        건강한 청년의 단시간 공해노출과 일회성 운동이 폐기능과 생리반응에 미치는 영향

        This study examined whether an acute exercise during a short-term exposure to a polluted environment affected pulmonary function and physiological responses in healthy young adults. Thirteen men (mean = 22.4 yrs) were randomly exposed to a polluted air (PA) or to clean air (CA) in a chamber. During the exposure, they rested for 30 min followed by a bench stepping exercise (40 cm height, 120 bpm) for 10 min. And they recovered from the exercise for 15 min in the chamber. The pulmonary function, heart rate, blood lactate, and blood pressure were measured during the exposure. At the end of 30 min resting, peak expiratory flow rate (PEFR) was higher in PA than CA. Immediately after the exercise, forced expiratory volume at 1 sec (FEV1) was higher in PA than CA. At 15th min of recovery, forced vital capacity, FEV1, and PEFR were significantly higher in PA than CA. No differences in cardiovascular and blood lactate responses were noticed. The results suggest that an acute intensive physical work out with breathing of volatile organic compounds induced changes of pulmonary function but not physiological adjustments.

        Megavoltage Cone-beam CT 영상의 변환을 이용한 변환 영상 정합의 정확도 향상

        적응 방사선 치료(Adaptive Radiation Therapy, ART)를 실행하기 위한 고도의 정확성을 갖는 변형 영상 정합 방법은 필수이다. 본 연구의 목적은 Megavoltage cone-beam CT (MV CBCT)영상의 Intensity 변화를 통한 영상 정합의 정확성의 향상이다. Intensity 변화 값을 도출 하기 위해 kilovoltage CT (kV CT)와 MV CBCT를 이용하여 12 종류의 전자 밀도 바를 제공하는 Cheese 팬텀의영상을 획득하고, 영상들로부터 kV CT와 MV CBCT의 Hounsfield Unit (HU)값들의 관계를 도출하였다. MV CBCT 영상의 잡음을 감소하기 위해 Gaussian smoothing 필터를 적용하였다. MV CBCT영상의 intensity는 마치 동일한 모달리티에서 획득된 영상과 같은 kV CT와 동일한 범위의 intensity로 변화시켰다. 이후 두 영상에 효율적이고 사용하기 쉬운 intensity 기반의 데몬 영상 정합이 적용되었다. 본 연구실에서 인체 내 폐를 모사하도록 제작된 변형 폐 팬텀을 이용하여 위와 같은 방법을 적용하여 영상 정합을 하였다. Cheese 팬텀 영상, 변형 폐 팬텀 영상을 이용한 변형영상 정합 결과는 상관 계수가 각각 6.07%, 18% 향상되었다. 변형 폐 팬텀 영상의 변형 영상 정합 정확성을 평가하기 위해 추가적으로 측정된 팬텀 내부에 삽입한 표적의 중심 좌표를 이용하여vector 차이를 계산하였다. 벡터 차이는 $2.23{\pm}1.19mm$, $1.39{\pm}0.97mm$였다. 본 연구에서 사용한 intensity 변화 방법을 통해 변형 영상 정합의 정확성이 향상됨을 확인 하였고, 본 연구는 영상 정합 정확성을 향상시키기 위한 해결 방법이 될 수 있다. 차후 연구 계획도 본 연구 내용에 의해 제안되었다. To perform the Adaptive Radiation Therapy (ART), a high degree of deformable registration accuracy is essential. The purpose of this study is to identify whether the change of MV CBCT intensity can improve registration accuracy using predefined modification level and filtering process. To obtain modification level, the cheese phantom images was acquired from both kilovoltage CT (kV CT), megavoltage cone-beam CT (MV CBCT). From the cheese phantom images, the modification level of MV CBCT was defined from the relationship between Hounsfield Units (HUs) of kV CT and MV CBCT images. 'Gaussian smoothing filter' was added to reduce the noise of the MV CBCT images. The intensity of MV CBCT image was changed to the intensity of the kV CT image to make the two images have the same intensity range as if they were obtained from the same modality. The demon deformable registration which was efficient and easy to perform the deformable registration was applied. The deformable lung phantom which was intentionally created in the laboratory to imitate the changes of the breathing period was acquired from kV CT and MV CBCT. And then the deformable lung phantom images were applied to the proposed method. As a result of deformable image registration, the similarity of the correlation coefficient was used for a quantitative evaluation of the result was increased by 6.07% in the cheese phantom, and 18% in the deformable lung phantom. For the additional evaluation of the registration of the deformable lung phantom, the centric coordinates of the mark which was inserted into the inner part of the phantom were measured to calculate the vector difference. The vector differences from the result were 2.23, 1.39 mm with/without modification of intensity of MV CBCT images, respectively. In summary, our method has quantitatively improved the accuracy of deformable registration and could be a useful solution to improve the image registration accuracy. A further study was also suggested in this paper.

        Megavoltage Cone-Beam CT 영상의 변환을 이용한 선량 계산의 정확성 향상

        적응 방사선 치료(Adaptive Radiation Therapy, ART)를 실행하기 위한 매 치료 마다 획득되는 Megavoltage cone-beam CT (MVCBCT) 영상을 이용한 재 선량 계산 과정은 필수적이다. 본 연구의 목적은 intensity 보정 방법을 적용한 MVCBCT 영상 기반의 선량 계산 결과와 kilo-voltage CT (kV CT) 영상 기반의 선량 계산 결과의 비교 및 MVCBCT 영상 기반의 선량계산 정확성의 향상이다. MVCBCT 영상의 intensity 교정을 위해 kV CT와 MVCBCT을 이용하여 12 종류의 전자밀도 바를 제공하는 Cheese 팬텀 영상을 획득하고, Cheese 팬텀 영상의 동일한 전자밀도 바에서 표현되는 kV CT 영상과 MVCBCT 영상의 intensity 관계를 도출하였다. 이후 kV CT, MVCBCT를 이용한 Rando 팬텀 영상을 획득하여 MVCBCT 영상은 3차원 강체 정합을 수행하였고 본 과정을 통해 MVCBCT 영상은 kV CT 영상과 마치 동일한 모달리티에서 획득한 영상과 같은 위치 및 intensity 분포로 변환되었고, MVCBCT 영상의 잡음을 없애기 위한 Gaussian smoothing 필터를 적용하였다. 위의 과정을 거친 MVCBCT 영상을 토대로 intensity 교정을 적용한 영상과, intensity 교정을 적용하지 않은 영상, kV CT영상을 기반으로 방사선 치료 계획 시스템을 이용한 선량 계산을 시행 하였다. 선량 계산의 결과는 선량 분포의 차이 및 Percentage difference로 평가되었다. Intensity 보정을 적용한 MVCBCT 영상의 선량 계산 결과의 경우 kV CT 영상 기반의 선량 계산 결과와의 Percentage difference가 두경부 영상의 경우 1.08%, 흉부 영상의 경우 2.44%였다. 본 연구에서 적용한 intensity 변환을 통해 MVCBCT 영상을 이용한 선량 계산의 정확성이 향상됨을 확인하였고, 본 연구 방법은 실제 선량 계산에 적용 및 사용의 편리성을 확인하였다. 차후 연구 계획도 본 연구 내용에 의해 제안되었다. The dose re-calculation process using Megavoltage cone-beam CT images is inevitable process to perform the Adaptive Radiation Therapy (ART). The purpose of this study is to improve dose re-calculation accuracy using MVCBCT images by applying intensity calibration method and three dimensional rigid body transform and filtering process. The three dimensional rigid body transform and Gaussian smoothing filtering process to MVCBCT Rando phantom images was applied to reduce image orientation error and the noise of the MVCBCT images. Then, to obtain the predefined modification level for intensity calibration, the cheese phantom images from kilo-voltage CT (kV CT), MVCBCT was acquired. From these cheese phantom images, the calibration table for MVCBCT images was defined from the relationship between Hounsfield Units (HUs) of kV CT and MVCBCT images at the same electron density plugs. The intensity of MVCBCT images from Rando phantom was calibrated using the predefined modification level as discussed above to have the intensity of the kV CT images to make the two images have the same intensity range as if they were obtained from the same modality. Finally, the dose calculation using kV CT, MVCBCT with/without intensity calibration was applied using radiation treatment planning system. As a result, the percentage difference of dose distributions between dose calculation based on kVCT and MVCBCT with intensity calibration was reduced comparing to the percentage difference of dose distribution between dose calculation based on kVCT and MVCBCT without intensity calibration. For head and neck, lung images, the percentage difference between kV CT and non-calibrated MVCBCT images was 1.08%, 2.44%, respectively. In summary, our method has quantitatively improved the accuracy of dose calculation and could be a useful solution to enhance the dose calculation accuracy using MVCBCT images.

        허약아의 수면 습관에 대한 연구

        Objectives The purpose of this study is to research on sleep habits of weak children groups using objective evaluation tools. Methods This study surveyed 108 children and adolescents of 4~10 years old who visited the department of Pediatrics of ${\bigcirc}{\bigcirc}$ Korean Medicine Hospital from June 19th, 2015 to August 28th, 2015. Weak Children Questionnaire and CSHQ (The Children's Sleep Habits Questionnaire) were used to research on sleep habits of children. We analyzed the data by using PASW Statistics 18.0 with independent t-test. Results CSHQ Score of Weak Children Group was significantly higher than CSHQ Score of Normal Children Group (p<0.05). CSHQ Score ($Mean{\pm}SD$) of Normal Children Group was $52.92{\pm}5.72$ and CSHQ Score ($Mean{\pm}SD$) of Weak Children Group was $56.23{\pm}5.66$. Conclusions Weak Children Group had more sleep habit problems than Normal Children Group.

        Sensitiv Imago<sup>TM</sup> 측정에 의한 틱 장애 환아의 검사 결과 보고

        Objectives The purpose of this study is to report test results in eight tic disorder children using Sensitiv $Imago^{TM}$. Methods Eight tic disorder children were tested using Sensitiv $Imago^{TM}$. Also, they were diagnosed with tic disorders by DSM-IV (Diagnostic and Statistical Manual of Mental Disorders, $4^{th}$ edition) and we evaluated tic disorder cases by Yale Global Tic Severity Scale (YGTSS). Results In 8 cases, 1 case was transient tic disorder, 3 cases were Tourette's disorder and 4 cases were unspecified tic disorder. In the result of Sensitiv $Imago^{TM}$, 4 cases showed the lowest score at filter ${\sharp}5^*$ and 4 cases showed the lowest score at filter ${\sharp}6^*$ in [Express Monitoring] of [Review of System Disorders of Homeostasis]. Filter #5 includes urogenital organs, liver, gallbladder, kidneys, urinary bladder and ureter and Filter #6 stands for organs of immune and respiratory systems. Conclusion We report test results in eight tic disorder children using Sensitiv $Imago^{TM}$. Further studies about the principle, repeatability, reproducibility of Sensitiv $Imago^{TM}$ are needed.

        소아·청소년의 알레르기 질환 (천식, 알레르기비염, 아토피피부염)과 기능성 위장관 질환과의 관계 (단면 조사 연구)

        Objectives The purpose of this study is to establish relationship between allergic diseases and functional gastrointestinal disorders in children and adolescents using objective criteria and questionnaires. Methods This study surveyed 237 children and adolescents who visited the department of Pediatrics of ${\bigcirc}{\bigcirc}$ Korean Medicine Hospital from September 23rd, 2013 to December 26th, 2013. The Korean version of International Study of Asthma and Allergies in Childhood (ISAAC) questionnaire and the Korean-translated Questionnaire on Pediatric Gastrointestinal Symptoms-Rome III (QPGS-Rome III) were used. We analyzed the data by using PASW Statistics 18.0 with Chi-square test, Fisher's exact test and Linear by linear association. Results There was no significant difference between prevalence of allergic diseases and functional gastrointestinal disorders. Also, there was no strong relationship between the number of allergic diseases and the functional gastrointestinal disorders. However, some parts showed significant relationships - such as asthma symptoms ever and belch; asthma symptoms last 12 months and belch; allergic rhinitis diagnosis ever and belly aches and abdominal pain around and below the belly button; allergic rhinitis treatment last 12 months and belly aches and abdominal pain around and below the belly button; and atopic dermatitis diagnosis ever and irritable bowel syndrome (respectively; p=0.046, p=0.008, p=0.004, p=0.029, p=0.035). And as the number of allergic diseases increases, the prevalence of functional gastrointestinal disorders are 19.4%, 31.3%, 41.7%, 31.0% respectively. Conclusions Although there was no significant relationship between allergic diseases and functional gastrointestinal disorders, some gastrointestinal symptoms were related to allergic diseases.

        천식 증상을 가진 변형 협심증 환자에 대한 한방치료 증례보고

        Objective: The purpose of this case study was to report the symptom improvement effect of integrative Korean medicine treatment based on Pyongsim Solution: Cheonwangbosim-dan in a patient with variant angina and asthma symptoms. Methods: This 68-year-old male patient was treated with Pyongsim Solution, Gamihyangsayukgunja-tang, Gwakhyangjunggi-san, acupuncture, pharmacopuncture, and cupping therapy for 24 days. The European Quality of Life-5 Dimensions-5 Levels profile, Numerical Rating Scale score, and pulse rate per minute were measured on admission, at two weeks of hospitalization, and upon discharge to assess the clinical outcomes. Results: After treatment, the "mobility" dimension profile of the European Quality of Life-5 Dimensions-5 Levels changed from 1 to 1, "self-care" changed from 1 to 1, "usual activity" changed from 3 to 2, "pain/discomfort" changed from 4 to 3, and "anxiety/depression" changed from 3 to 2. The Numerical Rating Scale score for chest pain decreased from 7 to 5, chest tightness decreased from 6 to 4, sleep disturbance decreased from 6 to 3, polydipsia decreased from 5 to 3, and stomach discomfort decreased from 5 to 3. All the profiles of these five dimensions were unchanged for 11 days in the period between the two weeks of hospitalization and discharge. There were also no changes in the profiles of either "mobility" or "self-care" throughout the entire 24-day hospitalization period. Conclusion: This case report shows that integrative Korean medicine treatment based on Pyongsim Solution can improve the symptoms of patients with variant angina and mild intermittent asthma, suggesting that Pyongsim Solution could be an important herbal medication for treatment.

        생식용 굴(Crassostrea gigas) 작업장의 위생안전성에 대한 모니터링

        This study assessed the safety of raw oysters (Crassostrea gigas) for consumption during processing in a processing plant. Bacterial contamination (e.g., viable cell counts, coliform groups, and pathogenic bacteria) and chemical contamination (e.g., heavy metals and shellfish toxins) were measured on raw oysters, a processing equipment, employees, and work areas. No total mercury, lead, paralytic shellfish poison, diarrheic shellfish poison, or norovirus was detected in any post-harvested oyster samples. However, the cadmium level ranged from 0.1-0.2 mg/kg. The viable cell count and Escherichia coli and coliform group levels in post-harvested oysters ranged from 4.00-4.54 log CFU/g, ND-210 MPN/100 g, and 110-410 MPN/100 g, respectively. The viable contaminating cell counts on employees, equipment, and work areas were in the range of 0.90-3.46 log CFU/100 cm<sup>2</sup>. Airborne bacteria in the work areas ranged from 0.60 to 1.81 log CFU/plate/15 min. Thus, no significant health risks were detected in the processing plant.
