        원주기독병원 응급실로 내원한 치과 응급환자에 관한 임상적 연구

        The appropriate care to the dental emergency patients is much important in the aspect of community dental service. To attain such a purpose, the sacred duty of the training of oral and maxillofacial surgeons is required. So, a retrospective study on the characteristics of dental injuries and diseases in emergency care unit will be very meaningful. This study was carried by reviewing the charts and radiographic films of 3,394 patients, treated for dental emergency at Wonju Christian Hospital, Republic of Korea, from January 1, 1993 to December 31, 2002. All patients were classified to 6 groups including trauma, toothache, infection, hemorrhage, TMJ disorder and the others. The clinical characteristics of diseases and treatment modalities according to each group were analyzed. The trauma (73.9%) was the most frequent cause in dental emergency patients, and acute toothache, odontogenic infection, oral hemorrhage, and TMJ disorder were next in order. Gender prediction was male (68%), there were many patients on May and December in the monthly frequency, and the most frequent age group was from 0 to 9 years. In the trauma group, male (68.6%) was predominant, and soft tissue injuries and primary closures were the most frequent type of injury and treatment. In jaw fractures, traffic accidents were the most cause and the weakest site was mandibular symphysis area, and mandibular angle, condyle, and body area were next in order. In the acute toothache group, the cause was dental pulpitis mostly and treatment for that was drug administration mainly. Buccal space abscess in infection group had the largest incidence (24.5%), and common treatments were incision and drainage and medications. In the hemorrhage group, a major cause was postoperative bleeding (60.3%) and hemostasis was obtained by pressure dressing, curettage and suture. For the TMJ disorder group, the peak incidence (63.8%) was shown in the post-traumatic myofascial pain dysfunction syndrome and its primary care was medication such as analgesics and sedatives. In the other group, the various specific symptoms were complained due to acute sialadenitis, trigeminal neuralgia, acute stomatitis, chemical burn, terminal stage neuritis of head and neck cancer, and foreign body aspiration. In conclusion, for the rapid and proper care of the emergency dental diseases, well-trained education should be presented to the intern and resident course of oral and maxillofacial surgery. And it is demanded that oral and maxillofacial surgeons must be prepared in knowledge and skill for such emergency care.

        두경부 악성 종양환자에서 조기 방사선치료를 위한 구강관리법에 대한 임상적 연구

        Introduction: Tooth requiring extraction before radiotherapy in head and neck cancer patients should be performed as long as possible before the initiation of radiation therapy. Conventionally, a minimum 2-week waiting primary healing period is recommended. Although the above 2-week period is ideal, it is not uncommon for the radiotherapist and cancer patient to feel an urgent need to proceed with radiotherapy despite the need for dental care. Therefore, alternative approaches for early radiotherapy, including conservative endodontic treatment and a 1-week waiting primary healing period after dental extraction at the time of radiotherapy were considered and applied based on a literature review Materials and Methods: The clinical study involved 120 head and neck cancer patients who were treated at Wonju Christian Hospital, Wonju College of Medicine, Yonsei University, from January 1995 to December 2004. Results: In the clinical study, there were no specific complications, such as, post-extraction wound infections, radiation osteitis and osteoradionecrosis over the recent 10 years despite the early radiotherapy. Conclusion: Based on the clinical study, a minimum 1-week waiting primary healing period for oral care before radiotherapy is suitable for early radiotherapy in head and neck cancer patients.

        Background: Teeth requiring extraction before radiotherapy in head & neck cancer patients should be removed as long as possible before the initiation of radiation therapy. Conventionally, a minimum 2-week waiting primary healing period is recommended. Although the above 2-week period is ideal, it was not uncommon for the radiotherapist and cancer patient to feel an urgent need to process with radiotherapy despite the need for dental care. Therefore, alternative approaches for early radiotherapy, including conservative endodontic treatment and a 1-week waiting primary healing period after dental extraction at the time of radiotherapy, were considered and applied based on the experimental study Materials and Methods: Eighteen dogs were processed for histopathologic wound healing. The effect of the primary endodontic treatment and extraction before early radiotherapy was examined. Results: No specific complication, such as, post-extraction wound infection, radiation osteitis and osteoradionecrosis, were encountered despite the early radiotherapy. Conclusion: Based on the experimental study, a minimum 1-week waiting primary healing period for oral care before radiotherapy is suitable for the early radiotherapy in head and neck cancer patients.

        마이크로 머시닝으로 제작한 기계적 가이드를 갖는 정전용량 선형 인코더

        Contact-type Linear Encoder-like Capacitive Displacement Sensor (CLECDiS) is a novel displacement sensor which has wide measurable range with high resolution. The sensor, however, is very sensitive to relative rotational alignment between stator and mover of the sensor as well as its displacement. In addition to, there can be some disturbances in the relative rotational alignment, so some noises occur in the sensor`s output signal by the disturbances. This negative effect of the high sensitivity may become larger as increasing sensitivity. Therefore, this negative effect of the high sensitivity has to be compensated and reduced to achieve nanometer resolution of the sensor. In this study, a new type capacitive linear encoder with a mechanical guide is presented to reduce the relative rotational alignment problem. The presented method is not only to reduce the alignment problem, but also to assemble the sensor to the stage conveniently. The method is based on a new type CLECDiS that has mechanical guide autonomously. In the presented sensor, when the device is fabricated by micro-machining, the guide-rail is also fabricated on the surface of the sensor. By the direct fabrication of the guide-rail with high precision micro-machining, errors of the guide-rail can be reduced significantly. In addition, a manual yaw alignment is not required to obtain large magnitude of the output signal after the assembly of the sensor and the stage. The sensor movement is going to follow the guide-rail automatically. The prototype sensor was fabricated using the presented method, and we verify the feasibility experimentally.

        파라메트릭 어레이를 이용한 공기 중 디지털 통신 연구

        This paper demonstrates the digital communication in air using the parametric array. The stepped-plate transducer which is suitable for high-power and high-efficient radiation is used to generate the difference frequency wave with the parametric array. The primary frequencies are selected to 83 kHz and 122 kHz and the resulting difference frequency wave at the frequency of 39 kHz is used for the communication. The modulation method is selected to On-Off Keying method. The waveform and signal-to-noise ratio (SNR) is measured and analyzed to see the characteristics of the digital communication using the parametric array. The proper distance for the communication using parametric array is about 3 m. The measured beam width of the 3dB SNR reduction was $14^{\circ}$. The possibility of the communication in air using the parametric array is confirmed and the high directional characteristic of the communication using the parametric array is expected to have the advantages for the multi path and the security problems. 본 논문에서는 파라메트릭 어레이를 이용한 공기 중 디지털 통신의 특성을 측정해 보았다. 2차 오더의 비선형 현상인 파라메트릭 어레이를 구현하기 위하여, 고출력, 고효율의 음파 발생이 가능한 요철 트랜스듀서가 사용되었다. 1차 음파 (primary wave)의 주파수는 트랜스듀서 공진 주파수에 맞추어 83 kHz, 122 kHz 로 결정하였고, 이를 통해 발생되는 39 kHz의 차음 (difference frequency wave)을 통신 신호로 이용하였다. 통신 변조방법으로 On-Off Keying 방식을 이용하였다. 실험은 파라메트릭 어레이를 이용한 디지털 통신의 특성을 측정하기 위하여 파형 분석을 하였고, 거리, 각도에 따른 SNR을 측정해 보았다. 실험 결과 파라메트릭 어레이를 이용한 디지털 통신은 3m이내의 근거리 영역에서 적합함을 알 수 있었고, 빔폭 (-3 dB)은 $14^{\circ}$로 측정되어 고지향성 통신이 가능함을 확인하였다. 위의 실험 결과를 통해 파라메트릭 어레이를 이용한 공기 중 음향 통신에 대한 가능성을 확인하였고, 고지향성 통신 특성은 다중경로 문제나 보안 문제에 강점이 있을 것으로 판단된다.

        맥스웰 응력텐서의 물리적 의미의 고찰

        Maxwell stress tensor is one of the methods which are generally used for electromagnetic force calculation. In this paper, it is presented that Maxwell stress tensor T and nㆍT have no physical meaning and therefore should not be used as sources of mechanical force for deformations or dynamics. The divergence of Maxwell stress tensor ∇ㆍT is the one which can acquire a physical identity and is electromagnetic body force density by an action at a distance like a gravity. This result can be derived from the principle of power balance, and also verified by some thought experiments. The virtual air-gap approach is proposed as a valid solution for the calculation of the body force.

        높은 동적영역을 갖기 위한 정전용량형 센서의 최적화

        The capacitive sensor has a simple structure, compact size and low cost, but a small dynamic range. The small range is caused by use of gap variation. If the sensor takes area variation type with one plate moving horizontally, it can have a large measurable range. While the area variation has relatively low sensitivity, some studies have found methods to improve the sensitivity. Even though the methods are effective, parameters of the results are limited and 2 dimensional. This study provides more practical and 3 dimensional analysis and suggests relations between parameters. Using the results, the optimized design parameters of a high dynamic range capacitive sensor can be found.
