We report a case of elderly patient with IgA-associated leukocytoclastic vasculitis who was successfully treated with dapsone. A 75-year-old Korean female patient who had suffered from asthma visited because of multiple purpura, vesicle, and bullae on her extremities. She also complained of pitting edema, especially on the lower extremities. She was diagnosed as IgA-associated leukocytoclastic vasculitis by skin biopsy and treated successfully with steroid. How-ever, skin lesions recurred after tapering steroid. We restarted high-dose steroid and the lesions showed improvement. However, skin lesions were refractory to lower-dose steroids, and also refractory to colchicine treatment. Dapsone (50 mg daily) induced dramatic improvement of her cutaneous vasculitis, proteinuria and edema. (Korean J Asthma Allergy Clin Immunol 2007;27:186-189)

Eosinophilic gastroenteritis is a rare disease of unknown etiology. It is characterized by massive tissue infiltration of eosinophils in the layer of any area of gastrointestinal tract, increased peripheral blood eosinophil count and various gastrointestinal symptoms. There are mucosal, muscular, and subserosal types, of which the subserosal type may result in the development of eosinophilic ascites. The cancer antigen (CA) 125 tumor associated antigen can be produced by normal cells of different tissues derived from the celomic epithelium. Serum CA-125 serum level can be increased in a diversity of physiological nonmalignant and malignant conditions. We experienced a 23-year-old female patient with eosinophilic gastroenteritis, who was presented with abdominal distention. The patient`s serum and ascitic fluid revealed increased CA-125 level. Serum CA-125 level was normalized following the administration of corticosteroids. We report this case with a review of relevant literature. (Korean J Asthma Allergy Clin Immunol 2006;26:167-171)

Background and Objective: A novel multiple antigen simultaneous test using ELISA assay (GENEDIA-ELISA) has been developed. It includes two spotted spider mite (TSM) and Japanese Hop pollen (Hop J), which are currently common allergens in Korea but were not readily available for test in the past. The aim of this study was to validate GENEDIA-ELISA assay and to compare it with MAST-Immunoblot assay which was one of the commonly used in vitro tests in Korea. Method: One hundred eight patients were evaluated with skin prick tests, MAST-Immunoblot and GENEDIA-ELISA assay. Using skin prick test results (A/H ratio>1) as the gold standard, the sensitivity, specificity, positive predictive value, and negative predictive value of GENEDIA-ELISA (cut-off=class 2) and MAST-Immunoblot (cut-off=class 2) were examined. The results were analyzed using receiver operating characteristic (ROC) analysis. Result: The overall sensitivity of GENEDIA-ELISA was higher than that of MAST-Immunoblot (61.4% vs. 38.4%; P<0.05) while the overall specificity of each test was similar (88.5% vs. 93.9%). There was a higher correlation between GENEDIA-ELISA and skin prick tests than that obtained between MAST-Immunoblot and skin prick tests (0.47 vs. 0.39). In ROC analysis, the overall result of GENEDIA-ELISA yielded an area under the ROC curve (AUC) of 0.75, which indicated that this assay is more accurate test than MAST-Immunoblot (AUC=0.66). The sensitivities for TSM and Hop J on GENEDIA-ELISA were 83.3% and 83.3%, respectively, and AUC were 0.73 and 0.89, respectively. Conclusion: GENEDIA-ELISA assay may be a sensitive and acceptable tool of specific IgE against common inhalant allergens in Korea. (Korean J Asthma Allergy Clin Immunol 2007;27:26-32)

Melkersson-Rosenthal syndrome, Orofacial edema Melkersson-Rosenthal syndrome (MRS) is a rare non-caseating granulomatous disease of unknown cause that is characterized by a triad of facial edema, facial palsy and furrowed tongue. Although most reported cases of MRS involve swelling of the perioral area, it rarely involves the periocular area. We report a case of MRS involving the periocular area for the first time in Korea. A 60-year-old man presented with recurrent orofacial swelling and facial palsy. Left facial palsy was developed one year ago. Recurrent swelling around the right orbital area and eyelid developed 6 months ago. Physical examination revealed a diffuse and firm edema of the face including the eyelids, periorbital region, and cheeks without erythema, tenderness or pitting. Upon neurologic examination, the left-sided facial palsy of peripheral nerve type was noted without abnormalities in taste sense or lacrimation. Biopsy performed from edematous cutaneous lesion revealed marked dermal edema with perivascular lymphocytic infiltration, telangiectasis and a few granulomatoid histiocytic collection, which was consistent with MRS. The patient was treated with oral antihistamines and corticosteroids without clinical improvement. Physicians should consider the possibility of MRS in the presence of any chronic facial swelling and recurrent peripheral facial palsy. (Korean J Asthma Allergy Clin Immunol 2007;27:268-272)

We here report a case with apple-dependent exercise-induced anaphylaxis, the first case in Korea. A 23-year-old Korean female patient presented with urticaria, angioedema, and dizziness during mountain climbing. A detailed clinical history revealed that she ingested apple 30 minutes before mountain climbing. Prick to prick skin test to apple showed a positive result. The exercise provocation test or the oral challenge test alone showed a negative result. However, the exercise provocation test after apple ingestion resulted in severe anaphylactic reaction. She was diagnosed as having food-dependent exercise-induced anaphylaxis to apples, and was instructed to avoid exercise after apple ingestion in the future. Immediate-type hypersensitivity responses to systemic corticosteroids are rare despite their widespread use. It is still controversial whether the responses rarely occur or are underdiagnosed in clinical settings. Many cases probably remain underdiagnosed because the symptoms often mimic symptoms of underlying diseases. This case report describes a 73-year-old man who had immediate hypersensitivity reactions after intravenous administration of hydrocortisone, which was primarily intended to prevent hypersensitivity reactions to contrast media. Whole body rash with pruritus developed shortly after the steroid injection. Since the patient already experienced rash and itching sense after contrast media and antihistamine injection, we conducted skin testing to figure out which drug triggered the hypersensitivity reactions. Intradermal skin tests revealed a positive response to hydrocortisone sodium succinate, which suggested his hypersensitivity reactions were developed by hydrocortisone. Being a common therapy for allergic reaction, corticosteroids themselves are rarely suspected of causing hypersensitivity reactions. Considering there is no typical symptom or standard diagnostic test, awareness of corticosteroid hypersensitivity reactions is of importance to make the diagnosis. Eosinophilic granulomatosis with polyangiitis (EGPA), also known as Churg-Strauss syndrome, is a rare systemic necrotizing vasculi¬tis affecting small- to medium-sized vessels. EGPA is associated with severe asthma and eosinophilia. The most frequently involved organs are skin and peripheral nerves; however, EGPA may involve other organs, such as the gastrointestinal tract, kidney, and heart. Antineutrophil cytoplasm antibodies (ANCAs)-related abnormal immune reactions are known to be associated with EGPA, but only 30%-40% of patients have a positive marker of ANCA. ANCA-negative patients are at higher risk of cardiac involvement than ANCA-positive patients. Cardiac involvement is one of the leading causes of mortality and could be resistant to conventional treatment. Early treatment with steroid plus cyclophosphamide is important because it could give chances of restoration of cardiac function. For patients undergoing heart transplantation, we should consider the severity of cardiac disease and the presence of systemic dis¬eases, including vasculitis. Here, we report a case of a 25-year-old EGPA patient with cardiac involvement who eventually received heart transplantation for progressive heart failure, although treated with systemic corticosteroid with cyclophosphamide. EGPA pa¬tients undergoing heart transplantion are rarely reported worldwide, and this is the first case report in Korea.(Allergy Asthma Respir Dis 2015;3:159-163)

Background: Allopurinol-induced hypersensitivity reactions are rare but increasing recently in clinical practice; however, little has been known about its clinical characteristics and underlying mechanism. Objective: The goal of this study was to obtain more accurate information on the clinical feature and course of allopurinol-induced hypersensitivity reactions. Method: We retrospectively reviewed medical records of 13 patients diagnosed with allopurinol-induced severe cutaneous adverse drug reactions between September, 2003 and April, 2006. Result: All patients (n=13) showed skin rash as an initial manifestation. Eleven patients had severe skin rash and multiple organ involvements such as peripheral blood eosinophilia, hepatitis, or renal dysfunction. They were diagnosed with drug hypersensitivity syndrome (n=5), Stevens-Johnson syndrome (n=5), or toxic epidermal necrolysis (n=1). There was one mortality with toxic epidermal necrolysis. Conclusion: Allopurinol-induced hypersensitivity is associated with significant morbidity, and potentially life-threatening. Therefore, attention should be given when prescribing allopurinol. Drug-induced aseptic meningitis (DIAM) is a rare manifestation of drug-induced hypersensitivity syndrome. Lamotrigine is a non-aromatic anticonvulsant drug with antidepressant activity and widely used nowadays. So far, several cases of lamotrigine-induced hypersensitivity syndrome have been reported. However, there has been only one case report of lamotrigine-induced DIAM worldwide. A 33-year-old woman with an 8-year history of bipolar disorder was hospitalized, complaining of fever, a generalized maculopapular rash, leucocytopenia, hepatic dysfunction, and lymphadenopathy which developed 10 days after initiation of lamotrigine treatment. In addition, she presented meningeal irritation signs, and cerebrospinal fluid examination showed pleocytosis, an increased protein concentration, and a normal glucose level. However, no evidence of an infectious condition was found. Based on clinical and laboratory findings, we diagnosed her as having lamotrigine-induced hypersensitivity syndrome with DIAM and administered methylprednisolone systemically. Her clinical symptoms quickly improved and abnormal laboratory findings were normalized. To the best of our knowledge, this is the first case of lamotrigine-induced DIAM in Korea.
