        전신성 홍반성 루푸스에서 병발한 시신경척수염 1예

        Purpose: We present a case of a patient with coexisting neuromyelitis optica and systemic lupus erythematosus (SLE). Case summary: A 26-year-old female was hospitalized in our medical center due to decreased visual acuity in her left eye; she had a history of gastric ulcers and herpes zoster infection. Steroid treatment was started under suspicion of optic neuritis, and she was diagnosed with SLE. After treatment, her vision improved, but eleven months later she was hospitalized with paresthesia on the abdomen and left flank progressing to the lower extremities. Spinal MRI showed transverse myelitis, suggesting multiple sclerosis. Fifteen months later, the patient was hospitalized due to decreased visual acuity and ocular pain in the right eye. Her vision was improved by steroid therapy. However, optic neuritis recurred in the right eye after five weeks, thus azathioprine was added to the treatment. Anti-aquaporin-4 Ab test was conducted based on the suspicion of neuromyelitis optica, and the serum was positive for anti-aquaporin-4 Ab (NMO-IgG). The patient was hospitalized again due to paraplegia after three months. Coexistence of neuromyelitis optica was verified because spinal MRI showed longitudinally extensive transverse myelitis. The symptoms were improved by high doses of steroids, a series of plasmaphereses, and rituximab. Optic neuritis was repeated in the right eye and the symptoms were improved with high doses of steroids. Myelitis recurred later and the symptoms improved with high doses of steroids and a series of plasmaphereses. Conclusions: Coexisting neuromyelitis optica should be considered in cases with relapsing events which have transverse myelitis without cranial lesions in autoimmune diseases such as SLE.

        코경유 눈물주머니코안연결술에서 폴리에테르 에스테르 우레탄의 임상적 효과

        Purpose: To evaluate the clinical effect of intraoperative application of polyether ester urethane (Nasopore?) in endonasal dacryocystorhinostomy. Methods: Endonasal dacryocystorhinostomy was performed on 77 eyes diagnosed with nasolacrimal duct obstruction between January 2010 and February 2011. Nasopore? was applied to the newly-formed internal lacrimal opening in the experimental group (41 eyes), and vaseline gauze was applied in the control group (36 eyes). For the evaluation of clinical effects regarding postoperative adhesion and stenosis of the internal lacrimal opening, subjective symptom improvement, lacrimal irrigation tests, and endoscopic examinations were performed at 1 week, 2 weeks, 1 month, 3 months, and 6 months, postoperatively. Results: In subjective symptom improvement and lacrimal irrigation tests, differences between the 2 groups were not statistically significant. In endoscopic findings, the Nasopore? group showed significantly lower severity of adhesion and stenosis of the internal lacrimal opening at 1 week, 2 weeks, and 1 month after operation (p < 0.05). Conclusions: Application of the Nasopore? may effectively reduce initial postoperative adhesion or stenosis of the internal lacrimal opening after endonasal dacryocystorhinostomy and has usefulness as an auxiliary method.         레이저 굴절교정 수술을 위한 웨이브프론트 맵의 선택에서 구면수차의 고려

        Purpose: To report the dynamic nature of human optical aberrations in the scotopic condition. Methods: A total of 20 eyes who were candidates for laser vision correction were included in the present study. Repeated wavefront data were obtained using WavescanTM (AMO/VISX). From the wavefront analysis data, the sphere, astigmatism, average pupil size, spherical aberration, coma and trefoil were selected and used to investigate any correlation among the parameters. Results: The sphere, spherical aberration, coma and pupil size showed a dynamic change in the scotopic condition. The spherical aberration and pupil size decreased by the amount of 0.10 ± 0.04 μm and 0.55 ± 0.37 mm as the sphere changed 1 D in myopic direction. There was significant positive correlation between the sphere and spherical aberration in 13 eyes of 9 patients (65%), between the sphere and pupil size in 5 eyes of 4 patients (25%), and between the sphere and coma in 3 eyes of 3 patients (15%). The spherical aberration decreased significantly in 4 eyes of 4 patients (20%) as the pupil size decreased. Conclusions: The optical aberration of human eyes showed a dynamic nature in the scotopic condition. In particular, there was significant correlation between the sphere and spherical aberration. The observed correlations have the potential to be used as helpful indicators to select the optimal wavefront data for the laser vision correction. J Korean Ophthalmol Soc 2011;52(2):147-156

        Purpose: Heat therapy by heat lamp after microvascular surgery is being used for preventing blood vessels's contraction and blood-flow's disturbance. As usually, incandescent lamp has been used. But there have been several problems and need for improvement in the existing heat lamp treatment. So we would like to introduce improved heat lamp to keep an appropriate temperature and intensity of illumination. Methods: The existing heat lamps are the ones of general light stands covered with newspaper, having 60 watt light bulb of incandescence and lampshade made of aluminum. We have tried to improve shortcomings of the existing heat lamps by enlarging the size of aluminum lampshade and attaching a curtain that can block heat and light. We conducted a comparative study between the existing and improved heat lamps. Under the assumption that there are several affected parts, we have also measured the distance from heat lamp to patients' eye region and then intensity of illumination. Result: The target temperature of surface was realized in 11 minutes with the maximum temperature reaching at 36.6 degrees C in 28 minutes at the existing heat lamp while the target temperature reached in 7 minutes with the maximum temperature reaching at 39.0 degrees C in 17 minutes at the improved heat lamp. The existing and improved heat lamp showed 38 lx and 0.1 lx of intensity of lumination, respectively. Conclusion: Using improved heat lamps, we can keep an appropriate temperature and we think we can make contribution to patients' treatment by making them and their neighbors able to sleep with minimized disturbance thanks to low intensity of illumination secured by blocking light.

        세로토닌 5-HT<SUB>1A</SUB>수용체 방사성 추적자 <SUP>18</SUP>F-Mefway의 합성과 소동물 뇌 PET 연구

        세로토닌 수용체 5-HT<SUB>1A</SUB>에 대한 방사성추적자로 알려진 <SUP>18</SUP>F-mefway를 개선된 방법으로 합성하고, 소동물 뇌 microPET 연구를 통해서 이 수용체에 선택적으로 결합하는 지를 확인하려고 한다. 기존 합성 방법을 수정하여 전구체를 합성하고, [<SUP>18</SUP>F]?과 130℃에서 30분간 교반하여 <SUP>18</SUP>F-mefway를 합성하여, Solid phase extraction(SPE)과 HPLC를 이용해 정제한 다음, 소동물 뇌의 microPET 다이나믹 영상을 얻었다. Oxalyl chloride와 LAH/diethyl ether을 사용해서 기존 합성수율 9%에서 34%로 <SUP>18</SUP>F-mefway 전구체를 향상된 수율로 합성할 수 있는 개선된 방법을 확립하였다. 이렇게 합성된 <SUP>18</SUP>F-mefway는 세로토닌 5-HT<SUB>1A</SUB>수용체 영상용 방사성 의약품으로서 신경정신질환 연구에 이용될 수 있을 것으로 사료된다. <SUP>18</SUP>F-mefway has been developed as radioligand for serotonin receptor 5-HT<SUB>1A</SUB>. The object of this study was to obtain the mefway precursor with the higher yield than previous method and to identify whether 18F-mefway can bind to 5-HT<SUB>1A</SUB> or not. from microPET imaging of small animal brain. Precursor was prepared by a modification of the reported procedure then [<SUP>18</SUP>F] labeling was performed by adding 18F ion at 130℃ in the hot cell for 30min. After purification of reaction mixture using alumina Sep-pak and HPLC, microPET images of small animal brain were determined. The chemical yield of precursor was increased from 9% to 34% using oxalyl chloride and LAH/diethylether. We synthesized a precursor which was successfully labeled with no-carrier-added <SUP>18</SUP>F-by new synthetic route. This research suggest that <SUP>18</SUP>F-mefway will be used a radiopharmaceutical for evaluation of central nerve system disorder as imaging a gent for 5-HT<SUB>1A</SUB> receptor.

        Although the adrenal gland is a common site of metastasis from hepatocellular carcinoma (HCC), adrenal metastases are rarely seen in clinical practice because of its lower metastatic potential compared to the other malignancies. Adrenal metastases usually were detected at the time of diagnosis of primary HCC or simultaneously with intrahepatic recurrence after curative management of HCC. It is very rare that only metastatic HCC is detected without evidence of intrahepatic recurrence. Hereby, we report two cases of adrenal metastasis from HCC without intrahepatic recurrence after hepatic resection. (Korean J Gastroenterol 2012;59:308-312)

Through the short-circuit tests for the power distribution system with the different application locations of the distribution generations, the application effects of SFCL on the operations of the protective devices as well as the reduction of the fault current were analyzed.

        치과구내용 X선발생기의 조사통 직경 변화에 따른 선량변화

        진단용방사선발생장치의 안전관리법에 의하면 치과 구내촬영용 X선발생기의 경우 조사통의 조사야 크기는 촬영면에서의 최대지름이 7 ㎝이하가 되게 기준 되어있다. 하지만 실제 두개부에 붙여서 촬영하는 근접촬영이기 때문에 X선 조사면적을 벗어날 염려가 없으며, 구내촬영에 사용되는 필름 또는 디지털 디텍터의 크기도 3×4 ㎠의 크기이기 때문에 훨씬 큰 편이며, 근거리 촬영에 따른 환자의 피부선량도 무시할 수 가 없다. 이에 조사통의 지름을 0.5 ㎝간격으로 줄이며, 환자선량, 해상력 및 화질의 영향을 알아본 결과 조사통 직경이 5 ㎝가 되어도 사용에 따른 큰 불편이 없으면서 환자선량 감소 및 사진의 대조도 증가 효과를 볼 수 있어 정책적으로 검토해볼 사항이라는 연구 결과를 얻었다 In case of the Dental X-ray apparatus, the diameter (or the field size) of the tip of the cone should be less than 7 ㎝ according to the Diagnosis Radiation Equipment Safety Management. However, deviation from the field is not expected to be big as photography is made at close range from the skin. Also, as the size of film or digital detector used in intra-oral photography is 3×4 ㎠, the size mentioned above can be considered to be much bigger. Furthermore, the patient dose by short-distance photography can not be ignored. Therefore, effect on the patient dose, resolution and image qualty was examined by reducing the cone diameter by 0.5 ㎝ interval. The result showed that the patient dose was reduced and a partial improvement in picture contrast was observed. Therefore, it can be concluded from these results that further investigation may be worthwhile in terms of policy.

        New Four-herb Formula Ameliorates Memory Impairments via Neuroprotective Effects on Hippocampal Cells

        본 연구는 동의보감을 근거로 선별된 백수오, 지황, 원지 및 석창포로 구성된 복합추출물의 해마신경세포에 대한 보호 및 기억력의 개선효과를 살펴보았다. 복합추출물의 신경보호효과를 검증하기 위해 HT22해마신경세포의 생존율, 세포독성 및 활성산소를 분석하였으며, 허혈성 뇌손상 마우스모델을 이용하여 기억에 대한 동물행동학적 변화와 단백질 발현을 측정하였다. 해마신경세포에서 복합추출물의 전 처리는 glutamate에 의해 유도된 활성산소의 축적을 억제하였으며 세포사멸을 감소시켰다. 허혈성 뇌손상 마우스모델에서 복합추출물은 동물행동학적으로 공간 및 단기 기억능력을 개선시켰다. 뇌허혈로 인해 증가된 p38 MAPK의 인산화는 복합추출물에 의해 현저히 감소하는 반면, 감소된 PI3K와 CREB의 인산화는 현저히 증가하였다. 이를 면역조직화학분석을 통해 복합추출물을 투여한 그룹이 해마에서 발현되는 CREB의 인산화가 현저히 증가되는 것을 확인하였다. 이상의 결과는 복합추출물이 CREB 단백질과 관련된 신경보호 신호기전을 조절함으로써 인지기능을 개선시키는 것으로 사료된다. The current study was conducted to evaluate beneficial effects of a new formula (CWC-9) using four traditional Oriental medicinal herbs, Cynanchum wilfordii, Rehmannia glutinosa, Polygala tenuifolia, and Acorus gramineus, on hippocampal cells and memory function. To examine the neuroprotective effects of a new four-herb extract, cell viability, cytotoxicity, and reactive oxygen species (ROS) assays were performed in HT22 cells and behavioral tests (Morris water maze and passive avoidance retention), Western blot, and immunohistochemistry were performed in a mouse model of focal cerebral ischemia. In HT22 hippocampal cells, pretreatment with CWC-9 resulted in significantly reduced glutamate-induced cell death with suppression of ROS accumulation caused by glutamate. In a mouse model of focal cerebral ischemia, we observed significant improvement of spatial and short-term memory function by treatment with CWC-9. Phosphorylated p38 mitogen-activated protein kinases (MAPK) in hippocampus of ischemic mice were decreased by treatment with CWC-9, but phosphorylated phosphatidylinositol- 3 kinase (PI3K) and cAMP response element binding protein (CREB) were significantly enhanced. By immunohistochemical analysis, we confirmed higher expression of phosphorylation of CREB in the hippocampal neurons of CWC-9 treated mice. These results suggest that new multi-herb formula CWC-9 mainly exerted beneficial effects on cognitive function through regulation of neuroprotective signaling pathways associated with CREB.

        신장 ; 복막 투석 환자에서 복막염의 원인균 동정에 이용되는 두 가지 배양방법의 비교

        목적: 복막염은 복막투석환자의 사망률과 유병률에 주요한 합병증으로 알려져 복막염의 원인균을 빨리 동정하는 것은 복막염의 치료에 있어 매우 중요한 일이다. 저자들은 복막염으로 내원한 환자들을 대상으로 복막액에 대해 재래적배양 방법과 혈액배양 방법을 모두 시행하여 원인균 동정양성률과 원인균 동정에 걸리는 시간을 측정하여 효율성을 비교하고자 하였다. 방법: 2007년 9월부터 2010년 2월까지 한림대학교 강동성심병원에서 복막 투석을 시행 받은 34명의 환자를 대상으로 총 38건의 복막염 발생 건수를 의무기록을 후향적으로 조사하였다. 모든 환자에게서 복막액을 재래적 배양 방법과 혈액배양 방법 모두 시행하였다. 재래적 배양 방법으로, 50 mL의 복막액을 3,000 g에서 15분간 원침하여 침사액을 혈액한천배지, MacConkey 배지 및 thioglycollate 액체배지에 배양하였다. 다른 방법인 혈액배양 방법으로, 10 mL의 복막액을 산소성과 무산소성 혈액배양용 배지(BACTEC)에 배양하여 원인균을 각각 동정하였다. 각각의 배양 방법으로 원인균 동정률과 원인균 동정에 걸리는 시간을 비교하여 효율성을 평가 하였다. 결과: 복막염의 원인균 동정 양성률은 혈액배양 방법이 재래적 배양 방법보다 더 높게 측정되었으며(86.8% vs. 57.9%, p=0.003), 원인균 동정 음성률은 혈액배양 방법이 재래적배양 방법보다 낮게 측정되었다(13.2% vs. 42.1%, p=0.003). 원인균이 동정된 그룹에서 균이 동정되는 시간은 혈액배양방법에서 재래적 배양 방법보다 짧게 측정되었다(90시간 vs. 109시간, p=0.03). 결론: 복막투석 환자에서 복막염의 원인균 동정에 있어혈액배양 방법이 재래적 배양 방법에 비해 원인균 동정 양성률과 동정된 시간을 비교한 결과 더 우위에 있어 효율적 이라고 볼 수 있겠다. Background/Aims: Peritonitis is the most frequent complication of continuous ambulatory peritoneal dialysis (CAPD). Prompt recognition and treatment of peritonitis is important. The purpose of this study was to compare the effectiveness of isolation of the microorganisms causing CAPD peritonitis by the BACTEC blood culture and conventional methods. Methods: We retrospectively reviewed 38 episodes of peritonitis in 34 CAPD patients between September 2007 and February 2010. Two methods of processing dialysate from patients on CAPD were used. Blood culture was performed using two 10-mL effluents, which were inoculated into a pair of BACTEC culture bottles. The conventional method was performed using 50 mL of centrifuged dialysate. The sedimented dialysate was inoculated onto blood agar and MacConkey agar plates or into thioglycollate broth. To evaluate effectiveness, we compared the rate of positive culture results and the time to identify the causative organism of the two culture methods. Results: Use of the BACTEC bottle method resulted in more positive culture results than did conventional culture (86.8 vs. 57.9% p=0.003). The time taken to identify the causative organism from culture-positive peritonitis was more rapid using the blood culture compared with the conventional culture method (90 vs. 109 hr, p=0.03). Conclusions: Blood culture using the BACTEC bottle is more effective than the conventional culture technique for detection of causative microorganisms in CAPD peritonitis.
