        양안에 발생한 홍채낭종 1예

        Purpose: Iris cyst is a rare condition and there has been no Korean report of bilateral iris cyst. The authors present the first Korean report of a patient with bilateral iris cysts. Methods: A 62-year-old man presented with intermittent visual field disturbance of the left eye when exposed to bright light. The patient underwent visual field examination and slit lamp examination. On slit lamp examination, a dark-brown mass was revealed along the lateral pupillary margin in the left eye. In the right eye, the shape of the iris was convex at the 8 o`clock direction. Ultrasound biomicroscopy, confirmed the presence of a mass. Results: The patient showed no specific abnormality in visual field examination. Ultrasound biomicroscopy showed the objects to be cystic, with a thin-walled homogenous hypoechoic lesion on the posterior surface of the iris in both eyes. There were two cysts in each eye measuring 4.35 mm×1.13 mm and 1.47 mm×1.11 mm in the left eye, and 1.05 mm×1.02 mm and 1.90 mm×1.05 mm in the right eye. Conclusions: Asymptomatic iris cyst does not need to undergo treatment, but careful examination is nevertheless required required at regular intervals in case a complication develops.

        다리뇌출혈로 인해 발생한 양방향 수평주시마비 1예

        Purpose: To report a case of bilateral horizontal gaze palsy associated with pontine hemorrhage. Methods: A 51-year-old man developed a sudden decrease in consciousness. Computed tomography reveal a pontine hemorrhage and infarctions of both basal ganglia. The mentality was recovered after conservative treatment, but he complained of difficulty in moving his eyes. We performed ophthalmic and radiologic evaluations to confirm a diagnosis of bilateral horizontal gaze palsy and to reveal the condition. Results: On ophthalmic examination, his eye showed complete bilateral horizontal gaze palsy, but convergence and vertical eye movements were normal. Radiologic evaluation revealed a pontine hemorrhage, and the hemorrhagic area was consistent the bilateral paramedian pontine reticular formation (PPRF).

        Vehicle aerodynamic performance is developed using both full-scale wind tunnel as well as quarter scale model at Hyundai-Kia Nam_yang R&D Center. The model wind tunnel is a relatively useful tool compared to the full-scale wind tunnel especially when the aerodynamic performance of a new vehicle is being predicted during the styling stage for a new vehicle. Model scale test can provide valuable aerodynamic data well before the prototype or pilot vehicle construction stage if the geometry and Reynolds similarities are considered properly for the scale model. The prediction accuracy of vehicle aerodynamic performance depends on the detailing of the test model such as underbody and engine room geometry.

      • An Experimental Study on the Aerodynamic Characteristics around Moving Bodies near the Ground

        The objective of present study is to compare the flow characteristics around moving bodies near the ground. The influence of the gap height and ground conditions, the moving and fixed grounds, on the aerodynamic forces as well as the dynamic behavior of the wake flow around the square cylinder near a ground is investigated. The model wind tunnel experiments were carried out in an open circuit low turbulence subsonic wind tunnel. The wind tunnel is equipped with moving belt system to simulate the ground effect. The mean velocity and the turbulent field around a square cylinder were measured using a two-component Particle Image Velocimetry (PlV). From the velocity profiles in wake region, the pressure recovery on a moving ground decreases due to the high momentum supply, finally causing the drag to increase from the fixed ground, while the lift has opposite pattern. Through the full scale wind tunnel testing, aerodynamic force difference occurs under ground conditions and increases as yaw angle increases. Therefore the adequate ground simulation is necessary to enhance the control stability under the transient crosswind.

        Cellular Localization and Translocation of Duplication and Alternative Splicing Variants of Olive Flounder Phospholipase C-δ1

        본 연구의 목적은 넙치 인지질가수분해효소(PLC- δ1) 3가지 타입의 세포내 특성을 규명하고자 하였다. 일반적으로 인지질가수분해효소(PLC)의 신호전달경로는 핵, 세포막, 세포질에 분포한다고 알려져 있으나, 핵내 위치 메커니즘은 여전히 불분명하다. PoPLC-δ1A, PoPLC-δ1B (Sf)과 PoPLC-δ1B (Lf)의 3타입의 유전자들은 각각 핵위치 신호(NLS)와 핵방출서열(NES)을 포함하고 있다. 본 연구에서는, 넙치 3가지 타입 인지질가수분해효소(PLC-δ1)의 세포내 위치이동 메커니즘 분석을 위해 GFP 벡터에 유전자를 삽입하여 ionomycin과 thasogargin처리 후 세포위치와 이동양상을 공초점 레이저 주사현미경으로 관찰하였다. PoPLC-δ1A는 PoPLC-δ1B (Lf)와 PoPLC-δ1B (Sf)가 원형질막에 국한되어 분포할때 세포질과 세포막보다 세포 소기관에 분포되어 있었다. PoPLC-δ1B (Lf) 및 PoPLC-δ1 (Sf)이 핵 세포질내 이동양상을 보이지 않을 때, PoPLC-δ1A은 ionomycin과 thapsigargin 처리에 의해 핵 내에 축적되는 양상을 나타냈다. 이런 결과는 손상되지 않은 기능적 NES 서열을 포함하는 PoPLC-δ1A가 어류에서 핵 세포질 내 왕복 및 이동의 주된 역할을 한다는 것을 보여주고 있다. The purpose of this study was to investigate the cellular characterization of phospholipase C-δ1 in olive flounders (Paralichthys olivaceus). In general, phospholipase C signaling pathways are distributed in nuclei at plasma membranes and in cytoplasms, although the pathways" nuclear localization mechanisms are unclear. P. olivaceus duplicates type-A PoPLC-δ1 (PoPLC-δ1A), which has a high similarity to the human isoform PLC-δ; type-B PoPLC-δ1 (PoPLC-δ1B [Sf]), which has a low similarity to the human isoform PLC-δ and the alternative splice variant PoPLC-δ1B (Lf), which has a nuclear localization signal (NLS) and a nuclear export signal (NES) for nuclear imports and exports, respectively. This study confirmed the effects of the cellular localization and translocation of GFP-tagged PoPLC-δ1A, PoPLC-δ1B (Sf) and PoPLC-δ1B (Lf). It administered treatments of Ca<SUP>2+</SUP> ionophore ionomycin and endoplasmic reticulum (ER)–Ca2+ pump inhibitor thapsigargin to hirame natural-embryo (HINAE) cells. A laser-scanning confocal microscope was used. GFP-tagged PoPLC-δ1A was distributed to the cellular organelles, rather than to the cytoplasms and cytomembranes, when PoPLC-δ1B (Lf) and PoPLC-δ1B (Sf) were localized at the plasma membranes. The treatments of ionomycin and thapsigargin showed the accumulation of PoPLC-δ1A in the nuclei when PoPLC-δ1B (Lf) nucleocytoplasmic shuttling and PoPLC-δ1B (Sf) nucleocytoplasmic shuttling were not observed. The results were the first evidence that PoPLC-δ1A, which contains functional, intact NES sequences, has a main role in nucleocytoplasmic shuttling and translocation in fish.

        서울지역 급성위장관염 환자에서 검출된 노로바이러스의 유전자형 분포

        2004년부터 207년까지 서울지역에서 산발적으로 발생한 급성위장관염 환자에 대하여 RT-PCR을 이용하여 노로바이러스를 검출하였다. 검출된 노로바이러스 258건에 대하여 염기서열분석을 실시한 결과 GII.1, GII.2, GII.3, GII.4, GII.5, GII.6, GII.8, GII.10, GII.12, GII.13, GII.15, GII.16, GII.17 등 13가지 유전자형이 검출되었으며 산발적으로 발생한 급성위장관염 환자에서 가장 많이 검출된 유전자형은 GII.4(76.7%)로 나타났다. 노로바이러스 유전자형의 다양성에 대한 연구로 서울지역 노로바이러스성 질환의 예방을 위한 기초자료로 활용될 것으로 기대된다. Fecal specimens from acute gastroenteritis in Seoul from 2004 to 2007 were collected and then tested for the presence of norovirus by RT-PCR. 258 noroviruses were subjected to be futher characterized to sequencing analysis. The sequencing analysis showed that thirteen genotypes were detected (GII.1, GII.2, GII.3, GII.4, GII.5, GII.6, GII.8, GII.10, GII.l2, GII.13, GII.l5, GII.l6, GII.l7) and predominant genotype was GII.4 (76.7%) in the cases of norovirus detected sporadic acute gastroenteritis in Seoul. By this molecular investigation, genotypic distribution associated with norovirus infections will be used for control and prevention of norovirus related diseases.

        유리체절제술 시 관류액병의 높이와 안압의 상관관계

        목적: 유리체절제술 시, 관류액병 높이와 안압의 상관관계에 대하여 알아보고자 한다. 대상과 방법: 23게이지 무봉합 유리체절제술을 시행한 24명 24안을 대상으로 하였다. 유리체절제술 후 관류액병의 높이를 trocar 위치에서 40 cm, 45 cm, 50 cm, 55 cm로 높이며 안압을 측정하였다. microcannulas를 막고 안압을TONO-PENⓇXL와 IcareⓇ PRO두 가지 안압계를 사용하여 각각의 높이에서 5회씩 측정하였다. 결과: 안압은 관류액병의 높이가 올라감에 따라 유의한 양의 상관관계를 보여 TONO-PENⓇXL 의 경우 선형모형에서 Y=0.65X-18.108(R2=0.794, p=0.000), 이차모형에서 Y=0.013X2-0.569X+10.446 (R2=0.801, p=0.000), IcareⓇ PRO의 경우 선형모형에서 Y=0.614X-15.842(R2=0.820, p=0.000), 이차모형에서 경우 Y=0.010X2-0.306X+5.688 (R2=0.825, p=0.000)로 나타나 TONO-PENⓇXL와 IcareⓇ PRO로 측정한 경우 모두에서 선형모형보다는 이차모형이 더 적합한 것으로 나타났다. 결론: 유리체절제술 시, 관류액병 높이에 따른 안압의 변화는 일정하게 증가하는 선형모형보다는 높이가 높아질수록 안압 상승폭은 더욱 증가하는 이차모형에 더 가까운 것으로 나타났다. 따라서, 시신경의 허혈성 손상이 우려되는 환자를 유리체절제술을 시행하는 경우, 관류액병의 높이를 적절히 유지하는 것이 추가적인 허혈 손상 예방에 중요하다고 생각한다. <대한안과학회지 2015;56(5):727-731> Purpose: To determine the correlation between intraocular pressure (IOP) and the bottle heights during vitrectomy using TONO-PENⓇXL applanation tonometer and IcareⓇ PRO rebound tonometer. Methods: Twenty-four eyes of 24 patients who underwent 23-gauge sutureless vitrectomy were evaluated. After complete vi-trectomy, the IOP was gradually increased by lifting the irrigation bottle height from the trocar insertion site by 40 cm, 45 cm, 50 cm, and 55 cm. The distance between the floor and patient's eye was consistent in all cases (105.5 cm). Before the removal of 23-gauge microcannulas, IOP was measured five times using each of the two methods, Tono-PenⓇXL and IcareⓇ PRO. Results: The mean IOPs were 8.25 ± 0.35 mm Hg for TONO-PENⓇXL and 8.96 ± 0.32 mm Hg for IcareⓇ PRO at 40 cm bottle height. As the bottle height increased, the differences in IOP was also increased, 10.71 ± 0.37 mm Hg at 45 cm, 14.18 ± 0.39 mm Hg at 50 cm and 17.93 ± 0.40 mm Hg at 55 cm for TONO-PENⓇXL and 11.48 ± 0.31 mm Hg at 45 cm, 14.64 ± 0.31 mm Hg at 50 cm and 18.13 ± 0.38 mm Hg at 55 cm for IcareⓇ PRO. In TONO-PENⓇXL, the linear equation was Y = 0.65 X - 18.108 (R2 = 0.794, p = 0.000) and the quadratic equation was Y = 0.013 X2 - 0.569 X + 10.446 (R2 = 0.801, p = 0.000). In IcareⓇ PRO, the line-ar equation was Y = 0.614 X - 15.842 (R2 = 0.820, p = 0.000) and the quadratic equation was Y = 0.010 X2 - 0.306 X + 5.688 (R2 = 0.825, p = 0.000). The results show correlation of the quadratic equation was stronger than the linear equation in both tonometers. Conclusions: The differences of IOP were positively correlated with bottle heights in the form of a curve during vitrectomy. Therefore, the patients who are susceptible to retina or optic nerve damage during vitrectomy should be closely monitored. J Korean Ophthalmol Soc 2015;56(5):727-731

        서울근교 약수터에서 분리한 Pseudomonas속균의 혈청생화학적 특성 및 항균제 감수성

        서울시내 약수에서 Pseudomonas속균의 균종분포, 생화학적 특성, 혈청학적 특성 및 항생제 감수성을 조사하기 위하여 약수 57개소에 대하여 봄, 여름, 가을에 걸쳐 조사하였다. 총 57개 약수를 조사한 결과, 33개 (57.9%)의 약수에서 Pseudomonas속균이 검출되었다. 봄, 여름, 가을에 모두 검출된 곳은 1곳(1.8%) 이었으며, 2회에 걸쳐 검출된 곳은 9곳 (15.8%)이었으며, 1회만 검출된 곳은 13곳 (22.8%)이었다. 지역별로는 청계산이 50%의 검출율을 나타내었으며, 북한산, 35.7&, 대모산 33.3%, 도봉산 29.6%, 수락산 25.9%, 우면산 22.2%, 불암산 7.4% 순이었다. 분리된 균종은 Ps. putidark 22주(50%)로 가장 많았으며, Ps. aeruginosa, Ps. fluorescens 및 Ps. mendocins 각 6주(13.6%), Ps.aureofaciens 4주(9.1%)이었다. 분리된 Ps. aeruginosa6주의 혈청형은 모두 5종으로 A형 2주 , B, C,E 및 G형 각 2주씩이었다. 분리된 Ps. aeruginosa의 항생제에 대한 내성은 Amoxicillin 90.9%, Chloramphenicol 84.1%, Carbenicillin 81.8%, Nalidixic acid 68.2%, Neomycin 38.6%, Streptomycin 31.8%, Gentamicin 4.6%, Kanamycin 4.6% 그리고 Colistin 2.3%이었다. Ps.aeruginosa는 다른 슈도모나스균에 비해 carbenicillin에 대한 감수성이 있으나 kanamycin에 대한 내성이 높았다. Ps. aureofaciens는 다른 슈도모나스균에 비해 streptomycin에 대하여 내성이 없었다. 분리된 Ps.aeruginosa의 항생제에 대한 다제내성양상은 5제내성이 31.8%로 가장 높았으며, 내성양상에서는 NA-CB-CTE-AMC가 18.2%로 가장 높게 나타났다. In order to investigate the species, serobiological characteristics and antibiotic sensitivity of Pseudomonas spp, we isolated Pseudomonas spp from 57 spring waters around Seoul area for spring, summer and autumn and identified Pseudomonas spp by biochemical characteristics and serological method. And also we tested the antibiotic sensitivity test by discdiffusion method. Of 57 spring waters tested, Pseudomonas spp were isolated from 33 spring waters(57.9%). Isolation rate of Pseudomonas spp in spring season was 28.1%, summer 21.1% and autumn 28.1%. Only 1 spring water was detected Pseudomonas spp in all seasons and 9 (15.8%) were detected for 2 seasons and 13 (22.8%) were for only 1 season. Isolation rate of Pseudomonas spp at Mt. Cheonggye was 50% and followed by Mt. Bookhan 35.7%, Mt. Daemo 33.3%, Mt. Dobong 29.6%, Mt. Surak 25.9%, Mt. Woomyun 22.2% and Mt. Bulam 7.4%. Of 44 Pseudomonas spp, 22 strains (50%) were identified by Ps. putida, Ps. aeruginosa, Ps. fluorescens and Ps. mendocina were identified 6 strains (13.6%), respectively. 4 strains (9.1%) were identified by Ps. aureofaciens. Of 6 Ps. aeruginosa, serotype A was 2 strains, B, E, G, and K was 1 strain, respectively. Of 44 Pseudomonas spp, resistance rate to amoxicillin was 90.9% and followed by chloramphenicol 84.1%, tetracycline 84.1%, carbenicillin 81.8%, nalidixic acid 68.2%, neomycin 38.6%, streptomycin 31.8%, gentamicin 4.6%, kanamycin 4.6% and colistin 2.3%. Ps. aeruginosa was more sensitive to carbenicillin than other Pseudomonas spp isolated from spring waters in Seoul area but more resistant to kanamycin, and Ps. aureofaciens was no resistant to streptomycin. Among multiple drug resistance, resistance to 5 drugs was 31.8%, 4 drugs 15.9%, 7 drugs 13.6%, 1 drug and 2 drugs 4.6%, and 8 drugs 2.3%, respectively. The multiple resistance patterns detected highestly were NA-CB-C-TE-AMC (18.2%), NA-CB-N-C-TE-AMC (13.6%), CBC-TE-AMC (11.4%) and NA-CB-N-C-TE-AMC-S (9.1%).

        서울지역 급성위장관염 환자 및 지하수에서 분리한 A형 로타바이러스의 유전자형

        2009년부터 2010년까지 서울지역에서 산발적으로 발생한 급성위장관염 환자에 대하여 효소면역측정법(ELISA)을 이용하여 로타바이러스를 검출하였다. 총 1,916건 중 로타바이러스는 354건(18.4%)이 검출되었다. RT-PCR을 이용하여 유전자형 분석을 실시한 결과 P6G[4] (35%), P8G[3] (28%), P8G[1] (24%), P4G[2] (10%), P8G[9] (3%)로 나타났다. 서울지역 지하수 70지점을 대상으로 효소면역측정법을 이용하여 로타바이러스를 조사한 결과 2 지점에서 양성으로 검출되었으며, RT-PCR을 이용하여 유전자형 분석을 실시한 결과 P8G[3]로 나타났다. 이러한 연구결과는 서울지역 로타바이러스 질환의 예방을 위한 기초자료로 활용될 것으로 기대된다. Among a total of 1,916 samples investigated, 354 samples (18.4%) were positive. The predominant genotypes of positive samples were confirmed as P6G[4] (35%), P8G[3] (28%), P8G[1] (24%), P4G[2] (10%), P8G[9] (3%), respectively. Among a total of 70 ground water samples investigated, 2 samples (2.8%) were positive. The genotypes of positive samples were confirmed as P8G[3] (100%). By this molecular investigation, genotypic distribution associated with rotavirus will be used for control and prevention of rotavirus related diseases.
