        정상인들의 Scapular Rhythm에 대한 연구

        This study was carried out to investigate the scapular rhythm of normal persons. 16 persons was no disease, injury and after-effect in period for July 1, 1996 to July 14, 1996. The statistical measures were performed by SPSS/PC t-test for classification. The result of this study were as follow : 1. There was a significant difference between the body median line and scapular superior angle from the mean distance in 83.4 mm of male and 86.0 mm of female to shoulder neutral position(p<0.05). 2. The mean distance of body median line between scapular inferior angle was 97.9 mm of male and 92.0 mm of female to shoulder neutral position. 3. There was a significant difference between the body median line and scapular inferior angle from the mean distance with male and female to shoulder abduction $90^{\circ}$ position(p<0.05). 4. The mean angle of body median line between scapular angle was $6.4^{\circ}$ of male and $4.4^{\circ}$ of female with shoulder neutral position. 5. The mean ratio of scapular rhythm was 5.6 : 1 in shoulder abduction of $90^{\circ}$ and 5.1 : 1 in shoulder abduction of 180.

        팔걸이 형태에 따른 편마비 환자의 보행 변수 및 비마비측 견갑대의 근활성도 변화

        The purpose of this study was to investigate the variations in gait pararreters in tenns of the type of ann sling used in hemiplegic patients. Ten patients with hemiplegia and ten healthy adults participated in this study and walked at self-selected speeds on a GAITRite-instn.urented carpet. The activities of the opposite shoulder girdle muscle including the latissimus dorsi, anterior deltoid, and posterior deltoid were sinmltaneously ra:orded using surface ElVIG during gait. They were randomly assigned a condition: without an ann sling, a single strap ann sling, a Harris hemi ann sling, a Rolyan hurreral cuff ann sling, and a Bobath roll ann sling. The following gait variables were analyzed: the temporo-spatial pararreters of velocity, step length, stride length, swing phase, stance phase, single SupjXlrt, step titre and toe in/toe out. The statistical analysis was one-way ANOVA with rer;eated rreasures to compare the variation of each variable. In comparison of pararreters in each trial in the hemiplegia group, the non-affa:ted side stride length, single support, and toe inltoe out resulted in statistically significantly changes (p<'C5). But without an ann sling group did not show any gait pararreter differences with ann slings. Ths study found that several ann slings varied gait patterns in patients with hemiplegia and in healthy adults. In the ElVIG analysis, the Rolyan hurreral cuff ann sling and the Bobath roll ann sling were higher muscle activity for the latissimus dorsi muscle than did the single strap ann sling. Further study should examine the problems that appeared in patients who worn ann slings by focusing on a larger number of subja: ts and by studying the variety of responses in rmre detail using an assessrrent tool that rreasures variation

        클램쉘 운동 시 골반 회전 조절 방법이 중간볼기근의 활성도와 골반 회전량에 미치는 영향

        Background Clamshell exercise is commonly used as a rehabilitation strengthening exercise for the gluteus medius muscle, which plays a large role in human walking, weight-bearing activities, and pelvic stabilization. Compensatory movements such as excessive horizontal rotation of the pelvis may occur during clamshell exercises. Various studies on pelvic control have been conducted, but studies on how to control pelvic rotation using tools are lacking. Objectives The purpose of this study was to investigate the effects on gluteus medius activity and pel-vic rotation when adjusting pelvic rotation using a smartphone-based measurement tool during clam-shell exercise. Methods This study recruited 27 healthy adult females. During clamshell exercise, gluteus medius muscle activity and pelvic rotation were investigated with and without the use of a smartphone-based measurement tool. Gluteus medius activity was measured with 4D-MT 2EM, and pelvic rotation was analyzed using the Kinovea® program. Statistical analysis was performed using t-test. Results The activity of the gluteus medius muscle significantly increased and the amount of pelvic rota-tion significantly decreased in the clamshell exercise with the use of the smartphone-based measure-ment tool compared to the clamshell exercise using the pelvic rotation control method without the use of a smartphone-based measurement tool (p < 0.001). Conclusions The use of a smartphone-based measurement tool during clamshell exercise can help the efficient activation of the gluteus medius muscle by minimizing compensatory actions such as un-wanted rotation of the pelvis.

        불안정 지지면에서 정적 서기 동안 구두 굽 높이의 변화에 따른 자세 조절 전략

        The purpose of this study was to evaluate the changes in the electromyographic (EMG) activity of the trunk and the lower limb muscles during quiet standing on an unstable surface while wearing low-heeled shoes (3 ㎝), high-heeled shoes (7 ㎝) and without footwear (0 ㎝) in 20 young healthy women. The subjects stood on an unstable surface for 30 seconds. We examined the differences in the EMG data of the erector spinae, rectus abdominis, biceps femoris, rectus femoris, tibialis anterior, and the gastrocnemius medialis muscle. A one-way repeated analysis of variance was used to compare the effects of shoe heel height on the EMG activity with the level of significance set at α=.05. The EMG activity of the erector spinae and the rectus femoris were significantly increased (p<.05) in the subjects who wore elevated heel height, while the tibialis anterior and the gastrocnemius medialis were significantly decreased (p<.05). However, the rectus abdominis and the biceps femoris exhibited no significant difference among the three conditions. The above results indicate that wearing high-heeled shoes may change the postural strategy. The findings of this study suggest that excessive heel height could contribute to an increased fall risk during quiet standing.

        백팩 무게가 일부 대학생의 머리척추각과 목 근활성도에 미치는 영향

        Background: A backpack is available equipment for moving some objects. Most studies have found that the appropriate weight limit of backpack for students is between 10% to 15% of their body weight (BW). However, Some students should carry a backpack heavier than 15% of BW. Therefore, It is associated with abnormal shoulder and neck posture on students. Objects: This study tested the effects that various amounts of weight carried by university students in their backpacks had on their cervical posture and electromyography of neck muscle. Methods: The subjects consisted of 12 students (6 male, 6 female) in university. There were three loading conditions tested: no backpack, a backpack that weighed 10% and 15% of the student’s BW. The dependent variables were the craniovertebral angle (tragus to C7) and the neck muscle activities (sternocleidomastoid, upper trapezius). All 12 subjects were asked to walk while wearing a backpack for 5 minutes and were then instructed to rest for 2 minutes. Results: When assessing the craniovertebral angle, the results of this study were significantly decreased in the order of 0%, 10%, and 15%. And then, there were significantly increased electromyography of neck muscles that comparison of the weight of 10% and 15% on 0%. It was found that as the weight of the backpack increased, the craniovertebral angle decreased (p < 0.05) and the muscle activities increased (upper trapezius p = 0.012, sternocleidomastoid p = 0.013). Conclusion: Our study recommended that some students shouldn’t carry on over the 15% backpack of own weight, and also they should distribute backpack load to equal on body for optimal posture.

        팔걸이 형태에 따른 정상인의 보행변수 변화

        본 연구의 목적은 4가지의 다른 형태의 팔걸이를 이용하여 상지의 움직임을 제한하였을 때, 보행양상에 미치는 영향을 알아보고, 기능적인 보행을 획득하기 위한 기초자료를 제공하는 것이다. 건강한 성인 남녀 20명을 대상으로 미착용 상태와 단일 끈형, 해리스 편측형, 뇌졸중용, 로리안 상완 커프형 팔걸이를 착용한 상태에서 무작위 순으로 GAITRite system을 통해 보행평가를 시행하였다. 팔걸이를 착용한 동측 하지의 보장에서 유의한 결과를 나타냈다(p=.002). 팔걸이를 착용하지 않았을 때보다 해리스 편측형 착용시에 팔걸이를 착용한 동측 하지의 보장이 감소되었다(p=.001). 신전형 팔걸이인 로리안 상완 커프형 착용시에 해리스 편측형 착용 시에 비해 동측 하지의 보장이 증가되었다(p=.01). 보행속도, 유각기, 단하지 지지기, 분속수에서는 통계적으로 유의한 변화가 없었다. 앞으로의 연구에서는 팔걸이를 착용하는 환자들에게 나타나는 보행변화에 대한 연구와 기능적인 보행패턴을 획득하기 위한 운동학적 측면의 연구가 이루어져야 할 것이다. The purpose of this study was to investigate the variations in gait parameters according to arm slings used in healthy adults. Twenty healthy adults (9 males, 11 females) participated in this study and walked at self-selected speeds on a GAITRite-instrumented carpet. They were randomly assigned conditions: without an arm sling, a care sling, a Harris hemi arm sling, a CVA sling, and a Rolyan humeral cuff sling. The following gait parameters were analyzed: the temporo-spatial parameters of gait velocity, swing phase, single support, cadence. In the comparison of parameters in each trial, step length was statistically significantly changed (p=.002). The right step length was significantly decreased in the Harris hemi arm sling and increased in the Rolyan humeral cuff sling when compared with no sling. This study found that several different types arm slings varied gait pattern in healthy adults.

      • 슬링 뉴렉 기법이 교각 운동 시 몸통 근육과 넙다리뒤근육의 근 활성도에 미치는 영향

        Background : Trunk stabilization exercises activate the stabilizing muscles of the trunk and essential for the correct function of the lumbo-pelvis. The bridge exercise increases the activity of the trunk stabilizing muscle, and is also used for the treatment of these muscles. Various studies related to sling exercise have been conducted, although none of them examined its effects on the muscle activity of trunk stabilizing muscles when the bridge exercise was performed by applying the sling neurac method. Objectives : The purpose of this study was to investigate which support surface the trunk and hamstring muscles high activity during bridge exercise using sling neurac method. Methods Seven teen healthy subjects, and the surface electromyography was attached to transverse abdominis, multifidi, and the dominant side hamstring muscles. Then, the maximum voluntary isometric contraction was measured. The bridge exercise was performed on either the floor, on a foam roller, and sling surface by randomly. A one-way repeated measures ANOVA was performed to determine the difference in muscle activity between the surfaces. Results : There was a significant difference in the activity of all muscles according to the difference in the supporting surface (p < 0.05). In the transverse abdominis, the sling surface showed significantly higher than the other support surfaces (p < 0.001). Conclusions : In this study, the more unstable the support surface, the highly muscle activity in all muscles. Especially, in the sling neurac method, a significant increase in the transverse abdominis muscle activity was observed compared to the other surfaces. These results suggest that exercise on a support surface applying the sling neurac method can simultaneously induce the contraction of the trunk stabilization muscles and hamstrings. Therefore, the bridge exercise using the sling neurac method proposed in this study is an effective way to strengthen the hamstring muscles while maintaining the stability of the trunk.

        부산광역시 낙상 실태조사

        본 연구의 목적은 부산광역시 시민들의 낙상 현황과 실태를 분석하고 문제점을 파악하여 시민들의 낙상 사고를 예방하기 위한 프로그램 개발과 정책수립에 기초자료를 제공하는 것이다. 부산광역시 시민의 낙상 발생 현황을 파악하기 위하여 통계청 사망원인 통계연보와 3개의 응급의료센터의 손상자료 및 부산광역시 16개 구군을 대표하는 17,088가구의 53,744명을 대상으로 역학조사를 실시하였다. 95% 신뢰수준 하에서 오차한계 ±4.0%로, 본 연구 결과는 국가통계(승인 번호 : 통계협력과 09007)로 승인받았다. 자료수집기간은 2009년 5월부터 6월까지 약 2개월간에 걸쳐 훈련된 조사원들이 가정을 직접 방문하여 구조화된 설문지를 이용하여 자료를 수집하였다. 수집된 자료는 연구 목적에 따라 SPSS WIN 17.0을 이용하여 분석하였으며, 빈도와 백분율로 조사하였다. 분석결과 부산광역시 시민들의 원인별 손상사망률은 자살(62%), 운수사고(21%), 낙상(11%) 순으로 나타났으며, 낙상 사망률은 노인층에서 가장 높았고 남성(16.1명)이 여성(5.7명)보다 더 높았다. 부산광역시 낙상사망률(6.4명)은 전국 평균(5.0명) 보다 높았고, 서울특별시와 6개 광역시 중 가장 높았다. 손상유형별 내원율은 낙상이 20.3%로 가장 높았으며 여성, 남성 모두에서 가장 높았다. 낙상으로 인한 응급의료센터 내원율을 연령별로 비교한 결과, 노인층이 30.1%로 가장 높았으며 여성은 모든 연령층에서 낙상으로 인한 내원율이 가장 높았고, 남성은 성인층과 장년층을 제외한 모든 연령층에서 낙상이 가장 높은 내원율을 보였다. 최근 부산시민의 인구 중 아동의 비율은 감소하는 반면, 노년층의 인구증가로 안전취약계층이 급증하면서 낙상으로 인한 사망률로 경제적 손실이 증가하고 그로 인한 사회 경제적 부담이 가중되고 있다. 이에 정부와 관련기관 및 단체는 낙상예방을 최우선 과제로 두고 실효를 거둘 수 있는 근거중심의 안전증진사업 개발 및 보급을 위하여 노력해야 할 것이다. Fall accidents are an important cause of death and disability. They also have enormous financial implications. This study aims to report the incidence of falls in Busan. Methods: A descriptive study has been conducted. Overall, 53,744 men and women from general population were recruited in Busan. Data collected was analyzed descriptive statistics SPSS/WIN 17.0. Results: Injury statistics show that in Busan, suicide ranks highest followed by traffic accidents, fall, drowning, homicide and others. Compared to other cities, Busan now ranks first in highest fall accident rates. This is why Busan and its people exclaimed the profound need for safety measures. The most prominent injury morbidity indicator is falls. Falls occur most commonly in the elderly group and infants. This high number shows that it is a major health problem that must be addressed by accident and falls prevention programs. Conclusion: To reduce the alarming rate of falls and related excessive mortality and morbidity, efforts must be made to detect persons at risk of falling and to prevent or reduce the frequency of falls. This study gave a very useful and important data to prepare accident prevention program and to prepare accident prevention strategies.
