      • PACS DATA CD의 호환성 평가 및 DICOM 적합성에 대한 검증을 통한 기준 제시

        Purpose To analyze the input and output error of data CD which records the image information and the problems of the server of the compatibility. And to report a compatibility assessment and verification of suitable to DICOM of PACS data CD with investigation of current situation of Korea METHOD AND MATERIALS Date CD of each 8 vendors in 30 hospitals was analyzed. We grasped a main verification element existence of a generation compatibility of data CD. The items of element are media identification, DICOM compression, DICOM viewer send, specified object information modify, auto-run, DICOM content type, etc, and give 1 point for each item. We divided the assessment about an each item into 5 levels. Verification about. DICOM conformance by using DICOM validation tool kit is shown to be classified pass or fail according to error occurrence of tag valus. Classify the prequency of tag occurrence as the item. RESULTS The average point of date CD compatibility is 8 point (very good), lowest is 5 point (6.6%), and highest is 10 point (23%_. Most high occurrence frequency's distribution is 7 point (36.6%). As a result of verification about DICOM conformance, PASS in 8 occurrence frequency's distribution is 7 point (36.6%). As a result of verification about DICOM maximum length numbers (14 items), DICOM error of modality (10 items), discord of pixel data length (6 items). etc.

        폐렴양 폐암에서의 전산화단층촬영의 중요성

        Chest CT is the most useful modality in the diagnosis of primary lung cancer, particularly in patients with recurrent pneumonia which are resistant to antibiotic therapy and in high risk age groups for cancer. The purpose of this examination is to assess the usefulness of CT in pneumonic types of lung cancer. The authors retrospectively analyzed 15 histologically proven cases of pneumonic type lung cancer, which had no evidence of primary lung mass, mediastinal lymphadenopathy or other metastatic lesion on plain chest radiography. On CT scan, a primary lung mass was identified in 13 patients(87%), bronchial obstruction was identified in 12 patients(80%), and metastatic foci was found in the bone(one patient), liver(one patient), and spleen(one patient). In Conclusion, CT scan is a useful method to detect the mediastinal lymphadenopathy or pulmonary mass and also should be the initial radiolographic procedure for diagnosis of lung cancer in patients with recurrent pneumonia on imple chest radiography.

        Percutaneous ureteral stent insertion is a treatment of permanent or temporary urinary diversion to maintain continuity and function of the obstructed and injured ureter. We performed 31 cases of percutaneous double pig tail ureteral stent insertion in 21 patients, included 13 patients with malignant ureteral obstruction and eight patients with injured ureter as well as benign inflammatory stricture. Satisfactory result was obtained in all patients but one, who need percutaneous nephrostomy one week later for urinary diversion. no significant complication was encountered. the authors conclude that percutaneous ureteral stent insertion, an interventional procedure alternative to urologic retrograde method, is an effective method for urinary diversion.

        PACS has been run at the Kyung Hee University Medical Center(KHMC) since 2001, and the installation and operation of PACS have contributed to automation and quantification of KHMC's medical environment During these five years our greatest concern is how to make our own guiding principle of diagnostic monitor QA which is adapted to international standards. In accordance with the terms of 'KHMC QA Guideline', 'AAPM TG18', 'SMPTE RP133', 'DICOM Part14', 'DIN V 6868-57', 'JESRA X-0093', 'JIS Z4752-2-5' and 'KCARE', concern about quality assurance of medical images are on the increase. With the investigation of acceptance testing and quality control of international standards for medical display devices, and data collection and analysis for recommended guideline, it is reported that acceptance testing(quality control), including geometrical distortion, display reflection, luminance response, luminance uniformity, display resolution, display noise, veiling glare and color chromaticity being adequate and effective to domestic hospital environments for medical display devices and assessment methods according to each performance. Accordingly, KHMC classified the checkpoint items by period, at the time of monitor setting, monthly, quarterly, half-yearly and annually. Periodic classification of checkpoint items for monitor QA makes a good guideline for image QA/QC and useful guideline for persistent good quality of monitor.

        Diagnostic display monitor QA according to AAPM TG18 is usually performed by PACS administrator, product manager and reading doctor, and for acceptance testing and periodic quality control evaluation, a combination of visual and quantitative tests can be used, as outlined in sections 5 and 6 of 'assessment of display performance for medical imaging systems'. Although many display tests can be performed visually, a more objective and quantitative evaluation of display performance requires special test tools. The required instruments vary in their complexity and cost, depending on the context of the evaluation(research, acceptance testing, or quality control) and how thorough the evaluation needs to be. Objective and reliable assessment of many display characteristics can be performed with relatively inexpensive equipment, So, we made 'AAPM TG18 guiding instrument' to ues variable purpose of the evaluation of 'geometrical distortions(quantitative"', 'veiling glare(visual)' and 'sensor calibration'. The spatial measurements for the quantitative evaluation of geometric distortions, and the measurement of the veling-glare ring response function which provides information regarding the spatial extent of the luminance spread, can be performed using the TG18 guiding instrument can be used to sensor calibration to standardize the basic rate of 0% luminance when periodic calibration.

        진단방사선 일반촬영에서의 X-ray Beam Collimator 사용 전국 실태조사

        방사선 검사에서 CR, DR의 도입으로 인해 X-선속 조사야의 사용이 경시되고 있는 실정이다. 일반방사선 검사에서 검사 부위별 조사야를 적절히 사용하고 있는지에 대해 현장 실태 조사와 설문 조사하고, 조사야의 기준의 필요성을 강조하며 표준 조사야를 제시하고자 한다. 서울, 경기도, 전라도, 충청도, 강원도, 부산 지역 총 333개 의료기관을 대상으로 X-선속 조사야의 사용 실태를 현장 조사하여, 검사부위별, 의료기관종별, 영상종류별로 조사야 조절의 사용 여부를 분석하였다. 또한 조사야 조절이 방사선피폭에 미치는 영향과 인식을 평가하기 위해 종합병원 10곳, 병원 10곳, 의원 10곳의 일반촬영실에 근무하는 168명의 방사선사를 대상으로 X-선속 조사야 조절이 환자피폭선량 저감화에 끼치는 영향에 대한 인식, 조사야 조절 방법과 실제 사용하고 있는 검사부위별 X-선속 조사야 크기를 설문 조사하였다. 검사부위별 적합한 조사야을 사용하고 있는 의료기관은 61.3%이었으나, 주요 장기가 밀집된 요추 검사의 경우에는 적합한 사용이 49.9%에 불과하였다. 의료기관종별에서는 종합병원이 약 69%, 영상종류별로 보면 DR을 사용하는 병원의 65.0%에서 적합한 조사야을 사용하였다. 설문 조사에서는 응답자의 97.6%가 조사야 조절이 환자에게 끼치는 선량을 줄일 수 있다고 인식하고 있었으나, 실제로 부위별 X-선속 조사야의 크기를 조절하여 검사하는 경우는 83.3%이었다. 일반방사선 검사에서 적절한 조사야를 사용하는 경우가 실태조사를 통해 낮게 나타났으며 이는 환자에 대한 방사선 피폭선량을 줄이기 위한 노력이 시급히 필요하며, 조사야에 대한 통일된 표준 규격이 필요하여 철저한 교육이 동반되어야 한다. Due to the introduction of CR and DR, it has been neglected the use of the X-ray beam collimator and field size. This study examines nationwide survey of the proper use of collimator and field size by area in a specific field of plain radiography and the current status. Authors emphasized the need for the field size criteria, and propose a standard reference field size in each specific radiologic examination. Total 333 medical institutions (included in Seoul, Gyeonggi-do, Jeolla, Chungcheong, Gangwon-do, Busan area), were investigated in relation to the status of the X-ray beam collimation field size, type specific inspection areas, medical facilities, and image analyses by type to figure out whether they use the adjustment of image field to the specific examination. To assess the awareness and the impact of radiation exposure to the collimation adjustable, 168 radiographers who was working in 10 general hospitals, 10 hospitals, and 10 clinics, were surveyed how they haver adjusted the actual field size. We examine that 61.3% of medical institutions used the "Proper collimation" and only 49.9% of them employed proper one in lumbar spine densely crowded by major organs. 69% among general hospitals, and 65% among hospitals using DR system were using proper collimation. Radiographers recognized that proper adjustment of collimation could reduce the harmful radiation dose on patients. In the survey, 97.6% of respondents were aware of this fact, but only 83.3% of respondents did the adjustment of the size of the collimation field. The using of proper collimation field was low in the nationwide survey, so the effort to reduce the radiation dose on the patients is urgently needed. A unified standard for the field accompanied by thorough education should be needed.

        외국의 컴퓨터 단층촬영 장치의 방어시설 문헌 조사

        전 세계적으로 컴퓨터 단층 촬영 장치(Computed Tomography, CT)의 임상 적용이 환자의 질병의 조기 진단에 매우 중요하게 사용되고 있으며 사용 빈도 또한 매년 급증하고 있다. 새로운 종류의 CT 장치들이 병변을 조기 진단하기 위하여 개발되고 있다. 이 방사선 발생 장치에 의한 환자나 작업 종사자들의 방사선 피폭이 불가피 할 수도 있다. 국내에서는 CT 장치에 대한 구체적인 방사선 방어나 방사선 시설에 대한 구체적인 규정이 확립되어 있지 않다. 본 논문에서는 국내에서의 CT 사용시설에 대한 방사선 방어 시설과 사용 기준에 대한 규정 마련에 토대를 이루기 위하여 외국에서의 CT 장비에 대한 방사선 방어 및 방사선 차폐에 대한 규정을 조사하였다. 조사방법으로는 구글 검색을 이용한 특정 키워드 검색과 방사선 관련 업무를 수행하는 특정 웹사이트를 직접 검색하는 방법을 사용하였다. 검색결과 캐나다, 미국, 영국 등의 국가에서 국가의 실정에 맞는 가이드라인을 사용하고 있었으나, 아직 이중 에너지 CT에 대한 가이드라인은 없는 것으로 나타났다. 한국에서도 국내 실정에 적합한 CT 장비에서의 방사선 방어, 차폐, 환자나 작업종사자 및 일반인에 대한 방사선 피폭에 대한 가이드라인의 설정이 필요하다고 생각된다. A computed tomography (CT) is a powerful system for the effectively fast and accurate diagnosis. The CT system, therefore, has used substantially and developed for improving the performance over the past decade, resulting in growing concerns over the radiation dose from the CT. Advanced CT techniques, such as a multidetector row CT scanner and dual energy or dual source CT, have led to new clinical applications that could result in further increases of radiation does for both patients and workers. The objective of this study was to review the international guidelines of the shielding requirements for a CT facility required for a new installation or when modifying an existing one. We used Google Search Engine to search the following keywords: computed tomography, CT regulation or shield or protection, dual energy or dual source CT, multidetector CT, CT radiation protection, and regulatory or legislation or regulation CT. In addition, we searched some special websites, that were provided for sources of radiation protection, shielding, and regulation, RSNA, AAPM, FDA, NIH, RCR, ICRP, IRPA, ICRP, IAEA, WHO (See in Table 1 for full explanations of the abbreviations). We finally summarized results of the investigated materials for each country. The shielding requirement of the CT room design was very well documented in the countries of Canada, United States of America, and United Kingdom. The wall thickness of the CT room could be obtained by the iso-exposure contour or the point source method. Most of documents provided by international organizations were explained in importance of radiation reduction in patients and workers. However, there were no directly-related documents of shielding and patient exposure dose for the dual energy CT system. Based international guidelines, the guideline of the CT room shielding and radiation reduction in patients and workers should be specified for all kinds of CT systems, included in the dual energy CT. We proposed some possible strategies in this paper.
