Background : the technical modality of esophagogastric anastomosis plays an important part in the factors leading to anastomotic leakage and stricture. Objective : The objective of this study was to compare the leakage and stricture rates between the different anastomosis techniques. Materials and Methods : A retrospective study was undertaken in the patients with esophageal cancer who underwent a esophagectomy. Hand-sewn anastomosis was performed in one layer with interrupted sutures(Manual group). The stapler group was divided into two subgroups, such as PCEEA group and Endo-GIA group. Results : The differences of preoperative patient profiles between the groups were not significant. Overall mortality fates were 5% in manual group, 5% in PCEEA group and 11.5% in Endo-GIA group. Fewer anastomotic leakage occurred in manual group(5%) than in PCEEA group(10%) or in Endo-GIA group(15.4%). The postoperative stricture rate was higher in PCEEA group(35%) than in manual group(5%) or in Endo-GIA group(0%) Conclusion : Statistically, there were no significant differences in the anastomotic leakage and stricture rates between the hand-sewn and mechanical anastomosis. But the stricture rate was lower in the group using the linear stapling device and the leakage rate was lower in the hand-sewn group than the other groups.

In the previous experiment, we have transiently analyzed a 307 bp cis-acting regulatory element of HOXA-7 in transgenic mice (TG). Since it was sufficient to direct a reporter gene to be expressed at the specific region of day 12.5 post coitum (p.c.) embryo with a sharp anterior boundary, we generated transgenic mice line using the NM307 (307 bp/tk promoter/LacZ/SV40 poly A) construct in order to see the temporal expression pattern following embryonic development. Through polymerase chain reaction (PCR) with LacZ specific primers and genomic DNAs isolated from ear tissue, 4 transgenic mice (founder mice, F0) were obtained and 3 exhibited characteristic expression pattern. The expression detected as early as day 8.5 p.c. in the neuroectoderm and epiblast at the posterior region; from the closed neural tube around the region S10-12 to the adjacent posterior neural fold, neural groove and to the posterior epiblast along the anteroposterior axis. At day 7.5 p.c., however, LacZ expression was not detected. As embryo develops, the expression was restricted in the neural tube from S10 area to the end of tail of day 9.5 p.c. embryo, among which rather low level of expression was detected particularly in the region of S10-13, and the overall expression pattern continued through the following day. From day 11.5 to 12.5 p.c. weak spreading of the expression was detected to the anterior of S10 and a repression at the tail region. These results altogether indicate that the 307 bp is a temporal as well as a spatial regulatory element of HOXA-7 initiating the function at around day 7.5.~8.5 p.c. and continuing through mid gestation.
정밀의료 생태계 구축을 위한 데이터 수집 및 연계 국내 동향

정밀의료 생태계 구축의 핵심 원동력인 개인의 의학적, 유전적, 분자적, 환경적 요인에 대한 데이터는 다양한 기관과 다수의 개인에게서 생성된다는 특징이 있기 때문에, 데이터를 활용하기 위해서는 데이터의 연계 및 통합이 우선되어야 한다. 본 종설은 정밀의료 생태계 구축을 위해 현재 진행되고 있는 데이터 수집에 대한 인프라, 정책, 시범 사업, 연구 등의 국내 동향을 살펴보는데 그 목적이 있었다. 데이터 수집 및 연계에 대한 국내의 동향을 살펴봤을 때, 기술적 인프라, 데이터 활용 · 연계 인프라, 인력적 인프라 등의 보완이 절실하다. 특히 데이터 수집 연계에 대한 정책 및 법률적 기반은 매우 미비한 상태로, 데이터 수집과 연계에 대한 필요성과 현재 법적 제한의 상충된 입장 내에서 여러 가지 한계에 직면하고 있는 실정이다. 현재 국가에서 시행되고 있는 데이터 플랫폼 시범서비스에 대해서는 향후 추진 성과를 지켜볼 필요가 있으며, 국가뿐만 아니라 기업 입장에서도 정밀의료 및 헬스케어산업에서 중요한 역할을 하는 전문 인력 양성에 적극적으로 참여해야할 필요가 있다. 현재로서는 정밀의료 생태계에서 큰 축을 담당하는 환자유래건강데이터(PGHD; patients generated health data)의 수집이 가장 미비한 것으로 판단된다. 향후 환자 유래건강데이터를 효율적으로 수집하고 이를 전자 의무 기록(EMR; electro medical record)과 연계하는 시도가 필요하다. Data on individual medical, genetic, molecular, and environmental factors is a necessary in a precision medical ecosystem. Since data is generated from various institutions and many individuals, the linkage and integration of data should be a priority for precision medical care. The purpose of this review was to examine domestic trends such as infrastructure, policy, pilot project, and research on data collection. The technical infrastructure, data utilization and linkage infrastructure, and human resources infrastructure are urgently needed. In particular, the policy and legal foundations for data collection linkages are inadequate and face various limitations. For the data platform pilot service currently in the country, it is necessary to observe the future progress. A healthcare companies should actively participate in the training of professional manpower. There is a lack of collection of patient generated health data(PGHD). In the future, patient-derived health data will need to be efficiently collected and linked to an electronic medical record(EMR).
