Polymyositis is a disorder of unknown etiology characterized by an inflammatory destruction of striated skeletal muscles, which dominated by lymphocytic infiltration. But two major etiologic possibilities exist: a viral infection and an abnormal recognition of self by the immune system of the body. Recently, we experienced a case of polymyositis associated with chronic persistent hepatitis who had evidence of hepatitis B virus infection. This 61-year old male patient was complaint of generalized muscle weakness, swallowing difficulty and arthralgia on multiple joints. The patient revealed characteristic findings of polymyositis on the face and the skin of both fingers, EMG and skin biopsy specimen. Also, proved to be have chronic persistent hepatitis by liver biopsy, viral markers and liver function test. So we report it with a review of the literature.

The air quality inside the cabin of an automobile is closely related to the health of those who spend a lot of time in their vehicles. Automotive indoor air quality can be improved in several ways; the most common and effective of which was to dilute the polluted air with fresh air through the vehicle"s HVAC. In this study, we considered switching between the outside air mode and the recirculation mode of the automotive HVAC, and how that change affected the concentration of pollution within the vehicle. The possible range between the outside air mode and the recirculation mode of the vehicle"s HVAC obtained by this experiment was applied to the CFD analysis. Furthermore, the actual vehicles were infused with gas over time and were compared for the purposes of feasibility review of numerical simulation and concentration trends of experimental data. In the CFD analysis, UDF(user defined function) was applied to our code in order to decipher the range of outside air and recirculation mode of the HVAC. Data generated through the full range between outside air and recirculation mode of the vehicle"s HVAC were analyzed under a variety of conditions which allowed the identification of characteristics which changed indoor air quality source concentration.

Background/Aims: Pelvic floor location and mobility vary according to the defecation posture. Authors investigated possible effects of the change of defecation posture on the patients with pelvic outlet obstructive constipation among patients with chronic idiopathic constipation. Methods: Subjects included in the study were 32 patients with pelvic outlet obstructive constipation (24 spastic pelvic floor syndrome, 8 perineal descent syndrome). From all the subjects, defecographies of both resting and straining phases were obtained for three different postures (sitting, leg elevating and squatting). Results: In the patients with spastic pelvic floor syndrome, the anorectal angles of straining phase in squatting position were 117.93±3.58캻nd 118.2±4.94?respectively. These values were significantly greater than those measured in sitting position (resting 93.75±6.74?straining 95.17±7.77?and leg elevating position (resting 99.42±4.79?straining 98.67±5.76?(p<0.01, p<0.01, respectively). In addition, in the patients with perineal descent syndrome, perineal descent in squatting position revealed no significant differences compared with sitting and leg elevating positions. Conclusions: In patients with spastic pelvic floor syndrome, squatting position increases the anorectal angle. Although further studies are needed to support our ideas, this study demonstrates that patients with spastic pelvic floor syndrome can have benefit from the Korean traditional defecation posture by increasing the anorectal angle. Acid ingestion causes a coagulative necrosis of the surface epithelium of the upper gastromtestinal tract. Its late sequence is luminal stenosis which frequently requires surgical repair. Nevertherless, in many cases, non-operative dilatation of luminal stenosis have been applicated, such as bouginations, balloon dilatations, endoscopic laser therapy, endoscopic electrocoagulation, endescopic microwave coagulation. These methods may give some considerable benefits in unoperable cases. Recently, we experienced a case of a 60-year-old man who had severe corrosive esophageal and gastric angular strictures by accidentally ingested hydrochloric acid and after many tiems of application of non-operative dilatation was able to have normal diet without dysphagia. So, we report this case with a review of literatures.
A Case of Trichilemmal Carcinoma

We report a case of trichilemmal carcinoma in a 79-year-old woman who presented an atrophic scar remaining after excision of a recurrent nodule on the right mandibular area. Her past history revealed that there had been a painful pea-sized brownish nodule on the same site for two years. Eleven months before presentation, it Lead been excised but recurred 9 months later. Histopathologic findings showed a clear cell neoplasm with trichilemmal keratinization. The tumor cells showed PAS-positive cytoplasm and cytologic atypia with a few mitotic figures. Imnoembryonic staining for high molecular weight, cytokeratin was positive but carcinoembryonic antigens and epithelial membrane antigens were all negative. To our knowledge this is the first report of trichilemmal carcinoma in Korea.
