        구상돌기절제술을 함께 시행한 코경유 눈물주머니코안연결술의 임상적 고찰

        Purpose: To evaluate the effects of uncinectomy for osteotomy method during endonasal dacryocystorhinostomy (DCR). Methods: This retrospective review study comprised 102 nasolacrimal duct obstruction patients that underwent endonasal dacryocystorhinostomy with uncinectomy at our hospital between 2003 and 2006. The patients were classified into two groups based on records of preoperative endoscopic examination. Group A consisted of patients who had no nasal cavity abnormality, and Group B comprised patients who had a nasal cavity abnormality but received no treatment. Results: Sixty-four patients were included in Group B. The success rates of endonasal DCR were 89.5% in Group A and 90.6% in Group B, with no statistically significant difference between the two groups (p>0.99). There were no cases of mucosal burn, orbital fat prolapse, spinal fluid leak, maxillary sinusitis, or delayed bleeding. Conclusions: Anterior resection of the uncinate process is the most important surgical step to expose the medial aspect of the lacrimal fossa and to form the precise location of osteotomy during endonasal dacryocystorhinostomy.         만성 중심장액맥락망막병증 환자에서 짧은 시간의 광역학치료의 효과

        Purpose: To evaluate the clinical results of short duration photodynamic therapy (PDT) using verteporfin in patients with chronic central serous chorioretinopathy (CSC). Methods: We retrospectively examined 15 eyes after PDT for chronic central serous chorioretinopathy. PDT for 83 seconds was evaluated for 8 eyes, and PDT for 60 seconds was evaluated for 7 eyes. Improvement in vision and changes in central macular thickness were evaluated with an optical coherence tomogram (OCT), and improvement in leaking was evaluated with a fluorescein angiogram (FA). Recurrence was also studied. Results: Visual acuity improved from 0.39 to 0.57 (p=0.04), and the mean central macular thickness decreased from 426.29 µm to 184.71 µm (p<0.01) in 7 eyes treated with PDT for 60 seconds. There was no significant change in visual acuity between the two groups (p=0.77) or in central macular thickness (p=0.52). Central macular exudation resolved completely in all eyes. After PDT, there was no recurrence during the follow-up period. Conclusions: Short duration PDT with verteporfin appears to be a beneficial treatment option for patients with chronic CSC.         홍채각막내피증후군 환자에서 섬유주절제술과 안내밸브삽입술 후 치료 성적 비교

        Purpose: To compare surgical outcomes and corneal endothelial cell densities in patients with glaucoma secondary to iridocorneal endothelial (ICE) syndrome who underwent trabeculectomy with mitomycin-C (MMC) versus those who underwent Ahmed Glaucoma Valve (AGV) implantation. Methods: Twenty-one patients with ICE syndrome who underwent either trabeculectomy with MMC or AGV implantation were included in this study. All patients had more than 12 months of follow-up after glaucoma surgery. Intraocular pressure (IOP), visual acuity (VA) and corneal endothelial cell density were compared according to the method of surgery. Results: Ten eyes had undergone a trabeculectomy with MMC, and 11 eyes had an AGV implantation. In eyes that underwent a trabeculectomy with a MMC, there were no statistically significant differences between preoperative and postoperative month 12 in IOP, VA or corneal endothelial cell density. In eyes that underwent AGV implantation, postoperative IOP, VA and corneal endothelial cell density were statistically significantly lower than the preoperative values. Conclusions: When determining the method of glaucoma surgery in patients with ICE syndrome, the additional procedures needed to maintain the surgical success of trabeculectomy with MMC and the corneal decompensation which is caused by a decrease in corneal endothelial cell density after AGV implantation should be considered.         생체적출안구에서 외안근의 해부학적 계측과 황반 및 시신경과의 관계

        Purpose: To make an anatomical evaluation of extraocular muscles, macular and optic nerve in enucleated eyes of live subjects and to compare the results with a previous autopsy study. Methods: Twenty-four surgically enucleated eyes were studied. The distance from the limbus to the muscle insertion site, muscle width, and the distance between muscle insertion sites were measured and compared with an Apt study. After an incision through the eyeball, a 26-gauge needle was passed perpendicularly through the macula to the sclera. We measured the distances between the oblique muscles and the macula and optic nerves from an external site of the eyeball. Results: The distance from the limbus to the muscle insertion site showed a significant difference in the superior and inferior rectus muscle. All measurements of muscle widths were significantly narrower than those of autopsy studies. The distances between the muscles were significantly closer between the lateral and superior muscles and between the superior and medial rectus muscles. From the optic nerve to the muscle was 3.99±0.62 mm, to the superior oblique muscle was 7.89±0.88 mm, to the inferior oblique muscle was 5.95±0.83 mm, and the macula to the inferior oblique muscle was 1.35±0.42 mm. Conclusions: Since most of the measurements were different from those of autopsy studies, we should consider the effects of preservation and fixation solution on autopsy eyes, race, and the differences among measurement instruments.         유아의 자아탄력성과 또래 놀이행동과의 관계

        본 연구의 목적은 유아의 성과 연령에 따른 자아탄력성 및 또래 놀이행동의 차이를 확인하고, 자아탄력성과 또래 놀이행동과의 관계를 알아보는 것이다. 이를 위해 서울에 위치한 어린이집에 재원 중인 만 3,4,5세 유아 305명을 대상으로 자아탄력성과 또래 놀이행동에 대한 검사를 실시하였다. 수집된 자료는 이원변량분석 및 상관관계 분석을 하였으며, 결과는 다음과 같다. 성과 연령에 따라 유아의 자아탄력성 및 또래 놀이행동에는 차이가 있었다. 또한 자아탄력성은 또래 놀이행동 중 놀이방해, 놀이단절과는 비교적 높은 부적상관이 있었으며, 놀이상호작용과는 높은 정적상관이 있었다. 자아탄력성이 높은 유아들이 또래들과 긍정적인 상호작용을 하는 것으로 나타난 본 연구의 결과는 또래와의 긍정적인 놀이가 가능하도록 유아 개개인의 자아탄력성을 증진시킬 필요가 있음을 시사한다. This study investigated the differences in ego-resiliency and peer play behaviors according to children`s age and gender, and the relationship between ego-resiliency and peer play behaviors of young children. Subjects of this study consisted of 305 children of 3 to 5 years of age who were in day care centers in Seoul. Children`s ego-resiliency was measured by DECA(Devereux Early Childhood Assessment), developed by LeBuffe and Naglieri(1999) and adapted by Koo(2010). For peer play, the Korean Version(Choi & Shin, 2008) of PIPPS(Penn Interactive Peer Play Scale; Fantuzzo et al., 1998) was used. The results of this study were as follows: There were significant differences in ego-resiliency and peer play behaviors according to children`s age and gender. And there were significant correlations between ego-resiliency and peer play behaviors. Specifically, the ego-resiliency was negatively correlated with play-disruption and play-disconnection of the peer play behaviors while positively correlated with play-interaction. It suggested that stimulating individual ego-resiliency makes children play with peers positively, which in turn has an influence on the social, emotional, and cognitive development of young children.

        Purpose: Purpose of the study has significance systematic phonological awareness intervention program investigates effect of phonological awareness ability, word recognition ability and speech sound production of child with mild intellectual disabilities and suggest evidence-based program that phonological awareness ability and basic reading ability improvement of child with intellectual disabilities. Methods: It is select to two children with mild intellectual disabilities - aged from full 6 to 8 - and syllable and phoneme level of counting, discrimination, synthesis, elimination, and confrontation task composing level of audio visual, visual, and audio stimulation accomplished one on one between researcher and child 50minutes of intervention in four sessions per week. After intervention, check degree of improvement accomplishing every periods phonological awareness evaluation and examining for word recognition and speech sound production compared and analyzed accomplishing pretest-posttest examination. Results: Research result show that systematic phonological awareness intervention program is improve syllable and phoneme level of phonological awareness ability and word recognition ability and influence speech sound production. Conclusions: Syllable and phoneme level phonological awareness show that result influence speech sound error of mild intellectual disabilities child that skip decrease and accurate phoneme production.

        감각사시의 편위 방향과 관련된 임상적 특징들

        Purpose: To assess the clinical features associated with the direction of deviation in sensory strabismus and postoperative stability of strabismus angles. Methods: The authors retrospectively reviewed the medical records of 98 patients diagnosed with sensory strabismus who were treated surgically. Among the factors analyzed were sex, age, direction of strabismus, preoperative visual acuity, age at onset of visual loss, cause of primary sensory deficit, refractive error, age at operation, deviation angle at 1 and 6 months postoperatively and after at least 5 years. Results: At baseline, 20 patients (20.4%) had esotropia and 78 (79.6%) had exotropia. Common causes of primary sensory deficit were corneal opacity (25.5%), vitreoretinal disorder (23.5%), and congenital cataracts (16.3%). The proportion of esotropia increased significantly as the refractive error of the sound eye became more hyperopic. The older the patients were when they lost their vision, the higher the proportion of exotropia. Patients with longer duration of vision loss were more likely to have a larger deviation angle with exotropia, but the association was not significant with esotropia. Conclusions: Patients with sensory strabismus tend to develop exotropia. Refractive error of the sound eye and age at onset of vision loss were associated with the direction of strabismus. Despite poor sensory fusion even after surgery, more than 50% of patients maintained a stable eye position. J Korean Ophthalmol Soc 2012;53(8):1138-1142

        안와에서 발생한 원발성 활액막 육종 1예

        Purpose: To report a rare case of primary orbital synovial sarcoma. Case summary: A 55-year-old male visited our clinic with a 2-month history of decreased visual field in the left eye. The best corrected visual acuity of the left eye was 1.0, and 2.5 mm of proptosis was observed. There was slight limitations in motility and a mild visual field defect in the inferotemporal quadrant of the left eye. Funduscopic examination of the left eye revealed papilledema. Orbital magnetic resonance imaging (MRI) demonstrated the presence of an orbital mass that extended inferotemporally into the left orbit. On magnetic resonance imaging, a well-demarcated and 2.7 × 2.0 × 2.2 cm sized tumor was identified with intermediate signal intensities on T1/T2-weighted scans with slight homogenous enhancement. We approached the orbital mass using a lateral conjunctival incision with left canthotomy and cantholysis and performed partial surgical resection of the mass because the mass encircled the optic nerve. The mass was diagnosed as synovial sarcoma based on the findings of both histopathology and immunohistochemical staining. Postoperatively, the patient underwent radiation therapy to the rest of the tumor site. Conclusions: Synovial sarcoma should be considered in the differential diagnosis of an orbital mass lesion encircling the optic nerve. Radiation therapy is useful to treat completely unresectable primary synovial sarcoma. J Korean Ophthalmol Soc 2012;53(5):716-720

        Flavor Components in the Bellflower Roots (Platycodon glaucum Nakai)

        도라지 뿌리의 향기성분을 규명하고자 도라지 뿌리의 향기성분을 상압 수증기 증류법으로 포집하여 중성, 염기성, 약산성 및 산성부분으로 분획하였다. Diazomethane 법으로 methyl ester화한 산성부분을 위시한 4부분은 모세관 column을 장치한 GC 및 GC-MS로 분석하였으며 함황화합물은 FPD 검출기로써 검출하였다. 도라지 뿌리로부터 aliphatic hydrocarbon류 6종, aromatic hydrocarbon류 10종, terpene hydrocarbon류 2종, alcohol류 12종, terpene alcohol류 8종, aldehyde류 17종, terpene aldehyde를 3종, ketone류 5종, ester류 5종, furan류 3종, thiazole류 2종, lactone류 2종, sulfide류 2종, phenol류 9종, 산류 12종 및 기타 5종을 위시하여 총 103종의 향기성분이 동정되었다. Carboxylic acid를 제외한 향기성분은 대부분 중성부분에서 동정되었고, 관능검사의 결과에서도 중성부분이 도라지 뿌리의 향기를 재현하는데 필요불가결한 것으로 나타났다. 관능검사의 결과, 중성부분에서 동정된 1-hexanal, trans-2-hexenal, 1-hexanol, cis-3-hexenol, trans-2-hexenol, 1-octenen-3-ol등은 도라지 뿌리의 풋냄새의 주성분인 것으로 생각된다. Flavor components were trapped by stimultaneous steam distillation-extraction method for investigating it in the bellflower roots and fractionated into four groups such as a neutral, a basic, a phenolic and an acidic fraction. An acidic fraction methylated with diazomethane solution and three others were analyzed by GC and GC-MS equipping a fused silica capillary column, and S-containing compounds in these were detected with a flame photometric detector (FPD). The total of one hundred and three compounds from the bellflower roots were identified: they were 6 aliphatic hydrocarbons, 10 aromatic hydrocarbons, 2 terpene hydrocarbons, 12 alcohols, 8 terpene alcohols, 17 aldehydes, 3 terpene aldehydes, 5 ketones, 5 esters, 3 furans, 2 thiazoles, 2 lactones, 2 sulfides, 9 phenols, 12 carboxylic acids and 5 others. The greater part of the others except carboxylic acids were identified from a neutral fraction of which was assumed to be indispensable for the reproduction of bellflower root odor in a sensory evaluation. As a result of a sensory evaluation. l-hexanal, trans-2-hexenal, 1-hexanol, cis-3-hexenol, trans-2-hexenol. l-octen-3-o1 and so forth identified in a neutral fraction were considered to be the key compounds of grass-like odor in the bellflower roots.
