N/A In order to evaluate myocardial energy fuels during fasting and exercise, the myocardial glycogen content and the level of plasma free fatty acid were measured. In the 24 and 48 hours fasted rats, the myocardial glycogen content was increased significantly compared with that of the normal rats. But in the skeletal muscle, rectus femoris muscle in lower limb, the glycogen content was decreased by fasting. And the level of plasma free fatty acid was increased up to two to three times of the level of control rats in the 24 and 48 hours fasted rats. The treadmill running, for 30 and 120 minutes, was loaded on the normal group and 24 and 48 hours fasted groups. The myocardial glycogen content was not changed by exercise in the normal rats. But the content of glycogen was increased proportionally to the duration of exercise loaded in 24 hours fasted rats. In 48 hours fasted rats, the glycogen content was decreased by exercise like that of skeletal muscle. The highest content of myocardial glycogen and the level of plasma free fatty acid were found in the 24 hours fasted, 120 minutes exercise loaded rats. It suggest that fatty acid is used in preference to carbohydrate when the carbohydrate and lipid are present together in heart muscle. During utilization of fatty acid in the conditions of fasting and exercise, the glycogen was accumulated in myocardium.

To estimate the incidence rate of coronary heart disease in Korea, of all residents in Taegu city aged 25 or above, those who had an acute MI or a fatal coronary event between 1 July 1996 and 30 June 1997 were registered. Seven hundreds and eight patients were registered during the study period(685 were identified at hospital and 23 were autopsy cases). Age-standardized annual incidence rate of men in city area was 93 per 100,000(95% CI; 61-142) and 35% CI; 16-67) in women(100 in men and 20 in women aged 35-64). The incidence was rapidly increased after age 40 in men, however, in women after age 60. Twenty-eight-days case fatality rate was 45% in men and 47% in women. However, in the age group of 45-59 case fatality rate in women was two times higher than that in men. In conclusion, crude annual incidence rate of CHD in city area was 73 per 100,000 in men and 33 in women. The age-standardized annual incidence of CHD in men(93 per 100,000) was 2 times higher than that in women (33 per 100,000) in Korea.
긴 미만성 관동맥병변에서 단일의 긴 스텐트 삽입 후 초기성적과 추적 검사상 시술성적에 대한 연구

N/A Background : The results and restenosis after long stent implantation for diffuse long coronary lesion has not been fully evaluated. We evaluated immediate and follow-up results of single long coronary stenting for long coronary disease. Methods : This study report on the use of follow-up examination is possible 48 patients with 50 lesions after stenting in total 70 patients with 72 lesions with long lesion. Diffuse long coronary lesion was defined as a lesion length longer than 20mm. Mean follow-up duration was 8.7±2.9 months and mean patients age was 58±10 years. Total patients was infused 8000-10000IU heparin and added bolus 3000-5000IU heparin for activating clotting time(ACT) was over 250 seconds during procedure. Restenosis was defined over 50% diameter stenosis in 6 months follow-up angiographic study. Results : Angiographic success was achieved 68/70 patients(97.1%) in this study. Mean reference diameter was 3.14±0.1mm and baseline, final, follow-up minimal luminal diameter(MLD) was 0.6±0.4mm, 3.0±0.4mm, 1.6±0.3mm on each occasion and each % diameter stenosis(% DS) was 78.7±0.4%, 4.9±0.4%, 47.4%±0.5% present. The overall restenosis rate was 42%(21/50). Procedure associated complication included 1 myocardial infarction and 1 death. The restenosis rate was significantly increased in diabetics patients(7/21 vs 3/29 p=0.04) and in long lesion length patients(33.9±4mm vs 30.4±0.4mm p=0.02) but, clinical diagnosis and indication of stenting, lesional location, stent length, stent size, reference diameter size were not associated with restenosis rate. Conclusion : Single long stent implantation for diffuse long coronary lesion shown excellent success rate but high restenosis rate present. The restenosis rate was significantly associated with diabetics and lesion length Some further study for improving restenosis rate is needed. (Korean J Med 58:48-56, 2000)
