소아 지속 발작의 임상 특성

목적: 5분 이상의 지속발작은 뇌 신경손상으로 이어지는 연속적인 발작으로 진행될 수 있기 때문에 발작이 지속되지 않게 빨리 치료되어야 한다. 이에 저자들은 지속발작과 연관된 임상 양상을 분석하기 위해 본 연구를 진행하였다. 방법: 2012년 1월부터 12월까지 30분 이하의 첫 발작을 주소로 원주세브란스기독병원에 내원한 환아 192명을 대상으로 하였다. 발작 시간 5분을 기준으로 5분 미만인 group A, 5분 이상인 group B로 나누어 성별, 나이, 과거력, 가족력, 발작유형, 영상검사, 뇌파검사, 혈액검사 등의 임상 양상을 비교하였고, 약 2년간의 추적 기간 동안 발작의 재발 및 뇌전증의 진단 유무를 비교하였다. 결과: group A는 110명, group B는 82명이었다. 칼륨 수치를 제외하고, 두 군에서 임상양상의 통계학적 차이는 없었다. 혈청 칼륨은 group A에서 4.46±0.49 mmol/L, group B에서는 4.30±0.45mmol/L 으로 유의미하게 차이가 있었다(P=0.0338). 발작의 재발은 group A에서 27.1%, group B에서는 36.4% 발생했고, 이중 group A에서는 13.1%, group B에서는 18.2%가 뇌전증으로 진행되었다(P=0.3819). 결론: 저자들은 지속발작 환아의 임상양상 중에 혈청 칼륨이 낮은 것을 제외하고는 통계적인 의의를 찾을 수 없었으나, 추적관찰에서 5분 이상의 지속 발작이 있었던 환아에서 발작의 재발 및 뇌전증 진단이 더 많은 경향을 볼 수 있어서 가능한 발작 후 5분 이내에 발작의 조절이 필요할 것으로 사료되었다. Purpose: Prolonged seizure, defined as seizures lasting more than 5 minutes, should be treated to prevent continuous seizures that cause brain injuries. In this paper, we will evaluate the clinical characteristics of prolonged seizures in children. Methods: Between the January and the December of 2012, 192 patients were admitted to Wonju Severance Christian Hospital due to epileptic seizures lasting less than 30 minutes. They were divided into two groups according to the duration of their seizures: less than 5 minutes (Group A), and between 5 and 30 minutes (Group B). We compared the clinical characteristics such as sex, onset age, family history, seizure types, cause, imaging studies, electroencephalography, laboratory findings, and prognosis between the two groups. Results: Group A consisted of 110 patients; group B consisted of 82. With the exception of serum potassium level, no statistically significant differences in clinical characteristics between the two groups were observed. Serum potassium level of the laboratory test was 4.46±0.49 in group A, and 4.30±0.45 in group B (P=0.0338). The rate of further seizure episodes was 27.1% in group A and 36.4% in group B. Among them, 13.1% of group A and 18.2% of group B were later diagnosed with epilepsy (P=0.3819). Conclusion: There were no statistically significant differences in the clinical characteristics between the two groups except for the serum potassium level; however, patients whose seizures lasted longer than 5 minutes tended to experience further seizure episodes and were more likely to be diagnosed with epilepsy.

새들백교회의 성장 원리 연구

새들백교회의 릭 워렌 목사는 이 시대에 있어 미국과 세계를 변화시키고 있다는 평가를 받고 있다. 그가 1980년 1월에 한 가정에서 개척한 새들백교회가 미국의 4대 대형교회로 성장했을 뿐 아니라 그 영향력은 미국과 세계에 미치고 있다. 그는 성장의 모든 원리들을 대학에서 학문적으로 정리해서 1993년에 박사 학위를 받았다. 그리고 그 내용을 1995년에 『목적이 이끄는 교회』로 저술했다. 그리고 2003년에는 교회와 비교인들의 삶의 의미를 찾는데 도움을 주기 위해 『목적이 이끄는 삶』을 저술했다. 그 책은 3,500만권 이상이 팔렸다. 새들백교회의 성장의 원리들은 어떤 것인가? 그리고 그 원리들은 전통적인 교회성장 원리들과 어떻게 다른가? 우선 성경에 나타난 교회성장 원리들을 살피고, 또한 전통적 교회성장 원리들을 함께 비교하여 살피고자 한다. 이상의 작업 등을 통해 논자는 공통적인 교회성장 원리들을 10 가지 즉 1. 설교 2. 기도, 3. 전도와 선교, 4. 영적인 예배, 5. 거룩한 교제, 6. 목회자 혹은 평신도의 리더십, 7. 교회의 행정과 관리, 8. 성령,9. 말씀 가르침과 제자 삼기, 10. 동질적인 연합으로 정리한다. 한편 새들백교회의 성장 원리들에 대해, 먼저 워렌은 ``변하지 않는 원칙 4가지``를 말한다. 즉 1) 교회성장에는 한 가지 방법 이상이 있고, 2)다양한 교회들이 다양한 종류의 사람들을 접촉할 수 있고, 3) 하나님이축복하는 것을 비난하지 말아야 할 것과, 4) 원리가 성경적이라면 어디서나 작동 한다는 것이다. 이 4 가지 기본 원칙 위에서 10 가지의 새들백교회의 성장 원리가 세워지고 있다. 그 원칙들은 새들백(S.A.D.D.L.E.B.A.C.K.)의 영문 알파벳으로 상징된다. 즉 단순 구조, 수용(용납), 전도대상자의 표준화, 교회 목적 규정, 평신도 목회 추구, 긍정적인 설교, 교회건물보다 사람을 먼저 세우고, 교회를 알리는 일에 투자하고, 시대에 맞는 예배를 드리고, 교회목회자와 스태프들이 계속적인 개인 성장을 추구하는 것이다. 그러나 열린 예배, 설교, 광고 등의 몇 가지 원칙들은 신학적인 비판을 받고 있기도 하다. 한편, 새들백교회의 특징적인 것은``야구장``프로그램과 ``믿음의 동심원``을 통해, 믿지 않는 자들을 그리스도에게 초청하고 훈련해서, 그들을 복음의 핵심에 헌신께 하는 것이다. 성공적인 교회 개척과 건강한 교회 성장을 위해, 그 원리들을 적용할 것을 워렌은 촉구하고 있다. This thesis is aimed at studying on the Principles of Saddleback Church Growth. Rick Warren started the Saddleback Community Church in 1980 with only one family. The Church has grown up to over ten thousands in worship attendance during the first 15 years, during which he moved to 79 places for worship, without owning a building. Rick Warren shared a five-part strategy that will make any church to grow. They are as follows: to be warmer through fellowship, to be deeper through discipleship, to be stronger through worship, to be broader through ministry, and to be larger through evangelism. Rick Warren adopted the 10 principles S.A.D.D.L.E.B.A.C.K. Strategy which means 1. Simple structure, 2. Acceptance, 3.Defined target, 4. Defined purpose, 5. Laity ministry, 6. Encouraging preaching, 7. Build up before build out, 8. Advertising, 9. Contemporary worship, and 10. Keep on growing. These 10 principles are mostly based on principles of the Bible. However, some principles are theologically criticized for being unbiblical. Meanwhile, Rick Warren also concentrated on the un-churched with the symbols of a baseball diamond and the five circles. 70% of his church members have been conversions from the un-churched. The four bases and pitcher`s mound correspond to the five purpose of the church. Rick frames them in terms of the 5Ms (Membership, Maturity, Ministry, Mission, Magnification). Saddleback Christian education program is purpose-driven. The goal is to help people develop a lifestyle of evangelism, worship, fellowship, discipleship, and ministry. The second symbol is composed of five concentric circles. The 5Cs( Community, Crowd, Congregation, Committed, Core) have a strong relationship to the bases in the first symbol. The goal of the church is to move people from the outer circle(low commitment/maturity) to the inner circle(high commitment/maturity). At Saddleback, this is called moving people from the community into the core.

The database of toxic chemicals for pollution and spills has also been fortified for the following terms: general characteristics, health hazard and response, fire hazard and response, chemical reactivity, physico-chemical properties, and other properties. The information and data running in this database are easily accessible via Internet and Korean telecommunications companies; it is also available KRISTAL databases.

Alto ShooterTM 로 조절된 말기 신부전증 환자에서의 다발성 표재성 위전정부 출혈

Diffuse or multifocal superficial gastrointestinal bleeding can be treated by endoscopic laser, argon plasma coagulation, heater probe, or electrocoagulation. However, these methods need relatively expensive equipments and clinical experience. Recently, Alto Shooter (Kaigen Co, Ltd, Osaka, Japan) which can spray 100% sodium alginate was developed to control gastrointestinal bleeding as a topical therapy. We herein present a 47-year-old man with end stage renal disease in whom multiple superficial gastric antral bleeding was successfully managed by the use of Alto Shooter. This method is easy and relatively cheap to use and seems to control superficial bleeding effectively from vascular lesions or after endoscopic biopsy.
