흉부 외상에서 폐좌상(Pulmonary contusion)과 외상성 폐낭종 (Traumatic Lung cyst)을 동반한 환자의 임상경과 및 예후 예측 인자

Purpose: A traumatic lung cyst (TLC) is a rare complication and is usually detected with a pulmonary contusion. This study attempted to identify the prognostic factors and the clinical characteristics for pulmonary contusion with TLCs. Methods: We retrospectively reviewed the medical records and chest CT findings of 71 TLC patients who visited our hospital from January 2006 to December 2007. Patients were assessed for any clinical characteristics. We evaluated significant differences between the survival and the death groups for patients with a traumatic lung cyst. Results: The male-to-female ratio of patients with TLCs was 54:17, and the mean age of the patients was 37.70±19.78 years with 36.6% of the patients being under 30 years forage. The cause of blunt thoracic trauma was mainly pedestrian traffic accidents (26.8%) and falls (25.4%). Associated conditions included pulmonary contusion in 68 patients (95.7%), hemopneumothorax in 63 patients (88.7%), and rib fracture in 52 patitents (73.2%). There was no consistent relationship between the number of TLCs and the pulmonary contusion score. The overall mortality rate of TLC patients was 26.8%. Death correlated with a need for ventilatory assistance, mean arterial pressure, worst mean arterial pressure in 24 hours, initial pH and base excess, worst pH and base excess in 24 hours, refractory shock, initial GCS score, and pulmonary contusion score. Conclusion: The presence of the aforementioned predictors indicate serious injury, which is the main determinant of the outcome for thoracic injuries with TLCs. Purpose: This hospital has operated a trauma system of the inclusive trauma system under the sponsorship of this hospital and with financial support from the government from 2011, and it has been designated as a specialized trauma center (candidate) since November 2008. Therefore, this emergency medical center evaluated the influence of the inclusive trauma system on the course of healing and on the results for trauma patients within the region. Methods: The medical records of all patients who were registered as trauma patients from among those who visited the emergency medical center of this hospital from April 2009 to May 2012 were retrospectively reviewed. The monthly and the annual averages of important indices, such as the time in the emergency room and preventable mortalities, were calculated, and patterns of change were sought. The preventable mortality rate was calculated by using the Trauma Injury Severity Score (TRISS) for each patient. Results: The total number of patients registered from April 2009 to May 2012 was 601, and male patients accounted for a larger proportion(432 males(71.88%) vs. 169 females(28.12%)). Their average age was 46.2 years, the average Revised Trauma Score (RTS) was 5.74 points, and the average Injury Severity Score (ISS) was 26.99 points. The preventable mortality rate during the entire period, which was calculated using the TRISS, appeared lower than the preventable mortality rates reported in past studies in the Republic of Korea. Conclusion: These results for the operation of a new trauma system are limited in that they are only for a local private university hospital. However, results show greater changes and developments in and out of the hospital due to multilateral endeavors by the trauma team and the hospital. These endeavors include increased communications among the departments and development of a complementary patient registration system.

This study investigated the cases of the domestic classification system and compared and analyzed them with the academic standard classification system to derive future improvement directions. The direction of future improvement of the academic standard classification system presented based on this is as follows. First, it seems necessary to clearly guarantee the operation of the classification system as a law for the continuous development of the academic standard classification system. Second, it is necessary to improve it to a comprehensive classification principle that satisfies both current issues and global universality so that domestic and foreign data can be collected and compared smoothly by producing a wide-ranging classification system. Third, it is necessary to select a clear revision cycle of the academic standard classification system, and it seems appropriate to proceed with the revision every five years in order to reflect the academic field across a vast field. Currently, research on such a domestic classification system is insufficient, and such investigations are continuously conducted in the future, requiring continuous interest and research on the domestic classification system.
학술 활동 건전성 제고를 위한 부실의심 학술활동 근절 방안 연구

본 연구에서는 국내외 부실의심 학술활동 사례조사와 설문 및 면담 조사를 기반으로 부실의심 학술활동 방지를 위한 사전예방절차 및 대처방안을 제시하고자 하였다. 첫째, 부실의심 학술활동 예방을 위한 대학(원)생 및 신진연구자 전문교육 실시방안, 대학 교원 및 연구책임자 전문교육 실시방안, 세미나/워크숍/콜로키움 실시방안, 혼용 교육프로그램 실시방안을 제시하였다. 둘째, 부실의심 학술활동을 예방하고 투명하고 건전한 학술활동 생태계를 조성하기 위해 연구자들이 논문 투고 및 학술활동 참석 시 점검할 수 있는 체크리스트를 제시하였다. 셋째, 부실의심 학술활동 예방을 위한 근절 방안에 대한 절차, 대처 방안, 예방 문화 조성 방안을 제시하였다. 마지막으로, 연구자들이 건전한 학술활동을 수행할 수 있도록 하기 위한 도서관의 부실의심 학술활동 점검 서비스 강화 방안 및 학술지 정보 및 오픈액세스 활용 서비스 강화 방안을 제시하였다. In this study, the authors tried to suggest preventive procedures and countermeasures to prevent suspicious academic activities based on case studies, questionnaires, and interviews of domestic and foreign suspicious academic activities. First, a plan for conducting professional education for undergraduate (graduate) students and young researchers, a plan for conducting professional education for university teachers and directors of research, a plan for conducting seminars/workshops/colloquiums, and a plan for implementing mixed education programs were presented to prevent suspicious academic activities. Second, in order to prevent suspicious academic activities and create a transparent and healthy academic activity ecosystem, a checklist that researchers can check when submitting papers and attending academic activities was presented. Third, procedures for eradicating measures to prevent suspicious academic activities, countermeasures, and prevention culture were suggested. Finally, a plan to provide library services was presented to enable researchers to conduct sound academic activities. A plan to strengthen the librarys suspicious academic activity inspection service and a plan to strengthen the service for using academic journal information and open access were suggested.
