        영아교사 상호작용역량 증진을 위한 예비교사교육 프로그램 모형 개발

        본 연구의 목적은 예비교사의 영아-교사 상호작용역량 증진을 위한 예비교사교육 프로그램을 개발하는 것이다. 이를 위해 영아-교사 상호작용 관련 국내외 선행 연구 및 상호작용관련 교사 지도서 등의 문헌고찰과 요구 조사를 기초로 프로그램안을 구성하고 이를 유아교육전문가 및 유아교육현장 교사를 대상으로 심층면담을 진행하여 프로그램 초안을 구성하였다. 개발된 예비교사교육 프로그램은 2년제 대학 예비교사 62명에게 180분씩 13회기 동안 실시되었으며, 교육 참여자인 예비교사가 작성한 포트폴리오 및 최종 질문지를 통해 수정·보완되었다. 최종 개발된 영아-교사 상호작용역량 증진을 위한 예비교사교육 프로그램의 목적은 영아-교사 상호작용역량인 영아발달지식, 하루일과에 상호작용 기술적용, 영아 중심적 민감하고 반응적인 태도 증진이다. 교육내용은 영아-교사 상호작용 원리, 영아의 학습과 정서를 지원하는 상호작용, 영아반 하루일과에 대한 이해로 구성되었다. 영아-교사 상호작용역량 증진을 위한 예비교사 교육내용으로 영아의 학습지원을 위한 상호작용을 포함하고 예비교사의 특성을 고려한 실제 영아-교사 상호작용 동영상 개발 및 제시, 현장 실습, 현장 동영상 분석, 마이크로티칭을 통한 모의수업 분석이 포함되었다. The purpose of this study is to develop a pre-service teacher education program to enhance the infant-teacher interaction capacity of pre-service teachers. For this study, a draft was made up based on the review of literature and data, and the program was drafted through in-depth interviews with early childhood education specialists and early childhood education teachers. The developed pre-service teacher education program was conducted for 13 sessions of 180 minutes for 62 two-year college pre-service teachers, and was revised and supplemented through the portfolio and final questionnaire completed by the pre-service teacher. The final purpose of the program was to help pre-service teachers gain knowledge of child development, develop interaction skills, and promote sensitive and responsive attitudes toward infants. The course content consisted of the principle of infant-teacher interactions, supports for infants’ learning and emotions, and understanding of daily routines. In order to help pre-service teachers promote infant-teacher interaction competency the program included students’ practicum, video taping of infant-teacher interaction and analyses of the videos, lesson demonstrations and analyses of the microteaching.

        방광파열의 진단에서 전산화단층촬영술의 의의

        Background: Computerized tomography(CT) is the method of choice for establishing patients with abdominal and/or pelvic trauma. However, the sensitivity of CT for detecting bladder rupture has been questioned. We investigated the roles of CT as the initial evaluation of abdominal and pelvic trauma in diagnosis of bladder rupture. Methods: We reviewed the medical records and radiographs of 53 patients with bladder rupture for last 5 years. And among them, all radiographs of 30 patients who underwent both CT and retrograde cystography were evaluated independently by two urologists who had no knowledge of the patients' final diagnosis. Results: Among 30 patients, all of patients were correctly diagnosed by retrograde cystography, but the CT diagnosis was correct in only 23 patients(76.7%), who were 17 patients(85.0%) with intraperitoneal rupture and 6 patients(60%) with extraperitoneal rupture. And of the 7 patients who were negative by the CT, all showed the sign of inadequate bladder distension. Conclusion: We suggest that CT, if properly performed with adequate bladder filling, is as sensitive for detection of bladder injuries as conventional cystography. Especially, in trauma patients with hematuria and suspected other organ injury, CT-cystography with retrograde filling may be as accurate as conventional cystography and obviate the need for an additional plain film cystography.

      • 인삼 사포닌이 하부요로와 음경해면체 평활근의 이완작용에 미치는 효과

        이상의 결과를 종합하면, 인삼 총사포닌은 본 실험의 조건에서 배뇨개선 효과가 있으며 이에 관여하는 총사포닌의 주 효능은 근위부요도를 이환시킴으로써 이루어지는 것으로 생각된다. 그리고 인삼 총사포닌은 혈류역동학적으로 안전하게 음경해면체 평활근을 이완시켜 음경발기를 유발시키는 것으로 생각된다. Background: Korean ginseng (KG) has been used as a general tonic, and for voiding dysfunction for a long time in oriental society. However, scientific basic studies on the use of KG, have been rare, especially for voiding and erectile dysfunction. This study was performed to investigate the effects of KG on voiding and erectile function by examining the effects of total saponin (TS) on the bladder, urethral and penile cavernosal smooth muscle. Materials and methods: To examine the effects of TS, NewZeland white rabbits were used to obtain tissue strips from the smooth muscle of the bladder, proximal urethra and corpus cavernosum. Adult Spargue Dawley rats were used to examine the changes in urodynamic findings and penile erection after administration of TS. Results: In proximal urethral strips, the rate of relaxation of the proximal urethra was increased from 9.0±2.9 to 33.7±4.8% in a dose-dependent manner when the concentration of TS was added accumulatively from 0.25 mg/ml to 4.0 ng/ml (p<0.05). However, no significant response was observed in the bladder strips within these concentration ranges. For the corpus cavernosal strips, the rate of relaxation ranged from 5.8±2.1 to 36.7±5.8%, increasing in a dose-dependent manner when TS was increased from 1.0 mg/ml to 4.0 mg/ml (p<0.05). After administration of 0.1 ml of TS (32 mg/ml) in the rat, the bladder pressure was 37.5±8.5 mmHg at 52.1±7.0 sec. during isovolumetric bladder contraction, showing no significant differences from 35.7±7.8 mmHg and 50.7±7.2 sec, respectively, before treatment. However, when 0.1 ml of TS (32 mg/ml) was administered, the relative reduction of urethral pressure was 6.9±0.5 mmHg at 62±7.5 sec, which was significantly higher compared to 4.6±1.1 mmHg at 45±10 sec before treatment (p<0.05). For the cavernosal injection study, the change in intracavernosal pressure (△ICP) was examined after administering 0.1 ml of TS. The cumulative additions of TS at concentrations from 0.5 mg/ml to 32 mg/ml increased △ICP from 1.3±0.5 to 21.3±7.8 mmHg in a dose-dependent manner (p<0.05). The duration of tumescence was from 0.3±0.1 to 5.2±0.2 min, showing dose-dependent increase (p<0.05). Furthermore, the cumulative addition of TS at concentrations from 0.5 mg/ml upto 32 mg/ml did not cause any significant change in systemic blood pressure. Conclusion: These results suggest that ginseng improves voiding functions, which is mainly achieved by TS relaxing the proximal urethra, the most important part of the bladder outlet function. In addition, ginseng safely induced a penile erection hemodynamically by relaxing the corpus cavernosum.

        대동맥내 기낭 펌프의 혈역학적 효과에 관한 연구

        Intra-aortic balloon pump (IABP) has been widely used since 1968 when Kantrowitz reported its design and clinical application for the patient with cardiogenic shock. However, information regarding clinical effectiveness of this device in terms of hemodynamic status had been limited. Consequently, using a simple random sampling method we reto\rospectively selected 23 (65±9.7 yr, mean±S.D.) ort of 223 cases in which IABP had been inserted for a period of 13 months (from Spetember, 1983 through October, 1987) in Yale-New Haven Hospital and evaluated their hemodynamic parameters. Six of 23 cases had IABP inserted in the coronary care units (CCU) and 17 had undergone cardiopulmonary bypass (CPB) for coronary artery bypass graft (CABG) except one case for aneurysmectomy and VSD repair. Nine of these 17 had IABP inserted for improvement of their preoperative cardiovascular functions and eight of them for successful weaning from CPB following CABG. While the diastolic pressure, cardiac output and cardiac index (CI) had increased from 62±11.0 mmHg, 4.4±1.40 l/min, and 2.5±0.80 l/min/m^2 to 69±10.8, 5.1±1.34, and 2.8±0.83, respectively, pulmonary arterial diastolic pressure (PAD) and pulmonary capillary wedge pressure (PCWP) decreased from 22±9.7 and 19±4.9 to 20±4.5 and 16±5.9 mmHg. respectively, after IABP counterpulsation. Despite their poor preoperative hemodynamic function (ASA Class IV or V), 18 out of these 23(78.3%) had survived during hospitalization. Under the condition of our study IABP appears to be an alternative beneficial therapeutic aid for the patient with cardiogenic shock.
