        10년 관찰기간 중 다수의 국소 재발을 보인 설암 환자

        Locoregional recurrence of tongue cancer is higher than that of other sites of the oral cavity. Locoregional control has shown improvement over the past 20 years, however, a high rate of recurrence and second primary tumor occurrence is still frequently reported. Leukoplakia is a clinical term, which describes a whitish lesion of the oral cavity. Clinicopathologic features may range from hyperkeratosis to malignancy. Because of its diverse pathologic characteristics, management of this lesion for diagnosis, treatment planning, establishment of a clear surgical margin, and periodic follow-up is difficult. We report on a case of successfully treated tongue cancer which developed from leukoplakia over 10 years. Periodic follow-up strategy and surgical planning are most important to management of locoregional recurrence.

        임베디드 시스템 기반 지능형 영상 감시 시스템 구현

        본 논문에서는 NIOS II 임베디드 플랫폼을 기반으로 하는 지능형 영상 감시 시스템을 구현하였다. 지금까지의 입베디 드 기반의 영상 감시 시스템들은 하드웨어의 의존도가 높아 특정한 목적에 제한되는 단점이 있었다. 이러한 한계를 개 선하기 위하여, 필자들은 그 응용의 목적에 따라 폭 넓게 적용 가능한 유연성이 높은 임베디드 플랫폼을 구현하였다. 소프트웨어 중심 프로그래밍 기법의 주요 문제점인 고속 처리를 위하여, 핵심 부분인 하드웨어 플랫폼에서 SOPC형 NIOS II 임베디드 프로세서와 영상처리 알고리즘을 소프트웨어 프로그래밍과 C2H(The Altera NIOS II C-To-Hardware(C2H) Acceleration Compiler) 컴파일러를 사용하는 하드웨어 프로그래밍을 통합하여 시스템의 성능을 향상 시켰다. 그리고 NIOS II 임베디드 프로세서 플랫폼을 중심으로 각각의 디바이스 인터페이스를 통합 관리하는 서버 시스템을 구축하고, 사용자의 접근 효율을 높이기 위해 네트워크상에서 제어하는 기능을 추가하였다.본 시스템을 영상 감시를 위한 지정된 구역에 설치하여 시험하고 그 성능을 평가하였다. In this paper, an intelligent visual surveillance system based on a NIOS II embedded platform is implemented. By this time, embedded based visual surveillance systems were restricted for a special purpose because of high dependence upon hardware. In order to improve the restriction, we implement a flexible embedded platform, which is available for various purpose of applications. For high speed processing of software based programming, we improved performance of the system which is integrated the SOPC type of NIOS II embedded processor and image processing algorithms by using software programming and C2H(The Altera NIOS II C-To-Hardware(C2H) Acceleration Compiler) compiler in the core of the hardware platform. Then, we constructed a server system which globally manage some devices by the NIOS II embedded processor platform, and included the control function on networks to increase efficiency for user. We tested and evaluated our system at the designated region for visual surveillance.

        정보 융합 칼만-Consensus 필터를 이용한 분산 센서 네트워크 구현

        무선 센서 네트워크에서 동적 시스템에 대한 consensus 알고리듬은 센서 네트워크의 데이터 융합을 위해 신축적인 알고리듬을 적용할 수 있다. 본 논문은 분산 센서 데이터 기반의 평균적인 consensus 특성을 이용하여 n개의 센서 계측치들의 평균을 추적하기 위해 센서 네트워크의 노드들로 구성되는 하나의 분산 데이터 융합 필터를 구현하였다. 본 consensus 필터는 센서 네트워크에서 분산 칼만 필터링에 의한 구조로 데이터 융합의 문제를 해결한다. consensus 필터의 최적 수렴특성, 잡음 전파의 감소 및 빠른 입력신호들의 추적 능력을 보여준다. 필터링 처리 결과를 확인하기 위해 지그비 통신을 이용하여 각 센서의 출력신호와 필터링 처리 결과 및 각 센서의 개별적 신호들을 통합하고 consensus 필터링 처리 결과를 보였다. In wireless sensor networks, consensus algorithms for dynamic systems may flexibly usable for their data fusion of a sensor network. In this paper, a distributed data fusion filter is implemented using an average consensus based on distributed sensor data, which is composed of some sensor nodes and a sink node to track the mean values of n sensors' data. The consensus filter resolve the problem of data fusion by a distribution Kalman filtering scheme. We showed that the consensus filter has an optimal convergence to decrease of noise propagation and fast tracking ability for input signals. In order to verify for the results of consensus filtering, we showed the output signals of sensor nodes and their filtering results, and then showed the result of the combined signal and the consensus filtering using zeegbee communication.

        CDR을 사용한 FPGA 기반 분산 임베디드 시스템의 클록 동기화 구현

        Time synchronization between distributed embedded systems in the Real Time Locating System (RTLS) based on Time Difference of Arrival (TDOA) is one of the most important factors to consider in system design. Clock jitter error between each system causes many difficulties in maintaining such a time synchronization. In this paper, we implemented a system to synchronize clocks between FPGA based distributed embedded systems using the recovery clock of CDR (clock data recovery) used in high speed serial communication to solve the clock jitter error problem. It is experimentally confirmed that the cumulative time error that occurs when the synchronization is not performed through the synchronization logic using the CDR recovery clock can be completely eliminated.

        노년기 우울증 환자의 하지불안증후군의 임상적 중요성

        Objectives Restless legs syndrome (RLS) is a sleep disorder characterized by uncomfortable and unpleasant sensations in the legs and an urge to move the legs, usually at night. The aim of this study is to investigate the incidence of RLS in patients with late life depression and its influence on various clinical outcomes such as severity of depression, sleep quality, cognitive function, and quality of life and accordingly, to elucidate the clinical significance of RLS in patients with late life depression (LLD). Methods This study enlisted 170 depressive patients aged 65 years or older from an outpatient clinic. Structured diagnostic interviews were performed using the Korean version of the Mini-International Neuropsychiatric Interview. All patients completed the questionnaires, including the International RLS Severity Scale, the Korean version of Short-Form 36-Item Health Survey (SF-36), and the Pittsburgh Sleep Quality Index (PSQI). The severity of depression was evaluated by the Korean form of the Geriatric Depression Scale (KGDS) and the level of global cognition was assessed by the Mini-Mental State Examination in the Korean version of The Consortium to Establish a Registry for Alzheimer's Disease Assessment Packet (MMSE-KC). Results The incidence of RLS was 17.6% in LLD patients. RLS was more prevalent among the subjects with major depressive disorder (MDD) than those with minor depressive disorder or subsyndromal depressive disorder. The RLS group showed higher score in the KGDS than the Non-RLS group but the difference did not reach the statistical significance (p = 0.095, Student t-test). The mean PSQI score was significantly higher in the RLS group than in the Non-RLS group (p = 0.001, Student t-test). The MMSE-KC score was also lower in the RLS group than in Non-RLS group (p = 0.009, analysis of covariance). But, there was no difference in the score of SF-36 between the RLS group and the Non-RLS group. Conclusions RLS is common in LLD patients, especially in the patients with MDD and is associated with poor sleep quality and cognitive dysfunction, indicating that RLS is clinically significant in patients with LLD. Therefore, RLS should be considered as an important clinical issue in the management of LLD.

        SA 508 Gr.3 Mn-Mo-Ni low alloy steel forgings thicker than 200 mm are used for reactor pressure vessels in nuclear power plants. The cooling rate difference along the thickness direction during the quenching process causes variation in the microstructure and mechanical properties. The microstructural variation and its influence on the fracture toughness of RPV steels were investigated in this study. The cleavage fracture toughness in the transition region were evaluated with the ASTM E1921 master curve method for samples at different locations from the inner surface to the center thickness of the RPV steel. The microstructural features, such as the area fraction, and the size and distribution of precipitates were quantitatively evaluated at each sampling position. Microstructure observations showed that at near the surface position, bainite laths are finer, and furthermore, the carbides are smaller and homogeneously distributed. The fracture toughness at the surface was better than those at deeper positions. The reference temperature T0 showed a linear relationship with the area fraction of the carbides bigger than a certain critical size. It is concluded that the size of the precipitates caused by the cooling rate gradient may have a dominant role in controlling the cleavage fracture toughness variation along the thickness direction for a very thick RPV steel.

        GPGPU를 사용한 다상분해 주파수 대역압축 및 복원기의 병렬화

        본 논문에서는 2차원 다상분해 영상신호의 주파수 대역 압축 및 복원 알고리즘을 PC기반에서 GPGPU를 사용하여 병렬구조로 설계하고, CPU, OpenCL(Open Computing Language) 그리고 CUDA(Compute Unified Device Architecture)로 구현하였다. 그리고 여러 고해상도 영상을 사용한 실험을 통해서 병렬화에 따른 처리 성능을 비교하였다. 그 결과 각각의 해상도에서 CPU 처리 속도 대비 OpenCL은 480~3800배, CUDA는 700~5000배 성능 향상을 보였다. CPU의 경우 해상도가 증가함에 따라 처리 시간이 지연되었으나, 병렬 처리 기반의 OpenCL과 CUDA는 본 실험 영상의 해상도 증가에 따른 처리시간 지연이 발생하지 않았고, OpenCL보다 CUDA가 더 효과적인 것을 확인하였다. In this paper, CPU, OpenCL (Open Computing Language) and CUDA (Compute Unified Device Architecture) algorithms are implemented by designing a frequency band compression and decompression algorithm of two-dimensional polyphase decomposed image signal in parallel structure using PC based GPGPU. Then, through experiments using various high resolution images. the processing performance by parallelization was compared. As a result, with each resolution, OpenCL gained 480 to 3800 times and CUDA increased 700 to 5000 times compared with CPU processing speed. In the case of CPU algorithm, the processing time was delayed as the resolution increased, but the processing time of OpenCL and CUDA algorithms did not cause delay in processing time accompanying the increase in the resolution of this experimental image data, and We confirmed that CUDA is more effective than OpenCL.

        하치조신경관으로 과충전된 근관치료 충전재에 의한 감각이상의 외과적 처치

        Damage to the inferior alveolar nerve(IAN) is a relatively infrequent complication in endodontic treatment. However, endodontic overfilling involving the mandibular canal may cause an injury of the inferior alveolar nerve resulting in sensory disturbances such as pain, dysesthesia, paresthesia or anesthesia. Two mechanism(chemical neurotoxicity and mechanical compression) are responsible for the IAN injury. When absorbent materials overfilled, it can be treated as a non-surgical procedure. But early surgical intervention required when mechanical, chemical nerve damage expected. We report surgical removal of overfilled gutta-percha and IAN decompression through sagittal split osteotomy in case of dysesthesia after overfilling of endodontic material into the mandibular canal. Dysesthesia recovered 3 months after surgical treatment.

        한방지역축제의 역사성 고찰

        Purpose : Local medicinal herb festivals present the history of the Korean medicine and cultural resources in the regions to play an important role in attracting tourists, vitalizing local economy, and improving the local image. Therefore, it is important to understand the origin of the festival and grasp historical and cultural meaning of local medicinal herb festivals. Methods : I compared the books and articles presented in the reference list. Results & Conclusions : Local medicinal herb festivals originates from traditional yangnyeongsi. Since the 17th century, yangnyeongsi has grown up as a special market. Implementation of the Daedong Act promoted commodity and monetary economy that helped commerce and industry develop and market grow up. It also made changes in the social status system and yangnyeongsi has been such a historical phenomenon appeared in this social background. The growth of yangnyeongsi contributed to the progress in the private medicine that triggered the gradual transfer of power in the medicine to the private sector which has long been held by the government. In yangnyeongsi, there were many cultural events to attract visitors. It’s the same case in China that preserves stages that were used for cultural events in the medicine market to pass down the historic sites while those in Korea are disappearing as yangnyeongsis are being pulled out of the city areas to the suburban areas due to the redevelopment projects. For this reason, restoration of the place for traditional yangnyeongsi should be taken into account through local medicinal herb festivals.
