      • 고전압맥동전류가 통각과민 백서의 통각역치 및 유해성 굴곡반사에 미치는 영향

        Purpose: This study conducted quantitative sensory test and nociceptive flexion reflex(NFR) measurement to examine degree of pain depending on polarity of high voltage pulsed current(HVPC) of hyperalgesia site in hyperalgesia rat by local thermal injury. mechanical pain threshold, thermal pain threshold and root mean square of NFR were measured. Methods: This study was conducted with control group I of hyperalgesia rat at hind paw by thermal injury and experimental groups divided into cathodal HVPC group II, anodal HVPC group III and alternate HVPC group IV. It measured pain threshold and root mean square(RMS) of NFR and obtained the following results. Results: Mechanical pain threshold of hyperalgeisa site was significantly increased at groups II, III and IV applying HVPC group compared to control group, but there was no difference among HVPC groups. Thermal pain threshold of hyperalgesia site showed a significant increase in group II. Group III showed significant difference after 4 days of hyperalgesia. RMS of NFR at hyperalgeisa site was significantly reduced in group II after 2 days of hyperalgesia. Group III showed significant decrease after 5 and 6 days of hyperalgesia. Conclusion: Consequently it was found that application of HVPC of hyperalgesia site increased pain threshold at hyperalgesia site by mechanical stimuli and thermal stimuli. NFR by electrical stimuli was similar to pain threshold by mechanical stimuli. Effects by polarity of HVPC showed the greatest reduction of hyperalgesia when cathodal electrode was used.

        The Effects of Hand Function on the Angle of Holding a Smartphone

        본 연구는 스마트 폰 사용 시 팔의 지지 각도에 따라서 엄지손가락과 나머지 손가락의 기능에 미치는 영향을 알아보고자 하였다. 이 연구는 2014년 4월 21일부터 27일 까지 진행되었다. 연구 대상자는 21명의 건강한 성인을 대상으로 하였으며, 총 3그룹으로 나누어 진행하였다. 지지대 없이 90도의 각도를 유지하고 스마트 폰 타자를 실행한 그룹, 슬링을 이용하여 90도 각도를 유지하고 스마트 폰 타자를 실행한 그룹, 슬링을 이용하여 120도 각도를 유지하고 스마트 폰 타자를 실행한 그룹으로 나누었으며, 각 그룹은 7명씩 나누었다. 손의 통증을 측정하기 위하여 핀켈스타인 검사를 하였으며, 시각 통증 척도를 이용하여 결과를 나타내었다. 또한 손의 악력과 엄지손가락의 핀치 그립을 측정하였다. 각 측정은 왼손과 오른손을 측정하였으며, 시작 전, 30분, 1시간, 1시간30분으로 총 4번 측정하였다. 본 연구의 결과는 왼손과 오른손의 통증, 악력, 핀치 그립의 시기에서 유의한 차이를 보였으며(p<.05), 오른손의 악력에서는 시기와 군의 교호작용에서 유의한 차이를 보였다(p<.05). 이 결과로 인하여 팔의 지지는 손의 스트레스를 줄여 손의 기능에 도움을 준다고 생각된다. The purpose of the study is to prevent pain and malfunction of the hand by correcting the angle of holding a smartphone. Subjects comprised of 21 healthy students. This research was conducted from April 21 2014 to April 27 2014. The three groups were as follows: first group included 7 students who held the phone at an angle of 90 degrees without a sling, and second group included 7 students who held the smartphone at an angle of 90 degrees with a sling; and the third group included 7 students who held the smartphone at an angle of 120 degrees with a sling. VAS was measured through the Finkelstein Test. Also, pain rating and muscle strength were assessed four times 30 minutes before and after the experiment for one and a half hour with pinch grip, dynamometer, and visual analogue scale. Smartphone was used. In all of the measurements, period showed a significant difference (p<.05) between both sides and a significant difference was not found between the group. In the time × group, pain score and grip strength were significantly different in the right side only (p<.05). According to the results of this experiment, providing support to the hand helps to increase the hand function by reducing the stress.

        백서의 적출창상에 대한 피하전기자극이 창상수축에 미치는 효과

        Purpose: This study investigated the effect of needle electrode stimulation at various frequencies on change in wound healing in excision wound rats Methods: Twenty-four Sprague-Dawley adult male rats were assigned to one of four groups: control (n=6), acupuncture group (n=6), low rate (2 Hz) percutaneous electrical stimulation group, high rate (100 Hz) percutaneous electrical stimulation group (n=6). We analyzed morphological effects by measuring the area of the excision wound, the contraction rate, and chromatic red. A digital camera and an image analysis program were used to measure and analyze the wound area,which was also used for the contraction rate. Chromatic red was obtained by calculating red, green, and blue (RGB) values of the wound area. Results: The electro acupuncture stimulation groupsshowed significant healing effects compared to the control and acupuncture groups. Conclusion: The results of this study showed that various frequencies of percutaneous electrical stimulation have a therapeutic effect on wound healing.

        정상근과 근막 유발점이 활성화된 근육의 초음파 영상의 비교

        Purpose: The objective of this study was to offer primary clinical data examining whether change of imaging structure and quantitative evaluation of muscle activity on myofascial trigger points can lead to implementation of an analytical technique for evaluation of myofascial pain diagnoses. In addition, we examined the effect of a variety of mediation techniques, in order to examine neuromuscular physiological characteristics of myofascial trigger points muscle by comparing differences in pressure pain threshold and ultrasound imaging. Methods: Participants in the study included 30 adults in their twenties. The subjects were divided into the normal and myofascial trigger points groups. Clinical outcomes were evaluated by pressure pain threshold for pain and ultrasound imaging was performed for evaluation of the structural characteristics of muscle. Independent t-test was used for statistical analysis. Results: The two groups showed statistical significance in the change in pressure pain threshold (p<0.05). Findings of ultrasound imaging analysis showed no significant differences, increased muscle thickness was observed (p>0.05). Findings of ultrasound imaging analysis showed significant differences, increased muscle echodensity was observed (p<0.05). Findings on ultrasound imaging analysis showed significant differences, increased muscle white area index was observed (p<0.05). Conclusion: From these results, active myofascial trigger points muscle showed quality deterioration on ultrasound imaging. Thorough evaluation of imaging structure and physiological characteristics can be useful quantitative analytical techniques for diagnosis of myofascial pain syndrome and a primary factor reflected in physical therapy intervention.

        양극 경두개직류자극에 의한 운동관련피질전위의 변화

        Purpose: Transcranial direct current stimulation (tDCS) is a useful method for modulating the brain activity. This study compared the effect of continuous and interrupted tDCS using the change in the movement related cortical potential. Methods: Thirty healthy participants (male: 18 and female: 12) were assigned randomly to three groups; sham tDCS, continuous tDCS, which the current continuously flowed for 10 minutes, and interrupted tDCS, which the interrupted current flowed for 10 minutes (repetition: 4sec stimulation and 5sec rest) at an intensity of 1mA with anodal polarity. The effect of tDCS on the right primary motor area was measured from the movement related cortical potential (MRCP) before and after the experiment. MRCP consisted of the bereitshaftspotential (BP) and negative slope potential (NS) at Cz and C4. Results: Continuous and interrupted tDCS showed a significant difference in the changes in the BP, NS at Cz and C4 compared to the sham tDCS. However, there was no significant difference between the continuous tDCS and interrupted tDCS. Conclusion: The change in cortical activity by continuous and interrupted tDCS results from an improvement in the MRCP. An interrupted tDCS may be a safe and useful modality for stimulating the cortical region.

        뇌졸중 환자와 정상인의 근 수축 유형에 따른 피질 신경원의 활성도 비교

        Purpose: In order to examine difference in the activity of potential of spinal neurons and cortical neurons according to muscle contraction type in post stroke hemiplegic subjects and healthy subjects, the present study conducted an experiment as follows. Methods: The subjects in the experimental group were 17 left-side hemiplegic subjects (9 female, 8 male; mean age, 63.41$\pm$9.86 years) with the right hand as the dominant hand selected among post stroke hemiplegic subjects, and 17 age matched healthy control subjects (10 female, 7 male; mean age, 64.12$\pm$12.07 years). Movement-related cortical potentials (MRCPs) were measured using surface electromyography and electroencephalography while concentric and eccentric movements were made alternately. Results: As to the pattern of the activity of MRCP, which indicates the activity of motor cortical neurons, we found that the amplitude is high (p<0.01), the length of excitement is short (p<0.01) and the ascending gradient of amplitude to the peak increases (p<0.05) in post stroke hemiplegic subjects' lesion sites different from healthy subjects. Conclusion: The activity of cortical neurons was no difference in activity according to contraction type was observed in post stroke hemiplegic subjects' lesion sites. This suggests that there is no distinction in the activity of cortical neurons between concentric contraction and eccentric contraction. Accordingly, if post stroke hemiplegic subjects' activity of motor cortex is analyzed by measuring MRCPs, it is considered useful in research on neural plasticity or as a ground of clinical effects in the area of physical therapy for the central nervous system.

        탄력밴드저항운동이 노인의 균형 능력 향상에 미치는 영향

        Purpose: This study examines the effects of elastic band of resistance exercise for balance control of the elderly. Methods: Thirty of eighty participants in experiment subjects who demonstrated balance-impairment through the use of primary screening tests including the one leg standing test (OLST), functional reach test (FRT) and timed up and go (TUG) were selected as subjects. Fifteen subjects that underwent muscle-strengthening exercise using an elastic band were selected as the exercise group and fifteen subjects were selected as a control group. Subjects undertook a home-based exercise program three times per week for 9 weeks. Muscle strength, functional assessment and a balance test were quantitatively measured before and after the exercise regimen. Results: After muscle strengthening exercises, changes in maximal voluntary isometric contraction (MVIC) showed a significant increase in all of the lower extremity muscles of the exercise group subjects. There were statistically significant differences between the exercise and control groups for changes in the OLST, FRT and TUG, which are functional assessments of balance ability, and changes of the unit path length and circumference area, measurement items of quantitative analysis. In addition, from examining correlations between MVIC, balance ability, it was found that an increase of muscle strength in the hip joint group of muscles among the lower extremity muscles had greater improvement in correlation with balance ability in this elderly population. Conclusion: Resistance exercise using elastic bands had significant effects on muscle strengthening in elderly subjects, with a resultant increase of lower extremity muscle strength with significant improvement of balance ability.

        Changes in the Spinal Motor Neuron Excitability Depending on Postural Changes in Post Stoke Hemiplegics

        Purpose: The purpose of this study was to measure changes in the H-reflex and V wave under loading conditions (e.g. prone and standing position) and to investigate whether postural change would affect the H-reflex and V wave in post stroke hemiplegic patients. Methods: Thirty persons with hemiplegia resulting from stroke (20 males, 10 females) participated in this study. Electromyography (EMG) was used to electrically stimulate and record the soleus H-reflexes and V waves under various loading conditions. The normality of the distribution of each variable (H latency, $H_{max}/M_{max}$ ratio, $V_{max}/M_{max}$ ratio) was tested using the Kolmogorov-Smirnov test. The means of normally distributed continuous data were assessed by independent t-test (${\alpha}$=0.05). Results: There were statistically significant differences in $H_{max}/M_{max}$ ratio (p<0.01), $V_{max}/M_{max}$ ratio (p<0.01), H latency (p<0.01) among the prone and standing position. Conclusion: We found that the H-reflex and V wave in standing position was more active to weight bearing load than prone position.
