        목적 : 본 연구는 암환자의 요구를 포괄적으로 포함하면서 실용적으로 측정할 수 있는 도구를 개발하기 위해 시도되었다. 방법 : 기존의 문헌들과 함께 Wingate와 Lackey(1989)의 내용분석 결과를 근거로 정보요구, 이용가능한 자원, 신체적 요구, 정서적 요구, 영적 요고, 그리고 법적/경제적 요구의 6개 영역을 선정하였고, 각 영역에 대한 문항 내용은 암환자 대상의 요고, 조사연구들를 참고로 하여 작성한 후 간호학 교수 3인의 자문을 받아 문항의 내용을 수정, 추가 또는 삭제하면서 예비문항을 작성하였다. 이를 통해 정보요구 7문항, 이용가능한 자원 5문항, 정서 요구 4문항, 영적요구 5문항, 법적/경제적 요구 4문항, 신체적 요구 7문항의 총 32문항의 예비도구를 작성하였다. 예비 도구의 내용 타당도를 확인하기 위해 강원도 소재 대학병원의 간호사 중 평균 경력 10년 이상인 암환자를 간호하고 있거나 간호한 경험이 있는 수간호사와 책임 간호사 9명을 대상으로 실시하였고, 구성타당도와 신뢰도를 검증하기 위해서는 강원도 충정도 및 경기도 소재 종합병원에 근무하고 있는 간호사 116명을 편의표출하였다. 수집된 자료를 SPSS 10.0 WIN을 이용하여 요인분석 및 Cronbach ${\alpha}$값을 구하였다. 결과 : 1) 내용 타당도 분석 결과, 전문가 합의률이 55.4%로 낮은 '유언장 쓰는 방법을 알고싶다'는 문항이 삭제되어 31문항이 선정되었다. 2) 요인분석을 실시한 결과 신체적 요구, 정보 요구, 영적 요구, 정서 요구, 이용가능한 자원, 그리고 법적/경제적 요구의 6요인이 추출되었고 전체 설명력은 61.840%였다. 제 1요인인 신체적 요구와 제 2요인의 정보요구는 각각 25.354%와 10.903%의 설명력을 나타내어 암환자 요구의 주요 영역으로 규명되었다. 3) 도구의 신뢰도는 내적 일관성을 나타내는 Cronbach ${\alpha}$값은 .90이었다. 결론 : 본 연구에서 개발된 도구는 간호사로 하여금 암환자의 요구를 포괄적으로 사정할 수 있게 하고 궁극적으로는 암환자의 요구에 맞는 총체적이며 포괄적인 간호중재를 마련하는 근거로 활용될 것이다. Purpose : This study was designed to develop an instrument that could be used for comprehensive and effective need assessment for patients with cancer. Methods : In the first phase, a conceptual framework for the instrument was established by Wingate & Lackey (1989). In the second phase, the preliminary instrument was drawn up through a review of the literature and in consultation with three professors in Nursing. In the third phase validity and reliability of the preliminary instrument were tested as follows; 1) an expert validity test of the preliminary instrument was done by nine head nurses and charge nurses who had over ten years experience caring for patients with cancer at Wonju Christian Hospital. 2) A construct validity test and reliability test was done for the instrument by 116 staff nurses selected by convience sampling from hospitals located in Kang-Won, Kyoung-Ki, and Choong-Chung Provinces. The collected data were analyzed using SPSS 10.0 WIN program. For the factors of the instrument, factor analysis was used. The reliability of the scale was analyzed by Cronbach's alpha. Results : The results of the experts' test of validity, showed that, of 32 items, only one item had less than 55.4%. It was then deleted and a total of 31 items was selected. On the basis of the results of the factor analysis, the following six components were identified: physiological, informational, spiritual, and emotional needs, available resources, and legal/financial needs. These factors explained 61.8% of the variance. In the factor analysis, the first factor (physiological needs) and the second factor (informational needs) explained 25.4% and 10.9% of the variance respectively, which were major factors for the needs of patients with cancer in Korea. Cronbach' alpha for the scale was .90 indicating internal reliability. Conclusion : This instrument can be effectively utilized for assessment of needs of patients with cancer in Korea. Use of the needs assessment instrument developed in this study will allow nurses to develop nursing interventions that provide comprehensiveness and continuity in meeting the needs of patients with cancer.

        심장질환자의 심리사회적 요인 및 증상 경험간의 관계 : 6개월 추적조사

        Purpose: The purpose of this study was to identify the relationship between psychosocial factors and symptom experience of patients with cardiovascular disease over a 6-month follow-up period. Method: Baseline data for each of the 138 patients were collected. Eighteen patients were dropped for a final total of 120 patients in the present study. To monitor patients' symptom experience after discharge, each patient was interviewed by telephone, and interviewed directly with a questionnaire at each clinic visit. Results: Mean scores for hostility, anger, anxiety, and depression were 51.63, 22.87, 48.95, and 41.21, respectively. Mean scores for the level of symptom experience at discharge, 3 months and 6 months after discharge were 32.83, 24.79, and 26.70, respectively. There were significant differences in the level of hostility by gender, BMI, job, and monthly income. Also the differences in the level of anxiety and depression were statistically significant according to gender and existence of spouse. In the regression analysis, depression was identified as the significant factor associated with symptom experience at discharge, 3 months, and 6 months after discharge. Conclusion: This study presented baseline data to elucidate whether psychosocial factors were associated with the progression of cardiovascular diseases. Accordingly, it suggests that nurses need to take into consideration psychosocial factor as well as physiological factor in symptom management for patients with cardiovascular disease.

        신규간호사의 이직경험: 질적 메타합성

        Purpose: The aim of this research was to identify, appraise and synthesize available evidence exploring new nurses" experiences of turnover. Methods: The qualitative meta-synthesis method suggested by Thomas and Harden was used. Qualitative research articles considered for inclusion in the review were identified through an extensive search of relevant literature in scientific databases. Data were extracted from the aforementioned articles, and qualitative research findings were pooled according to Thomas and Harden"s review process. This process involved the categorization of findings according to similarity of meaning and aggregation of these sub-themes to produce a comprehensive set of synthesized themes. Results: A total of 4 research articles met the inclusion criteria and were included in the review. The review process resulted in 30 sub-themes that were aggregated into 10 themes: ineffective education; overwhelming work; personal maltreatment by doctors, career nurses, patients and caregivers; lack of professional vision; suffering; fear; loss of confidence; isolation; cynical reaction to turnover; new beginning and growth. Conclusion: These findings illuminated the necessity of educational, organizational, emotional and social support for new nurses to help them accomplish their developmental tasks during their transition, as well as the need for this support to continue even after the turnover.

        노인장기요양보험 구강위생서비스의 활성화를 위한 재가장기요양기관 관리책임자 심층면담

        Objectives: This study was carried out to identify plans to activate home visit oral care services by dental hygienists in Long-term care insurance. Methods: In-depth interviews were carried out with 21 Long-term Home Care Center Managers as target. A total of 21 (27%) Home Care Centers were selected through convenience sampling among 78 Home Care Centers that are located in Won-ju city. The Managers were presented with questions and answered in 20-30 minutes in accordance with the interview instructions. The interview results were analyzed through content analysis, and their experiences and perceptions were classified into two themes and categorized again into four components. Results: The Home Care Center Managers suggested that dental hygienists should activate home-visit oral hygiene services. It is necessary to improve the management process and awareness of the elderly. Conclusions: To activate oral hygiene services, it is necessary to improve the service guideline and enhance the efficiency of the service process. This should be acceptable both to the elderly who need the services and the dental hygienists who provide them.

        시뮬레이션을 활용한 호흡곤란 응급관리 실습이 간호학생의 학습 성과에 미치는 영향

        Purpose: This study was conducted to evaluate the learning outcomes of simulation-based practice on emergency care for patients with dyspnea in nursing students. Methods: One group pre-post experimental design with 28 nursing students was used. Simulation-based practice on the basis of SimMan Human Patient Simulator including academic lectures, simulation lab exercises and debriefing was applied for four and half hours. The learning outcomes were assessed by measuring knowledge, critical thinking, problem solving process of cognitive skills, self-confidence and learning attitudes of affective domain. Furthermore, self reported clinical performance ability of psychomotor skills was examined. Results: After the completion of simulation-based practice, there was a significant increase in the mean of following measured variables: knowledge, critical thinking, problem solving process of cognitive skills, self-confidence, learning attitudes of affective domain and clinical performance ability of psychomotor skills. Significant positive relationships were found among learning outcome measurement variables. Conclusion: Simulation-based practice is an effective method to improve cognitive skills, affective domain and psychomotor skills of nursing students. Hence, Simulation-based practice should be applied for improving current limited emergency care training for nursing students and enhancing students' competency in clinical situations.

        우리나라 병동 간호사의 병원 특성이 재직률, 근무기간 및 퇴직에 미치는 영향: 건강보험심사평가원(HIRAS)자료를 이용한 후향적 코호트 연구

        Purpose: The purpose of this study was to investigate the effects of hospital characteristics on employment rate, working period, and retirement of ward nurses in Korea through a retrospective cohort study based on HIRAS data. Methods: Data were obtained from a report on medical care institutions of Health Insurance Review & Assessment Service (HIRAS). Data from 259,941 nurses who were working for a day or more from January 1, 2012, to December 31, 2016, at 2,942 medical care institutions were analyzed. Life table method analysis, Kaplan-Meier analysis, and Cox proportional hazard regression analysis were conducted. Results: The employment rates of 5 yeas and 10 years for the total sample were 38% and 28%, respectively. The estimated mean value of the working period was 3,642.7 days (SE: 17.4 days). Cox proportional hazard regression analyses revealed that nurses who were working at the general hospital/hospital, clinic, and nursing hospital were more likely to leave the hospital compared to those who were working at the 3rd general hospital. Nurses who were working at the medical institutions which were located in cities and countries, established by the private foundation, rated lower levels of nursing, and owned an insufficient number of beds, nurses and doctors were more likely to leave their workplace compared to those of the counterparts. Conclusion: This study indicates that hospital characteristics may play a significant role in retirement and working period of ward nurses in Korea. The improvement of hospital conditions to reduce ward nurses’ retirement are needed.
