        To evaluate the diagnostic findings of salivary gland tumors, we reexamined aspiration cytology smears of 7 cases of pleomorphic adenoma, 3 cases of adenoid cystic carcinoma, and 3 cases of mucoepidermoid carcinoma, performed during April 1986 to March 1990, which were comfirmed by surgical excision and histologic diagnosis. The results obtained are summarized as follows : 1. All cases of pleomorphic adenoma showed branching cellular clusters of epithelial and myoepithelial cells. Acellular elements including myxomatous and chondroid components were observed. There were no cellular pleomorphism and nucleoli. Keratinizing squamous epithelial cells and keratin pearls were noted. 2. The smears of adenoid cystic carcinoma showed cell bails or cell cords containing a central hyaline core. Nuclear atypism and the nucleoli were frequently observed. There were no keratinizing squamous epithelial cells. 3. The smears of mucoepidermoid carcinoma showed mainly sheets or clusters of intermediate cells and some mucin-producing cells. Some nuclear pleomorphism was observed. Mucinous material and many inflammatory cells were present in the background.

      • 폐종괴에 대한 경피적 세침흡인세포검사와 자동총부착 침생검의 비교

        To compare the diagnostic yields and complication rates of transthoracic fine needle aspiration cytology(FNAC) and gun biopsy in the diagnosis of pulmonary mass, a retrospective review was performed in 125 cases. Under the fluoroscopic guide, FNAC was performed by 20G Chiba needle in 91 cases, core biopsy was done by 18.5 G vaccum needle attached with automated biopsy gun in 74 cases and both procedures were done together in 37 cases. Overall sensitivity was 88.4% in FNAC and 87.5% in gun biopsy. For malignant pulmonary tumors, correct type correlation with final diagnosis was obtained in 33(76.7%) out of 43 cases by FNAC and 30(75.0%) out of 40 cases by gun biopsy. For benign pulmonary lesions, there were correct type correlation in 14(35.0%) out of 40 cases by FNAC and 14(53.8%) out of 26 cases by gun biopsy. The complication was pneumothorax and hemoptysis. Pneumothorax occured in 11.1% of FNAC, 10.9% of gun biopsy and 10.9% of both technique, among which chest tube drainages were necessary in one patient by gun biopsy and in three patients by both technique. Although no significant difference of diagnositc accuracy and complication rate was found between FNAC and gun biopsy, gun biopsy was more helpful in the diagnosis of pulmonary benign lesions than FNAC.

        The report of aspiration cytologic findings of epithelial-myoepithelial carcinoma(EMC) in the salivary gland is extremely rare. We present a case of fine needle aspiration cytology(FNAC) from EMC in the right submandibular gland of a 46 years old male patient. Neck CT scan revealed a confined lesion in the submandibular gland without enlargement of the regional lymph node. FNAC from the tumor showed several three-dimensional cellular clusters with admixed normal acinar cells. They frequently formed blanching tubular structures composed of two type of cells; darker cells haying eosinophilic scanty cytoplasm with round dense nuclei and clear cells having abudant pale cytoplasm with vesicular nuclei at the periphery of clusters. The tumor cells of both types did not show pleomorphism or mitoses. The resected submandibular gland showed an ill-defined whitish firm tumor, measuring $2{\times}1.5{\times}2cm$. The histology revealed an infiltrative tumor showing characteristic two cell types in a duct-like arrangement surrounded by thin basement menbrane. An inner layer of darker cells and outer layer of clear cells were postive for cytokeratin in the former and S-100 protein in the taller on the immunohistochemical stain.

        초등학생의 우유 및 유제품 섭취 양상과 소비 성향에 관한 조사

        We conducted a study to investigate milk and dairy products consumption among elementary school students to identify ways to increase their intake of milk and dairy products. We asked 518 students in Seoul to fill out questionnaire. Only 55.6% of the subjects consumed milk daily, and 43.7% of them drank at least 1 cup per day. The frequency and quantity of milk consumption were higher in male students than female students. The majority of the subjects consumed whole milk, and most students(59.5%) preferred flavored milk to whole milk. The reasons given for drinking milk included ``wanting to be tall``(66.1%) and ``for good health``(47.1%), while the main reason given for not drinking milk was stomach pain (68.4%). Records of having an unpleasant experience and upset stomach after drinking milk occurred at a higher rate for those who did not drink milk(84.2%) than for those who did(53.3%). Of the students questioned, 36.9% consumed dairy products other than milk 3~4 times per week, and liquid-type yoghurt(43.8%) and ice cream(35.4%) were the preferred dairy products among students. The reasons given for consuming dairy products were ``good taste``(56.6%), ``good to eat``(46.6%), ``wanting to be tall``(32.4%), and ``for good health``(25.4%). Among those students who did not consumed dairy products, the major reason given was ``family does not eat at home``(33.3%). After consuming the dairy products, 71.4% of participants thought that dairy products did not result in physical improvement. Most students had experienced purchasing milk and dairy products by themselves. When they bought milk or dairy products, the reasons given for making a particular selection were ``shelf-life``(61.6%) for milk, and ``favorite dairy products``(54.1%) for dairy products. According to participants, the factors that need to be considered for milk and dairy product consumption among elementary school students are ``higher nutrition`` (60.8%), ``better sanitation``(57.3%), and ``better quality and taste``(55.2%).

        진저요법이 화학요법을 받는 암 환자의 오심과 구토에 미치는 효과 : 체계적 고찰 및 메타분석

        본 연구는 체계적 고찰과 메타분석을 이용하여 진저요법이 화학요법을 받는 암 환자의 오심과 구토에 미치는 효과를 파악하기 위하여 수행되었다. 문헌검색을 위해 CINAHL, PubMed, Cochrane, EMBASE, Web of Science, Google Scholar 데이터베이스를 이용하였으며, 2009년부터 2018년까지 출간된 연구 396편 중 선정과 배제기준에 따라 최종 선정된 RCT 연구 8편에 대한 질 평가는 Revised Cochrane risk-of-bias tool 2(RoB 2)를 이용하였다. 메타분석은 RevMan 5.4 프로그램을 사용하였으며 오심과 구토의 발생빈도는 평균승산비(Odds Ratio, OR), 강도는 표준화된 평균 차이(Standardized Mean Difference, SMD)를 산출하였다. 진저요법이 오심에 미치는 효과로 발생빈도 OR= 0.92[0.65, 1.30], 강도 SMD= -0.12[-0.59, 0.35]로 통계적으로 유의하지 않았으며 하위집단 분석 결과 급성기와 지연기 모두 진저요법의 효과가 유의하지 않은 것으로 나타났다. 진저요법이 구토에 미치는 효과로 발생빈도 OR=0.89[0.62, 1.27], 강도 SMD= -0.45[-1.04, 0.13]로 통계적으로 유의하지 않았으며 하위집단 분석에서도 급성기와 지연기 모두 진저요법의 효과가 유의하지 않은 것으로 나타났다. 연구의 질 평가에 따라 민감성 분석 결과 고위험 연구의 효과 크기는 실제 효과에 비해 과장되었을 가능성이 있는 것으로 나타났다. 진저요법이 화학요법을 받는 암 환자의 오심과 구토 완화에 유의하지 않은 것으로 나타났으나 포함된 연구의 수가 충분치 않아 본 연구를 일반화하기에는 한계가 있어 추후 연구가 필요할 것으로 사료된다. This study was undertaken to assess effects of ginger on nausea and vomiting in cancer patients receiving chemotherapy, through systematic review and meta-analysis. CINAHL, PubMed, Cochrane, EMBASE, Web of Science, and Google Scholar databases were explored. From a total of 396 studies, 8 Randomized Controlled trials during the period 2009 to 2018 were selected and qualitatively evaluated using the Revised Cochrane Risk-of-Bias tool 2. Assessment of the frequency and intensity of nausea and vomiting was achieved by estimating the Odds Ratio (OR) and Standardized Mean Difference (SMD), respectively, using the RevMan 5.4. No significant effect of ginger was observed for nausea (OR=0.92 [0.65, 1.30] and SMD=-0.12 [-0.59, 0.35]), and subgroup analyses also revealed no significant effect on both the frequency and intensity in acute phase and delayed phase. Results also show that ginger has no significant effect on vomiting (OR=0.89 [0.62, 1.27] and SMD=-0.45 [-1.04, 0.13]), neither was any significant effect determined in both the acute and delayed phase through subgroup analyses. Sensitivity analysis revealed that the effect size of high-risk studies could have resulted in overestimation. Although the current study detected no significant effect of ginger on nausea and vomiting, the results cannot be generalized due to the limited number of studies included, which needs to be addressed in the future.

        중학생의 학교 우유 급식 실태 및 만족도 조사

        This study was conducted to investigate the consumption of milk, along with satisfaction for improvement of the school milk program in middle school. The subjects were 447 middle school students(265 male, 182 female) in the Gyeonggi area, who were asked to fill out the questionnaire. The collected data were analyzed by a SAS package. The results showed that 53.2% of the subjects were participating in the school milk program. Regarding the frequency of milk consumption, 77.7% of students reported daily milk consumption and 65.3% of students reported they drank milk when it was available. 56.8% of the students responded that they were satisfied with the school milk program. Male students not participating in the school milk program cited the reason as eating at home, while non-participating female students cited the reason as tasteless. When asked whether they accepted or rejected the school milk program, male students(97.7%) and female students(97.8%) answered it should be executed. To improve the school milk program, most of the students(72.2%) stated that it should be selected among flavored milk(strawberry, banana, chocolate, black bean, and husked rice milk) instead of white milk. Therefore, in order to increase the intake of milk, the students should develop the habit of drinking milk to gain the attention of parents. Also, schools should attempt to help through offering proper dietary education.

        비파(Eriobotrya japonica Lindley) 착즙액 첨가가 요구르트 균주의 산 생성 및 증식에 미치는 영향

        한국산 비파 과실의 활용도를 높이기 위하여 비파 착즙액을 첨가한 기능성 yoghurt를 개발하기 위한 기초 연구로 비파 착즙액의 농도별(10, 15, 20%) 첨가가 요구르트 균주의 산 생성 및 증식에 미치는 영향을 조사하였다. Str. thermophilus, Lac. acidophilus, Lao. casei를 단독균주 및 혼합군주로 접종하여 배양하면서 생균수와 pH 및 적정산도를 측정하였다. 비파 착즙액의 성분 특성은 수분 91.5%, 조회분 0.2%, 당 8.6$^{\circ}$Brix, 총산 0.34%, pH 4.11이었다. 단독균주의 경우 비파 착즙액의 첨가 농도(10, 15, 20%)를 달리하였을 때 Str. thermophilus는 착즙액 10% 첨가시 배양 12시간에 2.10${\times}$$10^{9}$ CFU/mL로, Lac. acidophilus는 착즙액 10-15% 첨가시 배양 15시간에 1.09${\times}$$10^{9}$-1.14${\times}$$10^{9}$ CFU/mL로 그리고 Lac. casei는 착즙액 10-15% 첨가시 배양 24시간에 1.47${\times}$$10^{9}$-1.54${\times}$$10^{9}$ CFU/mL로 가장 높은 균수를 보였다. 또한 이들 유산균의 산 생성도 비파 착즙액을 10% 첨가하였을 때 가장 많이 증가하였다. 혼합균주의 경우 Str. thermophilus와 Lac. acidophilus, Str. thermophilus와 Lac. casei는 모두 착즙액 10% 첨가구에서 배양 15시간에 각각 1.33${\times}$$10^{5}$ CFU/mL, 1.13${\times}$$10^{5}$ CFU/mL이 증가하면서 최대 균수를 나타내었으며, pH 저하와 적정산도 상승이 뚜렷하였다. 따라서 비파 착즙액 첨가 기능성 yoghurt 제조시 비파 착즙액을 10% 첨가하여 혼합 starter로 Str. thermophilus와 Lac. acidophilus 혼합균주를 사용하는 것이 이들 유산균의 증식에 가장 적합한 것으로 나타났다.타났다. This experiment was carried out to investigate the effect of loquat (Eriobotrya japonica Lindley) extract on the acid production and growth of lactic culture in reconstituted skim milk. The supplementation level of loquat extract to reconstituted skim milk was 10%, 15% and 20%. Reconstitued skim milk containing loquat extract was fermented by single of mixed culture of Streptococcus thermophilus, Lactobacillus acidophilus and Lactobacillus casei. General compositions of loquat extract, changes of viable cell count, pH and titratable acidity during fermentation were determined. Chemical compositions of loquat extract were 91.5% moisture, 0.2% crude ash, 8.6$^{\circ}$ Brix soluble sugar, 0.34% total acid, and 4.11 in pH. Supplementation of loquat extract stimulated acid roduction and growth of lactic acid bacteria. Among supplementation levels, a group that was fermented by a single culture of Str. thermophilus with 10% loquat extract was shown the highest viable cell count (2.10${\times}$10$\^$9/ CFU/mL) at 12 hours after inoculation. When loquat extract was added to reconstituted skim milk at the level of 10%, all mixed cultures of lactic acid bacteria showed higher acid production and the number of viable cell count than 3 kinds of single cultures. Especially, the growth of mixed culture of Str. thermophilus and Lac. acidophilus was promoted by the addition of 10% loquat extract. Therefore, it was suggested to manufacture the yoghurt with the addition of 10% loquat extract and the inoculation of mixed culture of Str. thermophilus and Lac. acidophilus for on the stimulation of growth of the lactic culture.
