        아토피피부염 환자에서 혈청 비타민 D 농도와 메티실린 저항성 황색포도구균 피부 집락화의 연관성

        Purpose: Bacterial infection with Staphylococcus aureus is a common complication of atopic dermatitis (AD) and involved in the worsening of this disease. Recent studies have revealed an increasing prevalence of community-associated methicillin-resistant S. aureus (MRSA) among patients with AD and the role of vitamin D in the immunopathogenesis of AD. We carried out a study to see whether levels of vitamin D correlate with MRSA skin colonization in AD. Methods: Total 60 patients with AD aged between 3 months and 6 years old were enrolled. We measured disease severity using the Scoring Atopic Dermatitis (SCORAD) index, serum levels of 25-hydroxyvitamin D (25(OH)D), total immunoglobulin E levels, eosinophil cationic protein and bacterial colonization by skin swab and culture method. Results: S. aureus was cultured in 20 patients of the total (33.3%). MRSA was comprised 35% of the S. aureus-positive group. SCORAD value was significantly higher in S. aureus-positive group. Serum levels (mean±standard deviation) of 25(OH)D were lower in MRSA group (22.6±11.5 ng/mL) compared with those of methicillin-sensitive S. aureus group (31.3±10.2 ng/mL) or S. aureus negative group (34.3±15.8 ng/mL). But there was no statistically significant difference. Conclusion: Our study shows that the serum levels of 25(OH)D were lower in MRSA group. But there was no statistically significant difference. Therefore further studies including large numbers of cases are necessary. (Allergy Asthma Respir Dis 2013;1:138-143)

        급성 소뇌염 1례

        저자들은 최근에 복시와 진전을 보인 소아에서 진단된 급성 소뇌염 1례를 경험하였기에 문헌 고찰과 함께 이를 보고하는 바이다. Acute cerebellitis, also known as acute cerebellar ataxia, is an inflammatory syndrome of cerebellar dysfunction that may reflect an infectious, post-infectious, or post-vaccination disorder. The clinical presentations are abnormal eye movement, truncal ataxia, dysarthria, nausea, headache, tremor, convulsion and altered mental status. Many patients with cerebellitis are supposed to have normal MRI findings. But hyperintense signals of cerebellar gray matter in T2-weighted sequences of MRI can be used as a strong indication of acute cerebellitis. We report here a case of acute cerebellitis with abnormal MRI findings in 7 years old patient with diplopia and tremor.

        경량 피스톤 로드를 위한 마찰용접 적용연구(SM45C/SM45C-Pipe 사용)

        Various research to reduce weight of a car is achieving. This research is tendencious to manufacture solid piston rod of shock absorber as hollow piston rod using friction welding. This study deals with the friction welding of SM45C to SM45C-Pipe that is used in car shock absorber, The friction time was variable conditions under the conditions of spindle revolution of 2,000rpm, friction pressure of 55MPa, upset pressure of 75MPa, and upset time of 2.0seconds. Under these conditions, the microstructure of weld interface, tensile fracture surface and mechanical tests were studied of friction weld, and so the results were as follows. 1. In tensile strength, the hole processing is better than non-hole processing. 2. When the friction time was 1.5seconds under the conditions, the maximum tensile strength of the friction weld happened to be 869MPa, which is 103% of SM45C's tensile strength and 91% of SM45C's Pipe. 3. When the friction time was 2.0seconds under the conditions, the maximum bending strength of the friction weld happened to be 1599MPa, which is 80% of SM45C's bending strength and 118% of SM45C's Pipe.

        장기간 사용한 12%Cr강 보일러 튜브의 산화스케일에 관한 연구

        The internal oxide scale of twelve superheater and reheater tubes were tested which were serviced for 30,000~120,000 hours in thermal power plants. The oxide scale was formed in three layers. The Cr-rich area was observed beneath the original metal surface. The hematite (Fe₂O₃) phase was formed on the outer surface. The intermediate layer was magnetite (Fe₃O₄). The thickness of Cr-rich layer was about half of the total scale. All layers grew during the operation hour of the plant. The thickness of thickest scale was 0.2㎜ in superheater tubes. This can increase the tube metal temperature about 7℃ more than initial state. 7℃ tube metal temperature can reduce tube life about 30%, but the boiler tube’s design margin is big enough therefore it has been analyzed that it would not effect on the life span.

        초초임계압(USC) 화력발전기술 개발

        화력발전은 지구온난화의 주범으로 인식되고 있지만, 산업화와 지속적인 국가경제성장에 의한 전력소요 증가는 에너지사용을 증대시켜 에너지 자원의 부족을 초래하고 환경오염물질의 과다배출로 지구환경문제를 유발하고 있다. 에너지원의 확보를 위한 국가와 지역간 경쟁이 심화되어 새로운 갈등의 원인이 되고 있으며, 지구온난화와 에너지 문제를 해결하기 위해 교토의정서 등 기후변화 협약이 체결되고 화석에너지에 재생에너지로의 에너지원의 전환과 다양화가 추진되고 있지만, 각국의 이해관계와 기술부족으로 완벽한 해결책을 제시하지 못하는 상황이다. 에너지 부족을 해소하고 CO₂ 배출량을 저감할 수 있는 가장 효과적인 방안으로 기존 화력발전 효율을 향상시킨 고효율 발전과 Near Zero Emission 수준의 저공해 기술이 결합된 고효율 석탄화력발전시스템을 개발현황을 논하고자 한다. For environmental reasons and because of our limited energy resources, high-efficiency power generation technology will be necessary in the future. Ultra-supercritical (USC) power generation technology is the key to managing the greenhouse gas problems and energy resource problems discussed in the Kyoto Protocol to the United Nations Framework Convention on Climate Change. Other countries and manufacturers are trying to build commercial power plants. In this paper, an efficient method of achieving near-zero emission operation of a high-efficiency fossil power plant using USC power generation is discussed. Development of USC power generation in Korea has been supported by the Korean government in two phases: Phase Ⅰ was USC key technology development from 2002 to 2008, and Phase Ⅱ is USC development and technology optimization from 2010 to 2017.

      • 소아 알레르기비염의 경제적 부담 평가를 위한 다기관 조사

        목적: 알레르기비염의 유병률은 전 세계적으로 급격히 증가하고 있으며, 그에 따른 경제적 부담도 증가하고 있다. 그러나, 소아 알레르기 질환이 미치는 경제적 부담에 관한 국내 연구는 제한적이어서 소아 알레르기비염이 환자와 그 가족에게 미치는 경제적 부담을 산출하고자 본 연구를 진행하였다. 방법: 2008년 7월 1일부터 9월 31일까지 서울시 6개 2-3차 의료기관을 방문한 18세 이하 소아 알레르기비염 환자 및 보호자를 대상으로 설문 조사를 진행하였다. 설문을 통해 직접 의료비 (병·의원 진료비, 및 약제비 한방 진료비 및 약제비, 보완/대체요법비), 직접비의료비 (교통비, 환경개선비), 그리고 간접비용 (월 평균 노동 손실)을 조사하였고 그 결과를 질환의 중증도 및 유병 기간에 따라 분석하여 그 차이를 비교하였다. 결과: 모집된 262명의 대상 중 174명(66.4%)이 남자였고, 평균 연령은 6.54세였다. 대상군의 연간 평균 직접 의료비는 177만 원이었고 직접비 의료비는 57만 원이었다. 비록 통계학적 유의성은 없었지만, 알레르기비염의 중증도가 증가할수록 직접 의료비가 증가하는 경향을 보였고, (P=0.053) 유병 기간 또한 직접 의료비의 증가와 유의한 양의 상관 관계를 보였다. (R=0.195, P=0.002) 대상 환자 보호자의 약 17%가 아이의 알레르기비염으로 인해 직장에 결근 또는 조퇴를 경험한 것으로 조사되어 사회적 간접비용을 미루어 생각할 수 있었다. 결론: 알레르기비염이 미치는 경제적 부담은 질환의 중증도가 심할수록, 그리고 유병 기간이 길수록 증가하며, 이는 특히 한방 진료 및 치료비와 보완/대체요법비의 증가에 기인한 것으로 조사되었다. 따라서 알레르기비염으로 인한 경제적 부담을 줄이기 위해서는 환자와 보호자를 대상으로 근거 중심의 치료를 할 수 있도록 지속적이고 체계적인 교육이 필요할 것으로 사료된다. Purpose: The prevalence of allergic rhinitis is rapidly increasing and results in relatively high socio-economic burden on their family and community. However, studies on the economic burden of pediatric allergic rhinitis in Korea are limited. Therefore, we conducted this study to investigate the impact of pediatric allergic rhinitis on economic burden. Methods: Two hundred sixty two children with allergic rhinitis were enrolled in 6 secondary or tertiary medical centers in Seoul from July to September, 2008. We collected data of the economic burden of allergic rhinitis (direct medical costs, direct nonmedical costs, and indirect costs) by face to face questionnaire survey. We compared the economic burden according to the severity and the duration of allergic rhinitis. Results: The mean age of subjects was 6.54 years, and male were 174 (66.4%). Direct medical costs (10,000 Korean Won/yr) were 177.75, and direct nonmedical costs were 57.92. Although, there was no statistical significance, direct medical costs showed increasing trends in severe allergic rhinitis. (P=0.053) In addition, direct medical costs were positively correlated with duration of allergic rhinitis.( R=0.195, P=0.002). About 17% of the parents who care the allergic rhinitis children experienced the work absence due to their child s illness. Conclusion: The economic burdens of allergic rhinitis were positively correlated with the severity and duration of illness. Particularly costs for alternative medicine including oriental medicine` were related with severity and duration allergic rhinitis. Therefore, special efforts for education with evidence based treatment strategy are necessary to decrease the economic burden of allergic rhinitis.

        보일러 배관용 P92 파이프강의 기계적 특성 및 미세조직에 관한 연구

        The hardness and strength test was performed to make the manufacturing process of SA213 P92 boiler pipe steel. And the microstructure change was studied to find out the cause of room temperature property of P92 steel, ie, low hardness and strength property. The room temperature property of P92 steel depends on the improper normalizing and cooling rate. Especially, Ferrite was formed and the steel had low hardness when the temperature was decreased slowly under the cooling rate 1℃/min after normalizing at the temperature around Ac1 to Ac3. The critical heat treatment temperature and cooling rate was over 900℃ and over 10℃/min to satisfy the minimum yield and tensile stress which was laid down by ASME Code.
