를 확인하고자 하였다. AHWE 및 AHEE의 미백효과를 확인하기 위해 tyrosinase 저해 활성 및 B16F10 melanoma 세포를 이용하여 멜라닌 생성 저해 효과, 미백 관련 인자인 MITF, TRP-1, TRP-2의 단백질 발현 억제 효과를 western blot으로 측정하였다. 그 결과 tyrosinase 저해 활성을 확인하였고, B16F10 melanoma 세포의 멜라닌 합성을 저해하는 것을 확인하였다. 또한, 미백 관련 인자 MITF, TRP-1, TRP-2 단백질 발현 측정에서도 모두 발현이 억제됨으로써 미백효능이 있음을 확인할 수 있었다. 따라서 AHWE 및 AHEE가 tyrosinase의 활성 및 멜라닌 생성의 억제를 통해 미백과 색소침착 완화제로 응용이 가능할 것으로 판단된다. Melanin is a natural pigment produced by cells that prevent damage caused by ultraviolet radiation. The overproduction or abnormal distribution of melanin results in skin disorders and cosmetic complications. Allium hookeri (A. hookeri) is a perennial herbaceous evergreen plant having various physiological activities, including anti-inflammatory, antioxidant, and anti-hyperlipidemia. However, the effect of A. hookeri extract on melanogenesis remains unknown. In this study, we investigate the inhibitory effect of A. hookeri extract on α-MSH-stimulated melanogenesis in B16F10 cells. Effects of the extracts at the cellular level were examined by first determining the viability of treated melanoma cells using the MTT assay. Non-toxic concentrations of the A. hookeri extracts were used in subsequent experiments. To determine the whitening effect, we measured the tyrosinase-inhibitory activity of A. hookeri hot water extract (AHWE) and 80% ethanol extract (AHEE). Exposure to both AHWE and AHEE resulted in increased tyrosinase inhibitory activity. Furthermore, both AHWE- and AHEE-treated melanoma cells showed a dose-dependent decrease in melanin content. We next determined the effects of both extracts on protein expression levels of tyrosinase-related protein 1 (TRP1), TRP2, and microphthalmia-associated transcription factor (MITF). Exposure to both extracts decreased the TRP1, TRP2, and MITF in the B16F10 cell line, resulting in lower levels of melanin production compared to the α-MSH-treated B16F10 cells. Taken together, our results indicate that both AHWE and AHEE are potential candidates for preventing and inhibiting melanogenesis in B16F10 cells.
