좌골의 결핵 감염은 드문 질환이며, 수술적 치료와 함께 항결핵제 사용을 병행하는 것이 추천된다. 본 증례의 환자는 우 둔부의 농양에 대하여 수술적 가료를 받았으나 항결핵제 치료를 받지 않았다. 8년 만에 동일 부위에 농양이 재발하여 수술적 변연 절제술과 함께 병리 조직학적인 결핵감염을 확진한 후 항결핵제 치료를 병행하였다. 1년 추시 관찰 결과 수술 부위의 통증이나 재발소견은 보이지 않았다. Tuberculous infection of the ischium is a rare condition. It is recommended that antituberculosis chemotherapy is combined with surgical intervention. The patient in this case had an abscess on his right gluteal region and he had undergone an operation without antituberculosis chemotherapy. After eight years, an abscess recurred in the same area and he was treated with surgical resection. Antituberculosis chemotherapy was administered following histopathological confirmation of tuberculosis. At 1 year postoperatively, the patient had no pain and there was no evidence of recurrence.

Pigmented villonodular synovitis (PVNS) is a rare proliferative disorder that affects synovium, tendon sheath and bursa. Although the condition can present in any joint, knee joint is the most commonly affected site and only 2.5% of cases occur in foot and ankle joint. PVNS occurs in two types: localized and diffuse. Localized type is characterized by focal involvement of the synovium with either nodular or pedunculated masses, Diffuse type affects virtually the entire synovium. Diffuse type has reported more recurrence rate. We have experienced a patient who has diffuse type PVNS of ankle joint and report an optimal method of surgical treatment.

Harris-Galante II형 비구컵을 이용한 무시멘트 고관절 전치환술

목적: 일차적 인공 고관절 전치환술에 사용되었던 Harris-Galante Ⅱ형 미세포말형 비구컵의 평균 10년 장기추시 결과를 살펴보고자 하였다. 대상 및 방법: Harris-Galante Ⅱ형 비구컵으로 인공 고관절 전치환술을 시행 받았던 98예(78명)를 대상으로 하였고, 평균 추시 기간은 121개월, 수술 시 평균 나이는 45세였다. 임상적, 방사선학적 평가와 폴리에틸렌라이너의 마모 정도 등을 측정하였다. 비구컵에 대해 Kaplan-Meier 방법에 의한 생존율을 분석하였다. 결과: 비구컵 주위의 골용해는 25예(25.5%)에서 관찰되었고, 1예(1.0%)에서 무균성 해리소견을 보였다. 폴리에 틸렌 라이너의 평균 마모율은 0.16 ㎜/year였으며, 폴리에틸렌 라이너의 분리가 총 9예(9.2%)에서 나타났다. 총 20예(20.4%)에서 비구컵의 재치환술을 시행하였으며, 비구컵의 10년 생존율은 87% (83-90%)였다. 결론: Harris-Galante Ⅱ형 미세포말형 비구컵은 장기 추시에서 골용해와 잠금 기전의 실패로 인해 비교적 높은 재치환율을 보였으나, 낮은 무균성 해리율은 fiber-metal 미세포말 표면 처리 방법이 장기간의 고정력에서 우수하였음을 시사한다. Purpose : To evaluate the average ten-year follow-up results of 98 total hip arthroplasties using Harris-Galante-Ⅱ porous-coated acetabular components. Materials and Methods : Ninety-eight hips were evaluated. The mean age of the patients was 45 years, and the mean follow-up was 121 months. The clinical and radiographic findings including the wear of the polyethylene liner were evaluated. The endurance of the acetabular component was analyzed using the Kaplan-Meier method. Results : Retroacetabular osteolysis, dissociation of the PE liner and aseptic loosening of the acetabular component was observed in 25 hips (25.5%), 9 hips (9.2%), and 1 hip (1.0%), respectively. The average rate of liner wear was 0.16 ㎜/year. Twenty acetabular components (20.4%) were revised, and the survival probability of the acetabular components at 10 years was 87% (95% confidence limits, 83-90%). Conclusion : In this long-term follow-up study, the revision rate of Harris-Galante-Ⅱ acetabular component was relatively high, and was associated with locking mechanism failure and etroacetabular osteolysis. However, the rarity of aseptic loosening suggests that excellent long-term fixation could be achieved with the surface treatment of a fiber-metal porous coating.

목적: 고관절 주위 골절이 있었던 노인 환자에서 대퇴골두의 관절 연골의 변성 정도를 육안적, 조직학적, 면역 조직 화학적 소견을 통해 평가해 보고자 하였다. 대상 및 방법: 고관절 주위 골절로 양극성 반치환술을 시행 받은 65세 이상 환자 중 수상 전에 고관절부 동통없이 보행이 가능하고 술 전 단순 방사선 사진상 퇴행성 변화가 없었던 30예를 대상으로 하였고, 평균 나이는 79.7세 (65-94)였다. 육안적 소견은 술 중 적출된 대퇴골두의 체중 부하부위 3구역(전상, 중상, 후상방) 및 비체중 부하 부위 1구역(하방)에서 시행한 후, 총 4구역에서 연골하골을 포함한 관절 연골을 채취하여 Hematoxylin & Eosin 염색 및 Safranin O 염색을 시행하여 조직학적 변성정도를 분류하였고, Ⅱ형 collagen 항체를 이용하여 면역 조직 화학염색을 시행하였다. 결과: Outerbridge 분류에 의한 육안적 소견은 grade Ⅰ 이하가 82.5%였고, Mankin 등급에 의한 조직학적 소견과는 유의한 상관 관계를 보였다. Mankin 등급에 의한 조직학적 소견은 24예(80%)에서 경도의 변성을, 나머지 6예에서 중등도의 변성을 보였으며, 각 구역별 변성 정도는 모두 평균 5점 이하로 경도의 변성을 보였다. 체중 부하부위와 비체중 부하부위 간의 변성 정도에는 유의한 차이가 없었다. Hollander 등의 분류에 의한 면역 조직 화학염색상 결괴는 grade Ⅰ 이하가 28예(93.3%)로 대부분 정상 또는 경미한 변성을 보였다. 결론: 고관절 주위 골절이 있었던 노인 환자에서 방사선적으로 고관절부의 퇴행성 변화 소견이 없는 경우 대퇴 골두 연골의 변성 정도는 대부분 경도의 변성을 보였으며, 부위와 상관없이 전반적인 변화를 보였다. Purpose: To evaluate the incidence, grade and extent of the degenerative changes in the femoral head articular cartilage in the elderly with hip fracture through gross, histological and immuno-histochemical studies. Materials and Methods: The study examined 30 femoral heads obtained from patients over 65 years with no arthrosis by radiograph, who had undertaken bipolar hemiarthroplasty due to hip fracture. The mean age of patients was 79.7 years and the study examined three weight bearing zones (anterior, middle and posterior) and one non-weight bearing zone (inferior) of each femoral head. Results: The gross findings showed significant correlation with the Mankin grade for each zone (p<0.05). Twenty four cases (80%) showed mild degeneration according to the Mankin grade and the others moderate. The mean Mankin score at each zone of all femoral heads was 5 points, which indicated mild degeneration. No significant difference was found between the weight bearing zone and the non-weight bearing zone (p=0.75). The distribution of type Ⅱ collagen antibody stainabilities mainly showed normal or mild degeneration (93.3%). Conclusion: Degeneration of the articular cartilage of the femoral heads in the elderly with hip fracture and no arthrosis by radiograph was mostly mild, and showed generalized changes regardless of the zone.
