상부위장관질환 환자에서 H . pylori 감염에 의한 위점막의 조직학적 변화와 혈청 Gastrin 및 혈청 IgG항체가의 상관관계

N/A Background/Aims: This study was performed to investigate the relationship between Helicobacter pylori infection and degree of gastric mucosal inflammation, IgG antibody titer against H. pylori, and fasting serum gastrin level. Methods: Patients were divided into 2 groups(H. pylori positive and negative) by identification of H. pylori in biopsied specimens with special staining. In H. pylori positive group, the density of gastric K. pylori colonization was further graded semiquantitatively from 1 to 3. The severity of gastritis in each group was scored according to the Sydney system from 0 to 3. Serum IgG antibody titer against H. pylori was detected by second-generation antigen based enzyme immunoassay(Cobas Core Anti-Helicobacter pylori EIA). Fasting serum gastrin level was measured by standard radioimmunoassay technique. Results: The severity of gastritis in H. pylori positive group was significantly higher than H. pylori negative group in mononuclear cell infiltration(pC0.001), activity of PMNL(pC0.001), and glandular atrophy(pC0.01). In H. pylori positive group, the density of H. pylori colonization was significantly correlated with mononuclear cell infiltration(r=0.67, p0.001), activity of PMNL(r=0.70, p0.001), and grandular atrophy (r=0.38, pC0.001). Neither density of H. pylori colonization nor severity of gastritis was correlated with fasting gastrin level and IgG antibody titer against H. pylori. Conclusions: H. pylori infection results in localized inflammatory reaction in gastric mucosa with relation to density of H. pylori colonization, but serum H. pylori IgG antibody titer does not reflect the severity of gastritis. Fasting serum gastrin level also has no relation with the density of H. pylori colonization and severity of gastritis. These findings suggest that the factor which cause inflammation in gastric mucosa may be different from that cause gastrin secretion or formation of IgG antibody. Simple camera-based system for evaluation of lower limb alignment as a part of an automated cycling fitting system was developed and verified in this study. Developed imaging system can evaluate lower limb alignment quantitatively during pedaling using a general camcorder and single marker attached on the knee. Threshold-based marker detection algorithm was proposed in this study. Experiment was carried out to compare the trajectory data from marker detection algorithm of the developed imaging system with the trajectory data from 3-D motion capture system. Results showed that average error between trajectories was 2.33 mm (0.92%) in the vertical direction and 0.62 mm (1.86 %) in the medio-lateral direction. There existed significant correlation between two measured values (r=0.9996 in the vertical direction and r=0.9975 in the medio-lateral direction). It can be concluded that developed imaging system be applied to evaluate lower limb alignment which is an important factor for dynamic bicycle fitting.
류마티스 관절염 환자에서 Phospholipase A2 활성도

목적: Phospholipase A2(PLA2)는 세포막 구성성분인 인지질로부터 각종 eicosanoid의 전구물질인 arachidonic acid 생성에 관여하는 속도조절효소(rate-limiting enzyme)로서, 저자들은 류마티스 관절염 환자에서 PLA2의 활성도를 측정하고 이를 질병활동도와 비교검토함으로써 그 의의를 조사하였다. 방법: 대상환자군은 1987년 미국류마티스학회가 제정한 류마티스 관절염의 진단기준에 부합되는 총 25명의 환자를 대상으로 하여 그들의 혈청과 슬관절에서 뽑은 관절액내 PLA2 활성도를 측정하였다. 정상대조군은 병력이나 직장신체검사상 특별한 이상이 없는 병원직원 14명을 대상으로 하였다. 결과: 1. 류마티스 관절염 환자군에서 혈청 PLA2의 활성도는 정상대조군에 비하여 유의하게 증가되어 있었다(p<0.001). 2. 류마티스 관절염 환자에서 혈청 PLA2 활성도는 관절액에서 측정한 PLA2와 유의한 상관관계를 보였다(p<0.05). 3. 치료 3개월 후 질병활동도(임상 및 검사실 소견)의 유의한 호전(p<0.001)과 함께 혈청 PLA2 활성도는 유의하게 감소하였다(p<0.001). 4. 혈청 PLA2의 유의한 감소는 임상적 호전이 있는 군에서만 관찰되었다. 또한 혈청 PLA2 활성도는 질병활동도와 관련이 있는 각종 지표들 중, ESR과 가장 좋은 상관관계를 보였다(r=0.493, p<0.05). 결론: 류마티스 관절염 환자군에서 혈청 PLA2의 활성도는 정상 대조군과 비교하여 증가되어 있었으며 이는 관절액에서 측정한 PLA2 활성도와 유의한 상관관계를 보였고, 치료 후에는 임상증상의 호전과 함께 감소하였다. 이상의 소견으로 PLA2는 이질환의 병태생리에 관여하며 질병의 활동도를 반영 할 수 있는 지표의 하나로 생각된다. Objectives: A limited retrospective study of patients with rheumatoid arthritis(RA) found that serum phospholipase A2(PLA2) activity correlates with disease activity. To assess the strength of this relationship we investigated prospectively 25 patients with RA using a double blind approach. Methods: Twenty five patients who fulfilled the 1987 American College of Rheumatology criteria for RA had clinical and laboratory assessments. PLA2 activity was measured before and after treatment of 3 months in patients with RA. Fourteen healthy individuals were also enrolled as controls. PLA2 activity was assayed using E. coli membrane phospholipid substrate labelled with [14C]-oleic acid. Results: 1) Serum PLA2 activity was significantly higher in patients with RA than that of normal healthy controls(p<0.001). 2) In patients with RA, synovial fluid PLA2 activity was higher than serum PLA2 activity, and a positive correlation between PLA2 in synovial fluids and matched sera was found in these patients(p<0.05). 3) After treatment, PLA2 activity was significantly decreased with improvement of clinical(morning stiffness and Ritchie index) and laboratory(ESR, CRP and rheumatoid factor) assessments(p<0.001). 4) Among the clinical and laboratory markers of disease activity, ESR showed the best correlation with serum PLA2 activity(r=0.493, p<0.05). 5) In the patients who did not respond clinically to treatment (n=5), there was no significant decrease in PLA2 activity. Conclusion: PLA2 activity significantly correlates with RA activity and may serve as an index of disease activity in RA.

Smart Thermal Energy Design platform (STED platform) has been developed for designing efficient thermal energy-intense industrial facilities. STED platform has Level 1 and 2 design schemes. Level 1 design scheme can approximately assess the performance of industrial processes, which consist of various thermal-related machines by modeling them with simple equations or correlations. Level 2 design scheme can design individual machines based on the calculated thermodynamic states from Level 1 design scheme with detailed equations and iterative calculations. Among Level 2 design modules in STED platform, the heat pump design algorithm will be discussed in this paper. The heat pump design module can treat 12 types of layouts. To validate the results from the heat pump design module, a heat pump simulator was manufactured, and a preliminary operation test of the simulator was completed.

분자선증착기 (Molecular beam epitaxy. MBE)를 이용하여 InP (001) 기판에 자발형성 (Self-assembled) InAs/InAlAs, InAs/InAlGaAs 양자점 (quantum dots, QDs)을 형성하고 구조 및 광학적 특성을 원자력간현미경(Atomic force microscopy, AFM), 투과전자현미경 (Transmission electron microscopy, TEM), 상온 포토루미네슨스 (Photoluminescence, PL) 실험을 통하여 분석하였다. AFM 측정을 통해 표면 형태를 분석한 결과 InAs 양자구조는 기저물질의 표면상태에 따라 양자대쉬, 비대칭적인 형태를 갖는 양자점, 대칭적인 형태를 갖는 양자점과 같이 다양하게 성장되었다. InAlGaAs 물질을 장벽층으로 하는 InAs 양자점의 평균크기는 폭이 대략 23 nm, 높이가 약 2 nm 이었다. 성장조건을 다양하게 변화시켜 광통신시스템에 중요한 파장중의 하나인 $1.55{\mu}m$ 발광파장을 갖는 InAs 양자점을 형성하였다. Self-assembled InAs/InAl(Ga)As quantum dots (QDs) were grown on InP substrates by a molecular-beam epiaxy, and their structural and optical properties were investigated by atomic force microscopy (AFM), transmission electron microscopy (TEM), and room-temperature photoluminescence (PL). AFM images indicated that the InAs quantum structures showed various shapes such as quantum dashes, asymmetric and symmetric QDs mainly caused by the initial surface conditions of InAl(Ga)As with the intrinsic phase separation. For the buried InAs QDs in an InAlGaAs matrix, the average lateral size and height of QDs were 23 and 2 nm, respectively. By changing the growth conditions for the QD samples, the emission wavelength of $1.55{\mu}m$ was obtained, which is one of the wavelength windows for fiber optic communications.

연소기, 공업로, 건조기, 히트펌프 및 열교환기와 같은 공정 산업 기기들을 기술 수요자가 자유롭게 구성하고, 그에 따른 공정해석 결과를 바탕으로 에너지효율 향상을 위한 스마트 설계 플랫폼이 현재 한국에너지기술연구원에서 개발하고 있다. 스마트 플랫폼의 계산 단계는 크게 Level 1과 Level 2로 나뉘며, Level 1은 다양한 설비들이 연계돼있는 공정을 신속하고 강건하게 해석하는 단계이고, Level 2는 Level 1에서 계산된 공정 정보에 기반하여 각각의 설비들을 상세히 설계하는 단계이다. 일반적으로 히트펌프를 설계할 때에는 냉매, 과열도, 과냉도 등을 포함하여 다양한 설계 변수가 있으며 설계 알고리즘 또한 많은 반복계산이 포함돼있기 때문에 기존의 설계 방법론을 스마트 플랫폼 Level 1에 적용하는 것은 적절치 않다. 따라서 이번 연구에서는 수백 개의 히트펌프 Level 2 설계 결과를 역카르노 COP식으로 곡선 맞춤하여 냉매의 선택, 과열도, 과냉도와 같은 히트펌프 설계변수 없이 공정 입력온도만으로 합리적인 COP를 도출할 수 있는 Level 1 히트펌프 설계 방법론을 제시하고자 한다.
