The essential hypotension is considered to be not a disease but a constitutional and usually healthy with rather longer longevity except occasional some complaints. Since there are very few studies on essential hypotension in Korea, auther studied the incidence of essential hypotension in Korea, and geographic difference in 20,075 apparently healthy Koreans (male 11,354 persons, female 7,402 persons) with age range of 15-85 years, relation to Korean constitution (obesity index, height, weight, thoracic circumference-height ratio and Kaup's index) and clinical observation on 52 clinical cases. The two criteria of systolic blood pressure less than 100 mmHg and less than 90 mmWg without preceding illness to cause hypotension were used for the diagnosis of essential hypotension and following results were obtained; 1. The incidence of essential hypotension among the total 20,075 Koreans were 6.4% and 1.1 % by the criteria of less than 100mmHg and less than 90 mmHg systolic blood pressure respectively. There was higher incidence in female with the incidence of male 3.9%, female 9.7% (less than 100 mmHg) and male 0.7%, female 1.6% (less than 90mmHg) respectively. There was no significant age difference of incidence in persons with age above 20 years. These data are similar to 3-7% of incidence in Japan. 2. There was higher incidence in rural area (islands) than in urban area (Seoul city) with the incidence in urban area 5.6% (less than 100 mmHg), 0.9% (less than 90 mmHg) and in rural area 12.3% (less than 100 mmHg), 2.3% (less than 90 mmHg). There were no geographic difference of the incidence among rural areas. 3. There was high incidence of essential hypotension in persons with constitutions of obesity index Less than 0.89, thoracic circumference-hight ratio less than 49 and Kaup index less than 1.99. These data are indicative of constitutional characteristics of underweight, narrow thorax and underdevelopment in essential hypotension. 4. The major symptoms of essential hypotension in symptomatic cases are dizziness, indigestion, palpitation, fatigability, dull headache, weakness, insomnia, dyspnea, chest discomfort, night mare, sweating and cold extremities, epigastric fullness and flushing in order. 5. Auther recommended that a criteria of essential hypotension in Korea is systolic blood pressure persistently less than 100 mmHg in adult without preceding illness or medication.
